사스 격리전담 병원 직원들의 사스에 대한 이해와 태도 조사 (SARS quarantining hospital employees's knowledge about SARS and attitude to SARS control)

  • 연구배경 : 국내에서는 2003년의 사스 유행기에 사스 방역에서 만족할 만한 성과를 거두었으나, 유행이 다시 시작되거나, 다른 새로운 점염병의 가능성이 있다. 사스 전담 병원 중의 한 병원에서 직원들을 대상으로 시행한 사스에 대한 설문조사를 통해 병원 직원들의 사스에 대한 이해정도와 태도에 미치는 영향을 파악하여 향후의 유사한 상황에 대비한 기초자료를 얻고자 하였다. 방 법 : 강원대학교병원의 전 직원을 대상으로 인구 사회학적 사항, 사스에 대한 지식 측정, 진료 참여 의향, 사스의 사회적 파장에 대한 의견, 정보 취득에 대한 내용으로 설문조사를 시행하였다. 사스에 대한 지식 정도와 사스에 대한 태도와의 관련성, 인구 사회학적 특성과 사스에 대한 태도와의 관련성에 대해 분석하였다. 결 과 : 280명 중 251명의 직원이 응답하였고, 사스지식에 대한 평균점수는 $72.0{\pm}13.7$이었다. 사스전담 팀참여에 대해 50.6%에서 참여하겠다고 응답하였다. 본원이 격리전담을 담당하는데 반대하는 의견은 23.1%였다. 사스 지식에 대한 점수와 사스 관리에 대한 태도사이에는 관련성이 없었다. 교육에 참여한 사람과 참여하지 않은 사람들 사이에 지식 점수의 차이는 없었다. 교육 시 알고싶은 내용으로 사스환자 관리요령과 개인별 예방법에 대한 것이 가장 많았다. 결 론 : 대중 매체를 통한 사스환자 관리요령과 개인별 예방법에 대한 정확한 국가적 정보제공이 중요할 것으로 사료되며, 사스에 대한 지식수준과 태도와의 관련성이 없었다.

The anti-climacterium effects of red clover dry extracts combined with pomegranate concentration powder in ovariectomized rats

  • Objective : In this study, the addition of dried pomegranate concentrate powder (PCP) was affected the anti-climacterium activity of red clover dry extracts (RC) in ovariectomized (OVX) rats. Materials and methods : After bilateral OVX surgery, RC 40 mg/kg, PCP 20 mg/kg and RC:PCP 2:1 mixture (g/g) 120, 60 and 30 mg/kg (of body weight) were orally administered, once a day for 84 days, and then the changes on the serum estradiol levels, abdominal fat pad and uterus weights were observed for estrogenic effects. In addition, liver weights, serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels were also evaluated for hepatoprotective effects, and serum total cholesterol (TC), low density lipoprotein (LDL), high density lipoprotein (HDL) and triglyceride (TG) levels were monitored for hypolipidemic effects. Results : As a result of OVX, the estrogen-deficient climacterium symptoms, increments of abdominal fat pad weights, serum AST, ALT, TC, LDL and TG levels with decrease of uterus and liver weights, serum estradiol levels, were demonstrated. However, these estrogen-deficient climacterium symptoms induced by bilateral OVX in rats were significantly inhibited by continuous oral treatment of RC 40 mg/kg, PCP 20 mg/kg and RC:PCP 2:1 mixture (g/g) 120, 60 and 30 mg/kg, respectively. Conclusion : The results suggested that RC:PCP 2:1 mixtures synergistically increased the anti-climacterium effects of RC in OVX rats. It, therefore, is expected that RC:PCP 2:1 mixture will be promising as a new potent protective agents for relieving the climacterium symptoms.

의료보험(醫療保險) 의약품(醫藥品)의 요양취급기관(療養取扱機關) 종별(種別), 약효군별(藥效群別), 상병별(傷病別) 사용(使用)에 관(關)한 조사연구(調査硏究) (Study of the Amount and Share of Drug Cost in the Total Medical Fee under Medical Insurance Scheme)

  • To grasp the idea about how drugs are used under Medical Insurance Scheme, the amount and share of drug cost in the total medical fee have been reviewed and analyzed for different types of patients (in-patient out-patient), medical institutions and frequently seen diseases and following findings were revealed. In 1986, drug cost took 32.78% of total medical fee for in-patients and 32.98% for out-patients averaged over 30% share as a whole. When drug cost per case in 1980 be indexed to 100, it has shown steady growth to become 200 for in-patients and about 150 for out-patients in 1986. The contribution of drug cost to the total medical fee is, regardless of patient type-in-patients and out-patients, the highest in University hospitals and followed by General hospitals, Hospitals and Clinics in decending order That for the most frequent 10 diseases came out the highest,79 a with the essential benign hypertension of out-patients in the General hospitals, 61% for the gastric ulcer of out-patients in Hospitals and 33% for the female genital diseases of out-patients in Clinics. The drug cost of oral formula was contributed the most, 7.93% by cardiovascular agents followed by hepatic detoxicants(5.47%) and out-patients(4.93%), and that of injectable formula was contributed the most by antibiotics(24.17%), followed by protein amino-acid preparations(6.19%). The order of drug usage by specialty for the in-patients was the highest with internal medicine followed by general surgery and E.N.T, and that for the out-parients was in the order of Internal medicine, neuropsychology and Ob/Gy. This study revealed that the drug dependency was characteristically different to specialty. In view of the fact that drug cost on average exceeds over 30% of total medical fee, proper drug administration appears to be vitally important for the stabilization of the financial standing of the Medical Insurance Scheme. As a consequence, drug usage guidelines including antibiotics usage shall be established first of all and the voluntary participation for the regulation of drug usage and propagation of the guidelines to medical institutions are strongly coerced.

Burkhoderia sp. AK-17에 의한 잎들깨 재배의 생물학적 조절 (Biological Control of Perilla Culture by Burkhoderia sp. AK-17)

  • 시설원예단지의 재배환경은 고온다습으로 많은 병해가 발생하며, 이를 방제하기위해 과다한 농약을 사용하므로 농약 잔류성에 대한 피해가 심각한 우려를 낳고 있다. 미생물을 이용한 생물학적 방제법 개발로 청정 시설원예작물을 생산할 목적으로 길항균을 분리하였다. 잎들깨 재배토양과 식물체로부터 길항세균 18종을 분리하였으며 분리한 길항세균 중에 AK-17이 가장 활성이 뛰어나 이를 이용하여 주요 식물병원균에 대한 항균스펙트럼을 조사하였다. 그 결과 잿빛곰팡이병원균의 Botrytis cinerea와 균핵병원균의 Sclerotinia sclerotiorum 및 줄기썩음병원균인 Rhizoctonia solani에 대한 항균효과가 뛰어났다. 잎들깨의 주요 병에 대한 생물학적 방제실험은 병발생 억제효과와 병 방제효과를 pot 실험으로 실시했다. 그 결과 균핵병은 55%의 병발생 억제호과와 92%의 방제효과가 있었고, 잿빛곰팡이병은 40%의 병발생 억제효과와 78%의 방제효과를 확인할 수 있었다. AK-17의 식물 생육촉진효과는 신장이 120%, 생체중이 164% 증가되었으며, 엽수와 엽면적은 각각 120%와 220%의 증가효과를 보였다. 그리고 AK-17을 생리 생화학적방법과 유전학적 방법으로 동정한 결과, Burkhoderia sp.로 확인되었다.

프로바이오틱스의 면역조절을 통한 알레르기 예방 및 치료효과 (The Preventive and Therapeutic Effects of Probiotics in Allergic Diseases Via Immune Modulation)

  • 프로바이오틱스는 인체 내에서 정상 균총의 역할을 하며 건강에 도움을 주는 미생물을 의미하며, 식품이나 미생물총에서 유래한 비교적 안전한 균주이다. 프로바이오틱스로 많이 사용되는 젖산균은 소화를 돕고 장내환경을 보호해줄 뿐 아니라 면역조절기능 또한 가지고 있다. 대표적으로 $Th_1$ 반응을 유도하여 $Th_1/Th_2$ 균형을 이루게 하는 것을 중심으로, 알레르겐에 반응하는 IgE 및 침윤된 비만세포와 호산구를 감소시키고 면역 억제 기능이 있는 Treg을 유도함으로 알레르기 반응을 완화시킨다고 알려져 있다. 하지만 프로바이오틱스를 이용한 알레르기 질환에 대한 치료 및 예방 효과는 질병 별로 다르게 나타났다. 아토피 피부염에 대해서는 예방 및 치료 효과가 검증되고 있고 알레르기 비염과 음식물 알레르기에서는 예방 효과는 미미했지만 치료효과에서 긍정적이었다. 기관지 천식의 경우 프로바이오틱스의 예방 및 치료 효과를 거의 볼 수 없었지만, 최근에는 특정 균주가 기관지 천식 환자에서 임상 증상을 향상시킨다는 보고가 있다. 균주의 장내 생존율을 높이고 재조합 유산균을 만드는 백신 기술과 함께 프로바이오틱스는 미래의 안전한 알레르기 예방 및 치료제로서 기대할 수 있을 것이며 지속적인 시장의 확대를 통해 차세대 건강기능식품으로 떠오를 것이다.

21세기 초 한국의 인구구조 및 질병양상 (Perspective on Population Characteristics and Health Problems of Koreans in 21st Century)

  • In order to anticipate disease pattern and health problems of Koreans in the 1st part of 21st century (by the year 2020), transition of population characteristics, mortality and morbidity data during the last 30 years Koreans have experienced were reviewed. On the actual basis of epidemiologic transition process that has undergone during last 30 years since 1960 along with socioeconomic development and successful implementation of selective national health policies (family planning, medical insurance and etc.), following changes can be expected in the 21st century in Korea, under the assumption that the current rate of progress is maintained. The population of South Korea alone will be doubled the population of 1960 by the year 2013 : aged Population older than 65 years will be increased from 3.3% in 1960 to 11.4% in 2020 with increased average age of the population from 23.6 year in 1970 to 39.2 year in 2020; urban population from 28% in 1960 to 83% in 2005. GNP/capita has increased tremendously from U.S. $120 in 1970 to $6,749 in 1992, and the government estimated it would be 519,350 in 2010 and $29,460 in 2020. Growth and developmental indices of children, educational achievement and social status of women also showed a remarkable improvement and anticipated to make futher progress. Leading causes of mortality and morbidity have shown a striking change during the last 30 years, from infectious diseases to chronic degenerative diseases and man-made injuries. Occurrence of communicable diseases may become minimal although viral hepatitis, venereal diseases Including AIDS, and well adapted herpes virus infections will maintain their endemic level. Newly evolving infectious agents, however, should be carefully monitored because of rapidly changing environments and human behaviours. Tuberculosis may increase up to the epidemic level when AIDS prevails. Ischemic heart diseases may increase steadily with increasing occurrence of hypertension and diabetes mellitus whereas cerebrovascular diseases may be decreased slowly. Musculaskeletal diseases which contribute a lot to the disability of aged people may be a major health problems due to increased aged population. Mental diseases, particularly that caused by alcohol and drug abuse, and senile dementia may become a prominent health problem. On the other hand injuries caused by traffic and industrial accidents that have shown most striking increase till now may be decreased considerably by intensive intervention. The health policies in the 21st century will be oriented to the health promotion for good quality life rather than life-savings.

유산균을 이용한 대두 발효 추출물이 면역계 활성에 미치는 영향 (Effects of soybean extracts fermented with Lactic acid bacteria on immune system activity)

  • Objectives : NK cells are spontaneously cytotoxic lymphocytes. These are not only important parts in the first line of defence against bacterial and viral infections of outside, but they may also play a critical role in chronic viral diseases. NK cells kill their targets spontaneously, without the need for prior sensitization and class I MHC restriction by the regulation of cytolytic functions and secretion of a variety of cytokines, such as interleukin-12(IL-12), MCP-1, IL-6, TNF-${\alpha}$, IFN-${\gamma}$. In addition, macrophage and NK cells cooperate through the production of cell mediates. These cooperation and modulation are one of major factors to prevent for evading immune surveillance of cancer. Hence, it could be assumed that if any candidate to enhance activities of macrophage and NK cell, it is considered as a potentially useful agents against cancer. Methods : In our study, to investigate effect of fermented soybean extracts by Lactic acid bacteria (SFE, soybean fermented extracts) work on intestinal immune cell to maintain general immune modulating and anti-cancer activity. We analyzed NK cytotoxicity assay and gene expressions of cytokine related with macrophage and NK cell activity. Results : In vitro experiment, SFE was verified as safety material for cell toxicicty to tumor cell strain without any toxicity of tumor growth inhibition and various cell strain. Effects of macrophage activity stimulating directly by SFE measured induced cytokine. The studies showed that IL-12 production by stimulation of SFE depended on concentration from 0.16mg/mL to 0.63mg/mL with non toxicity to cell, and it was the best activity at 0.63mg/mL. Besides, the effective concentration of SFE producing TNF-${\alpha}$ is similar to IL-12, but it was the best activity at 1.25mg/mL. The level of MCP-1, IL-6 and IFN-${\gamma}$ depended on concentration from 0.16mg/mL to 10mg/mL, IFN-${\gamma}$ showed the best activity at the effective concentration of 0.63mg/mL. With the result of NK cell activity measurement, the spleen cell of mouse injected SFE had 1.5 times higher killing effect than non injected cell. Conclusions : The result of this studies is that Soybean fermetated extracts(SFE) has possibility to immune aided material for the function not only inhibition of microbial infection to macrophage but also activity of adaption immune and cellular immune system.

천마 괴경썩음병 방제에 효과적인 유기농업자재 선발 (Screening for Effective Organic Farming Materials for the Control of Tuber Rot of Gastrodia elata Caused by Fusarium spp.)

  • 본 연구는 Fusarium spp.에 의해 발생하는 천마 괴경썩음병에 대하여 유기농업자재의 방제효과를 검정하기 위해 수행되었다. 유기농업자재 15품목을 대상으로 실내에서 항균활성 검정과 실내시설에서 천마 자마에 포자현탁액 접종을 통한 자마검정을 실시하였다. 이중 미생물제인 7품목의 유기농업자재가 Fusarium spp.의 균사생장을 억제하였다. 천마 자마를 이용한 검정에서 3품목의 유기농업자재가 70% 이상의 우수한 방제효과를 나타냈다. 그 중에서 sulfur가 유효성분인 광물질 제제의 억제효과가 가장 뛰어났다. 그 다음으로 3품목의 유기농업자재를 선발하여 예방적 효과와 치료적 효과를 검정하기 위해 적용하였는데, 예방적 처리를 했을 때 3품목 모두 병 발생에 대한 억제 효과가 상당히 높았다. 특히 sulfur와 B. subtilis의 예방적 효과는 각각 93.2%, 86.9%로 탁월하였으며, 치료적 효과는 73.3%, 60.2%로 상대적으로 낮게 나타났다. 반면, citronella + paraffin oil + ethyl alcohol은 예방적 효과와 치료적 효과가 각각 73.3%, 67.0%로 비슷한 효과를 나타냈다. 이 연구를 통해 시설하우스 내 연작 재배지에서 잘 발생하는 천마 괴경썩음병은 유기농업자재로 충분히 방제될 수 있으며, 예방적 처리가 더욱 중요하며 효과적임이 확인되었다.

일개 도농복합지역 지역응급의료기관 환자의 급성 중독 실태 (Research of Acute Poisoning on a Local Emergency Medical Facility in an Urban-Rural Area)

  • Objectives: The aim of this study was to investigate the characteristics of acute poisoning cases admitted to a local emergency department in an urban-rural area. Methods: This retrospective study included all patients with acute poisoning who admitted the emergency department between June 2004 and May 2005. We analyzed the medical records for etiological and demographical characteristics of the acutely poisoned patients. Results: There were 155(1.0%) poisoning cases among 15,457 admitted to the emergency department. Of the admitted cases, 51.0% were female and 49.0% male. The poisoning made a peak in the age range of 30-39 and over 60. The seasonal distribution in poisoning patients suggested a peak in spring(38.1%) and summer(24.5%). The majority of exposures(95.5%) occurred in patient's home, 81.8% were deliberate. The most common agents of acute poisoning in deliberate causes were drugs(p<0.01). All of deliberate cases were suicidal attempts. The most common poisoning cases were pesticides in male, but in female were medications(p<0.01). Overall mortality rate of total cases were 12.9%. The overall mortality rate was higher in male(p<0.05), increasing age(p<0.05) and intentional ingestion(p<0.05), pesticides poisoning(p<0.05). Conclusions: The study forms a foundation for future work on poisoning epidemiology which hopefully will facilitate the allocation of preventive services in order to reduce the incidence as well as morbidity and mortality of toxic exposures.

Cedrela sinensis Leaves Suppress Oxidative Stress and Expressions of iNOS and COX-2 via MAPK Signaling Pathways in RAW 264.7 Cells

  • Overproduction of reactive oxygen species (ROS), including nitric oxide (NO), could be associated with the pathogenesis of various diseases such as cancer and chronic inflammation. Inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) are known to play key roles in the development of these diseases. Cedrela sinensis leaves have been used in Asian countries as a traditional remedy for enteritis, dysentery and itching. In the present study, we investigated the anti-inflammatory effects of Cedrela sinensis leaves in lipopolysaccharide (LPS)- stimulated RAW 264.7 macrophages. Powder of C. sinensis leaves was extracted with 95% ethanol and fractionated with a series of organic solvents including n-hexane, dichloromethane, ethyl acetate, n-butanol, and water. The dichloromethane (DCM) fraction strongly inhibited NO production possibly by down-regulating iNOS and COX-2 expression, as determined by Western blotting. Hydrogen peroxide-induced generation of reactive oxygen species (ROS) was also effectively inhibited by the DCM fraction from C. sinensis leaves. In addition, C. sinensis inhibited LPS-mediated p65 activation via the prevention of IκB-$\alpha$ phosphorylation. Furthermore, mitogen-activated protein kinases (MAPKs) such as ERK 1/2 and p38 were found to affect the expression of iNOS and COX-2 in the cells. Taken together, our data suggest that leaves of C. sinensis could be used as a potential source for anti-inflammatory agents.
