Is It Useful and Safe to Maintain the Sitting Position During Only One Minute before Position Change to the Jack-knife Position?

  • Background: Conventional spinal saddle block is performed with the patient in a sitting position, keeping the patient sitting for between 3 to 10 min after injection of a drug. This amount of time, however, is long enough to cause prolonged postoperative urinary retention. The trend in this block is to lower the dose of local anesthetics, providing a selective segmental block; however, an optimal dose and method are needed for adequate anesthesia in variable situations. Therefore, in this study, we evaluated the question of whether only 1 min of sitting after drug injection would be sufficient and safe for minor anorectal surgery. Methods: Two hundred and sixteen patients undergoing minor anorectal surgery under spinal anesthesia remained sitting for 1 min after completion of subarachnoid administration of 1 ml of a 0.5% hyperbaric bupivacaine solution (5 mg). They were then placed in the jack-knife position. After surgery, analgesia levels were assessed using loss of cold sensation in the supine position. The next day, urination and 11-point numeric rating scale (NRS) for postoperative pain were assessed. Results: None of the patients required additional analgesics during surgical manipulation. Postoperative sensory levels were T10 [T8-T12] in patients, and no significant differences were observed between sex (P = 0.857), height (P = 0.065), obesity (P = 0.873), or age (P = 0.138). Urinary retention developed in only 7 patients (3.2%). In this group, NRS was $5.0{\pm}2.4$ (P = 0.014). Conclusions: The one-minute sitting position for spinal saddle block before the jack-knife position is a safe method for use with minor anorectal surgery and can reduce development of postoperative urinary retention.

  • A local limit theorem for large deviations for the i.i.d. random variables was given by Richter (1957), who used the saddle point method of complex variables to prove it. In this paper we give an alternative form of local limit theorem for large deviations for the i.i.d. random variables which is essentially equivalent to that of Richter. We prove the theorem by more direct and heuristic method under a rather simple condition on the moment generating function (m.g.f.). The theorem is proved without assuming that $E(X_i)=0$.

  • Pure-strategy Nash Equilibrium (NE) is one of the most important concepts in game theory. Tae-Hwan Yoon and O-Hun Kwon gave a "sufficient condition" for the existence of pure-strategy NEs in matrix games [5]. They also claimed that the condition was necessary for the existence of pure-strategy NEs in undominated matrix games. In this short note, we show that these claims are not true by giving two examples.

  • In this paper, a class of set-valued mappings is introduced, which is called uniformly same-order. For this sort of mappings, some minimax problems, in which the minimization and the maximization of set-valued mappings are taken in the sense of vector optimization, are investigated without any hypotheses of convexity.

  • In this paper, we suggest a H center .inf. generalized predictive control(H center GPC) which guarantees $H_{\infty}$-norm bounds. THe suggested control is obtained by solving the min-max problem in nonrecursive forms. The stability conditions of the suggested control are derived in a somewhat simple form and it is not required for the derived solution to be a saddle point solution. It is also shown that the suggested control guarantees the $H_{\infty}$-norm bounds under the same conditions of stability.

  • This paper presents sufficient conditions7 for monotonicity of the saddle point value for receding-horizon H$\infty$ control(RHHC). The resulting monotonicity is used to prove the stability of the closed-loop. Under these sufficient conditions the well-known terminal equality condition is handled as a special case and the condition on the state weighting matrix is weakened so as to include even the zero matrix. The whole procedure is much simpler than the previous results and thus is expected to be easily extended for constrained delayed and/or nonlinear systems with the RHHC.

  • Flow over a protruding surface is investigated using numerical simulation. We consider flow between two parallel plates with a cube mounted on one side of the channel. As the flow approaches the cube, the adverse pressure gradient produces three-dimensional boundary-layer separation, resulting In the formation of horseshoe vortices. The objective of our study is to clarify both the steady and the unsteady characteristics of the vortex system. As the Reynolds number increases, the structure of the vortices near the cube becomes complex and the number of vortices increases. The distribution of skin friction on the cube-mounted wall reflects the effect of the horseshoe vortices. All these results are consistent with the experimental findings currently available.

  • This paper presents improved direct method for calculating the closest saddle node bifurcation (CSNB) point, which is also applicable to the selection of appropriate load shedding, reactive power compensation point detection. The proposed method reduced dimension of nonlinear equation compared with that of Dobson's direct method. The improved direct method, utilizing Newton Iterative method converges very quickly. But it diverges if the initial guess is not very close to CSNB. So the direct method is performed with the initial values obtained by carrying out the iterative method twice, which is considered most efficient at this time. Since sparsity techniques can be employed, this method is a good choice to a large scale system on-line application. Proposed method has been tested for 5-bus, New England 30-bus system.

Optimization of Solvent Extraction Process on the Active Functional Components from Chinese Quince (모과내 기능성 유용성분 용매추출공정의 최적화)

  • In this study, various active functional components in Chinese Quince were extracted by solvent extraction method. A central composit design for optimization was applied to investigate the effects of independent variables such as solvent to sample ratio ($X_{1}$), extraction temperature ($X_{2}$), and extraction time ($X_{3}$) on the soluble solid contents ($Y_{1}$), total phenols ($Y_{2}$), electron donating ability ($Y_{3}$), browning color ($Y_{4}$) and reducing sugar contents ($Y_{5}$). It was found that extraction temperature and extraction time were the main effective factors in this extraction process. The maximum soluble solid contents of 35.77% was obtained at 26.38 mL/g ($X_{1}$), 72.82$^{\circ}C$ ($X_{2}$) and 74.86 min ($X_{3}$) in saddle point. Total phenols were rarely affected by solvent ratio and extraction time, but it was affected by extraction temperature. The maximum total phenols of 20.70% was obtained at 22.61 mL/g ($X_{1}$), 84.49$^{\circ}C$ ($X_{2}$), 77.25 min ($X_{3}$) in saddle point. The electron donating ability was affected by extraction time. The maximum electron donating ability of 94.12% was obtained at 10.65 mL/g ($X_{1}$), 67.78$^{\circ}C$ ($X_{2}$), 96.75 min ($X_{3}$) in saddle point. The maximum browning color of 0.32% was obtained at 23.77 mL/g ($X_{1}$), 87.27$^{\circ}C$ ($X_{2}$), 96.68 min ($X_{3}$) in saddle point. The maximum value of reducing sugar content of 10.55% was obtained at 26.83 mL/g ($X_{1}$), 82.167$^{\circ}C$ ($X_{2}$), 81.94 min ($X_{3}$). Reducing sugar content was affected by extraction time.
