시뮬레이션에서의 간호역량 측정도구 개발 및 평가 (Development and Preliminary Testing of the Nursing Competency Scale in Simulation for Nursing Students)

  • Purpose: The purpose of this study was to develop and validate a Nursing Competency Scale in Simulation (NCSS) for nursing students. Methods: A preliminary version of the NCSS of 14 items was derived from the literature. A panel of seven experts reviewed the preliminary version for content validation and developed 15 items scale. A convenient sample of 195 nursing students completed the survey and two evaluators measured the performances of nine teams in a simulation scenario with NCSS. The data were analyzed using exploratory factor analysis, descriptive statistics, independent t-test and Cronbach's alpha. Intraclass correlation coefficient (ICC) was used in order to estimate the degree of inter-rater reliability. Results: An exploratory factor analysis demonstrated that two factor structures of the NCSS explained 51.1% of the total variance. Two factors were named psychosocial skills and cognitive and psychomotor skills. The mean scores of NCSS between third and 4th grade were significantly different providing support for its known-group validity. Cronbach's alpha was .90 and ranged from .79 to .88. The overall ICC for inter-rater agreement was 0.89 (95% CI 0.03 to 0.98). Conclusion: This scale shows preliminary evidence for validity and reliability. It could be a useful instrument for measuring learning outcome in simulation for nursing students' clinical competency.

임상판단모델에 근거한 디브리핑 적용이 임종간호 학습성과에 미치는 효과: 비무작위 대조시험설계 (Effect of Debriefing Based on the Clinical Judgment Model on Simulation Based Learning Outcomes of End-of-Life Care for Nursing Students: A Non-Randomized Controlled Trial)

  • Purpose: This study was conducted to identify effects of debriefing based on the clinical judgment model for nursing students on their knowledge, skill performance, clinical judgment, self-confidence and learner satisfaction during simulation based end-of-life care (ELC) education. Methods: Simulation based ELC education was developed in six steps as follows: selection of learning subjects and objects, development of learning tools, a trial run of simulation-based education, students' skill training, and evaluators' training. Forty-eight senior nursing students (25 in the experimental group and 23 in the control group) participated in the simulation-based ELC education using a high-fidelity simulator. Debriefing based on the clinical judgment was compared with the usual debriefing. Results: ANCOVA showed that there were differences in knowledge (F=4.81, p=.034), skill performance (F=68.33, p<.001), clinical judgment (F=18.33, p<.001) and self-confidence (F=4.85, p=.033), but no difference in satisfaction (t=-0.38, p=.704) between the experimental and control groups. Conclusion: This study found that debriefing based on the clinical judgement model is effective for supporting nursing students for reflecting on clinical judgment and improving their diverse competencies in complex clinical settings such as ELC.

간호학생의 학습성과 달성과 연계된 시뮬레이션 실습 설계와 운영 (Planning and Applying Simulation-based Practice for the Achievement of Program Outcomes in Nursing Students)

  • Purpose: This study was conducted to plan and apply simulation-based practice for raising the achievement of program outcomes (POs) in nursing students. Methods: Using convenience sampling, 95 nursing students participated in this descriptive study. A self-reported questionnaire was used to measure the achievement of POs (self-directed learning, problem-solving ability, critical thinking) and usefulness of curriculum in addition to observing tracheal suction skills using a checklist. Results: A scenario with a pneumonia patient was developed to observe tracheal suction skills during simulation-based practices. Self-directed learning, problem-solving ability, and critical thinking were then scored. The mean scores of performance skill, self-directed learning, problem-solving ability, critical thinking were $37.82{\pm}6.03$. $3.61{\pm}0.38$, $3.61{\pm}0.33$, and $3.73{\pm}0.32$, respectively. All students passed the simulation-based practice in terms of performance ability and met the required achievement level for the POs at this university. Students with good suction skills showed significant differences in problem identification (p=.044) and alternative development (p=.019), which are components of problem-solving ability, compared to students with only fair skills. Conclusion: These findings indicate that simulation-based practice related to an adult nursing respiratory course was useful for evaluating the achievement of POs in nursing students. Further study is needed to develop a universal method of outcome measurement.

가상현실 시뮬레이션 교육이 간호과정 수행능력에 미치는 효과 (The Effect of Virtual Reality Simulation Education on Nursing Process Competency)

  • 본 연구의 목적은 간호대학생의 가상현실 시뮬레이션 교육 전후의 간호과정 수행능력, 비판적사고성향과 자기효능감의 차이를 파악하여 효과적인 실습교육의 기초자료를 마련하고자 함이다. 2021년 2월부터 4월까지 간호대학생 31명에게 URL설문지를 이용하여 자료를 수집하였고, 수집된 자료는 SPSS 22.0으로 분석하였다. 연구결과, 가상현실 시뮬레이션 교육 후 간호대학생의 간호과정 수행능력(t=-3.776, p=.001), 비판적사고성향(t=-3.608, p=.001), 자기효능감(t=-3.580, p=.001)이 교육 전보다 유의하게 높아졌다. 또한 전공만족도, 임상실습만족도가 높은 집단과 임상실습만족도가 보통인 집단과 학업스트레스가 높은 집단에서도 가상현실 시뮬레이션 교육 후 간호과정 수행능력, 비판적사고성향, 자기효능감이 교육전보다 유의하게 높아졌다. 가상현실 시뮬레이션 교육이 간호대학생의 간호과정 수행능력 향상에 효과적임이 확인되었다. 따라서, 가상현실 시뮬레이션 교육이 간호과정 수행능력 향상을 위한 임상실습 전략으로 활용될 수 있으며, 이를 위해 다양한 시나리오 개발과 후속연구가 이루어져야 할 것이다.

  • 본 연구의 목적은 시뮬레이션 기반 흉관배액관리 간호교육이 간호학생의 시나리오 경험에 대한 반응, 학습에 대한 자신감 및 문제해결능력에 미치는 효과를 검증하는 것이다, 본 연구는 단일군 전후설계 실험 연구로 133명의 3학년 간호학생을 31개조로 시뮬레이션 교육 중재를 시행하였다. 연구결과, 시뮬레이션 실습교육 시행 전보다 시행 후에 시나리오 경험에 대한 반응은 긍정적인 결과로 나타났고, 학습에 대한 자신감 및 문제해결능력은 유의하게 향상된 것으로 나타났다. 이는 간호학 실습교육에 있어서 시뮬레이터를 이용한 시뮬레이션 교육이 효과적임을 보여주는 결과라고 할 수 있다. 본 연구는 현장중심의 실습교육 및 임상실무의 질을 향상시키는데 기여할 수 있을 것이다.

문제중심학습 기반 가상현실 시뮬레이션 교육이 간호대학생의 비판적 사고능력, 문제해결능력 및 자기효능감에 미치는 효과: 유사실험 연구 (Effects of a virtual reality simulation integrated with problem-based learning on nursing students' critical thinking ability, problem solving ability, and self-efficacy: a non-randomized trial)

  • Purpose: This study analyzed the effects of virtual reality simulation-based problem-based learning on nursing students' critical thinking ability, problem-solving ability, and self-efficacy in the nursing care of women undergoing induction of labor. Methods: A nonequivalent control group pretest and posttest design was employed. The study participants included 52 nursing students (24 in the experimental group and 28 in the control group). The experimental group took a problem-based learning (PBL) class in the first week, and then engaged in self-directed learning using virtual reality simulation. In the second week, lectures about emergency nursing care for induction of labor and drug administration were given. The control group participated in PBL in the first week and lectures in the second week. The study was conducted from April 17 to May 19, 2023. Data were analyzed using the chi-square test, Fisher exact test, analysis of variance, and the independent t-test. Results: Before-and-after differences between the two groups were statistically significant in problem solving ability (t=-5.47, p<.001) and self-efficacy (t=-5.87, p<.001). Critical thinking ability did not show a statistically significant difference between the two groups. The score for satisfaction with the virtual reality simulation program was 3.64±5.88 out of 5 in the experimental group. Conclusion: PBL education using a virtual reality simulation was found to be an effective way of teaching. Although convenience sampling was used, PBL education using virtual reality can be used as an educational strategy to enhance nursing students' problem-solving ability and self-efficacy.

분만간호에 대한 융복합 기반 시뮬레이션 실습 효과 및 요구도 (The needs and effects of convergence-based simulation practice for obstetrical nursing)

  • 본 연구는 분만간호에 대한 융복합 기반 시뮬레이션 실습의 효과 및 요구도를 파악하기 위해 실시하였다. 연구대상은 충남 소재 일개 대학 간호학과에 재학 중인 3학년 학생 47명을 대상으로 하였다. 자료수집은 2012년 2월 13일에서 25일까지 실시하였으며, 자가보고형 설문지를 이용하였다. 분만간호에 대한 융복합 기반 시뮬레이션 실습은 1일 8시간씩 총 2일 동안 4개 영역으로 나누어 실시하였다. 시뮬레이션 실습 후 분만간호에 대한 자기효능감(t=-14.74, p<.001) 및 임상술기능력(t=-17.51, p<.001)이 유의하게 증가하였으며, 실습만족도가 실습스트레스에 비해 높게 나타났다. 시뮬레이션 실습필요도는 10점 만점에 7.96점이었으며, 실습만족도가 높을수록 실습필요도가 높게 나타났고, 임상실습 전에 시뮬레이션 실습을 실시하는 것에 대한 요구도가 임상실습 후 시뮬레이션 실습을 실시하는 것에 비해 높게 나타났다. 따라서 분만간호에 대한 임상실습을 보완하는 방안으로 시뮬레이션 실습을 활용하는 것이 필요하며, 시뮬레이션 실습은 임상실습 전에 실시하고, 시뮬레이션 실습필요도에 대한 학생들의 인식을 증가시키기 위한 전략 수립이 필요하다고 할 수 있다.

간호대학생의 임상 시뮬레이션 학습동기가 학습성취도에 미치는 영향: 학습몰입의 매개효과 (The impact of clinical simulation learning motivation on nursing student learning achievement: The mediating effect of learning immersion)

  • Purpose: This study aimed to examine the mediating effect of learning immersion in clinical simulations on the relationship between nursing student learning motivation and achievement in clinical simulation. Methods: This study was conducted using a cross-sectional survey with 184 nursing students from two universities who participated in clinical simulation between September and December 2022. The participants completed a self-administered questionnaire, and the collected data were analyzed using independent an independent t-test, Mann-Whitney U-test, one-way ANOVA, Pearson's correlation coefficient, and multiple regression analysis to identify the mediating effects of learning immersion on the relationship between nursing student learning motivation and achievement. Results: Among the subvariables of nursing student learning motivation, task value and self-efficacy for learning and performance had a significant effect on learning immersion (respectively, β=.36, p=.001; β=.31, p<.001) and learning achievement (respectively, β=.48, p<.001; β=.38, p<.001). With the input of learning motivation variables, the direct effect of learning immersion on learning achievement was significant (β=.20, p=.003), and the effects of learning motivation and task value and self-efficacy on learning achievement was reduced after controlling for learning immersion, which is a mediating variable (respectively, β=.41, p<.001; β=.32, p<.001). The bootstrapping test to confirm the mediating effect of learning immersion was also significant (task value 95% confidence interval [95% CI], 0.02~0.20; self-efficacy 95% CI, 0.01~0.12). Conclusion: The results of this study suggest that simulation educators should consider learners' motivation and immersion when organizing and operating clinical simulations.

시뮬레이션 기반 허약노인 방문간호 교육 요구도 (Educational needs for the development of a simulation module of home visiting care for the frail elderly)

  • Purpose: This study aimed to identify the educational needs of nurses and nursing students for the development of a simulation module of home visiting care for frail, elderly people. Methods: Focus group interviews were conducted with 15 home visiting nurses working in public health centers and 14 nursing students who experienced home visiting from September 10 to October 10, 2018. Results: Bloom's taxonomy of learning objectives, namely, cognitive, affective, and psychomotor domains was used as a framework for data analysis. The defined educational needs for each domain were as follows: "understanding frail, elderly people" for the cognitive domain; "intervention for mental health" and "building a therapeutic relationship" for the affective domain; and "nursing skills", "health education for healthy lifestyles", "referral to the community resource connection", "protection for visiting nurses" for the psychomotor domain. Conclusion: Based on the findings of this study, a simulation module of home visiting care for frail, elderly people can be developed and used for nursing students and nurses to strengthen the capacity for home visiting care.

SimBaby 시뮬레이션 학습 시나리오의 개발 및 평가 -응급실 내원 발열환아를 중심으로- (Development of a Scenario and Evaluation for SimBaby Simulation Learning of Care for Children with Fever in Emergency Units)

  • 간호 실무 능력의 개발을 위해서는 임상 환경에서의 지속적인 경험이 중요하지만 간호학생의 임상 경험의 기회는 줄어들었고 모든 학생들이 충분히 임상 경험을 한다는 것은 현실적으로 어렵다. 이러한 문제를 해결할 수 있는 방안으로 시뮬레이션 학습을 생각해 볼 수 있는데, 성인대상의 간호 시나리오 개발이 주로 이루어졌고 아동대상의 시나리오 개발은 거의 없는 실정이여서 아동간호와 관련된 시뮬레이션 시나리오 학습내용을 개발하고자 시도되었다. 시나리오의 내용은 응급실 내원 발열환아 간호과정을 중심으로 구성하였다. 시뮬레이션 학습 시나리오 적용 결과, 학생들은 실습에 대한 자기효능감, 수업태도와 수업에 대한 만족도가 증가하였다. 따라서 더욱 다양한 사례의 아동관련 시뮬레이션 시나리오를 개발하여, 아동간호와 관련된 실제와 유사한 임상 상황들을 학생들에게 적용한다면 아동과 그 가족에 대한 질 높은 간호를 제공할 수 있을 것이다.
