중학생의 학교 우유 급식 실태 및 만족도 조사 (Survey of Middle School Students' Intake of Milk and Satisfaction with School Milk Program)

  • This study was conducted to investigate the consumption of milk, along with satisfaction for improvement of the school milk program in middle school. The subjects were 447 middle school students(265 male, 182 female) in the Gyeonggi area, whowere asked to fill out the questionnaire. The collected data were analyzed by a SAS package. The results showed that 53.2% of the subjects were participating in the school milk program. Regarding the frequency of milk consumption, 77.7% of students reported daily milk consumption and 65.3% of students reported they drank milk when it was available. 56.8% of the students responded that they were satisfied with the school milk program. Male students not participating in the school milk program cited the reason as "eating at home", while non-participating female students cited the reason as "tasteless". When asked whether they accepted or rejected the school milk program, male students(97.7%) and female students(97.8%) answered "it should be executed". To improve the school milk program, most of the students(72.2%) stated that "it should be selected among flavored milk(strawberry, banana, chocolate, black bean, and husked rice milk) instead of white milk". Therefore, in order to increase the intake of milk, the students should develop the habit of drinking milk to gain the attention of parents. Also, schools should attempt to help through offering proper dietary education.

G시 보건소에 등록된 미숙아의 영유아기 성장과 발달 상태 (Growth and Development in Infants and Children Born Prematurely Who were Registered at the Public Health Center in G City)

  • Purpose: The purpose of this study was to evaluate and compared the growth and development of premature and full-term infants during the 2 years after birth. Method: The participants were 102 infants, 51 each for premature infants, and for healthy full-term infants. Participants in the premature group accounted for 17.5% of all premature infants who were registered at the public health center in G city. Developmental status was evaluated using the Korean Denver II. Results: The catch-up growth of the premature was 100% in weight and in height. Suspicious developmental delay according to the Korean Denver II was 3.9% in normal infants and 31.2% in premature infants. Factors related to the suspicious developmental delay in premature infants were their age and health state at birth. The rate of suspicious developmental delay was higher in infants over 6 months and infants unhealthy at birth. Conclusion: A premature follow-up program, which includes nutrition education to achieve catch-up growth and to prevention obesity, along with continuous developmental screening test for infants and children born prematurely is recommended. Provision for home visits and telephone counseling for premature infants and their families who do not to use the public health center should also be included.

Lifestyle intervention might easily improve blood pressure in hypertensive men with the C genotype of angiotensin II type 2 receptor gene

  • BACKGROUND/OBJECTIVES: Recent studies have reported an association of the angiotensin II type 2 receptor (AT2R) 3123Cytosine/Adenine (3123C/A) polymorphism with essential hypertension and cardiovascular diseases. The purpose of the study was to investigate whether the AT2R 3123C/A polymorphism affects blood pressure for free-living hypertensive men during a 5-month intervention period. SUBJECTS/METHODS: The subjects were free-living hypertensive Japanese men aged 40 to 75 years who agreed to intervention in the period from 2004 to 2011. Detection of the AT2R 3123C/A polymorphism was determined by polymerase chain reaction. The dietary intervention was designed to decrease salt level and to increase potassium level through cooking instructions and self-monitoring of the diet. The exercise session consisted of activities such as stretching, resistance training, and walking. Blood pressure, urinary sodium and potassium excretion, dietary and lifestyle data, and non-fasting venous blood sample were collected at baseline and after the intervention period. RESULTS: Thirty nine subjects were eligible for participation and the follow-up rate was 97.4%. The C allele proportion was 57.9%. AT2R 3123C/A polymorphism was X-chromosome-linked, therefore we analyzed the C and A genotypes. At baseline, no significant differences were observed between the genotype groups. After the intervention, there were no significant differences in lifestyle habit between the groups. Nevertheless, the estimated salt excretion (g/day) was significantly decreased only in the C genotype (13.0-10.3, P = 0.031). No significant change was observed in systolic blood pressure (SBP) (mmHg) in the A genotype, but a significant decrease was observed in the C genotype (150.0-141.5, P = 0.024). CONCLUSTIONS: In the C genotype, it might be easy to improve SBP through lifestyle intervention in free-living hypertensive Japanese men, however generalization could not be achieved by the small sample size.

21세기 삶의 질 향상을 위한 건강증진방안 (The Health Promotion Programme and Quality of Life in the 21 Century)

  • The ultimate goal of national health promotion services is the improvement of quality of life and health longevity through the implementation of health promotion services. The approach strategy for national health promotion summariged as follows: 1) A model for health promotion should be developed by the level of government. 2) Roles and functions between central government and autonomous local governments should be defined to carry out the health promotion services effectively. 3) New manpower for health promotion such as health educator should be trained and activated at hospitals, health centers, industries, school, and related community agencies. 4) School health education should be strengthened in order to teach: various health subject(smoking & alcohol, drug abuse, accident and safe, nutrition, environmental pollution and preservation, population & family planning, personnel hygiene, physical growth, stress, sex education, communicable disease, physical exercise etc) students through appointing health teachers at school base. 5) Health promotion services in industries should be activated using manpower such as health educator, exercise instructor, dietist and counsellor, 6) Health promotion services for the elderly should be activated. 7) Health screening services in the medical insurance and his/her family should be activated for health promotion services. 8) Health education material development center for health promotion should be established and the materials should be made to distribute to related groups, agencies and institutions (health conte.5, hospitals, schools, pharmacies, industries etc). 9) The pilot health promotion center in each automous local governments(large cities, provinces, Guns and Gu level) should be established and operated for community people. 10) The mass media such as TV, radio, newspapers and magazines should be used effectively. 11) Periodic evaluation of health promotion services should be carried out in order to help effective and successful planning for community health promotion in the future.

혈중 납이 비흡연자들의 혈압에 미치는 영향 (The Effects of Blood Lead on Blood Pressure Among Non-smokers)

  • Objective: The effect of lead on blood pressure remains controversial in spite of the numerous studies which have been conducted in the recent years. The aim of this study was to evaluate the effects of exposure to lead on blood pressure among non-smokers. Methods: In this cross sectional study, 1416 male and female non-smokers were enrolled, aged 20 years or older, from the Korean National Health and Nutrition Examination Survey 2008. Blood pressure, blood lead levels (BLLs), height, weight, and cotinine level were measured for all subjects. Results: Geometric mean BLLs of the participants was 2.20 ${\mu}g$/dl. BLLs were higher in the older, male, and lower education groups than the younger, female and higher education groups. After adjusting for age, sex, education and BMI through multiple regression analysis, a significant positive association between systolic blood pressure (p = 0.0357), diastolic blood pressure (p = 0.0111) and BLLs. Also, among the normal BMI group (18.5 kg/$m^2$ < BMI < 25 kg/$m^2$), we also found a significant positive association between diastolic blood pressure and BLLs (p = 0.0370). Conclusion: The present study showed that blood lead serves as a good predictor of blood pressure changes and that there was a statistically significant association between blood lead and blood pressure, especially diastolic blood pressure.

학교보건사업을 통한 건강증진 사업에 대한 연구 (A Study on School Health Promotion Services)

  • The study was designed to gain necessary basic data in order to grasp the health knowledge, attitude, and practice level of students and teachers of elementary, middle and high schools. This study was conducted through interviews of 3,400 students and 1,022 teachers attending 14 different schools large, middle and small cities and rural towns during a period of nine months (from Oct. 2 1995 to Jun. 30 1996). By the results of this study, the recommendations can be summarized as follows: 1. A school health development committee should be established of 10 members: school health related teachers (physical trainers, nurses, and teachers in charge of health), parents, persons related to health administration, local medical doctors, and student reprensentatives in order to support and immplement school health development plans. 2. Like advanced countries, a health class of 2~4 hours should beplaced in middle and high schools. A nurse majoring in health from a university should be the teacher. 3. A curriculum of health should contain the following: education on health, sex, alcohol, tabacco, the misuse of the drugs, the structure and function of human body, the growth of the body, mental health, safety and emergency care, the prevention of disease, proper eating habits and nutrition, daily health life, family health education, society health, community health, environmental pollution and individual responsibility. 4. Create a school health promotion center, with a nurse's office, and a sports center which has health machines (bars, aerobics, training, twist machine, belt massage, running machine, bench press, chest waist, hack hip extension machine) as well as a physical strength measuring machine (muscular strength, alertness, flexibility, endurance, lung functions and so on), so that the teaching staff and students can use them and train their bodies. 5. Through a refresher education program, urge teachers to understand school health promotion services. 6. Regulate a standard and establish a system of monitoring the physical enviroment of the school (the height of desks and chairs, illumination facilities, ventilation facilities, safe drinking water). 7. Create a check list of health to evaluate improvement.

정상체중과 비만인 대학생의 UCP 2 유전자 다형성, 식습관, 비만도 및 체성분의 비교 연구 (Comparisons of UCP2 Polymorphism, Dietary Habits, and Obesity Index in Normal and Obese University Students)

  • This study was carried out to compare UCP2 polymorphism, dietary habits, and obesity index in normal and obese university students. The survey was carried out using self-questionnaires collected from the 126 normal and 60 obese university students. The results are summarized as follows. Breakfast was skipped in 43.7% of normal and 49.3% of obesity students and it appeared obese students eat faster than normal students. The percentage of weight control experience were 49.2 and 71.0 in the normal and obese students, respectively. Blood levels of lipid profiles(triglyceride, LDL cholesterol, and HDL cholesterol), hemoglobin, AST and ALT were anaylzed. In UCP2 genes, the frequency of deletion homozygote(DD) was 71.5%, heterozygote(DI) was 26.9% and insertion homozygote(II) was 1.6%. Plasma levels of triglyceride, total cholesterol, LDL cholesterol, and HDL cholesterol of normal students were 79.06, 172.25, 100.86 and 57.03 mg/dl, and those of obese students were 93.06, 173.22, 101.22 and 54.39 mg/dl, respectively. Blood parameters were in normal range in both group. Plasma triglyceride, total cholesterol and LDL cholesterol levels of obese students were higher than those of normal students. On the other hand, plasma HDL cholesterol levels of obese students were lower than those of normal students. Plasma levels of AST and ALT were in normal range in both group. However, AST and ALT levels of obese students were higher than that of normal students. Thus, it was recommended for them to have a nutritional education program to improve their dietary and living habits for obese students’health. Nutritional education program should also be organized practically and systematically.

영양 상담서비스 품질에 대한 환자의 만족도와 인식조사 (Patient Satisfaction and Perception on Nutritional Counseling Services Quality)

  • 전국의 43개 상급종합병원과 20개 상급병원의 환자 1,095명을 대상으로 환자의 영양 상담서비스에 대한 인식을 조사하고, 만족도는 서비스 품질 개념을 적용하여 분석하였다. 환자의 영양 상담서비스 인식 평균점수는 4.54/5.00점이었다. 환자의 영양 상담서비스 만족도를 요인 분석한 결과 서비스 품질 5개 영역으로 추출되었고, 각각 공감성, 반응성, 유형성, 신뢰성, 전문성으로 명명, 분류하였다. 5개 영역의 전체 평균 점수는 4.56점/5.00이었고 공감성 4.62점, 반응성 4.52점, 유형성 4.53점, 신뢰성 4.65점, 전문성 4.61점이었다. 학력에 따른 차이에서 영양 상담서비스 품질 만족도 총 평균은 대졸 이상이 중졸 이하보다 유의적(P<0.001)으로 높았다. 수도권 병원의 만족도 점수는 비수도권 병원보다 전체 평균 점수와 반응성, 공감성, 전문성, 신뢰성(P<0.001), 유형성(P<0.05)에서 유의적으로 높았다. 상담횟수에서는 반응성 영역에서 상담을 1회 받은 군이 2회보다 유의적(P<0.01)으로 높은 만족도를 보였다. 환자의 영양 상담서비스 인식과 만족도 간에 신뢰성(r=0.721), 반응성(r=0.697), 공감성(r=0.690), 전문성(r=0.678), 유형성(r=0.622)에서 양(+)의 상관관계를 보였다. 이상을 살펴볼 때 비수도권 병원에서는 임상영양사 수의 확보를 통해 환자의 영양 상담서비스 품질을 향상시키는 것이 필요하겠다. 또한, 영양 상담이 반복될수록 환자의 기대치가 높아질 수 있으므로 임상영양사는 영양 상담 내용 및 기법에 변화를 주고 다양한 자료를 활용하여 영양 상담서비스 품질을 향상시킬 수 있을 것이다. 영양 상담서비스 품질 만족도와 영양 상담 인식 간에는 높은 상관관계가 있는데 서비스 품질 만족도에서 유형성과 반응성은 다른 영역보다 상대적으로 낮은 점수를 보였으므로, 상담실 분위기를 쾌적하게 하고 다양한 교육매체를 활용하며 환자와의 여유 있는 상담시간 및 선택적 상담시간 확보 등 해당 영역에 대한 관리가 특히 필요할 것으로 여겨진다.

C시 가정어린이집 운영자의 영유아급식 알레르기 관리에 대한 융합적 연구 (Convergence Analysis on Food Allergy Management of Home Childcare center's Directors in C city)

  • 어린이들의 식품 알레르기가 매년 증가하고 있고, 여성의 사회진출 증가로 어린이집에 재원하는 어린이의 비율 또한 증가하는 추세이다. 이에 본 연구에서는 C시에 등록된 가정어린이집 운영자인 원장을 대상으로 어린이집 급식의 식품 알레르기 관리현황과 이에 대한 지식, 태도, 실천을 조사하고 이를 융합적으로 해석하여 어린이들의 안전한 급식제공을 위한 기초자료를 마련하고자 하였다. 연구결과 가정 어린이집 운영자에 비하여 교사와 조리원의 식품알레르기에 대한 교육의 기회가 많이 부족하였고, 알레르기 교육을 받은 운영자의 식품 알레르기 관리에 대한 지식과 태도, 실천이 바람직한 것으로 나타났다. 따라서 어린이집 운영자와 교사, 조리원을 위하여 수준별 맞춤식으로 식품 알레르기에 대한 교육을 계획하고, 다양한 콘텐츠와 매체를 활용하여 제공하여야 하며, 어린이집 급식의 식품 알레르기 관리가 제대로 이루어질 수 있도록 지속적인 홍보와 지원을 아끼지 말아야 할 것이다. 본 연구는 어린이들의 안전한 급식제공을 위해 기여할 수 있으며, 향후 영유아 급식의 알레르기 관리현황의 변화에 대한 지속적 조사와 연구가 진행되어야 할 것이다.

양육인의 교육 및 수입정도에 따른 이유기 식생활관리에 대한 실태조사 (A Study on Infant Weaning Practices Based on Maternal Education and Income Levels)

  • 영유아 보육의 실질적 책임자인 어머니들의 이유기 급식, 이유식 생활관리에 대한 지식과 인식도를 조사$\cdot$연구하고자, 보건소를 방문한 양육인 103명을 대상으로 설문조사를 실시하였으며, 그 연구결과를 요약하면 다음과 같다. 1) 영유아의 이유기 식생활관리와 이유식 공급에 대한 필요성과 중요성에 대하여 고졸과 대졸양육인 간에 인식도의 차이를 보였으며 (p<0.01), 가계소득 월 150만원을 기준으로 저소득층의 $47.6\%$와 고소득층의 $41.7\%$가 올바른 이유관리의 중요성을 인지하였다. 2) 단계별 이유식의 종류, 형 태, 조리 법, 급여 횟수와 양 등에 대한 지식과 관련하여 양육인의 $76.0\%$ 정도가 올바른 이유상식 없이 이유를 실시하였으며, 학력과 소득수준이 높을수록 이유식 도입시 발생할 수 있는 알레르기 반응에 대한 지식을 갖추고 있었다. 3) 이유정보의 출처는 대중매체 $(57.3\%)$, 또래 엄마$(34\%)$, 가족$(3.9\%)$, 보건의료인$(2.9\%)$, 기타$(1.9\%)$ 등이었으며, 대졸자는 대중매체 $(72.5\%)$를, 고졸자는 대중매체$(47.6\%)$와 또래 엄마$(39.7\%)$를 통하여 주로 정보를 얻었다. 또한 학력과 소득수준이 높을수록 이유정보와 교육의 기회를 갖고자 하는 욕구가 강하였다. 4) 시판이유식 구입시 양육인의 학력과 소득이 높을수록 TV광고에 대한 영향을 적게 받았으며, 대졸자는 영양측면에서 시판 이유식이 가정식에 비해 월등하지 않다고 응답하여 학력 수준에 따른 유의 성 이 확인되었다. 또한 선식이 영유아에게 좋거나 보통정도의 이유식으로 인식하는 비율이 양육인의 $88\%$ 정도로 나타났다. 5) 학력 에 따른 유의성은 없었으나 고졸자에 비하여 대졸자가 영양표시 라벨을 좀 더 적극적으로 인지하고 있었으며, 식생활과 건강에 대한 관심은 양육인의 교육과 수입에는 무관하게 3.5점 이상으로 높은 관심도를 보였다. 6) 영양교육의 기회가 주어지면 교육과 소득수준이 높은 양육인이 좀 더 적극적으로 활용하였으며, 식품$\cdot$영양관련 기초지식에 대하여 양육인의 $55.3\%$가 보통정도, $43.7\%$가 지식이 부족하다고 스스로 평가하였다. 양육인 중에 특히 대졸자 또는 고소득자가 본인 스스로의 지식에 대해 높은 신뢰도 점수를 보였다. 이상의 연구결과를 통하여 이유기의 식생활과 영양에 대해 실질적 책임자인 어머니들이 올바른 이유지식과 영양정보를 습득할 수 있는 환경조성이 절실하다고 판단된다. 즉 구체적이며 전문적인 이유관련 교육과 계몽이 이루어지기 위해서는 지역사회의 건강을 주도하며 지역 모자보건사업을 실시하는 보건소를 중심으로 한 영유아의 급식과 집단영양지도에 대한 교육 프로그램의 개발과 운영이 요청된다. 이 때 양육인의 사회적 배경과 기초 지식정도를 참조한 영유아 월령별 이유지도가 이루어져야 하며, 영양균형을 갖춘 식재료 선택과 조리법 등에 대한 올바른 이유식 실무교육도 병행되어야 할 것으로 사료된다. 지역사회를 기반으로 한 지속적이며 장기적인 영양지도교육은 양육인의 식품${\cdot}$영양 관련지식을 향상시키며, 이유기의 섭식 개선을 통하여 영양취약계층인 영유아의 이유 식생활관리와 영양개선에 기여할 수 있으리라 본다.
