Safety of low-dose anticoagulation in extracorporeal membrane oxygenation using the Permanent Life Support System: a retrospective observational study

  • Background: Bleeding and thrombosis are major complications associated with high mortality in extracorporeal membrane oxygenation (ECMO) management. Anticoagulant therapy should be adequate to reduce thrombosis. However, related studies are limited. Methods: We retrospectively reviewed all patients supported with ECMO at a single institution between January 2014 and July 2022 and included those on all types of ECMO using the Permanent Life Support System. Patients were classified into two groups according to their measured mean activated partial thromboplastin time (aPTT) during ECMO management: a high-anticoagulation (AC) group (aPTT, ≥55 seconds; n=52) and a low-AC group (aPTT, <55 seconds; n=79). The primary outcome was thrombotic or bleeding events during ECMO. Results: We identified 10 patients with bleeding; significantly more of these patients were in the high-AC group (n=8) than in the low-AC group (15.4% vs. 2.5%, p=0.01). However, thrombus events and oxygenator change-free times were not significantly different between the two groups. Four patients in the high-AC group died of bleeding complications (brain hemorrhage, two; hemopericardium, one; and gastrointestinal bleeding, one). One patient in the low-AC group developed a thrombus and died of ECMO dysfunction due to circuit thrombosis. Conclusion: Heparin did not significantly improve thrombotic outcomes. However, maintaining an aPTT of ≥55 seconds was a significant risk factor for bleeding events, especially those associated with mortality.

  • In this study, the disaster management and disaster medical improvement was described in underground space(specifically underground shopping center) in case of fire disaster occurred. Firstly, statue and system was discussed concerning building law, safety, fire services, refuge. The underground in Seoul management is classified as to space and form and management agent is different according to the type form. Because the difficulty of emergency rescue arises due to individual management system unified system needs to be established and improvement of facility management agent is necessary. For the patient to be transfer on the ground, corporation between command head quarter and emergency rescue team are essential. And disaster information and emergency medical information are also need to be considered. Therefore, effective countermeasures for emergency saving is urgent considering distribution of medical institution and medical treatment.

  • Since the late 1950s, concerted efforts to reduce the accident rate in aviation have yielded unprecedented levels of safety. Although, the overall accident rate has declined considerably over the years, unfortunately reductions in human error-related accidents in aviation have failed to keep pace with the reduction of accidents due to environmental and mechanical factors. Today, a very large percentage of all aviation are attributable, directly or indirectly, to some form of human error. As a result of many study, a range of prevention of human error have been developed. but each of kind is lack of a precision, effectiveness and seem to be considered for aspect of deficiency as an systematic accessibility. So, we're going to analysis the most effective and systematic prevention of human error and study on consolidating method for human error and aviation safety. In this study, several alternatives for the prevention of human errors a priority to understand and solve problems by identifying the implications for human error to be presented.

Security Problem of National Major Facility's Parking Lot and its Improvement Method -Focused on Doonchi(Waterside) Parking Lot of National (국가중요시설의 주차장 보안의 문제점과 개선방안: 국회둔치주차장을 중심으로)

  • National Assembly is a constitutional institution that is required to first consult the will of the people and it should do its effort continuously so that security of citizens using parking lot would be enhanced at the same time while improving parking service in order to increase customer satisfaction of the people. Under this recognition, in this study, Doonchi parking lot of National Assembly under consigned management was first reviewed in a perspective of criminal prevention through environmental design(CPTED) and particularly, fence installation and reinforcement work for securing 'territoriality' and operation of all round shooting camera and installation of No-trespassing warning board at entrance were suggested. Second, it was recommended to change independent control system in which CCTV security system of National Assembly Doonchi parking lot is operated separately from National Assembly safety situation room and integrate it with National Assembly safety situation room(revised to double safety system) and performance of CCTV camera was made to be increased to over 2m. In addition, video recording mode was converted to NVR mode for application to IP camera in the future and in order to avoid dead zone of security monitoring area and based on site inspection result, addition 3 places of newly installing CCTV were indicated. Third, it was recommended to introduce parking fare billing and management system through unmanned equipment in parking lot management and operation.(specialized management of professional parking service provider was reviewed). By doing so, risk of cash handling by charging personnel was removed by reducing current 7 working personnel to 3 and particularly, by converting parking lot management mode being operated temporarily from 9 A.M. to 9 P.M. at present to 24 hours operation mode and providing more specialized parking service, citizens visiting National Assembly were provided with convenience and image of National Assembly was also enhanced. This study was carried out in parallel with various literature and case studies, including data from the Office of the Defense Protection in the National Assembly.

Optimal Exposure Conditions according to Detector Type in Chest Digital Radiography (디지털흉부X선촬영에서 검출기 방식에 따른 최적의 노출조건)

  • The aim of this study was to set up the optimal exposure condition according to detector type considering image quality (IQ) with radiation dose in chest digital radiography. We used three detector type such as flat-panel detector (FP) and computed radiography (CR), and charge-coupled device (CCD). Entrance surface dose (ESD) was measured at each exposure condition combined tube voltage with tube current using dosimeter, after attaching on human phantom, it was repeated 3 times. Phantom images were evaluated independently by three chest radiologists after blinding image informations. Standard exposure condition using each institution was 117 kVp-AEC at FP and 117 kVp-8 mAs at CR, and 117 kVp-8 mAs at CCD. Statistical analysis was performed by One way ANOVA (Dunnett T3 test) using SPSS ver. 19.0. In FP, IQ scores were not significant difference between 102 kVp-4 mAs and 117 kVp-AEC (28.4 vs. 31.1, p=1.000), even though ESD was decreased up to 50% ($62.3{\mu}Gy$ vs. $125.1{\mu}Gy$). In CR, ESD was greatly decreased from 117 kVp-8 mAs to 90 kVp-8 mAs without significant difference of IQ score (p=1.000, 24.6 vs. 19.5). In CCD, IQ score of 117 kVp-8 mAs was similar with 109 kVp-8 mAs (29.6 vs. 29.0), with decreasing from $320.8{\mu}Gy$ to $284.7{\mu}Gy$ (about 11%). We conclude that optimal x-ray exposure condition for chest digital radiography is 102 kVp-4 mAs in FP and 90 kVp-8 mAs in CR, and 109 kVp-8 mAs in CCD.

  • For the introduction of the electricity of the special design and supervision system to Protect the faulty-workmanship of the electricity equipment and to reserve the electricity safety, last 1995 "Electricity Technology Management Act" were carried out enactment. March 2002, we introduced the pre-Qualification system that is selected the competent company about the electricity design and supervision service over so much capacity which is ordered by the Public institution such as the nation, local autonomous entity, government investment institute and so on. December 2005, the electricity equipment over so much capacity was taken the housing construction plan's approval by the housing act in case, the municipal or district governor with the authority to approve who selected the electricity supervision company according to the pre-Qualification system. Due to the introduction of the system, we could expect the rights and interests increase of the consumer by eradicating dumping of the close relationship between supervision company and the builder, reduction and concurrent position of the supervisor, according to confirm the quality of the electricity equipment and electricity safety. In spite of introducing of the system, the problems occured as reducing the electricity designer's task range compared with different field, modifing the supervisor's rank system rationally, because of researching about the electricity design and supervision technique lacks. Also, KEEA(Korea Electric Engineers Association) manage the caller of the electricity design and supervisor(company or people), but the problems occured as opening to the public about the curler management, introducing automatic bidding system and We need the solution plan about this. Consequently, we consider the present situation of the domestic and outside and problem about the electricity design and supervision system in this paper, try to present rational operation plan through this.

Comparison of Job Tasks and Task Elements of Korean Nurse Anesthetists by Type of Medical Institution: Hospital, General Hospital and Higher General Hospital (의료기관 규모에 따른 마취전문간호사의 직무관련 특성, 직무 및 직무요소의 수행빈도 차이 비교)

  • Purpose: This study was done to identify job tasks and task elements of Korean nurse anesthetists according to type of medical institution. Methods: A job task scale which consisted of 9 job tasks and 40 task elements was developed. Data were collected from December, 2009 to February, 2010 from 182 nurse anesthetists who were working in medical institutions (response rate: 75.8%). Results: Forty-eight percent of nurse anesthetists were independent from anesthesiologists in anesthetic practice. Preanesthetic nursing assessment was much more frequent in small hospitals than in general hospitals (p<.05), and anesthetic nursing intervention, administering the anesthetics, monitoring the patient's status during anesthesia, and provision of safety and compliance with anesthetic ethics were much more frequent in general hospitals than medical centers (p<.001). There were no differences among the medical institutions for job tasks in post-anesthetic nursing interventions (p=.229), administering anesthetics (p=.354) and monitoring patients' status during anesthesia (p=.099), providing safe anesthetic environment (p=.896), and management of ancillary personnel/equipment (p=.617). Conclusion: Results indicate that nurse anesthetists contribute significantly to anesthetic practice in small hospitals and general hospitals. Therefore, it recommended that nursing leaders make efforts to enact legal nurse anesthetist-related policies for safe and high quality anesthetic nursing care.

  • Due to the increase in awareness of school safety after the Sewol ferry disaster, it became necessary to improve the capability to prevent a safety accident at school and to respond to an accident in the case of emergency as well as the awareness of safety issues. Therefore, it is necessary for educational institutions to reinforce safety measures autonomously, and the importance of supports and cooperation from agencies related to the safety of local community is also emphasized. However, it is quite difficult for schools and the Office of Education to prevent safety accidents only with their own capability, and safety accidents can be prevented through the supports from agencies involved in safety of local communities. Therefore, the status and cooperation system of educational institutions and relevant authorities were reviewed theoretically in this study. In addition, a field investigation was carried out to examine the awareness of disaster and safety, and cooperation status of officials in charge of practical disaster-related works. Moreover, this study intended to present a measure to establish the safety cooperation system between educational institutions and agencies involved in safety of local community in the future based on the findings.

The Analysis of radioactivity Concentration in drainage when using a radioactive Iodine (방사성옥소 사용 시 배수 중 방사능농도 분석)

  • Purpose With regard to the use of radioiodine in domestic medical institution, the case of exceeding the allowance of nuclear safety Act about radioactive concentration in drainage was found. Through understanding the cause of exceeding case and analyzing radioactive concentration in drainage, evaluating the relationship of the public waters in surroundings and usefulness. Materials and Methods From November 1, 2014 to April 30th, 2015, the research is aiming at domestic twenty hospitals for six months. By using a HPGe gamma-ray spectrometer(Canberra DSA-1000) and GENIE-2000 Analysis software for comparative analysis, measuring a radioactive concentration of radioiodine in drainage. Consequently, we confirm the excess of radioactive concentration of radioiodine in seven medical institutions. Results Conducting a survey of twenty hospitals and average radioactive concentration of radioiodine in drainage appears $42,100Bq/m^3$. The features of domestic hospitals where show a high radioactive concentration are a number of medical treatment patient when using radioactive iodine and the absence of private rest room. During I-131 whole body scan, the pretreatment procedure of urinating is considered emission of residual Iodine. In public waters, the cause of exceeding detect on radioactive concentration in drainage suppose a diagnostic radioactive iodine. Conclusion We confirm the importance of enhanced education, providing a safety control instructions and installing a private rest rooms for patients who injected a low capacity radioiodine. Also, constructing institutional and legal management system is considered about the Emission management standard in drainage.

A Survey on the Sanitary Management with Step-by-step Working Process in Food Service Institutions (집단급식소의 작업 단계별 위생관리에 대한 실태 조사)

  • Sanitary management with step-by-step working process and dietician's perception against them at 98 food service institutions located in Jeollabuk-Do were surveyed. Food service institutions included 13 hospitals, 38 schools,40 enterprises, and 7 others. Practice ratio of sanitary management items at purchasing and receiving step of food and food materials in food service institutions were 59.2-98.0%, and they were higher than those of other steps. In pre-treatment step, practice ratio of 'Undoing treatment of foods on the ground' in hospital was higher (53.8%) than those of enterprise (32.5%) and school (34.2%), and needed the improvement of pre-treatment procedure in enterprise and school. Practice ratio of all sanitary management items in cooking step were below 30%, and needed the improvement of cooking procedure. In storage step, the improvement of cooking procedure in school and enterprise were needed. Practice ratio of all sanitary management items in distribution step were low, and needed the improvement of this working procedure. However, in spite of low practice ratio on these items, dietician's perception against sanitary management items in all steps was low. Therefore, we estimated that the improvement of working processes and the conversion of dietician' perception on sanitary managements in food service institutions were needed.
