Purpose : The purpose of this study was to investigate functional disability for the elderly in rural Korea and to identify influencing factors of functional disability. Methods : The data were collected for 76 community-dwelling elderly in rural area and above 65 years. We assessed cognitive function, functional disability, depression using LACLS, WHODAS 2.0, SGDS-K. Data were analyzed using descriptive statistics for general characteristics of subjects, Spearman's correlation among LACLS, SGDS-K, WHODAS 2.0, age, educational levels. And multiple regression was used to find influencing factors of functional disability. Results : As a result of this study, we identified that WHODAS 2.0 total score was 50.59, summary score was 9.94 and functional disability of the elderly in rural area is in the 70th percentile. The highest level of disability occurred in areas related to 'life activities (household)', 'participation in society' and the lowest level of disability occured in areas of 'self-care', 'getting along with people'. Functional disability was significantly correlated with age (r=.398), cognitive function (r=-.547), depression (r=-.563) but not educational levels (r=-.215). Finally, we confirmed that depression (𝛽=.371), cognitive function (𝛽=-.263), widowed status (𝛽=.303), age (𝛽=.272), non-participation of community program (𝛽=.165) was significantly influencing factors of functional disability and the explanatory power of these factors was 52.80 %. Conclusion : This study revealed important factors of functional disability. Therefore, we need to consider these factors when we developed program related to health for the elderly (aged > 65 years) in rural Korea. Further, we need to standardize WHODAS 2.0 in order to enhance its applicability in clinical practices. -
Purpose : The purpose of this study was to investigate the effect on lung capacity of healthy men and women in their twenties by performing an intervention using the chest extension exercise and the bridge exercise, which are respiratory muscle strengthening exercises. Methods : Thirty adult men and women in their 20s participated in this study. All subjects participated in the study after hearing the explanation of the purpose and method of the study, filling out a consent form. All subjects were randomly assigned to the chest extension exercise (CEE) group and the bridge exercise (BE) group of fifteen each. Each exercise was performed twice a week for 4 weeks. Lung capacity was measured by forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) using spirometry. Lung capacity was measured before and after exercise. The measured data were compared through the dependent t-test and the independent t-test. The statistical significance level was set at .05. Results : After the intervention, the CEE group showed a significant increase in FVC and FEV1 compared to before the intervention (p<.05). After the intervention, the BE group also had a significant increase in FVC and FEV1 compared to before the intervention (p<.05). However, there was no difference in FVC and FEV1 between groups before and after the intervention (p>.05). Conclusion : There was no difference between groups in lung capacity after exercise. However, both the chest extension exercise and the bridge exercise increased FVC and FEV1, which was thought to be because both exercise methods were effective in increasing lung capacity. Therefore, both chest extension exercises and bridge exercises can be effectively applied as a way to increase lung capacity.

Purpose : Therapeutic exercise should improve the health outcomes of rehabilitation in children with neurodevelopmental disability. The purpose of this study was to investigate the feasibility of primitive reflex integration exercises on forward head posture, balance ability, and concentration in children with neurodevelopmental disability. Methods : This study included 10 children with neurodevelopmental disability. Primitive reflex integration exercises were performed for 40 minutes, twice a week for 8 weeks (16 sessions). The reflective markers were placed at the center of the shoulders and on the ears. A caliper was used to measure the distance between the attachments of the reflective markers to assess the forward head posture. Pediatric balance scale was used to quantify balance ability. Their abilities in terms of changeless sitting, looking at the teacher, putting children's hands on their knees, and looking at immovable and movable objects, were assessed to quantify concentration. Results : There were significant improvements in forward head posture after the intervention (p=.005). Primitive reflex integration exercises significantly improved balance ability of children with neurodevelopmental disability (p=.027). There were also significant improvements in changeless sitting (p=.005), looking at the teacher (p=.004), putting children's hands on their knees (p=.005), and looking at the immovable (p=.004) and movable (p=.004) objects. Conclusion : This study showed that primitive reflex integration exercises were a useful intervention to improve forward head posture, balance, and concentration in children with neurodevelopmental disability. Therefore, primitive reflex integration exercises may also promote and improve their general development. Further studies with appropriate sample size and control group are needed to conclude the effectiveness of primitive reflex integration exercises on improving posture, motor function, and concentration in children with neurodevelopmental disability.

Purpose : This study compared the differences in power and agility of athletes in each sports using visual response speed test (VRST) scores after conducting 10 weeks of body stability exercise (BSE) on elementary school athletes in handball, football, volleyball and conducted a post-hoc test on the measured values. The subjects of this study were baseball (n=27), taekwondo (n=22), and football (n=23) athletes with at least two years of athletic experience. A total of 72 elementary school athletes were measured by VRST after 10 weeks of BSE under the same conditions. Methods : For VRST measurement of the upper extremity, the right and left hands were alternately touched in the order the blazepod equipment lights were turned on. The number of touches for 15 seconds and response touch were measured. In the case of the measurement of lower extremity the left lower extremity was measured first when the Blaze pod equipment light came on. The average value was obtained by measuring 3 times using a measurement sensor with the position indicated in order to measure the upper arms and legs the same. Results : This study confirmed homogeneity among sports and that VRST improved after implementing BSE for sports. However, no statistically significant difference was identified when comparing VRST improvements between sports, and post-hoc test results showed no significant differences either. Conclusion : After applying the BSE program under the same conditions for 10 weeks to elementary school students who can improve their power and agility the most, the results of the examination using the Blaze pod showed that the power and agility of baseball, taekwondo, and soccer players were similarly improved. From the fact that there was no significant difference among sports, it could be inferred that the BES training program could improve VRST without being limited to some sports. -
Background : Klippel-Trenaunay-Weber syndrome (KTS) is a rare congenital medical condition characterized by complex vascular malformation. KTS consists of a classic triad of capillary malformation (hemangioma), venous malformations and bone or soft tissue hypertrophy causing limb asymmetry. The aim of this report is to describe management for gait disturbance and foot pain in a Patient with KTS using custom-made total contact insole. Case presentation : A 32-year-old man with KTS presented with a 3-year history of gait disturbance on hard surface due to right first toe pain and Achilles tendon tightness. The patient had soft tissue hypertrophy, varicose veins and port-wine stains over the right lower limb associated with KTS. True leg length discrepancy was 2 cm. We prescribed custom-made total contact insole to protect his deformed foot and correct leg length discrepancy. The insole of right side included wedge shaped heel lift and the insole of left side included full length lift to add extra support on unaffected side. Also, we provided compression stocking and physiotherapy including manual lymphatic drainage for lymphedema and stretching exercise for tightness in right lower extremity. At 3 years follow-up, postural alignment including pelvic obliquity was improved using a custom-made total contact insole. The degree of scoliosis and foot pain were also reduced. Conclusion : An individualized and multidisciplinary approach is essential regarding the complexity of comorbidities in patients with KTS. For patients with KTS, orthotic management should be considered to prevent and correct deformities related to KTS. Active orthotic management, compression stocking and physiotherapy can enhance the quality of life and function in patients.

Purpose : Forward head posture (FHP) is one of the most common postural malalignment of the cranio-cervical region. Previous studies have reported that FHP might affect both temporomandibular joint (TMJ) and cervical muscles, but still remains unclear. The purpose of this study was to compare the changes of craniovertebral angle (CVA) and muscle properties after smartphone use in healthy individuals with and without FHP. Methods : Fifteen healthy individuals aged 18 to 22 years were included. CVA was evaluated using Dartfish motion analysis, and the subjects were divided into two groups according to their CVA: a FHP group (n = 7, CVA less than 48 °) and a control group (n = 8, CVA more than 48 °). MyotonPro was used to measure muscle properties of masseter, digastric and sternocleidomastoid muscles (SCM). Each subject underwent 15-minutes of smartphone task (web browsing or video watching) in relaxed sitting posture. CVA and muscles properties were assessed both before and after the smartphone task. Results : There were significant changes in post measurements of CVA between the groups. Masseter muscle showed significant differences in pre and post measurements of all muscle properties, and digastric muscle showed significance only in muscle tone. Amount of changes (post-pre), however, showed no significant difference in this study. Conclusion : 15-minutes of smartphone task did not affect CVA and muscle properties of masseter, digastric and SCM in both groups, however, there were significant changes in pre and post measurements of CVA and some muscle properties of masseter and digastric muscles. Therefore, CVA, masseter and digastric muscles might be significantly changed in a heavy duration of smartphone usage more than 15-minutes. Further studies are needed regarding duration of smartphone task, assessments in other various TMJ muscle groups, and participants with pathological FHP conditions. -

Purpose : This study was conducted to investigate whether phonetic complexity affected the type and frequency of articulation errors and the acoustic duration of consonants and words produced by children with functional articulation and phonological disorders. Methods : The participants in this study were 20 children with functional articulation and phonological disorders and 20 children without such disorders who were between 3 years 7 months old and 4 years 11 months old. The participants were asked to name what they saw in pictures and their responses were recorded. The types and frequencies of articulation errors and the acoustic duration of words were analyzed and words were categorized as being of either 'high' or 'low' phonetic complexity. The acoustic duration of initial and final consonants and vowels following initial consonants were compared between the groups according to articulatory complexity. Results : Children with functional articulation and phonological disorders produced articulation errors more frequently when saying high complexity words and had longer word duration when saying low-complexity words than children without such disorders. There was no major difference in initial and final consonant duration between the groups. but the main effect of articulatory complexity on the duration of both consonants was significant. Conclusion : These results suggest that the articulatory-phonic structure of words influences the speech motor control ability of children with functional articulation and phonological disorders. When articulating consonants, children with functional articulation and phonological disorders had speech motor control ability similar to that of children without such disorders.

Purpose : Chronic ankle instability can lead to problems in balance and gait due to weakness of the ankle muscles and decreased proprioceptive sensation. Balance training that stimulates proprioceptors is necessary to improve ankle stability. We aimed to compare the effects of unstable support balance exercises using whole body sonic vibration and an aero-step (TOGU) on proprioception and balance in individuals with unilateral functional ankle instability. Methods : Thirty-six participants with unilateral functional ankle instability were randomly recruited and divided into two groups (group 1 = sonic whole body vibration, group 2= TOGU). Individuals in each group participated in training for 5 weeks, 40 minutes per session, 5 times per week, and performed weight-bearing exercises in five postures on different unstable support surfaces. Proprioception was measured by digital inclinometer (Dualer IQ), and balance was measured by force platform (Biodex balance system). Results : Significant differences were observed in proprioception before and after intervention within both group (p<.05). Significant differences were also observed in the balance index before and after intervention within both groups (p<.05). Conclusion : As a result of this study, it is suggested that for individuals with ankle instability, unstable support surface training using a whole body sonic vibrator and TOGU can have a positive effects on proprioception and balance ability. -
Purpose : There is a lack of research on this field in the Republic of Korea, especially those that have seen the effect of interaction between transcranial direct current stimulation and computerized cognitive rehabilitation therapy. This study divided 30 dementia patients into a treatment group, which received transcranial direct current stimulation and computerized cognitive rehabilitation, and a control group, which received pseudo-transcranial direct current stimulation and pseudo-computerized cognitive rehabilitation. This study evaluated the effects of these treatments on the visual perception, cognition functions, and daily activities of dementia patients. Methods : Fifteen subjects were allocated to the treatment group and the other 15 subjects were allocated to the control group. Treatments were given at intervals of five sessions per week (30 minutes per session) for six weeks (30 times in total). This study used the Neurobehavioral Cognitive Status Examination (NCSE) to examine cognitive functions, MVPT to evaluate visual perception, and FIM to test daily living activities before and after applying the treatments. Results : The results of this study showed that cognitive functions, visual perception, and daily living activities significantly (p<.05) improved after the intervention in the treatment group and the control group. The changes in cognitive functions, visual perception, and daily living activities due to the treatments were significantly different between the groups (p<.05). Conclusion : The results indicated that transcranial direct current stimulation and computerized cognitive rehabilitation therapy improved visual perception and daily living activities by increasing cognitive functions. Consequently, it was found that the simultaneous application of transcranial direct current stimulation in conjunction with a computerized cognitive rehabilitation program was an intervention method that could positively affect the visual perception, cognitive function, and daily living activities of dementia patients. Based on the results of this study, the study of arbitration protocols for demential will have to be more active.

Purpose : The purpose of this study was to examine the effect of task-oriented circuit training using unstable support surface on balance, gait ability, and balance confidence in subacute stroke patients. Methods : Forty-five patients with subacute stroke were randomly divided into the three following groups of 15: 1) TOCT-US group; task-oriented circuit training using unstable surface (experimental group 1), 2) TOCT-SS group; task-oriented circuit training using stable surface (experimental group 2), and 3) CON group; conventional physical therapy (control group). All patients participated in one of the three training programs for 6 weeks, 30 minutes per session, 3 times per week. Patients' balance ability was assessed using the BT-4, BBS (berg balance scale), TUG (time up and go test), and LOS (limit of stability). Gait speed was measured to examine gait ability. K-ABC (activities-specific balance confidence scale) was also used to assess the level of patients' confidence in daily activities. Results : After the intervention, the sway area in experimental groups 1 and 2 decreased, but that in the control group increased. Experimental group 1 showed significant improvement compared with experimental group 2 and the control group. BBS, TUG, and LOS scores of experimental group 1 were significantly improved compared with those of experimental group 2 and the control group. Also, gait speed significantly improved in experimental group 1 compared with experimental group 2 and the control group. Experimental groups 1 and 2 showed significant improvement in K-ABC scores after training. Conclusion : Patients with subacute stroke had significantly improved balance, gait, and level of confidence in performing activities of daily living following task-oriented circuit training using the unstable surface. This indicates that task-oriented circuit training using unstable surfaces can be an effective treatment method for the recovery of balance and gait in subacute stroke patients.

Purpose: Since coronavirus disease 2019 (COVID-19) spread around the world, non-face-to-face services have emerged as the new normal. The functioning of children with developmental disabilities and cerebral palsy improves when parents, therapists, and institutions cooperate. We aimed to investigate the difficult environment of pediatric physical therapy caused by COVID-19, and grasp parents' perceptions of home exercise programs. We intend to design home exercise programs for pediatric treatment in the future and create a non-face-to-face pediatric rehabilitation service system based on our findings. Methods: The first survey was conducted on 30 parents who had children with cerebral palsy and developmental disabilities. It was produced after consultation with relevant experts and discussion with the research team, based on a survey conducted in a previous study. The second survey was produced by revising and supplementing the opinions of parents who completed the first survey and consulting experts. Thereafter, an online survey was conducted on 118 parents who had children with cerebral palsy and developmental disabilities. Results: 61.01 % of the parents reported that they were anxious or afraid for their children to be treated for infectious diseases. 83.90 % of the parents reported that the role of parents had increased. 50.00 % of the parents complained of a lack of skill. 85.59 % of the parents reported that they needed a home exercise program provided by the organization. As a non-face-to-face exercise management method, 35.59 % of the parents wanted a real-time video recording electronically mailed to them, and 34.75 % wanted real-time education using video conferencing programs such as zoom. Conclusion: The threat of COVID-19 has further emphasized the importance of social cooperation and management, leading organizations to enter a new era of non-face-to-face rehabilitation services. It is necessary to collect and systematize related studies to reflect more diverse opinions and improve the perceptions of therapists and parents.

Purpose : This study was to compare the effect changes after physical therapy (extracorporeal shock wave therapy + high-load strengthening exercise) or corticosteroid injection in patients diagnosed with chronic plantar fasciitis. Methods : A total of 40 patients were randomly assigned to each group of 20. According to the intervention method, "Group 1. Physiotherapy" was performed for 12 weeks, and "Group 2. Corticosteroid injection" was performed only once. As a pre-intervention test, plantar fascia thickness, pain intensity I, II (What is the most painful moment of the day?, How painful is the first step in the morning?), and functional performance were measured. To compare the effects of each group, the tests 3, 6, and 12 weeks after were also performed using the same measurement method. Also, after 12 weeks, patient satisfaction was also compared. Results : There was no significant difference between the groups in the change in the thickness of the plantar fascia during all periods. However, pain intensity I, II was significantly lower in Group 1 than in Group 2 at only 12 weeks and functional performance was also significantly increased in Group 1 compared to Group 2 at only 12 weeks. Also, there was no significant difference between groups in patient satisfaction. Conclusion : The physiotherapy protocol, which consisted of extracorporeal shock wave therapy and high-load strengthening exercise, showed excellent results, especially after 12 weeks, compared to corticosteroid injection. It is recommended as a more effective treatment method as it is possible to safely return to daily life by reducing pain and improving functional performance.

Purpose : The purpose of this study is to investigate the effect of mid frequency electrical stimulation during squat exercise on the muscle activity and muscle activity ratio of vastus medialis oblique and vastus lateralis, and to prepare scientific basic data for exercise intervention using mid frequency electrical stimulation. Methods : This study was conducted with students from University C located in Busan, and among a total of 123 subjects, 12 subjects who complained of knee joint dysfunction between 80 and 90 points using the Kujala patellofemoral score (KPS) were used. All subjects participated in the experiment for 3 days, and MVIC values were measured for normalization of muscle activity values on the first day. For the two days, participants participated in the experiment and performed squat exercise or squat exercise receiving mid-frequency electrical stimulation in random order. Measurements were taken in the squat position immediately after the squat exercise, and muscle activities of vastus medialis oblique and vastus lateralis were measured. The measured data were compared through the dependent t test, and the statistical significance level was set to .05. Results : According to the results of this study, in the case of applying mid-frequency electrical stimulation together in the ratio of vastus medialis oblique and vastus lateralis muscle activity during squat exercise, higher values were observed compared to the case of not applying mid-frequency electrical stimulation together, and statistically significant. Also, when mid-frequency electrical stimulation was applied to both vastus medialis oblique and vastus lateralis activities during the squat exercise, higher values were shown compared to the case where the mid-frequency electrical stimulation was not applied together, but there was no statistically significant difference. Conclusion : The results of this study reported that mid-frequency electrical stimulation provided to vastus medialis oblique increased the muscle activity ratio of vastus medialis oblique and vastus lateralis. Therefore, the improvement of coordination due to the enhancement of the activity of vastus medialis oblique through mid-frequency electrical stimulation will be more helpful in the treatment of patellofemoral pain syndrome patient. In addition, it is hoped that the electrical stimulation method applied to exercise will be widely used.

Purpose: The Pedisole is a newly developed shoe-mounted wearable assessment system for analyzing balance and gait. This study aimed to determine the reliability and validity of the parameters provided by the system for static balance and gait analysis of healthy adults. Methods: This study included 38 healthy adults (22.4±1.9 years) with no history of injury in the lower limbs. All participants were asked to perform balance and gait tasks for undertaking measurements. For analysis of balance, both the smart Pedisole and Pedoscan systems were concurrently used to analyze the path length of the center of pressure (COP) and the weight ratio of the left and right for 10 s. Gait was measured using the smart Pedisole and GaitRite walkway systems simultaneously. The participants walked at a self-selected preferred gait speed. The cadence, stance time, swing time, and step time were used to analyze gait characteristics. Using the paired t-test, the intra-class coefficient correlation (ICC) was calculated for reliability. The Spearman correlation was used to assess the validity of the measurements. In total, data for balance from 36 participants and the gait profiles of 37 participants were evaluated. Results: There were significant differences between the COP path lengths (p<.050) derived from the two systems, and a significant correlation was found for COP path length (r=.382~.523) for static balance. The ICC for COP path length and weight ratio was found to be greater than .687, indicating moderate agreement in balance parameters. The ICC of gait parameters was found to be greater than .697 except for stance time, and there was significant correlation (r=.678~.922) with the GaitRite system. Conclusion: The newly developed smart insole-type Pedisole system and the related application are useful, reliable, and valid tools for balance and gait analysis compared to the gold standard Pedoscan and the GaitRite systems in healthy individuals. -
Purpose : The purpose of this study was to compare the ankle pronation squat with the general squat and investigate the effect on the geun varum and the muscles around the knee. Methods : Subjects were chosen as the target for squat exercise with the distance between the knees more than 5 cm. The selected 30 students were randomly divided into 15 pronation squat group and 15 general squat group, and performed five sets movements 20 times. Global postural system (GPS) and digital goniometer were used to check the distance between the knees and the Q angle, and muscle activity was measured with EMG during squat exercise. Results: The result is as follows. First, as a result of analyzing the change in the distance between the knees, the distance between the knees decreased and the Q angle increased in the pronation squat group after exercise. Second, as a result of analyzing the change in muscle activity of the peroneus longus, the muscle activity increased in the pronation squat group after exercise, and it was more effective than the general squat group. Third, as a result of analyzing the change in the VMO (vastus medialis oblique) and VL (vastus lateralis) muscle activity ratio, the activity ratio of the pronation squat group increased after exercise, and the imbalance in the VMO/VL muscle activity was decreased. Conclusion: The pronation squat exercise applied to the ankle will greatly affect what is made into a balanced leg as decreased of the distance between knees, increased of Q angle, increased of the muscle activity of the peroneus longus and the ratio of VMO/VL.
