Surgical Decision Making for the Elderly Patients in Severe Head Injuries

  • Objective : Age is a strong predictor of mortality in traumatic brain injuries. A surgical decision making is difficult especially for the elderly patients with severe head injuries. We studied so-called 'withholding a life-saving surgery' over a two year period at a university hospital. Methods : We collected data from 227 elderly patients. In 35 patients with Glasgow Coma Score 3--8, 28 patients had lesions that required operation. A life-saving surgery was withheld in 15 patients either by doctors and/or the families (Group A). Surgery was performed in 13 patients (Group B). We retrospectively examined the medical records and radiological findings of these 28 patients. We calculated the predicted probability of 6 month mortality (IPM) and 6 month unfavorable outcome (IPU) to compare the result of decision by the International Mission for Prognosis and Analysis of Clinical Trials in TBI (IMPACT) calculator. Results : Types of the mass lesion did not affect on the surgical decision making. None of the motor score 1 underwent surgery, while all patients with reactive pupils underwent surgery. Causes of injury or episodes of hypoxia/hypotension might have affected on the decision making, however, their role was not distinct. All patients in the group A died. In the group B, the outcome was unfavorable in 11 of 13 patients. Patients with high IPM or IPU were more common in group A than group B. Wrong decisions brought futile cares. Conclusion : Ethical training and developing decision-making skills are necessary including shared decision making.

Development and Effects of a Simulation-Based Convergence Practicum Education Program for Nursing Seniors (간호대학생 대상 시뮬레이션 기반 융합실습교육프로그램 개발 및 효과)

  • The purpose of this study was to develop and evaluate the effects of a simulation-based convergence practicum education program among senior nursing students. A repeated measure experimental design was used. A total of 77 senior nursing students were recruited from a university located in G city. Data was collected at baseline, program completion, and 2 month following completion of the practicum. Repeated measure ANOVA was used to evaluate the effects of the program. There were statistically significant improvements between baseline and program completion and between the baseline and 2 month follow-up. However, there was no significant differences between the program completion and 2 month follow-up. The study demonstrated that simulation-based convergence practicum education was an effective strategy to increase study outcomes and reduce clinical practice stress among senior nursing students. Further studies are needed to evaluate the effects of the program using randomized controlled trials.

Effects of a New-Nurse Education Program Utilizing E-learning and Instructor Demonstration on Insulin Injection Practices (이러닝 교육(인슐린 주사방법)을 통한 신규 간호사 교육 프로그램의 효과)

  • Purpose: The purpose of this study was to develop and evaluate a new-nurse education program utilizing both e-learning and instructor demonstration. Methods: From August to December in 2009, the e-learning education program about insulin injection was developed. The control (C) group was educated via instructor demonstration from April 15 to October 6 in 2009, and the experimental (E) group was educated via both e-learning and instructor demonstration from January 5 to October 13 in 2010. After each education, knowledge and educational effectiveness were checked. Results: Satisfaction with the education contents in the E group was significantly higher than those of the C group (Z=-3.72, p<.001), and satisfaction with the education method in the E group was higher than those of the C group (Z=-2.98, p=.003). Usefulness (Z=-3.33, p=.001), application (Z=-2.62, p=.009), and confidence (Z=-2.61, p=.009) in the E group were all higher than those of the C group. 78.9% in the E group reused the e-learning program after the experimental education. Conclusion: Combined educational program with e-learning and instructor demonstration had both merits of online efficiency and face-to-face education. It would be useful especially for new-nurses to improve their nursing skills in accomplishing their roles.

The Phenomenological Study on Parents Consultation Experiences of Pediatric Occupational Therapists (아동작업치료사의 부모상담 경험에 관한 현상학적 연구)

  • Purpose : This study phenomenologically explores and describes the experiences of pediatric occupational therapists during parents consultation. Method : In this study, Colaizzi's phenomenological research method was applied. Telephone interview was conducted with six people who had more than eight years of clinical experience in pediatric occupational therapy and were working at a private center. Each interview was recorded with the consent of the subject and analyzed after transcription. Results : In this study, 21 themes, 4 theme clusters, and 2 categories were derived. The categories were the importance of parents consultation and factors influencing parents consultation. The theme clusters were the meaning of parents counseling, the reciprocity of the parent, the child and the therapist, the efforts of the therapist, and the competencies required of the occupational therapist. Conclusion : The results of this study may be helpful for first-time pediatric occupational therapists who are having difficulties with parents consultation. This work can also be used as basic data for developing educational programs and materials for improving the parental consultation skills of pediatric occupational therapists in the future.

  • The purpose of this study is to investigate various opinions on changes and revisions in the Medical Technician Act, to compare the curriculum of radiological technologist and physicians or dentists, and to compare the definitions and scope of work of radiological technologist in Korea and abroad. From the information, The goal is to review whether the phrase 'guidance of a doctor or dentist' specified in the definition of the 'Act on Medical Technicians, etc.' is realistically appropriate. radiological technologist receive specialized college education on radioligical science & medical imaging. The training hours for radiolgical science student are greater than medical students. In addition, radiological technologists are continuously developing their competencies for new knowledge and skills through continuing education in clinical fields. In particular, radiological technologist are making steady research efforts to reduce patient exposure and improve medical image quality. As a result of this investigation, it is considered that the term "guided by a doctor or dentist" as currently defined in the 'Act on Medical Technicians, etc.' may need to be revised in consideration of the professionalism of the radiological technologist.

Influence of Critical Thinking and Empathy on Resilience of Senior Nursing Students (졸업학년 간호대학생의 비판적 사고 성향과 공감역량이 회복 탄력성에 미치는 영향)

  • This study is a descriptive research study to investigate the effects of critical thinking and empathy on resilience of nursing students. The subjects of this study were 234 nursing students in the 4th grade and the data were collected from June 3 to 19, 2019. Data was processed by descriptive statistics, t-test, correlation analysis, and multiple regression method. As a result of the study, critical thinking and empathy showed a significant positive relationship with resilience. Critical thinking and empathy were factors that influence the resilience of nursing students and the resilience was explained as 70.3%. Therefore, in order to increase the resilience of nursing students it is considered that an education program that strengthens internal factors can cultivate critical thinking and empathy skills is necessary.

  • A resolution adopted by the World Health Assembly in 2014 stated that all nurses should be equipped with palliative care skills in order to integrate palliative care into a day-to-day healthcare system. This article introduces the palliative nursing competency that was developed for the Korean environment by the Korean Hospice Palliative Nursing Research Network based on its study of overseas cases where this competency and competency-based training were developed. This is the first step towards the development of competency-based palliative nursing education, and active efforts should be made to integrate this competency into the undergraduate nursing curriculum.

The Effect of Voice Therapy Applying Self-Regulation Concepts on Dysphonia Patients (자기 조절 개념을 적용한 음성치료 기법이 발성장애 환자에게 미치는 효과)

  • Background and Objectives : The goal of this study is to present a strategy for improving the self-regulation (SR) ability and facilitating the change of vocal behavior by applying voice therapy using the SR concept to the patients with vocal cord nodule and muscle tension dysphonia. Materials and Method : The subjects were 80 patients and 80 patients who were diagnosed with muscle tension dysphonia and vocal nodules. As a control group, the results were compared among patients with the same dysphonia without using SR strategies. The concept of SR before voice therapy was explained to the patients, and the treatment was divided into three stages according to the goal of voice therapy. The treatment stages consist of 1) skill acquisition, 2) habit formation, and 3) habit changes. voice therapy was performed by applying SR strategies such as goal implementation intentions and a less routine behavior. Patient's dropout rates were measured to compare the adherence of voice therapy. Results : Significant improvement was seen in all groups receiving voice therapy. However, in the group using the SR strategy, the voice analysis results showed a relatively low dropout rate of voice therapy. In the generalization confirmation stage, patients who applied SR concept showed better results. SR strategy did no longer be necessary to maintain newly adopted vocal behavior. Conclusion : The results of this study show that SR is one of the cognitive factors that can have a significant impact on the outcome of voice therapy, and also has a positive impact on the acquisition and generalization of new skills. A better understanding of SR and the development of therapeutic strategies using it will play an important role in solving voice problems in clinical settings.

Development of a Problem-Based Learning Module for Preschoolers' Growth & Development (학령전기 아동 성장발달의 PBL 모듈 개발)

  • Problem-Based Learning (PBL) is a student-centered pedagogy that integrates nursing knowledge, skills, and attitudes into clinical nursing practice. This pilot aims to apply a PBL module on preschoolers' growth and development in the nursing curriculum. This quantitative study was performed to develop a PBL module following Dick and Carye's program development process (planning, development, application, and evaluation phases), and to evaluate its effects using structured questionnaires among sophomore nursing students. These students formed teams of four or five people each and spent 40 minutes participating in the PBL module. Data were analyzed using descriptive statistics, t-tests, and content analysis. Metacognition level increased significantly. There was no significant difference in team efficacy between pre-test and post-test. Post-test learning satisfaction was high. Students reported obtaining knowledge and problem-solving ability with respect to preschoolers' growth and development and were satisfied with teamwork. This finding offers fundamental knowledge concerning the application of a PBL module in nursing curricula.

No Association between Polymorphisms of Vitamin D and Oxytocin Receptor Genes and Autistic Spectrum Disorder in a Sample of Turkish Children

  • Objective: Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by impairment in social skills and communication with repetitive behaviors. Etiology is still unclear although it is thought to develop with interaction of genes and environmental factors. Oxytocin has extensive effects on intrauterine brain development. Vitamin D, affects neural development and differentiation and contributes to the regulation of around 900 genes including oxytocin receptor gene. In the present study, the contribution of D vitamin receptor and oxytocin receptor gene polymorphisms in the development of ASD in Turkish community was investigated. To our knowledge, this is the first study examining these two associated genes together in the literature. Methods: Eighty-five patients diagnosed with ASD according to DSM-5 who were referred to outpatient clinics of Child and Adolescent Psychiatry of Başkent University and Mersin University and 52 healthy, age and gender-matched controls were included in the present study. Vitamin D receptor gene rs731236 (Taq1), rs2228570 (Fok1), rs1544410 (Bsm1), rs7975232 (Apa1) polymorphisms and oxytocin receptor gene rs1042778 and rs2268493 polymorphisms were investigated using real time polymerase chain reaction method. Results: No significant difference between groups in terms of distribution of genotype and alleles in each of polymorphisms for these genes could be found. Conclusion: Knowledge of genes and polymorphisms associated with the development of ASD may be beneficial for early diagnosis and future treatment. Further studies with larger populations are required to demonstrate molecular pathways which may play part in the development of ASD in Turkey.
