A Study on the Change of Physical Environment in Seoul Medical Center by Providing Comprehensive Nursing Service (간호간병통합서비스 도입에 따른 서울의료원 병동부 시설환경변화 연구)

  • Purpose: The research looks at differences between the 'General ward' and the 'Comprehensive nursing ward' in Seoul Medical Center, regarding the facility improvements and changes in nursing services. It investigates and analyzes spatial problems and improvement needs through a survey and conduct investigation of staffs. It is to propose the primary data for the architectural planning of the future ward with the comprehensive nursing service. Methods: Targeting the comprehensive nursing ward, changes of the physical environment and spatial problems are analyzed through a field survey, behavior investigation, present-condition investigation, and floor plan analysis. Results: The workforce is increased by approximately twice the amount of the nursing staffs in the comprehensive nursing ward, compared to the pre-general ward. When utilizing the general ward, various spatial problems arise due to the restrictions of the facility condition. Because Sub N.S is an important facility as a part of the nursing work function in the ward of the comprehensive nursing service, the opinions of staffs must be considered when selecting a location and composing a space. Implications: It can be used as a primary data for the comprehensive nursing ward when architectural planning of a new hospital.

Comparison of Patient Needs, Patient Satisfaction, and Communication Type of Nurses between Integrated Nursing Care Service Ward and General Ward Patients (간호·간병통합서비스병동과 일반병동 환자의 간호요구도, 간호만족도 및 간호사의 의사소통 유형 비교)

  • Purpose: The purpose of this study is to provide patient-centered nursing by comparing the patient needs, patient satisfaction, and communication type of nurses for patients admitted to integrated nursing care service ward and general ward. Methods: Data were collected by self-report questionnaires between September 7 and October 9, 2021. Patients were patients recruited from a hospital in D city, 100 each from comprehensive nursing care service ward and general ward. The data were analyzed using descriptive statistics, 𝜒2-test, t-test, ANOVA, and Pearson's correlation. Results: There was no significant difference in patient needs between the patients of integrated nursing care service ward and the general ward (t=-1.12, p=.263). However, patient satisfaction was higher among the patients in integrated nursing care service ward than in the general ward (t=-3.55, p<.001). Pertaining to the communication type of nurses, the informational communication type (t=-4.02, p<.001) and the friendly communication type (t=-3.92, p<.001) were stronger in the integrated nursing care service ward than the general ward. It was, hence, confirmed that the educational requirements were positively correlated with patient satisfaction in the reliability domain and technical-professional domain, and that the nurse informational communication type and friendly communication type had a significantly positive correlation with patient satisfaction. Conclusion: The expansion of the integrated nursing care service ward may be considered in the future, with an informational and friendly communication method as an effective response that can be adapted in practice as a reflection the type of communication among nurses.

A Study on the Architectural Planning for Integrated Nursing Care Service in Ward of Public Hospital - Focused on the Regional Public Hospital in Korea - (간호·간병통합서비스를 위한 국내 공공병원 병동부에 대한 건축 계획적 연구 - 국내 지역거점 지방의료원을 중심으로 -)

  • The study is to identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing service has expanded nationwide from 2016 but 10 out of 34 public hospitals are currently unable to operate as nurses due to the shortage. Analyze each floor plan and space program of ward and provide the basic data for the construction of the architecture plans for Integrated nursing care service. For this study, selecting 14 wards of public hospital in Korea and then analyzes space organization, distance from NS to bedroom and NS to core(Elevator) and spatial hierarchy of them. Among the 14 public hospitals, there are 7 types of double-loaded corridor, 4 types of double corridor and 3 types of single corridor. If NS is placed centrally, the control of the outsider and the patient's control would be efficient, but most of NS are lean on one side of floor. the placement of NS should be independent of individual arrangements, but it should be smooth alignment with patient spaces, staff spaces, and public spaces. This study could serve as basic research for the architectural plan for future integrated nursing care in ward of public hospital.

A Study on Ward of Public Hospital for Spatial Composition of Efficient Integrated Nursing Care Service - Focused on the Regional Public Hospital - (효율적 간호·간병통합서비스 공간 조성을 위한 공공병원 병동부에 관한 연구 - 지역거점 지방의료원을 중심으로 -)

  • The purpose of this study is identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing care service has been expanding on a national scale from 2016 but most of public hospitals are currently unable to operate due to nursing shortage. In this study 8 wards of public hospital have been chosen and analyzed. The measure of space program and area distribution(patient area percentage, staff area percentage, circulation area percentage), nurses's walking distance(distance from ns to patient room, distance from ns to core) were calculated by depthmapX and autocad programs. The result of this study is as follows. Nurse's walking distance is more than 24m so the efficiency of nurse's patient care is reduced. The percentage of patient area in double-corridor is higher and the patient feels more comfortable but the Nurse's walking distance is longer and the accessibility is lowered. NS should be located in the center of the ward and close to the core but some wards are not composed of proper space-separation and flow of human trrafic is overlapped. This study may serve as basic research for the architectural plan for future integrated nursing care ward.

Comparing the Emotional Labor, Job Stress, and Nursing Performance of Nurses in the Comprehensive nursing care Ward and the General Ward (간호·간병통합서비스병동과 일반병동간호사의 감정노동, 직무스트레스 및 간호업무수행)

  • Purpose: This study aimed to improve nursing practice efficiency by analyzing and comparing the emotional labor, job stress, and performance of nurses in comprehensive nursing care wards and in general wards. Methods: The study surveyed 151 nurses in comprehensive nursing care and general wards. The results were analyzed by ${\chi}$ 2test, ANOVA, and Pearson's correlation coefficient. Results: There were no statistically significant differences in terms of emotional labor, job stress, and performance for the two different types of nurses. Statistically significant positive correlations were noted between job stress and emotional labor, nursing performance, emotional labor, and job stress in the comprehensive nursing care wards, and between job stress and emotional labor, and nursing performance in the general wards. Conclusion: A longer duration of direct nursing in comprehensive nursing care wards tended to increase, emotional labor and job stress. However, the quality of nursing provided to patients tended to improve. Therefore, it is necessary to establish an institutional plan for carrying out efficient nursing while reducing emotional labor and job stress among nurses in comprehensive nursing care wards.

Comparison of Nurses's Fatigue, Sleep Quality and Interpersonal Relationship Stress between Comprehensive Nursing Care Service Wards and General Wards (간호·간병통합서비스병동과 일반병동 간호사의 피로, 수면의 질, 대인관계 스트레스 비교)

  • Purpose: The purpose of this study was to compare the differences in fatigue, sleep quality and interpersonal relationship stress levels between nurses in comprehensive nursing care wards and in general wards. Methods: Data were collected using self-reported structured questionnaire from 14 August to 17 September 2018. The participants were 220 nurses from 6 hospitals in Seoul and Gyeonggi-do. The data were analyzed using the IBM SPSS 23.0 program, descriptive statistics, t-test, ${\chi}^2-test$, Fisher's exact test, and Pearson's correlation coefficient. Results: The subjects' fatigue, sleep quality, and interpersonal relationship stress were not significantly different between the comprehensive nursing care services wards and the general wards. The interpersonal stress with a superior was statistically higher among general wards than comprehensive nursing care services wards. The relationship stress between patient and caregiver was higher in comprehensive nursing care services wards than in general wards. Conclusion: A follow-up study is needed to develop a strategic method to reduce interpersonal stress in nurses.

Relationships between Psychosocial Work Environment and Health Outcomes among Nurses in Integrated Nursing Care Wards and General Wards (간호·간병통합서비스 병동과 일반병동 간호사의 심리사회적 업무환경과 건강결과와의 관련성)

  • Purpose: This study aimed to examine the differences in nurses' psychosocial work environment (PWE) in integrated nursing care wards (IWs) with higher nurse staffing and general wards (GWs) with lower staffing, and to analyze the effect of PWE on their health outcomes. Methods: Data were collected from 151 nurses in IWs and 156 nurses in GWs at a tertiary hospital in Seoul in 2023. The Korean version of the Copenhagen Psychosocial Questionnaire III was used to measure PWE and health outcomes. Regression analyses were conducted to examine the effect of PWE on health outcomes. Results: The most negative aspects of the nurses' work environment were found in the work demand domain. IW nurses (vs. GW nurses) reported lower work demand, better work organization and job contents, and a more positive work-individual interface. Nurse health outcomes were also better in the IWs. Lower work demand and better work-individual interfaces were positively associated with improved health outcomes, while ward type (IW vs. GW) was not significantly related to health outcomes. Conclusion: Reducing work demand by expanding integrated nursing care services nationwide and improving nurse staffing in GWs is crucial for improving health outcomes.

  • Prescription, pickup, and acting information from the ward are stored in a hospital database, and can be accessed and retrieved by the relevant departments. Frequent inquiries from many departments can cause a great load on the hospital information system. When the nursing and care services are integrated, the real-time pickup and acting tasks are also increased, which may lead to an increase in database inquiries, thereby increasing the amount of information being transferred. To effectively solve this inefficiency problem, we aimed to develop a nursing and care integrated information system that excludes database inquiries and incorporate a method that transmits pickup and acting information in real-time. Because the new system increases the workload and responsibility, we developed a ward acting dashboard so that every ward employee can determine all the acting situations of patients in real-time to improve the quality of services. We designed a database by concentrating on the pickup and acting business procedures, and applied real-time web techniques to enable pickup and acting information to be delivered instantly. Through our implementation, we were able to reduce the inquiry time and transmission amount significantly compared with the existing method.

Comparative Study of Clinical Nurses' Role Conflict, Emotional Labor and Nursing Performance between Comprehensive Nursing Care Service Wards and General Wards (간호·간병통합서비스 병동과 일반병동 간호사의 역할갈등, 감정노동, 간호업무성과 비교)

  • Purpose: The purpose of this study was to compare the role conflict, emotional labor, and nursing performance outcomes among nurses in comprehensive nursing care service wards and general wards. Methods: Data were collected 10 hospitals located in B city that operated a comprehensive nursing care service for more than one year. A total of 237 nurses were included in this study: 118 in comprehensive nursing care service wards and 119 in general wards. Data were collected using questionnaires and statistically analyzed with $x^2$ test and t-test. Data were analyzed using SPSS win 22.0 program. Results: Role conflict was lower among nurses in comprehensive nursing care wards than in general wards, and the difference was statistically significant. Conclusion: Therefore, organizational support interventions, such as sufficient understanding of others' roles and on-the-job training on nursing, should be planned to reduce role conflict and emotional labor, and systems and policies should be established to improve nursing performance outcomes.

Predictors of Posttraumatic Stress in Psychiatric Nurses (정신과 병동 간호사의 외상 후 스트레스 영향요인)

  • Purpose: This study was performed to identify the impact of aggressiveness of patients and a sense of coherence on posttraumatic stress in psychiatric nurses. Methods: After collecting data from 162 psychiatric nurses, we carried out a t-test, ANOVA, Pearson's correlation coefficient, and multiple regression using IBM SPSS Statistics 19.0. Results: The mean score of posttraumatic stress was $20.75{\pm}16.59$ points. Verbal aggression, aggressiveness about property, aggression toward oneself, and aggression toward others had a positive correlation with posttraumatic stress, while a sense of coherence had a negative correlation with post-traumatic stress. It was concluded that the significant predictors of posttraumatic stress in psychiatric nurses were aggression toward oneself, a sense of coherence, and aggression toward others, all of which accounted for 38.9% of the variability. Conclusion: The results of this study indicate that the factors influencing posttraumatic stress in psychiatric nurses were aggression toward oneself, aggression toward others, and a sense of coherence. Therefore, education programs should be developed in consideration of the fact that aggressive behavior against the patient himself and against others intensify the posttraumatic stress of the psychiatric nurse, but the integration force mitigates it.
