  • 합성한 5종의 6-Exomethylene penamsulfone 유도체의 Type I, Type II, Type III, TypeIV, TEM 효소에 대한 $\beta$-lactamase저해효과를 spectrophotometric assay방법으로 측정하였다. 또한 여기서 우수한 효소억제활성을 보여준 3종의 화합물을 가지고 In vitro antibacterial activity를 Agar dilution method법으로 27종의 $\beta$-lactamase생성균주에 대하여, Sulbactam, Ampicillin 및 Cefoperazone과 병용투여하여 MIC를 측정하였다.

  • The resistance of bacteria against ${\beta}$-lactam antibiotics is mainly caused by the production of ${\beta}$-lactamase enzymes. ${\beta}$-Lactamase inhibitors are used in combination with known antibiotics to overcome the growing problem of bacterial resistance. We prepared 6-(thienylmethylene)penam sulfones for the development of potent ${\beta}$-lactamase inhibitors and evaluated their ${\beta}$-lactamase inhibitory activities.

Antibiotic Susceptibility and Distribution of Vibrio parahaemolyticus Isolated from the Seafood (어패류에서 분리한 장염비브리오균의 분포 및 항균제 감수성)

  • Vibrio parahaemolyticus was investigated for analysis of distribution and antibiotic susceptibility at the markets, Seoul in 2004. The average isolation rate was 26.8%(306/1,143) and the highest rate with 64.9% was obtained in August. The isolation rates from December to April was 0.0%. It is more effective to inoculate into the enrichment medium, alkaline pepton water by swabbing a sample with a cotton ball rather than pouring mixed sample. The isolation rates were from 24.4% to 37.1% in domestic samples and from 9.8% to 26.8% in imported ones. All of the isolates were resistent to ampicillin and ticarcillin and most of them were sensitive to chloramphenicol, tetracycline, nalidixic acid, amoxicillin, ceftriaxone, streptomycin, kanamycin, trimethoprim/sulfamethoxazol, ampicillin/sulbactam, ciprofloxacin.

Susceptibility tests of oral antibiotics including cefixime against Escherichia coli, isolated from pediatric patients with community acquired urinary tract infections (소아 원외 요로감염 환아에서 분리된 E. coli에 대한 cefixime을 포함한 경구 항생제의 감수성 연구)

  • Purpose : Urinary tract infection(UTI) is one of the most frequent infections in children. E. coli is the most frequent etiological micropathogen in pediatric community UTI, and E. coli has developed resistance to many antibiotics, highlighting the need for regular surveys of this organism resistant patterns in the community. The aim of this study was to determine the oral antibiotic susceptibility patterns of E. coli, isolated from pediatric patients with uncomplicated community acquired UTI. Methods : E. coli isolates, obtained from pediatric patients with uncomplicated community acquired UTI between October in 2004 to September in 2005. And minimal inhibitory concentrations(MICs) of oral aminopenicillins and beta-lactamase inhibnitors(ampicillin, amoxacillin, ampicillin-sulbactam), oral cephalosporins(cefaclor, cefixime) and sulfa drug(trimethoprime-sulfamethoxazole) were performed according to the National Committee for Clinical Laboratory Standards(NCCLS) guide line. Results : Total 211 organisms were isolated from pediatric out-patients with community UTI. E. coli was the most common organism(89 percent), followed by E. fecalis, Proteus species, S. aureus, M. morganii, and P. aeruginosa. The resistant rates of aminopenicillins and beta-lactamase inhibitors, cefaclor and sulfa drug to E. coli were very high. But, the resistant rate of cefixime was markedly low, and ESBL strains were isolated with small rates. Conclusion : Our study results suggest that aminopenicillins, cefaclor and sulfa drug may not be useful as first line empirical antibiotics to treat pediatric patients with community UTI in Korea. But, 3rd generation cephalosporin such as cefixime can be used as effective second line antibiotics after primary treatment failure, also may be useful as an empirical first line antibiotic. Finally, we conclude that a continuous surveillance study to monitor susceptibility patterns of E. coli in community UTI will be needed for the standard guide lines of empirical oral antibiotic treatment.

Trend of Antimicrobial Susceptibility and Multiple drug Resistance Patterns of Salmonella entericia Serovar Enteritidis Isolated from Foodborne Patients in Seoul between 2001 and 2005 (최근 5년간 서울시내 식중독 환자에서 분리한 Salmonella enterica serovar Enteritidis의 항생제 감수성 및 다제 내성 특성)

  • Antimicrobial susceptibility and multiple drug resistance patterns have been carried out on total of 364 isolates of Salmonella enterica serovar Enteritidis isolated from foodborne patients in Seoul from 2001 to 2005. Overall, the highest percentage of resistance was found to the following antimicrobial agents: streptomycin (46.7%), ampicillin (37.3%), ticarcillin (36.7%), tetracycline (36.0%), nalidixic acid (20.7%), chloramphenicol (13.3%), amoxicillin/clavulanic acid (6.7%) and Ampicillin/sulbactam (4.0%). Seventy five percentage of isolates were found to be resistant to one or more of the antimicrobes tested. The resistant rates to nalidixic acid and chloramphenicol in S. Enteritidis tested were annually increased but the resistant rate to tetracycline was decreased and the resistant rates to streptomycin, ampcilin and ticarcillin were remained steadily. The most frequent patterns of multiresistant isolates were only nalidixic acid resistant (18.0%) and streptomycin-tetracycline (18.0%), streptomycin-ampicillin-ticarcillin (10%), and ampicillin-ticarcillin (5.5%). Overall the resistant rates of 1 drug was 19.3%,2 drugs 24.7%, 3 drugs 6.7% and 4 or more drugs 24.0%. The resistant rates of 1 drug and 2 drugs in 2005 were increased dramatically.

Characterization of Antimicrobial Resistance Patterns and Integrons of Nontyphoid Salmonella Isolates from Infants in Seoul (서울지역 소아에서 분리된 Nontyphoid Salmonella의 항생제 내성과 Integron의 특징)

  • A total of 105 nontyphoid Salmonella isolated from infants in Seoul from 2003 to 2009 was investigated for their serotype, antimicrobial resistance, characterization of integron, and the patterns of Pulsed-field gel electrophoresis (PFGE). Eighteen serotypes were detected in 105 isolates, and the two most common serotypes were S. Enteritidis (47.6%) and Montevideo (15.2%). Among the Salmonella serovars, a high level of antimicrobial resistance was found to ampicilin (60%), tetracycline (46.7%), streptomycin (35.2%) and nalidixic acid (28.6%). In the multi-drug resistance patterns, the predominant patterns were only nalidixic acid (15.7%), ampicillin-ampicillin/sulbactam-tetracycline (14.5%), and ampicillin-streptomycin-chloramphenicol-tetracycline (10.8%). PCR and DNA sequencing analysis revealed the presence of class 1 integron in 20 isolates (19%). Of the class 1 integron positive isolates 20% harboured the integron-associated gene cassettes : aadA2, blaP1, dfr17-aadA5, dfrA12-aadA2, and aadA7. PFGE was carried out to examine the genetic relatedness among S. Enteritidis isolates. Except for three strains, fifty strains were divided by three pulsotypes.

Antibiotic Susceptibility of Vibrio parahaemolyticus Isolated from Commercial Marine Products (시판 어패류에서 분리한 장염 비브리오균의 항생제 감수성 특성)

  • Ninety three strains (4.2%) of Vibrio parahaemolyticus were isolated from 2,239 samples of commercial marine products during 2006-2008 in Seoul, Korea. We examined 16 antimicrobial susceptibilities of 93 V. parahaemolyticus isolates. Antibiotic resistance of V. parahaemolyticus was most frequently observed to ampicillin (93.5%), followed by cephalothin (90.3%), streptomycin (87.1%), ticarcillin (55.9%), and amikacin (40.9%). Antibiotic susceptibility was most frequently observed to nalidixic acid, chloramphenicol and ampicillin/sulbactam (100%), followed by trimethoprim/sulfamethoxazole (98.9%), gentamicin and tetracycline (82.8%), and ceftriaxone (63.4%). In addition, the isolates also displayed intermediate resistance to kanamycin (79.6%), ciprofloxacin (64.5%), amikacin (47.3%) and cefoxitin (43.0%). All isolates were resistant to more than two drugs. The most prominent multiple drug resistance was 3 drug resistance (37.6%), followed by 4 drug (24.7%), 5 drug (17.2%), and 6 drug resistance (11.8%). The most prominent multiple drug resistance pattern was the cephalothin-streptomycin-ampicillin resistance pattern (22.6%), followed by cephalothinticarcillin-streptomycin-ampicillin (18.3%) and cephalothin-ticarcillin-streptomycin-ampicillin-amikacin (9.7%). Multiple drug resistance patterns of V. parahaemolyticus from marine products require continuous monitoring.

A Clinical Observation on Childhood Shigellosis Occurred in Mass Outbreak (소아에서 집단 발병한 세균성 이질의 임상적 고찰)

  • Purpose: The incidence of shigellosis had been decreased due to the use of antibiotics and the improvement of environmental sanitation but recently increases again. Shigellosis occurrs in mass outbreak through unsanitized meal preperation refered from welfare facility and school. We observed epidemic aspect and clinical coarse of childhood shigellosis. Methods: from December 2001 to January 2002, 22 inpatients with shigellosis in Dongbu Municipal Hospital were observed epidemiologically, clinically and microbiologically. Results: 1. The sexual ratio was 1:1 and mean age was $5.5{\pm}1.4$ years (14 months to 11 years). 2. The clinical manifestations were following: diarrhea (95.5%), high feve (10%) and asymptomatic (4.5%). The mean duration was $3.9{\pm}2.1$ days (1 to 12 days). All patients had no complication and normal serologic test. 3. S. sonnei was cultured in rectal swab, and was resistant to TMP/SMX and ampicillin, but susceptible to ampicillin/sulbactam and the 3rd generation cephalosporins. 4. The patients were treated by antibiotics with conservative treatment including electrolytes and fluid therapy for 5 days, resulting in improvement confirmed by negative reaction on stool culture. 5. These cases were all occurred in mass outbreak in day care center and were suspected to be secondary infection by members of family. Conclusion: This shigellosis occurred in day care center was secondary infection due to S. sonnei and had mild clinical coarse and improvement after antibiotic treatment.

Antimicrobial Resistance and Multi-Drug Resistance Patterns of Pathogenic Bacteria Isolated from Food Poisoning Patients in Incheon (인천지역 식중독 환자에서 분리한 병원성 세균의 항생제 내성 및 다제 내성 양상)

  • Antimicrobial resistance and multi-drug resistance patterns have been carried out on total of 210 isolated of Salmonella spp. and pathogenic E. coli isolated from food poisoning patients on January through December 2012 in Incheon, Korea. The highest percentage of antibiotics resistance was found to the following antimicrobial agents: tetracycline 43.8%, ampicillin 34.8%, nalidixic acid 23.8%, sulfamethoxazole/trimethoprim and chloramphenicol 12.4%, and ampicillin/sulbactam 11.4%. The highest percentage of resistance was 37.5% to ampicillin for Salmonella spp. and 59.0% to tetracycline for pathogenic E. coli. Overall the multidrug resistance rates of 1 drug was 26.2%, 2 drugs 9.0%, 3 drugs 9.5%, 4 drugs 7.1%, and 5 or more drugs 12.46%. The multi-drug (MDR) strains to four or more antimicrobial agents among the resistant organisms were quite high: 15.9% and 22.1% for Salmonella spp. and pathogenic E. coli, respectively. The study implies that limitation of unnecessary medication use is pertinent in order to maintaining the efficacy of drugs.

A Case on Streptococcal Pneumonia Associated with Leptomeningitis, Osteomyelitis and Epidural Abscess in a Patient with AIDS

  • Patients with acquired immunodeficiency syndrome (AIDS) are at higher risks of bacterial pneumonia than the general population, and the pathogen is the most commonly involved Streptococcus pneumoniae. We hereby report a case of pneumococcal pneumonia associated with leptomeningitis, osteomyelitis and epidural abscess in a patient with AIDS. He is being successfully treated with ampicillin/sulbactam and clindamycin. And because the pneumococcal infection is usually associated with morbidity and mortality rates in the setting of AIDS, we should consider for pneumococcal vaccinations among the AIDS populations.
