The Change of perception according to the fidelity of simulation in Objective Structured Clinical Examination for Procedural Skill of 4th Medical Students (의전원 4학년 학생의 과정기술 객관구조화진료시험에 도입된 시뮬레이션의 충실도에 따른 인식 변화)

  • The 4th year medical students were randomly divided and designated as group HF and LF, representing high and low fidelity simulations respectfully. Both groups performed OSCE on 2 topics(adult intubation and IV cannulation) with each composed of the same scenario. Each scenario was developed by a faculty in charge of clinical skill education with supervision from 2 other faculty members. High fidelity simulations were designed as in-situ simulations in real OR. Low fidelity simulations had the same scenarios but the environment was simulated only with a simple task trainer in the classroom. All students participated in series of survey using a Likert scale before and after the simulations. The provided data was anlayzed with paired T-test and Mann-Whiteney test(p<0.05). The post simulations self evaluation score for group HF was lower than pre-simulation self confidence score for both topics of adult intubation and IV cannulation where as for group LF, it was lower only for adult intubation(p<0.05). In group HF, the needs for clinical training on both topics increased after the simulation. In group LF, there was no significant change of needs for clinical training between pre and post simulation survey. This suggest that fidelity may be a helpful factor to improve the motivation of the students and further study on economical aspect should be addressed.

Effect of a Self-Evaluation Method Using Video Recording on Competency in Nursing Skills, Self-Directed Learning Ability, and Academic Self-Efficacy (비디오 녹화를 통한 자가평가 학습법이 간호술기 수행능력과 자기주도적 학습능력, 학업적 자기효능감에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate the effect of a self-evaluation method using video recording on competency in nursing skills, self-directed learning ability, and academic self-efficacy in nursing students. Methods: The study design was a non-equivalent pre-post quasi-experimental design. The experimental and control groups were randomly assigned with 35 participants in each group. Interventions for the experimental group were video recording and students' self-evaluation of what they did. Nursing skills included in the study were tube feeding, intradermal injection, subcutaneous injection, and intramuscular injection. Competency in nursing skills was measured one time at the end of the study using a checklist. Self-directed learning ability and academic self-efficacy were measured 3 times (pre-, mid-, and post-intervention) over the 8 weeks. Independent t-test, chi-square test, and repeated measures ANOVA were used for data analyses. Results: There was no statistically significant difference for competency in nursing skills and self-directed learning ability over the 8 weeks of the practice session. There was a significant difference in academic self-efficacy by groups over time. Conclusion: Results indicate that self-evaluation method using video recording is an effective learning way to improve academic achievement in nursing students.

Empowerment on Clinical Nursing Skills Core Program (임상 시나리오 중심 핵심 간호기술 강화 프로그램의 효과)

  • Critical thinking includes creative thinking, open-mindedness, inquisitiveness. Critical Thinking Disposition was significantly higher in experimental group that was enhanced by the scenario-based learning contents than that of the control group(t=4.102, p<.001). Problem Solving process was significantly higher in experimental group that was enhanced by the scenario-based learning contents than that of the control group(t=5.082, p<.001). However, there was no significant difference between the two groups in Clinical Competence(t=2.646, p=.009). But experimental group was significantly higher score than that of the control group score decreased. These results support that the implementation of a scenario-based learning contents need to be continued as an effective educational material but we need more research to establish how this can be effectively implemented in a students' practicum.

Effects of Mentoring Senior Core Fundamental Nursing Skills Education on Clinical Competence and Critical Thinking Disposition in Nursing Students (핵심기본간호술 선배멘토링교육이 간호대학생의 임상수행능력과 비판적 사고성향에 미치는 효과)

  • The purpose of this study was to identify the effects of mentoring senior core fundamental nursing skills education on clinical competence and critical thinking disposition in nursing students. The research design was a one group pre-posttest design and the participants were 57 nursing students. The contents consisted of 3 core fundamental nursing skills of vital sign, nelaton catheterization and subcutaneous injection and total duration of 6 hours. Mentoring seniors conducted to rapport forming & education and evaluated them after exercise and evaluation under the guidance of professor. Data were collected using self-administered questionnaires in pre-test and post-test, the collected data were analyzed using paired t-tests. The result shows the scores of clinical competence and critical thinking disposition were increased significantly after mentoring senior core fundamental nursing skills education in nursing students. The results indicate that this study was effective in improving nursing students clinical competence and critical thinking disposition in core fundamental nursing skills and is expected to be utilized in nusing practice education. Additional studies including those on control groups are recommended to compare difference between the experimental group and control group.

Effects of Open Laboratory Self-directed Practice on Knowledge, Self-confidence, and Skill Competency of Intramuscular Injection (실습실 개방 자율실습이 간호학생의 근육주사법에 대한 지식, 자신감 및 수행능력에 미치는 효과)

  • This study examined the effects of open laboratory self-directed practice on knowledge, self-confidence, and skill competency of intramuscular injection in nursing students. This was a quasi-experimental study using a non-equivalent control group pre-posttest design, and the participants were 32 students comprising an experimental group and 30 students comprising a control group. The experimental group was allowed to perform an open laboratory self-directed practice and traditional practice, and the control group performed traditional practice only. The score of knowledge of the control group was significantly higher than that of the experimental group (F=3.59, p=.019), and those of the self-confidence (F=2.26, p=.016) and skill competency (F=9.08, p<.001) of the experimental group were significantly higher than those of the control group. Open laboratory self-directed practice was effective in improving the nursing students' self-confidence and skill competency. Therefore, students should be encouraged to actively participate in open laboratory self-directed practice and the effective strategies should be developed.

Effect of Human Patient Simulator-based Education on Self-directed Learning and Collective Efficacy (환자시뮬레이터활용교육에서의 자기주도적 학습능력과 집단효능감의 변화)

  • The purpose of this study was to identify the effect of human patient simulator(HPS)-based education on self-directed learning(SDL) and collective efficacy(CE) for nursing students. This study design was one group pre-posttest. The subjects were 2nd grade 92 students enrolling in the integrated practice. They have no previous experience of HPS-based education. HPS-based education included team based learning, skill training, taking a high-fidelity simulation with Medical Education Technologies, Inc (METI) simulator and being debriefed during 12 weeks. The pretest and posttest were conducted to understand the improvement in SDL and CE. After the subjects had participated in the HPS-based education, they showed statistically significant higher SDL(t=4.24, p=0.000) than before. However, there was no significant change in CE. Based on the results, this study suggests that SDL for nursing students were significantly improved by HPS-base education.

Effect of pre-educational evaluation on CPR education of the General population (일반인의 심폐소생술 교육에 대한 사전 교육 평가의 영향)

  • The purpose of this study is to determine how the pre-test performed before training affects the results of CPR performance. In the case of the pre-test group(PTG), a pre-evaluation was performed for 1 minute before training, and the group that did not perform the pre-test(NPTG) performed only regular education. In both groups, skill test was performed for 1 minute after training. As a result of comparing the pre and post-test of PTG, there were statistically significant changes in chest compression depth, rate, and compression recoil. There was a statistically significant difference only in the chest compression rate in the chest compression performance results of the two groups after training. There was a statistically significant difference in the results of confidence after training in both groups. It is judged that the pre-test conducted before training has a good influence not only on the results of chest compression, but also on confidence improvement. Therefore, it is judged that it is necessary to develop additional programs such as pre-education test in order to increase the concentration of CPR education for the general population.

Comparisons of Nursing Skills, Self-Directed Learning Ability, and Problem Solving Competency, and Satisfaction by Learner-led vs. Faculty-led Fundamental Nursing Practice (학습자 주도와 교수자 주도 기본간호학실습에 따른 술기수행능력과 자기주도 학습능력, 문제해결능력, 실습만족도 비교)

  • This study was done to compare the effects of learner-led and faculty-led fundamental nursing practice on skill performance, self-directed learning, problem-solving competency, and satisfaction. The learner-led practice was consisted of pre-class open lab and peer supervision. The faculty-led practice was consisted of faculty supervision for a student individually without pre-class open lab. The study was a pre-posttest, non-synchronized, quasi-experimental design between two groups. Data from 134 nursing students in either faculty-led (n=54) or learner-led fundamental nursing practice group (n=80) was collected using a checklist and self-reported questionnaire. Nursing skill performance score and class satisfaction of the learner-led group were significantly higher than those of the faculty-led group. This study showed that learner-led fundamental nursing practice consisting of pre-class open lab and peer supervision was an efficient practice method to improve students' skill performance and satisfaction maintaining their self-directed learning ability and problem-solving competency.

  • Purpose: This study was done in order to identify the effectiveness of open laboratory self-directed practice on knowledge, self-confidence, skill competency, and satisfaction of intravenous infusion in nursing students. Methods: This is a quasi-experimental study using a non-equivalent control pre-posttest design. The participants were 32 students comprising an experimental group and 30 students comprising a control group. The experimental group was allowed to perform open laboratory self-directed practice only. Pretest and posttest was performed through questionnaires of knowledge and self-confidence, and competence and satisfaction were assessed using checklists at posttest. Results: The score of Knowledge in the posttest showed no significant difference between both groups (F=1.28, p=.290), and those of self-confidence (F=3.87, p=.014) and competency (F=9.55, p<.001) of the experimental group were significantly higher than those of the control group. Conclusion: Open laboratory self-directed practice was effective in improving nursing students' self-confidence and competency of intravenous infusion. Therefore, students should be encouraged to actively participate in open laboratory self-directed practice and the effective strategies should be developed.

The effects of out of hospital ACLS simulation training on the paramedic's duty ability (구급대원의 전문심장소생술 시뮬레이션훈련이 직무수행융합능력에 미치는 영향)

  • The purpose of this study is to analyze the effects of the simulation-based professional cardiac resuscitation training on the performance of professional cardiac resuscitation performed by paramedics in the pre-hospital stage and to provide basic data for effective cardiac resuscitation. This study is an experimental study of the design before and after the control of non-equality. The subjects of this study were 16 newly recruited paramedics from K firefighting school. The simulation training program and evaluation papers used as the evaluation tool were reviewed and commented by 6 ACLS simulation experts (2 emergency medical doctor, 2 emergency medical professors, 2 KALS instructors)Respectively. The training consisted of 30 minutes of theory and 150 minutes of practical training. The lecturer first demonstrated for 5 minutes, and after training by individual debriefing after individual training, individual and team education was conducted The evaluation scale was given a 5 - point Likert scale. The SPSS 22.0 program for Windows was used. The general characteristics of the subjects were analyzed for frequency, the examination of homogeneity between the experimental group and the control group wasfulfilled by t test, and the difference test between the groups of the two groups was performed using the paired t-test. The homogeneity test was able to confirm the homogeneity between experimental group and control group. In the evaluation of six ACLS techniques, it was proven that the experimental group that received the simulation training had better performance in all aspects than the non - training control group. The following are the technical items to be performed. 1. Electrocardiogram 2. Specialized instrument 3. Treatment of fluid 4. Leadership and teamwork 5. Medical guidance 6. Evaluation during transfer. It was proved that paramedics who received simulation training were improved on their job performance ability than general lecture and training group. Therefore, if simulation training and education are applied to a student in the synthetic course or an emergency resident who is engaged in clinical practice, he / she will be able to perform his / her duties more proficiently. It is expected that emergency services provided to patients with cardiac arrest will be improved.
