  • Granular cell tumor(formerly named to be granular cell myoblastoma) was first described by Abrikossoff in 1926 and is a rare tumor thought to be of Schwann cell origin. It can occur at any soft tissue of the body, but most cases are found at the tongue, skin, breast and GI tract Only 6% of them occur in respiratory system. We report two cases of asymptomatic granular cell tumor of the bronchus that were detected incidentally by bronchoscopy. One patient had aspiration pneumonia, the other had immotile cilia syndrome. The former patient was simply observed and bronchoscopic extirpation of the tumor was done in the latter patient.

  • Churg-Strauss syndrome(CSS) is a systemic vascular disorder that has an unknown cause with multiorgan involvement and diverse presentations. The three main histologically distinct phases were necrotizing vasculitis, tissue eosinophilia and extravascular granulomas. A diagnosis of CSS can be made on four or more of the following six criteria : 1) asthma, 2) peripheral eosinophilia >10% on the differential leukocyte count, 3) mononeuropathy (including multiple) or polyneuropathy, 4) paranasal sinus abnormalities, 5) nonfixed pulmonary infiltrates and 6) biopsy evidence of extravascular eosinophils in the skin, the nerves, or the lungs. CSS has a good prognosis with systemic steroid therapy. The 5 year survival is approximately 70%. We experienced a 66-year-old man who presented with cough, sputum, edema and numbness in both legs. He presented with all of the 6 CSS criteria. A nerve and muscle biopsy confirmed the diagnosis. Here, we report this case with a review of the relevant literatures.

Study on the modulation of immune system of CPS in atopic dermatitis induced animal models (외치방인 청기패독산(淸肌敗毒散)의 아토피피부염 동물 병태 모델에서의 면역조절작용에 관한 연구)

  • In order to study the effect of CPS in the treatment of atopic dermatitis (AD), its role on various immune related cytokines were tested. Levels of liver and kidney function markers such as ALT, AST, BUN in NC/Nga mice were all normal indicating no toxicity in CPS treated group. Significant recovery from AD could be observed in CPS treated group through naked eye observation. Dermatitis index was significantly decreased after 11, 12, and 13 weeks of treatment. CD4+, CD8+, CD3+ /CD69+ immune cell ratio in DLN were decreased significantly by 37.2%, 49%, 20.8% in CPS treated group. CD4+ and CD11b+ /Gr-1+ immune cell ratio in dorsal skin were decreased significantly by 50.8% and 59.2% in CPS treated group. Expression of IL-4, IL-5, IL-6, IL-13 and TNF-${\alpha}$ in spleen were decreased by 78.8%, 97.8%, 64.7%, 73.6%, and 68.4%, respectively in CPS treated group. The results above strongly indicated the significant immune modulatory effect of CPS and thus clinical application of CPS on AD treatment.

  • A 3-year-old, 38 kg, male Golden Retriever dog was referred to Veterinary Teaching Hospital of Konkuk University because of chronic formation of crust on nasal bridge and planum nasale. Abnormalities of physical examination included hyperkeratosis on the footpad, symmetrical alopecia and erythma around olecranon, and crust on nasal bridge and planum nasale. Results of the hematological examination showed a mild leukopenia with neutropenia, monocytosis, and mild lymphocytosis. In addition, the result of serum chemistry and thyroid gland profile were normal. Results of fungal and bacterial culture was negative. Acanthocytes in cytological evaluation of nasal crust were observed in direct microscopic examination. Examination of skin biopsy exhibited vacuolation of basal cell layer, degeneration and necrosis of basal cell with defluxion, mild monocytes filtrations between epidermis and dermis, and mild acanthosis with hyperkeratinization. Based on results of examination described above, Discoid Lupus Erythematosus (DLE) was diagnosed.

  • Anaplastic large cell lymphoma(ALCL) is a recently recognized large cell lymphoma characterized by large pleomorphic cells, with prominent nucleoli, expressing the CD30(Ki-1) antigen. It is often misdiagnosed as metastatic carcinoma, malignant histiocytosis or Hodgkin's disease. Primary pulmonary ALCL is extremely rare although it frequently involves extranodal sites, e.g. skin, bone, gastrointestinal tract, soft tissue and lung. We report a case of ALCL suspected as primarily involving lung.

  • A 35-year-old male presented high fever and cough. The pateint showed three, discrete, "punched-out", shallow ulcers appearing as pyodermic gangrenosum on the trunk and the back, and a painless subcutaneous nodule on the medial side of the left thigh. The chest X-ray showed multiple cavities on the both lungs. The diagnosis of Wegener's granulomatosis was established by pathology of the skin and the lung, radiologic findings of the chest and positive result of c-ANCA test.

Inhibitory Effect of Artemisiae Asiaticae Herba on Degranulation, Production of Cytokine, and $Fc{\varepsilon}RI$ Expression in RBL-2H3 Cells (애엽(艾葉)의 RBL-2H3 비만세포 탈과립, cytokine 생산 및 $Fc{\varepsilon}RI$ 유전자 발현에 미치는 영향)

  • Artemisiae Asiaticae Herba (AAH) has been used to remedy of symptoms such as bleeding, dysmenorrhea, eczema and itchy skin in Oriental Medicine. In this study, we investigated the protective effect of AAH on allergic response. The effect of AAH was analyzed by ELISA and RT-PCR in RBL-2H3 cells. We investigated cell viability, ${\beta}$-hexosaminidase and histamine as markers of degranulation, production of IL-4 and TNF-${\alpha}$, and gene expression of HDC2, cytokines and $Fc{\varepsilon}RI$ ${\alpha}{\beta}{\gamma}$ subunit. We found that AAH suppressed ${\beta}$-hexosaminidase and histamine release, the production of IL-4 and TNF-${\alpha}$ in RBL-2H3 by the anti-DNP IgE plus DNP-HSA stimulation. AAH also significantly decreased cytokine mRNA expressions, such as IL-$1{\beta}$, IL-2, IL-3, IL-4, IL-5, IL-6, IL-12, IL-13, TNF-${\alpha}$, and GM-CSF, and increased cytokine mRNA expressions of IL-10 in RBL-2H3. In addition, AAH suppressed mRNA expression of $Fc{\varepsilon}RI$ ${\alpha}{\beta}{\gamma}$ subunit on cell surface. Our results indicate that AAH protects against allergic response and exerts an anti-inflammatory effect through the inhibition of degranulation and production of cytokines and expression of $Fc{\varepsilon}RI$ ${\alpha}{\beta}{\gamma}$ subunit.

  • Langerhans\` Cell Histiocytosis(LCH) is a disease of unknown origin, characterized by abnormal proliferation of Langerhans\` cells. Previously, it has been called histiocytosis X, which included eosinophilic granuloma, Hand-Sch ller-Christian\`s disease, and Letterer-Siwe disease. Any organs or tissues such as skin, lymph nodes, bone and bone marrow can be involved. However, LCH of chest wall is rarely reported in our country. We experienced a 18 month old male child, who had osteolytic lesion involving the rib with axillary lymph node metastasis. The tumor was confirmed as LCH after surgery.

  • Scolpendra subspinipes (centipede) is pungent in flavor, activates the channels to stop pain, and, removes toxins and resolves masses. So it has been used as a folk remedy. We researched folk remedies which used centipede as a single medicine and results of its prior clinical and experimental study, and figured out that centipedes are effective on ulcerative disease, herpes zoster, clavus, insect bite, inflammatory disease, furunculus, pain disease by external therapy, and oral administration is effective on anti-cancer such as hepatoma and uterine cancer, cerebrovascular disease, inhibition of atherosclerosis, sedation of central nervous system, antipyretic action, and anti-inflammation, and centipede Herb-Acupuncture is effective on inflammatory disease and pain disease. Centipede preparation materials for external therapy were sesame oil, perilla oil, vinegar, salt, and, burnt alum. Fumigation of centipede treated paronychia, pertussis, and hemorrhoids. For the external therapy, the way of centipede powder mixing with other materials has been used. It is needed that we use dusted powder to make skin absorb its ingredients more easily, and we study intensely minor ingredients to make centipede components absorbed more effectively and do pharmacological action. Also, fumigation, a kind of another external therapy, is considered to be effective treatment of upper respiratory infection like pertussis, so it needs an in-depth study on centipede fumigation.

Antioxidative and Anti-inflammatory Effect of Ethanol Extract from Duchesnea chrysantha (사매 에탄올 추출물의 항산화 및 항염증 효과)

  • Oxidative stress has been implicated in cutaneous damage in various inflammatory skin diseases, including atopic dermatitis. The present study was undertaken to investigate the antioxidative and anti-inflammatory activities of the extract of Duchesnea chrysantha (DCE). DEC was prepared by extracting with 80% ethanol. Total flavonoids and polyphenols were measured by a colorimetric assay. The free radical scavenging activity of the extract was analyzed by the DPPH (1,1-diphenyl-2-picryl hydrazyl), ABTS (2,2'-azino-bis(3-ethylbenzthiazoline-6-sulfonic acid) and Griess reagent assay. An oxidative product of nitric oxide (NO), was measured in the culture medium by the Griess reaction. The level of prostaglandin $E_2$ ($PGE_2$) was measured by enzyme-linked immunosorbent assay. The expressions of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) were measured by Western blot analysis. Total flavonoid and polyphenol contents of DCE were included $24.73{\pm}0.45$ and $178.77{\pm}2.65$, respectively. DCE significantly increased electron donating ability (DPPH), nitrite scavenging (NO) and ABTS reducing activity in dose dependant. We investigated the anti-inflammatory effects of DCE on lipopolysaccharide (LPS)-stimulated RAW 264.7 cells. DCE significantly suppressed NO and prdstaglandin $E_2$ ($PGE_2$) in dose dependant. Furthermore, the levels of iNOS and COX-2 protein expressions were markedly suppressed by the treatment with DCE in a dose dependent manner. These results suggest that DEC may has value as natural product with its high quality functional components, antioxidative and anti-inflammatory activities.
