The applicability of Diabetes Mellitus(DM) in Oriental Medicine might be placed under the category in Wasting-thirst(消渴, WT). However, because of WT having so various concepts, classification, and names in Oriental Medicine, this resulted in confusion in applying WT treatment to DM. WT occurs mainly due to dryness-heat(燥熱) which by depleting 'Fluid and Humor'(津液) makes symptoms of thirst(口渴) swift digestion with rapid hungering(消穀善饑) frequent urination(小便頻數) according to the "Internal Classic"(內經). WT was also called Sodan(消癉), Bidan(脾癉), or Sojoong(消中) in the "Internal Classic", which was caused by not only the disorder of 'five viscera' (五臟), but also the failure of 'three yin and three yang'(三陰三陽). However, the doctors of those days had a tendency to manage WT (including DM) with herbal drugs of dryness-heat or with drugs that recover the pancreas and the kidney, that have a tendency to appreciate that WT is the disease of 'five viscera'(五臟). Accoding to the "Internal Classic"(內經) and "Jinguiyaolue"(金匱要略), WT was appreciated that WT is also due to the disorder of Meridian including 'three yin and three yang'(三陰三陽) theory. Disorder of 'yang brightness'(陽明) or 'reverting yin'(厥陰) would cause Wasting-thirst(消渴), so the 'three yin and three yang'(三陰三陽) theory should be considered in WT treatments. Furthermore, 'yang brightness'(陽明) and 'reverting yin'(厥陰) all belong to 'the door'(闔) in the bolt-door-pivot(關闔樞) theory, and the mean of 'the door'(闔) is the function of storage, so we could understood WT is the wasting disease due to the disorder of 'the door'(闔) system in Meridian. WT should also be understood by the point of the bolt-door-pivot(關闔樞) theory in addition to the 'three yin and three yang'(三陰三陽) theory.

Scutellaria baicalensis Georgi, which has been known to be able to clear away heat and remove dampness, was used for febrile disease. It is now clear that Advanced glycation end products (AGEs) play major roles in the pathogenesis of diabetic complications such as atherosclerosis. In this study, we examined whether Scutellaria baicalensis Georgi suppress the AGE mediated inflammatory responses in the THP-1 cells. AGE treatment increased the gene expression of pro-inflammatory cytokines such as tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), interleukin-$1{\beta}$ (IL-$1{\beta}$), monocyte chemotactic protein-1 (MCP-1) and cyclooxygenase-2 (COX-2). Reverse transcriptase-polymerase chain reaction and Western blot analysis revealed that S. baicalensis had inhibitory effects on the expression of pro-inflammatory genes and protein levels in AGE-treated THP-1 cells. S. baicalensis had also reduced the production of ROS in the AGE-treated THP-1 cells. These results suggest that S. baicalensis has inhibitory effects for the development of diabetic vascular complication.

The present study was designed to investigate the effect of Rehmannia glutinosa on phosphorylation of extracellular signal-regulated kinase(ERK) and cAMP response element-binding protein(CREB) in the acute cocaine-treated rats. Rats orally received vehicle or extract of Rehmannia glutinosa 1 h prior to saline (1 ml/kg, i.p.) or cocaine hydrochloride (20 mg/kg, i.p.) treatment. Rats were sacrificed 15 min after a single intraperitoneal injection of saline or cocaine. Rehmannia glutinosa at dose of 50 mg/kg significantly decreased phosphorylation of ERK, CREB and Elk-1 in the nucleus accumbens and striatum of the cocaine-treated rat brain by immunocytochemistry. These results suggest that Rehmannia glutinosa may contribute to the effects of cocaine on gene expression and on behaviors.

The results of this study intended to clarify the apoptotic effects of curcumin on Epstein-Barr virus transformed human B lymphoma (EBV-B) cells are summarized as follows: It was found that curcumin induced endoplasmic reticulum(ER) stress as well as apoptotic cell death in EBV-B cells, although the magnitude of action was insignificant. When EBV-B cells activated by pokeweed mitogen (PWM) were treated with the same concentrations of curcumin, it was found that higher ER stress (GRP78, P-PERK, XBP-1, ATF6, and CHOP expressed) increased unfold protein response (UPR) and thus, apoptosis attributed to ER stress, compared to non-activated EBV-B cells In conclusion, it is expected that curcumin will play an important role for leukemia treatment.

Inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) are important inflammatory mediators implicated in pathogenesis of inflammation and certain types of human cancers. The present study was designed to determine whether Red Ginseng (RG) could modulate $I{\kappa}B$-kinase, iNOS and COX-2 gene expression and immune responses in RAW 264.7 macrophages stimulated with lipopolysaccharide (LPS). RG extract suppressed the expression of LPS-induced $I{\kappa}B$, iNOS, COX-2, and immune responses in a dose-dependent manner. It also showed an anti-inflammatory effect by inhibiting NF-${\kappa}B$ immune response induced by LPS treatment. Inhibitory effect of RG on LPS-induced inflammation was mediated by suppressed phosphorylation of ERK, JNK and p38 through the regulation of the mitogen-activated protein kinase (MAPK) pathway leading to a decreased production of NO, iNOS, COX-2 and NF-${\kappa}B$. The results implied the role of RG as an inflammation regulator and its possible application for curing inflammatory diseases.

The purpose of this study is to investigate effects of Anisi stellati Fructus Water Extract on hydrogen peroxide production in RAW 264.7 mouse macrophages. Anisi stellati Fructus were extracted by hot water. Effects of Anisi stellati Fructus water extract (AS) on hydrogen peroxide production in RAW 264.7 were measured by dihydrorhodamine 123 assay after 20, 24, 28, 44, 48, and 52 h incubation at the concentrations of 10, 25, 50, and $100{\mu}g/mL$. For 20 h incubation, AS significantly increased hydrogen peroxide production in RAW 264.7 cells by $108.6{\pm}1.56%$, $109.5{\pm}1.94%$, $108.4{\pm}1.14%$, and $107.3{\pm}3.06%$ at the concentrations of 10, 25, 50, and $100{\mu}g/mL$ (P < 0.05) respectively. For 24, 28, 44, 48, and 52 h incubation, AS also significantly increased hydrogen peroxide production in RAW 264.7 cells at the concentrations of 10, 25, 50, and $100{\mu}g/mL$ (P < 0.05). These results suggest that Anisi stellati Fructus has the immune - enhancing property related with its increase of hydrogen peroxide production in macrophages.

Herbal medicines are medicinal products containing a single or a mixture of two or more different herbal substances or herbal preparations as active principles. Recently, much attention has been paid to developing various kinds of fermented herbal extracts, a new type of traditional herbal medicine in the field of Korean traditional medicine. The fermentation of medicinal herbs is intended to exert a favorable influence on bioestability, bioavaliablilty and pharmacological activity of herbal extract in the gastrointestinal tract as well as intensifying the nutritional and pharmacological aspects of the medicinal herbs. The purpose of this study was to investigate biological activities of fermented Rehmanniae Radix by lactic acid bacteria at $30^{\circ}C$ for 3 days in comparison with those for Rehmanniae Radix The fermented Rehmanniae Radix exhibited different chemical profile to Rehmanniae Radix generated with HPLC, indicating production of new ingredients during fermentation. Rehmanniae Radix served as good nutritional sources for the growth of lactic acid bacteria showing increased number of bacteria during fermentation. Toxic effect of the fermented Rehmanniae Radix to cells were not seen judged by the MTT assay. The fermented Rehmanniae Radix exhibited better antioxidant effect than non-fermented Rehmanniae Radix analyzed by a SOD-likely assay. Both hypoglycemic and hypotensive effects of the fermented Rehmanniae Radix were also detected and better than those for Rehmanniae Radix in showing dose-dependent inhibitory effects on alpha-glucosidase and ACE, respectively. In conclusion, fermented Rehmanniae Radix appears to have more biological activities than non-feremented Rehmanniae Radix showing not only antioxidant effect but also cardiovascular protection.

Itching is one of the major diagnostic criteria of atopic dermatitis (AD) and one of its most troublesome symptoms that provokes the desire to scratch. Effective control of itching is believed to be one of the basic approaches in controlling AD. The purpose of this study was undertaken to investigate the antipruritic effect of ethanol extracts from Perillae Japonicae Semen leaves (PJSL) and Chaenomelis Fructus (CF) on the scratching behavior induced by pruritogen such as compound 48/80 or substance P in hairless mice. PJSL or CF treatment inhibited histamine release in HMC-1 stimulated compound 48/80 or substance P in a dose-dependant manner. In particularly, co-treatment PJSL ($50{\mu}g/mL$) plus CF ($100{\mu}g/mL$) significantly inhibited histamine release in HMC-1 stimulated compound 48/80 or substance P. PJSL, CF or PJSL plus CF was administered orally for 2 h and then compound 48/80 ($50{\mu}g/site$) or substance P ($100{\mu}g/site$) was injected into rostral back, and scratching of the injected site by the hind paw was counted for 1 h. PJSL or CF administration reduced the scratching behavior induced by compound 48/80 as well as substance P in a dose-dependent manner. Furthermore, co-administration of PJSL and CF markedly suppressed the scratching behavior induced by compound 48/80 as well as substance P. These suppressive effects were synergistically increased by their combination. From the preliminary observations, we considered that ethanol extracts from PJSL and CF could be an effective natural materials for itching treatment.

Osteoporosis is an important public health issue in postmenopausal women. It is a major public health concern and is widely believed that osteoporosis results from imbalance between bone resorption and bone formation. Recently natural products from plants have been extensively studied as therapeutic drugs to treat and prevent various diseases. Hoelen (scientific name, Poria cocos) is a mushroom that is used in traditional Chinese medicine. Hoelen exhibits anti-inflammatory activity and has a protective effect on tumor progression. However, the effect of hoelen in osteoclast differentiation remains unknown. Thus, we examined the effect of hoelen in receptor activator of nuclear factor-${\kappa}B$ ligand (RANKL)-induced osteoclast differentiation. Hoelen significantly inhibited RANKL-induced osteoclast differentiation in bone marrow-derived macrophages (BMMs) in dose dependent manner without toxicity. Also, we showed that hoelen significantly inhibited the mRNA expression of tartrate-resistant acid phophatase (TRAP) and nuclear factor of activated T cells 1 (NFATc1) in BMMs treated with RANKL. In Particular, hoelen greatly inhibited the protein expression of NFATc1. Ectopic expression of NFATc1 partially reverses hoelen-mediated inhibition of osteoclast differentiation. Taken together, our results demonstrated that hoelen may be useful treatment option of bone-related disease such as osteoporosis, reumatoid arthritis, and periodontitis.

The present study investigated the antimicrobial properties of medicinal herbs including Scutellariae Radix (SR: dried root of Scutellariae bicalensis Georgi). Among hot-water extracts of medicinal herbs tested in this study, SR extract showed the most potent antimicrobial activity with minimum inhibitory concentration (MIC) of 0.625 mg/mL. In particular, synergistic effects of antimicrobial activity were observed upon combined application of SR and chitooligosaccharide as indicated by MIC of 0.125 mg/mL and FIC (fractional inhibitory concentration) index of 0.45. Thermal stability analysis indicated that the components responsible for antimicrobial activity was stable for 8 months at $45^{\circ}C$. Antimicrobial activity was proven to be effective in foods as well as in cosmetics as comparable to that of the chemical preservatives.

This study was performed to assess the Effect of SCF(Schisandrae Chinensis Fructus) on Keratinocyte Damage by UV irradiation. The effect of SCF were determined in UV irradiated HaCaT. We measured LDH release and NO release from HaCaT to elucidate the effect of SCF. And iNOS, TNF-${\alpha}$, COX-2, Bax, Bcl-2, Bcl-xL, c-jun, c-fos gene expression were determined in HaCat using real time PCR method. The results are as follows. SCF inhibited LDH-release, NO production in UV irradiated HaCaT. SCF increased the gene expression Bax, Bcl-2 and Bcl-xL protein in UV irradiated HaCaT. SCF suppressed the gene expression TNF-${\alpha}$ in UV irradiated HaCaT. SCF suppressed the gene expression iNOS, c-fos, and c-jun in UV irradiated HaCaT. SCF not affected the suppression of the gene expression COX-2 in UV irradiated HaCaT. The study showed SCF inhibited the cell damage in UV irradiated HaCaT.

Oryeong-san which was first recorded in Shanghanrun describing the treatments of acute febrile disease is one of the frequently used oriental medicines. Oryeong-san has been prescribed for the treatment of symptoms accompanied by edema. The purpose of this study was to examine the diuretic effects of Oryeong-san by different routes of administration. Oryeong-san (100 mg/kg body weight) was administrated by three different routes in Sprague-Dawley rats: intravenous infusion, intraperitoneal injection and oral intake. Oral intake of Oryeong-san significantly increased urinary volume and excretion of $Na^+$, $Cl^-$, and $K^+$ compared to vehicle-treated control group. The effects were concentration-dependent. Intravenously administrated Oryeong-san increased urinary volume and electrolyte excretion but without significance in hydrated (0.02 ml/min/rat for 90 min) anesthetized rats. Similarly, intraperitoneally injected Oryeong-san had no effects on water and urine electrolyte excretion compared with saline control group. These findings suggest that Oryeong-san has different effects on water and electrolyte balance by routes of administration.

Previously, we showed that Dangguisoo-san (DGSS), an herbal formula that has been traditionally used for the treatment of blood stagnation, is also applicable for inflammatory lung diseases. Activation of Nrf2, an anti-inflammatory transcription factor, and suppression of NF-${\kappa}B$, a pro-inflammatory transcription factor, were suggested as an underlying mechanism. However, the constituents responsible for these activities remain unidentified. To this end, we prepared the water extracts of the 9 constituents of DGSS and tested for their effect on Nrf2 by using an Nrf2-Luciferase reporter cell line and western blot analysis. Results show that Carthamus tinctorius L.(CT), one of the 9 constituents of DGSS, strongly activated Nrf2. Similarly, when measured the effect of the 9 constituents on NF-${\kappa}B$ by using an NF-${\kappa}B$-Luciferase reporter cell line and western blotting for nuclear p65, indicative of activated NF-${\kappa}B$, most constituents were capable of suppressing NF-${\kappa}B$ in various degrees. However, CT and Cyperus rotundus L. (CR) strongly suppressed NF-${\kappa}B$ activity elicited by LPS. Of note, CT activated Nrf2 and suppressed NF-${\kappa}B$ strongly as well. Our results contributes to corroborating the anti-inflammatory effects of DGSS by identifying CT and CR as two major herbs responsible for activating Nrf2 and suppressing NF-${\kappa}B$. These results suggest that CT and CR represent some of the effects of DGSS in the regulation of inflammation.

This study aims to evaluate the student satisfaction of problem-based learning(PBL) in Korean medical curriculum. A questionnaire was given to clinical clerkship students of Pusan National University School of Korean medicine. These items covered overall evaluation of module & learning environment, tutoring, individual learning, group learning and effectiveness for clinical clerkship in PBL. By most of all respondents, PBL was recognized as a more effective learning method. Most respondents were satisfied with the group learning, individual learning and effectiveness for clinical clerkship. However, satisfaction was lower for tutors and module. The results of this study demonstrated that the students had a high level of satisfaction in PBL. It might be concluded that PBL was successfully implemented into the Korean medical curriculum.

There have been many studies to find out association between the face characteristics and some diseases or the personality and the health state, but there have been few association studies between the face characteristics and the yin/yang personality. We would like to suggest that the yin/yang personality is associated with the face characteristics. We collected 1,471 subjects from the February, 2008 to July, 2010. We divided into three groups of subjects by a yin/yang personality score and analyzed the face characteristics after adjusting for age. The Analysis of Covariance and Scheffe test was conducted to certify the difference among three groups. P-value for significance was under 0.05. The significant face characteristics were SH36 in men, and FD50_51, SH36, FArea04, FAreaRatio4, FR01, FR05 in women. There is some association between the face characteristics and the yin/yang personality.

The goal of current study is to make a standard pattern identification for post stroke depression using a delphi method. Finally, ten experts of oriental medicine, especially of stroke or depression, participated in Dephi examination. At the first meeting, experts conducted free discussion and determined to use the previous published questionnaires of Deficiency-Excess identification and Cold-Heat identification. From the second round, experts participated in evaluating and correcting the questionnaire by email. New seven questions were added to the questionnaire of Deficiency-Excess identification through the second round. Finally, the standard pattern identification of Cold-Heat or Deficiency-Excess is composed 20 questions and 11 questions, respectively. These pattern identifications for post stroke depression will contribute to research and treatment of oriental medicine.

This study was designed to evaluate the effects of traditional korean herbal medicine(Jaumkunbi-tang) therapy on a hyperthyroidism patient. Clinical data was analyzed on a patient with hyperthyroidism whose main symptoms were fatigue, palpitation, up flame, hyperhidrosis, hypersomnia and dyspepsia. The patient visited our hospital 9 times from November 10, 2011 and remianed until January 30, 2012. He was treated with Jaumkunbi-tang, the traditional korean herbal medicine. After treatment, improvement was seen in clinical symptoms(such as fatigue, palpitation, up flame, hyperhidrosis, hypersomnia and dyspepsia) and thyroid function test. The study suggests that Jaumkunbi-tang, one of traditional korean herbal medicines, could be effective on the treatment of hyperthyroidism.

We investigated the eradication rate and the adverse effects of combined treatment with western and oriental medication on Helicobacter pylori infected patients. We studied the eradication rate and the adverse effects of 1-week-triple therapy and 2-week-oriental medication(Banhasasimtang or Hyangsapeungeuysan) on Helicobacter pylori infected patients confirmed by urea breath test. Comparing 5 patients who took combined treatment with western and oriental medication, the results of this study showed that 4 patients got eradication effect of Helicobacter pylori with no influence on liver and kidney function. A patient had diarrhea and another patient had satiety after meals because of an adverse effect. Despite some possitive effect, the treatment did not make the Quality of Life improved. The findings of this study supports the adverse effects of combined treatment with western and oriental medication for the treatment of Helicobacter pylori infection.

The purposes of this study were to investigate the difference on activities of daily living(ADL), instrumental activities daily of living(IADL) and quality of life of aged woman with dementia reported by aged woman with dementia and caregivers. From December 2009 to February 2010, 64 subjects in the institutionalized aged woman with dementia, 22 caregivers were surveyed through structured questionnaires. There was no difference ADL and quality of life of aged woman with dementia reported by aged woman with dementia and caregivers. But, there was significant differences in IADL. Also, ADL was positively related to IADL in aged woman with dementia and caregivers. ADL and IADL were negatively related to quality of life in aged woman with dementia and caregivers. In daily activity and life quality, there is no difference recognized by aged woman with dementia and caregivers. So it's not matter for the nursery to recognize the demand of the aged woman with dementia and to care them. It is necessary to study the elderly with dementia at home, and to compare the patients in accordance with severity.

This research aims to analyze the ethical requirements and to find the problems for the publication guidelines of oriental medicine journals. A total of 13 domestic journals, related to oriental medicine and registered in 'National Research Foundation of Korea' list, were selected for the analysis of ethical requirements. We inquired the revised year of the publication guideline and the status of ethics committee of selected journals. The ethical requirements of publication guidelines were evaluated by categorizing them into Institutional Review Board(IRB), Helsinki Declaration, Informed Consent, confidentiality and anonymity of subjects, and conflict of interest. In the case of revised year of the publication guidelines, the year of the most recently revised journal was 2011. However, the revised year of publication guidelines were not announced for three journals and a few journals haven't revised the publication guidelines since 2002. In the case of ethics committee status, four journals out of 13 journals maintained the ethics committee. In the result of ethical requirement analysis, nine journals included the information for confidentiality and anonymity of subjects, and four journals included the information for Helsinki Declaration and Informed Consent. However, only one journal included the information for IRB and conflict of interest. Recently, the importance of clinical researches has been increased to prove the effect of oriental medicine scientifically and objectively. The clinical researches should be considered in scientific side and ethical side as well, because they have potential risks to human subjects. In this sense, announcing the ethical requirements in the publication guidelines can be one way to secure the morality of researches. Thus, it is required to prepare correct publication guidelines and ethical requirements in oriental medicine journals.

To evaluate age and gender differences in the relation of cardiovascular risk factors, cognitive impairment, and subclinical carotid atherosclerosis from aged people using by a cross sectional method. Sixty-nine healthy elders living in the community who had not previously undergone carotid ultrasonography were included. We conducted life style surveys, and cognitive function tests including Korean-mini-mental state examination (K-MMSE) and clinical dementia rating-Korean. Various biomarkers from blood were assessed; fasting insulin-like growth factor-1, lipid-profile, high sensitivity C-reactive protein, total homocysteine, glucose, insulin, Homeostasis model assessment (HOMA) for insulin resistance index, vitamin B12, and folate level. Carotid intima-media thickness (C-IMT), and plaques were measured using carotid ultrasonography and aortic ultrasonography, a valid index of atherosclerosis. For the elderly subjects (aged 65-82 years), cognition impairment was more prevalent in females while subclinical atherosclerosis was more prevalentin males. Increased C-IMT has been kept in males, and C-IMT shows increasing trend and the peak at about 80 year-old in females with increasing age. The significant correlations between C-IMT and many vascular risk factors including age, triglyceride, abnormal homocysteinein male, and K-MMSE, insulin, HOMA index and abnormal aortic ultrasonography in female were different in each gender, with the exception of homocysteine (p<0.05). This data suggests that there were differences of age and gender characteristics in terms of subclinical atherosclerosis, cognitive impairment and vascular risk factors in community-living elders. Further larger and longitudinal studies across entire age are required to better understand the effects of risk factors on subclinical atherosclerosis.
