병원 근무 간호사의 경험 (A Phenomenological Perspective and Discovery of Meaning in Nurse's Experience in Clinics)

  • Purpose: The ultimate aim of the inquiry was to discover the essence of nurse's experience and promote understanding. Method : Guided by Colaizzi's method - 1. Description of the phenomena of interest by the reader. 2. collection of subject's description of the phenomena, 3. Reading all the subject's descriptions of the phenomenon, 4. Returning the original transcripts and extracting significant statements, 5. Trying to spell out the meaning of each significant statements, 6. Organizing the aggregate formalized meanings into clusters of themes. 7. Writing an exhaustive description, 8. Returning to the subjects for validation of the description, 9. If new data fare revealed during the validations incorporating them into an exhaustive description. The participants in this study were eight are nurses working for clinics. This strategies for data collecting were needed : deep face to face interview. Results : 6 cluster of themes are : 1. the heavy pressure, 2. the pride and the royal summons, 3. the powerlessness, 4. the hope, 5. the tiresome. 6. the distressed feeling. Conclusion : The results of the this study would help us to understand nurses in clinics, make direction for nursing education, and identify need for continuing inquiry.

전문간호사 교육과정생의 실습소속감 경험: 학습연계과정 (Experience of Belongingness at Apprentice Course for Advanced Practice Nurse: Learning-connected Process)

  • Purpose: This study was to explore the process of belongingness experienced during the apprentice course for advanced practice nurses. Methods: Data were collected through in-depth interviews with 15 people, who attended the apprentice course for advanced practice nurse, from three schools in Seoul from Jan. 19 until Feb. 25, 2010. The constant comparative method was adapted for data analysis. Results: The core category of this study was the 'learning-connected process' and this process was categorized into three stages. These stages were: going along with the atmosphere, exchanging, and integrating. During the course, the 'uncomfortable participation' as the central idea meant a sense of responsibility and a tension about practice learning of the participant and was influenced by the quality of interaction and the distinct instruction of learning contents. Belongingness was characterized by the Joyful and happy participation which linked to the motivation of new learning opportunities. Conclusion: The findings indicate that there is a process to belongingness and a close relationship between belongingness and learning. Further studies would suggest exploring the components of belongingness, a concept analysis and incorporating the belongingness scale with other qualitative research on this topic.

임상간호사에서 의료급여관리사로의 역할 전이 과정: '척박한 땅에 뿌리내리기' (Role Transition from Clinical Nurse to Case Manager for Medical-aid Beneficiaries: Taking Root in a Barren Land)

  • Purpose: The purpose of this study was to explain the role transition process to nurse case managers (NCMs) for Medical-aid beneficiaries in Korea. Methods: Fourteen NCMs were interviewed regarding their experiences of becoming proficient in the new role of case manger. Data were analyzed through the application of grounded theory. Results: 'Taking root in a barren land' was the core category explaining the role transition process of NCMs. They engaged in four stages: launching, trial and error, proficiency, and wait-and-see stages. NCMs showed not only fear but also passion for case management practice. Despite their passion and effort, NCMs went through a period of trial and error. After becoming skilled, NCMs went through a stage of wait-and-see often because of job insecurity related to temporary position or few opportunities for promotion. Factors influencing NCMs' role transition process included their understanding of client characteristics, belief in case management, and support from their colleagues and families. Conclusion: NCMs experience many challenges in the process of becoming proficient NCMs. To help with their role transition, there is a need for education programs, preceptorship programs, research on their roles and functions, and regulation for securing NCMs' employment and career stability.

Transformational Experience of a Student Nurse with Diabetes: A Case Study

  • Purpose. The current study was aimed to investigate the transformational experience of a female student nurse living with type 1 diabetes. Methods. A case study of a 24-year-old diabetes patient was conducted, with interviews concerning the evolving process she had lived through during the period from her later high school years to her graduation from nursing college. Results. Followings were identified as 5-transformation process: With her diabetes-related limitation, the participant experienced 'conflict involving choosing a college and major'. The participant tried to be in charge of managing her diabetes and stepped forward to 'adaptation to college life as a new environment', and she learned more about the process of 'evolving awareness of caring' and developed herself further through the process of 'integration of the nurse identity into self-identity', and finally through the process of 'progression and preparation for getting a job' she achieved her goals, being positive about the future. Conclusions. The results of the study can provide individuals with diabetes a way of self-management and help the patients and their families in diabetes education. Further research will be needed to refine the results of this study and to learn more about the experiences of patients with type I diabetes in college years.

근대(1876-1945) 한국사회의 전염병 인식과 간호사의 융합적 역할 (Awareness of Epidemics and Convergence Role of Nurse During the Modern Society(1876-1945) in Korea)

  • 본 연구는 근대(1876-1945) 한국사회의 전염병에 대한 사회적 의미와 변화 과정 속에서 간호사의 역할에 대하여 규명함으로써 간호역사를 이해하고 간호의 새로운 방향을 제시하는 데 목적이 있다. 이를 위해 근대 사회의 전염병의 시대적 의미와 관리에 관하여 살펴 보았다. 지난 역사 속에서 전염병은 사회에 많은 문제점을 일으켰고 피해를 최소화하기 위해 많은 노력을 지속적으로 실시하였다. 전염병 관리를 위한 노력이 다른 나라에 비해 늦게 시작 되었지만 지속적인 노력 끝에 위생이라는 개념이 인식되기 시작하면서 전염병의 의미 및 간호의 의미가 다르게 해석되었다. 또한 전염병 관리를 위한 지속적인 노력 속에서 간호사의 역할과 존재감이 새롭게 조명되었다. 근대 한국사회에서 간호사의 역할이 초반에는 보조자의 역할로 인식되었다. 하지만 전문적 교육과 훈련을 통해 단순한 돌봄이 아닌 전문적 지식을 바탕으로 이루어진 전문가로써 인식이 넓혀지기 시작하였다. 본 연구는 과거와 현재 공존하면서 다양한 문제를 일으키고 있는 전염병에 대한 역사적 재탐색을 하였다는 것에 의의가 있다. 또한 간호사의 새로운 역할과 사명감을 새롭게 조명했다는 것에 의의가 있다.

2차 병원 신규간호사의 직무스트레스와 자아탄력성이 간호업무성과에 미치는 영향 (The Influence of Job Stress and Ego-Resilience on Nursing Performance New Nurse In Secondary Hospitals)

  • 2차 병원 신규간호사의 직무스트레스와 자아탄력성이 간호업무성과에 미치는 영향을 규명하기 위한 서술적 조사연구이다. 연구대상은 일 지역 6개의 2차 병원에 재직 중인 6개월 이상의 신규간호사 198명으로 편의표집하였고 2016년 2월 10일부터 4월 30일까지 자료수집을 하였다. 연구 결과 대상자의 간호업무성과정도는 높게 나타났고, 직무스트레스 정도와 자아탄력성 정도는 중간 정도로 나타났다. 대상자의 직무스트레스와 간호업무성과는 음의 상관관계가 있는 것으로 나타났고, 대상자의 자아탄력성과 간호업무성과는 양의 상관관계가 있는 것으로 나타났다. 대상자의 간호업무성과 정도에 가장 영향을 미치는 요인은 자아탄력성 정도, 직무스트레스 정도, 간호학을 택한 동기 순이었으며, 전체 설명력은 33.6%이었다. 따라서 2차 병원 신규간호사의 간호업무성과를 향상시키기 위해서는 2차 병원 신규간호사의 직무스트레스를 줄이고 자아탄력성을 증진시키는 프로그램의 개발과 적용이 필요하다고 사료된다.

신규간호사의 현실충격, 조직몰입 융복합연구 (New Nurse's on Reality Shock and Organizational Commitment Convergence Study)

  • 본 연구의 목적은 종합 병원 신규 간호사의 코칭리더쉽이 현실충격, 조직몰입에 영향을 미치는 것을 검증하고자 한다. 자료분석은 기술 통계, t- 검정, ANOVA, Pearson 상관 계수 및 표준 다중회귀분석(Standard Multiple Regression)을 사용하여 SPSS / WIN 21.0 프로그램을 통해 분석되었다. 연구결과는 조직몰입 대한 회귀모형은 유의하였고(F= 6.17, p<.001), 조직몰입에 대한 코칭리더십의 설명력은 27.3%(Adj R2= 0.273)였다. 현실충격에 대한 회귀모형은 유의하였고(F= 13.96, p<.001), 현실충격에 대한 코칭리더쉽의 설명력은 39.2%(Adj R2= 0.392)였다. 본 연구결과는 신규간호사의 코칭리더쉽은 현실충격을 낮춰주며, 조직몰입을 높이는 것으로 나타났다. 따라서 이 연구결과를 바탕으로 신규간호사의 코칭리더수비프로그램 개발과 신규간호사대상의 코칭리더쉽연구의 기초자료를 제공하고자 한다. 본 연구 결과를 토대로 신규간호사의 현실충격을 완화시키고 적응을 돕고 조직몰입을 높여 이직을 예방하여, 조직적 차원에서 인적, 물적 관리를 위한 방안의 기초자료를 제공하고자 한다.

중환자실과 응급실 초보간호사를 위한 맞춤형 시뮬레이션 학습프로그램 개발 및 효과 (The Development and Effects of a Tailored Simulation Learning Program for New Nursing Staffs in Intensive Care Units and Emergency Rooms)

  • Purpose: The purpose of this study was to develop a tailored simulation learning (SL) program and to evaluate the effects of the program on the clinical competency, clinical decision-making competency, and communicative competency of new nursing staffs in intensive care units (ICU) and emergency rooms (ER). Methods: In this quasi-experimental study, fifteen nurses were assigned to the experimental group and fifteen to the control group. The experimental group was given the SL program of four sessions, whereas the lectures of four sessions were given to the control group. Data were analyzed using a $x^2$-test, Mann-Whitney U test, and Kruskal-Wallis test with an SPSS program. Analysis of covariance was used to treat the covariate of pre communicative competency between the experimental and control groups. Results: Based on the education needs of new nursing staffs in ICUs and ERs, three learning scenarios and one evaluating scenario were developed for the SL program. The score for clinical competency, clinical decision-making competency, and communicative competency were significantly higher in the experimental group compared with the control group. Conclusion: The SL program is an effective learning strategy for new nursing staffs in ICUs and ERs. These findings suggest that an SL program be offered as an alternative for new nurse orientation and continuing nurse education.

신규간호사의 현실충격과 영향요인 (Factors associated with New Graduate Nurses' Reality Shock)

  • Purpose: The purpose of this study was to identify factors that affect reality shock in new nurses. Methods: Participants were 216 newly graduated nurses with less than 1 year experience in 5 university and 10 general hospitals in Busan, Ulsan and Gyeongnam, A self-report questionnaire was completed by the nurses between November 18 and December 25, 2013. Data were analyzed using t-test, ANOVA and hierarchial multiple regression analysis with the SPSS/WIN 21.0 Program. Results: Average scores for work environment and environmental reality shock were $2.63{\pm}0.33$ and $2.66{\pm}0.43$ points respectively. Regression analysis showed that with the nurses' demographic and work characteristics controlled, work environment explained 4.2% of the reality shock. Factors significantly affecting reality shock included nurses' changing residence because of job (${\beta}$=.21, p=.001), whether they were able to work on the unit of their choice (${\beta}$=-.13, p=.031) and whether they had a choice in days off (${\beta}$=-.14, p=.038). Conclusion: When these factors are considered, reduction in nurses' reality shock requires improvement in work environment, placing new nurses in a department of their choice and allowing them a choice in off-duty days. These measures would also help achieve organizational goals and develop the new nurses as professional nurses.
