Certification and Management of Occupational Health Nurse in Korea (한국의 산업전문간호사 자격인증 및 제도운영방안)

  • The purpose of this study was to develop the certification system, analyze the job profile of occupational health nurses, plan the education program and suggest the political recommendations. Many materials of KAOHN (Korean Association of Occupational Health Nurses) were collected and analyzed and many times of meetings and workshops were held to coordinate the various opinions from related organizations. The results were as follows; 1. The certification of occupational health nurse will be available to the nurses who have the careers more than three years, graduate the education program for certified occupational health nurse, and pass the certified occupational health nurse examination. 2. Two types of education program will be needed, one will be the one-year program for diploma nurses and the other will be master program for baccalaureate nurses. 3. The plan was made for 2000 occupational health nurses to be certified by 2010. By 2007, annually 200 occupational health nurses will graduate from one-year program and 40 occupational health nurses will graduate from master program. After than, 260 occupational health nurses will graduate from master program and 40 or 80 occupational health nurses will graduate from one-year program. 4. To facilitate the usage and motivation of certified occupational health nurse, political recommendations were suggested; revision of the related laws and regulations, the incentives for employers, and financial supports for the certified occupational health nurse educations by the government.

Mediating Effect of Self-Esteem on the Relationship between Perceived Discrimination Occupational Stress of School Nurse (보건교사의 차별인식과 직무스트레스와의 관계에서 자아존중감의 매개효과)

  • Objectives: This study examined the relationship between discrimination perception and occupational stress of school nurses to verify the mediating effects of self-esteem on the relationship between the perception of discrimination and occupational stress. Methods: This study was a secondary analysis of the data collected by the Public Health Nurses Association of D city from regular school nurses working in elementary, middle, and high schools in September 2017. The data were analyzed using the SPSS 25.0 Statistics Program, and multiple regression analysis was used to analyze the effects of self-esteem on the relationship between the perception of discrimination and occupational stress. Results: The occupational stress of school nurses was higher with higher discrimination and lower self-esteem, and the moderating effects of self-esteem (β=-.152, p=.016) was confirmed between the perception of discrimination and occupational stress (F=30.117, p<.001). Conclusions: Based on the results of this study, it is necessary to prepare a method for managing occupational stress due to job demands of school nurses as well as organizational measures to reduce discrimination in organizations. In addition, efforts to develop various programs focusing on improving the self-esteem of school nurses are required.

The Current Status of Student Mental Health Programs and the Needs for Related Training: Focusing on School Nurses and School Counselors (학생정신건강사업 현황 및 관련 교육 요구도: 초중고 보건교사와 상담교사 대상)

  • Purpose: The leading cause of death among teenagers in Korea is suicide. Also, it is known that mental health of Korean adolescents identified based on the levels of perceived stress or depression is very poor. The purpose of this study was to examine the current status of student mental health programs in Korea and the associated problems and to explore ways to make the programs more effective. Methods: The subjects of the study were a total of 115 school nurses and school counselors in elementary, middle and high schools. The study examined the current status and the associated problems of school mental health programs as well as the needs for teacher training programs related to student mental health. Results: Few mental health programs other than counseling for at-risk students were being carried out. The problems associated with student mental health management were, in terms of management, 'accountability in case of incidents such as student suicide' and 'parents uncooperative in their kid's mental health management (visit to hospital, etc.)' and, in terms of manpower, 'unclear R&R between school nurses and school counselors' and, more fundamentally, 'the lack of human resources.' The needs for teacher training programs related to student mental health were very high and the content required most was 'ways to manage stress of students and teachers themselves,' followed by 'interpersonal conflicts management & conservational skills,' and 'ways to manage student mental illness.' Conclusion: In conclusion, to increase the effectiveness of student mental health programs, we should move on from the current system which focuses only on early discovery and intervention to a more comprehensive system that includes preventive approaches, while having clearer R&R and more workforce dedicated to student mental health programs and attracting more parental cooperation and support.

Relationship between Critical Thinking Disposition and Clinical Decision-Making Abilities in Home Health Advanced Practice Nurses (가정전문간호사의 비판적 사고성향과 임상의사결정능력과의 관계)

  • Purpose: Clinical decision-making carried out by nurses is a complex process that influences the quality of care provided and various patient outcomes. This study examined the relationship between critical thinking disposition and clinical decision-making abilities in home health advanced practice nurses. Method: The study had a non-experimental correlational design. Data were collected from 100 home health advanced practice nurses in 20 hospitals. Results: The mean critical thinking disposition score was $3.69{\pm}.39$ out of 5 and the mean score for clinical decision-making abilities was $3.48{\pm}.22$ out of 5. In this correlation analysis, a significant positive correlation (r=.58, p<.001) was found between critical thinking disposition and clinical decision-making abilities of home health advanced practice nurses. Conclusion: In order to improve the clinical decision-making ability of home health advanced practice nurses, we need to improve their critical thinking disposition. In order to make this change, appropriate training program are needed to increase the critical thinking disposition and clinical decision-making abilities of home health advanced practice nurses.

Factors Affecting Cultural Competency of Elementary School Nurses (초등학교 보건교사의 문화적 역량 영향요인)

  • Purpose: The aim of this study was to investigate the factors affecting the cultural competency of elementary school nurses. Methods: Data were collected from a convenience sample of 109 school nurses who were recruited from elementary schools across three large cities in South Korea. The daata were analyzed with independent t-tests, a one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. Results: School nurses who had experience caring for multicultural students showed a significant difference in the cultural competency (t=8.75, p=.004). Factors affecting their cultural competency were awareness of multicultural families (${\beta}=.17$, p=.006) and transcultural self-efficacy (${\beta}=.69$, p<.001), which explained 58.6% of the total variance. Conclusion: The findings suggest that it is necessary to provide more training and educational programs for school nurses to improve their awareness of multiculcural families, including interventions to enhance transcultural self-efficacy, in order to enhance their cultural competence and to help them provide tailored care for multicultural students with diverse cultural backgrounds.

Impact of Psychosocial Factors on Occurrence of Medication Errors among Tehran Public Hospitals Nurses by Evaluating the Balance between Effort and Reward

  • Background: Patient safety and accurate implementation of medication orders are among the essential requirements of par nursing profession. In this regard, it is necessary to determine and prevent factors influencing medications errors. Although many studies have investigated this issue, the effects of psychosocial factors have not been examined thoroughly. Methods: The present study aimed at investigating the impact of psychosocial factors on nurses' medication errors by evaluating the balance between effort and reward. This cross-sectional descriptive study was conducted in public hospitals of Tehran in 2015. The population of this work consisted of 379 nurses. A multisection questionnaire was used for data collection. Results: In this research, 29% of participating nurses reported medication errors in 2015. Most frequent errors were related to wrong dosage, drug, and patient. There were significant relationships between medications errors and the stress of imbalance between effort and reward (p < 0.02) and job commitment and stress (p < 0.027). Conclusion: It seems that several factors play a role in the occurrence of medication errors, and psychosocial factors play a crucial and major role in this regard. Therefore, it is necessary to investigate these factors in more detail and take them into account in the hospital management.

Type and Contents of Health Records and Recording Tendencies of Nurses in Small-Scale-Enterprises (소규모 사업장 보건관리 산업간호사의 업무서식과 기록행태)

  • This study was carried out to investigate the types of health documents for nurses, the content of informations in the documents, and writing behaviors of occupational health nurses. Health documents were collected from 7 nurses who were working in 7 group occupational health agencies (GOHA) located at Seoul and Inchon area in Korea. The collected health documents written in January to June 1999 were analyzed, and revealed the following results. 1) The occupational health nurses were using 9 to 18 different types of health documents. The contents of the documents were considered quite similar to each other with slightly different headings and items to record. Among different types of health documents. Health Management Card for Workplace', Nursing performance sheet and Workplace environmental checklist were in common among nurses and were used for content analysis. 2) The 'Health Management Card for Workplace' was the only formal sheet of small-scale-enterprises (SSE) for health management, in which health and safety related information was recorded. The information on nursing services were recorded on the Nursing performance sheet, which has slightly different names on each type with similar contents. The Workplace environment checklist was for the information on general work environment management and mainly status of workplace hygiene. This checklist is to be used by or with nurses among the 3 types health professional team such as doctor, hygienist, and nurse, but it seemed not being used frequently by nurses. 3) Analysis on recording tendencies of nurses revealed that the writing styles of occupational health nurses were associated with 'memo' using a few number of words and short sentences. The amount of information by this kind of recording style was considered not enough for health management situation. The possible reasons for nurses to use this writing style might be insufficient time for recording and improper designed format of health documents. Because nurses working in SSE spend more time on the roads to visit workplaces, nurses may not found enough time for recording properly within their working time. In addition, the health records were designed to focus on the frequency of nurse's performance in certain types of work rather than on the method they used to deal with health problems. In conclusion, this study suggests that some steps are necessary to develop health documents and recording system which is appropriate to occupational health nurses. The educational need for nurses on appropriate recording behavior is also recommended.

Attitude toward Health behaviors in Student Nurses (간호대학생의 건강행위에 대한 태도 조사연구)

  • Health perception is a very important issue for student nurses to be future health professionals. The concept of health has changed with the emphasis of 'health promotion', and thus nurses now have a demanded role as a health promoter. According to health promotion theories, health-promoting behaviors are affected by cognitive variables, and human behaviors are related to thoughts and perception. This study was conducted to recognize the relationships of attitude toward health behaviors to other health related variables. The data was collected using a self administered survey. The instrument used in this study was attitude of health behavior scales that were developed by Okayama medical school. The subjects were 512 student nurses recruited from 1 college and 3 universities. The data was analyzed using SPSS pc program with mean, correlation. and multiple regression technique. The results were as follows: 1. The mean score of attitude of health behaviors of the student nurses was 8.11, showing a relatively high level. A high score of health belief was reported in 'self-responsible type' with the score of 4.80, and a high score of 'think for health or disease' was reported in passive type with the score of 3.12. 2. Specifically, there were significant positive correlations between the level of 'attitude of health behaviors' and other health related variables such as health perception, think for health or disease, and age. 3. Stepwise multiple regression analysis revealed that active thought, leisure, self-responsible perception and depression accounted for 13%of variance of attitude toward health behaviors. In conclusion, although student nurses certainly perceived the performance of health behavior, they need to learn in more systematical way in order to be a better health promoter, one of the role of nurses. The results of the study suggest that further studies need to be done on attitude toward health behaviors and performance of health behavior by student nurses.

Current Status of Home Visit Programs: Activities and Barriers of Home Care Nursing Services (재가간호서비스 제공자의 업무 수행 현황과 장애요인)

  • Purpose: The purpose of this study was to examine the current status of home care nursing services provided by community health nurses and to identify barriers to the services. Methods: A cross-sectional survey was conducted with three types of community health care nurses. Participants were 257 nurses, 46 of whom were hospital based home care nurses, 176 were community based visiting nurses, and 35 were long term care insurance based visiting nurses. A structured questionnaire on 7 domains of home care nursing services with a 4-point Likert scale was used to measure activities and barriers to care. Data were analyzed using SPSS WIN 21.0 program. Results: Hospital based home care nurses showed a high level of service performance activity in the domain of clinical laboratory tests, medications and injections, therapeutic nursing, and education. Community based visiting nurses had a high level of service performance in the reference domain. Long term care insurance based visiting nurses showed a high level of performance in the service domains of fundamental nursing and counseling. Conclusion: The results show that although health care service provided by the three types of community health nurse overlapped, the focus of the service is differentiated. Therefore, these results suggest that existing home care services will need to be utilized efficiently in the development of a new nursing care service for patients living in the community after hospital discharge.

The Effects of Job Demands and Emotional Intelligence on Emotional Exhaustion in School Nurses -Focusing on the Mediating Factors: Stress Response and Job Satisfaction- (보건교사의 직무요구와 감성지능이 정서적 고갈에 미치는 구조모형: 스트레스 반응과 직무만족 매개 요인을 중심으로)

  • Purpose: This research was conducted to investigate the characteristics of emotional exhaustion that Korean school nurses suffer, to suggest policy implications to address this issue, and to propose preventive and supervisory measures regarding emotional exhaustion experienced by school nurses in elementary, middle, and high schools in Korea. Methods: A survey was conducted under the authorization of Chungcheongnam-do Office of Education. Out of 400 copies first distributed, 376 copies were returned. Among the 376 copies, 117 copies which were judged insincere or from teachers with less than 1 year of experience were exclded from the study. Through this process, only 259 copies were finally selected. Results: As a result of assessing the fit of the model, it was found that 𝑥2=67.247 (df=28, p<.001), GFI=.953, AGFI=.911, TLI=0.936, CFI=0.965 and RMSEA=.074. Emotional exhaustion was found to have a direct effect on job demand (β=.26) and an indirect effect on stress response (β=.31). The total effect was β=.57. There was a positive complete indirect effect of β=.52 between emotional intelligence and emotional exhaustion. The total effect was not significant, respectively. Conclusion: Since job demands of school nurses affect emotional exhaustion, it is necessary to reconstruct their jobs and operations to alleviate their job demands.
