Purpose: This study was conducted to investigate health problems and factors related to work impairment of nurses working at community health centers. Method: Data were collected from May 2008 to July 2008 using a structured, self - reported questionnaire for general characteristics, work-related characteristics, health problems and work impairment. The subjects were composed of 434 nurses from 25 community health centers in Seoul. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and multiple regression analysis. Results: For present health problems, shoulder, back and neck pain were ranked the highest at (54.6%), followed by cold, GI trouble, and visual disorders. The average score of work impairment was 24.71($\pm7.14$) out of 50.0. Multiple regression analysis revealed that the major factors that affect work impairment were age and number of health problems, and that these factors explained 15% of the work impairment. Conclusion: Age and number of health problems are the factors that have the greatest influence on work impairment. These findings indicate that the health condition of nurses at community health centers must be considered to improve the overall quality of nursing service.
The purposes of this study were to investigate the perception and needs of community nutrition programs for 379 community residents of 23 health centers where the pilot community nutrition programs are intervening. The awareness rate of the nutrition programs was 54.3% and the reason of the awareness was mainly happened to know when visiting health centers'. More than 90% of the respondents responded that public health nutrition services are necessary. But the residents who experienced the nutrition services showed higher needs of the programs(97.3%) and improved the impression about the roles of health centers(93.6%). They also showed a higher rate of balanced dieting, stronger intentions to change their inappropriate eating style and a higher practicing rate. The more they believed in the provided nutrition information, the more they showed concerns about their diet and practicing rate of the advices from nutritionists. These results show the positive and successful impact of the pilot nutrition programs on the community residents. We need strategies for a more active improvement of the programs and to maintain more qualified public health nutritionists to carry out targeted community nutrition programs.
Health centers provided intensive health care services for local residents according to changes in the times and environment. Public health centers were given various roles such as medical treatment, administration, and service, and the demand for functional reorganization has emerged. We analyzed the literature on the functional restructuring of public health care institutions. In addition, the current status of medical services, which is the main function of institutions, will be analyzed through health insurance statistical data, and detailed contents will be analyzed according to regional types and income levels. As a result of the analysis of medical services at institutions, the total number of patients was 2,238,000, and the number of visits was 11,806 times. Total medical expenses were 169.6 billion won, of which 132 billion won was found to be benefit. When analyzing the number of patients per institution, public health centers had the largest number of 4,326, and the share of benefit was also the highest at public health centers. It should focus on the function of providing local health and medical services related to health promotion and disease prevention in the community. This functional reorganization of public health centers can contribute to forming cooperative relationships with private medical institutions in the local community. For this, first, to establish the role, essential functions for public health centers for preventive health management are established. Secondly, regular manpower expansion and flexible manpower management are required in the human resources sector. Finally, in the organizational sector, it is necessary to establish a step-by-step organizational system according to environmental changes.
Although breastfeeding is recognized to be vital to the health and well-being of children and women, the rate of breastsfeeding among Korean women has continuously decreased. One barrier to breastfeeding has been identified to be associated with health care providers. Health care professionals do not give sufficient advice and encouragement, next do hospitals provide supportive environments for breastfeeding by separating infants from mothers or providing formula. The purpose of this study was to investigate prenatal breastfeeding education and infant feeding practices in public health centers and baby-friendly hospitals. A telephone survey was carried out on 57 public health centers located in the Seoul and Chungcheong areas and 13 baby-freindly hospitals from February to April in 2000. Among the public health centers, 43.8% offered periodic prenatal education for pregnant women who visited the centers. Most of them used leaflets or pamphlets developed by the Ministry of Health and Welfare or UNICEF. Twenty six percent of the public health centers developed their own educational materials. All of the 13 baby-friendly hospitals gave additional fluids to infants, did not allow mothers and infants to stay together 24 hours a day, and did not foster the establishment of breatfeeding support groups among the mothers. Most of the baby-friendly hospitals gave artificial teats, did not help mothers initiate breastfeeding within a half-hour of birth. Findings indicate that current practices even in the baby-friendly hospitals are not consistent with the '10 steps to success breastfeeding'. Therefore, deliberate efforts should be made to incorporate adequate breastfeeding education into prenatal program in both public health centers and hospitals, and create environments to support breastfeeding in hospitals, even in baby-friendly hospitals.
The purpose of this study was to assess the needs for nutrition programs in the public health centers by general characteristics and body mass index (BMI) of community residents. Information of general characteristics of study participants including age, education, income, marital status, residence, and job, and resident's interest in nutrition programs, the preferred educational methods and the willingness to participate in nutrition programs were collected by an interview. Weight and height were measured and were used to calculate the BMI. The program that resident's were most interested in was 'diet therapy for chronic disease and counseling' ($28.5\%$), followed by 'nutritional management for the elderly' ($21.1\%$), and 'obesity and weight control' ($17.1\%$). 'Education and counseling by nutrition professionals' was the most preferred educational method. Among the programs that the subjects would participate in, if they were offered in public health centers, $65.8\%$ subjects would participate in 'diet therapy for chronic disease and counseling' programs, $64.9\%$ would participate in 'nutritional management for the elderly' programs and $52.2\%$ would participate in 'obesity and weight control' programs. The contents of programs that the subjects were interested in, the preferred methods and their willingness to participate nutrition programs differed significantly by age, income, education, marital status, and body mass index. The results imply that the planning of nutrition interventions in the public health centers must be tailored and targeted group specific by taking the participants general characteristics and body mass index into consideration. Purpose: To explore the barriers and solutions of immunization programs for children among vulnerable classes in the position of service providers in Community health centers. Method: Data were collected through focus group interviews with 3 groups (17 providers), and were analyzed according to the analytic method of Morgan (1998) and Krueger (1998). Results: The following five categories were identified: difficulties in obtaining a list of children among vulnerable classes, difficulties in first contact with them, difficulties in up-to-date immunization, various difficulties according to the types of vulnerable family and suggestions of solution. These results indicated that there are many difficulties in providing expanded immunization service for children among vulnerable classes in terms of manpower, cooperation of departments in public health centers and linkages between health and welfare. Finally, the characteristics of the types of vulnerable family need to be elucidated. Conclusion: To increase immunization rates among children of vulnerable classes, policies need to be prepared at the public health organization itself and the linkages between health and welfare and need for hiring additional personnel.
Purpose: This study was conducted to identify factors of COVID-19 stress, impulsiveness, and family strength on suicidal ideation of public nursing officials in community health centers. Methods: This study was a descriptive analysis study the subjects of this study were a total of 145 public nursing officials from community health centers in Busan. The data were analyzed using IBM SPSS/WIN 23.0 version. Result: The result of the predictor analysis showed that the motor impulsiveness (β=.383, p<.001), workload (β=.222, p=.003), cognitive impulsiveness (β=-.205, p=.012), bond (β=-.169, p=.033). The regression model showed an explanatory power of 24.1%. Conciusion: it is necessary to increase impulse and stress control ability, and to adjust the workload. In addition, it is necessary to systematically guarantee a working environment where practical vacation can be used, but it may not be a realistic alternative in a disaster crisis such as COVID-19, so indirect alternatives such as reinforcing infectious disease experts, improving the work environment, and psychological support to prevent suicide in advance are required.
Nursing centers are nurse-managed organizations that give the client direct access to professional nursing services. Academic nursing centers are faculty-created and -organized nursing centers integrated into nursing school or cooperated with community nursing center. Academic nursing centers are needed for providing services to the forgotten or underserved populations in the community, providing learning opportunities for nursing students and practice opportunities for faculties. The intent of this study is to identify the elements needed in developing process and operations of acedemic nursing center for elderlies and the disabled, and to present the desired model for academic nursing center. The processes of my study were : 1) The articles of the academic nursing centers in U. S. were reviewed and analysed. 2) The academic nursing center for elderlies and the disabled was developed and operated in my paper. 3) Desired model for academic nursing center was presented in my paper. The followings are the results of my study: 1. Elements needed in developing process of academic nursing center were philosophy and goals, the community support, assessment of the validity of the service and health needs, identification of the service contents, roles and responsibilities, communication lines, finances for facilities and operations, cooperation with resources, and developing record system. 2. Elements needed in operations of academic nursing center were the structural organizations, realization of the above philosophy and goals, development of policy and nursing standards, faculty participation, continuity of services, and financial solutions. 3. The desired model was presented according to the process and operations.
Purpose: The aim of this study was to identify a conceptual link between role conflict, job satisfaction, and organizational commitment, with a specific examination on how job satisfaction mediated the relationship between role conflict and organizational commitment, among community healthcare workers. Methods: Participants were 119 community healthcare workers of the Seoul Preventive Project for Metabolic Syndrome at 15 community health centers. Descriptive analysis, correlations analysis, and multiple regression analysis were used for statistical analysis of our data. Additionally, Sobel's test was used to verify the significance of the mediating effect. Results: An inverse association between role conflict and organizational commitment was found (beta=-0.53, p<.001). This relationship was fully mediated by job satisfaction (z=-6.43, p<.001). Conclusion: Job satisfaction mediates the effects of role conflict on the organizational commitment of healthcare workers in community health centers. Strategies for improving the components of job satisfaction are needed to achieve better outcomes in organizational commitment in the Seoul Preventive Project for Metabolic Syndrome and possibly other work settings.

Purpose: The purpose of this study was to identify nursing interventions performed by public health nurses in health centers. Method: Data was collected by the taxonomy of Nursing Intervention Classification(NIC 3rd: 486 nursing interventions) from 131 public health nurses in health centers and analyzed using descriptive statistics. Result: As its result, more than 50% of public health nurses performed 137 nursing interventions at least monthly. The most frequently used intervention class was 'activity and exercise management', followed by 'physical comfort promotion', 'community health promotion', 'life span care', 'coping assistance', 'Self care facilitation', 'information management', 'nutrition support', 'community risk management' and 'patient education'. One hundred twenty nursing interventions were rarely performed by 90% or more of the nurses. Most of them were the physical complex domain. Conclusion: In conclusion, 137 interventions were performed by public health nurses at least monthly. NIC is helpful to build a standardized language for public health nursing.
