암 치료를 받은 간호사의 직장복귀 경험 (Return-to-Work Experiences among Nurses after Receiving Cancer Treatment)

  • 본 연구의 목적은 암 진단을 받아 치료를 받은 간호사가 경험하는 직장 복귀 경험에 대한 의미와 본질을 탐색하고자 암 치료를 받은 간호사 6명을 대상으로 2016년 1월 5일부터 1월 29일까지 자료를 수집하여 Colaizzi의 현상학적 연구방법을 적용한 질적 연구이다. 연구결과 암 진단을 받아 치료를 받은 간호사의 직장 복귀의 경험은 27개의 주제와 9개의 주제모음, 3개의 범주로 도출되었다. 최종 도출된 주제모음과 범주는 '힘들게 버텨나감'의 범주에서 '암 치료에 대한 불편함', '불안과 불확실성이 공존하는 생황', '암으로 인한 관계의 문제'의 3개의 주제모음이 도출되었으며, '삶의 전환점에서 중심잡고 살아가기'의 범주에서는 '성숙한 삶의 원동력이 됨', '강해짐', '내려놓음', '나의 필요에 민감해지기'의 4개의 주제모음이 도출되었다. 마지막으로 '새롭게 태어남'의 범주에서는 '암치료 경험이 간호의 도구가 됨', '직장에 대한 새로운 정의를 내림'의 2개의 주제모임이 도출되었다. 암 진단을 받아 치료 후 직장에 복귀한 간호사의 경험은 일과 치료의 병행에 대한 어려움을 호소하면서도 질병경험을 통해 더욱 성숙한 삶을 살아가고 있었으며, 일을 통하여 자신의 존재감을 경험하였다. 따라서 일과 치료의 병행을 위한 업무 환경의 개선과 지속적인 사회적 지지와 지원이 필요할 것으로 사료된다.

진료환경개선을 위한 우선적 전략과제 설정 및 그 적용 (A Strategic Quality Initiative and Its Opportunities to Improve Healthcare Environment)

  • Background : Strategic planning is an organizationwide or systemwide, ongoing look into the future usually of 2~3 years, based on objective analysis of the current environment and trends, but it can incorporate both short-term and long-term goals. The strategic planning process includes external analysis, internal analysis, issue analysis, development of mission, vision and values, and lastly development of organizational goals and objectives. As a part of the strategic quality planning process, certain service lines, important organizationwide functions, or key processes supporting these functions can be prioritized to expedite and roll out certain strategic goals. This is called strategic quality initiatives. Methods : We organized a quality improvement team, a subgroup of 21st century vision planning corps of our medical center, and pursued QI activities for improvement of healthcare environment, particularly in the admission setting. We developed a strategic quality initiative based on the results of patient satisfaction surveys, and carried out functions of self-directed work team. Results : The strategic goal was to be the benchmark for peer group hospitals in Korea for providing cost-effective best-practice. The QI team included 3 medical doctors, 1 nurse, 1 social worker, and 1 QI consultant as well as many operational members to support services and quality initiatives met every Tuesday for 18 weeks. Outcome objectives were to improve patient satisfaction score. The issues included in the objectives were comfort, temperature, noise, cleanliness of the admission wards, quality and education of patient meals, matters regarding the admission process, and an appurtenant facility such as restaurant or convenience store. Every issue was discussed and recommendations, conclusions and opportunities were implemented. Conclusions : By developing a strategic quality initiative as a part of the strategic quality planning process, and pursuing a self-directed work team, certain sen/ice lines, important organizationwide functions, or key processes supporting these functions can be improved effectively within a short period. Strategic quality initiatives serve to support, or roll out, certain strategic goals that are relevant to performance improvement and development of specific measurable outcome objectives, and associated performance measure for each initiative. Each strategic quality initiative should include a statement of intent outcome objectives, and performance measures. We will come back with follow up of the strategic quality initiative, for improvement of healthcare environment, and results of patient satisfaction re-survey.

수술실 간호사의 방사선 노출 경험: 현상학적 연구 (Paper Operration room nurses Experience in Using Radiation Suegery: Phenomenological Research)

  • 본 연구의 목적은 방사선 노출 경험을 참여자의 관점에서 확인함으로써 수술실 간호사의 방사선 노출 경험에 대한 구성요소를 확인하고 의미구조를 밝히는 데 있다. 연구의 참여자는 일 광역시 종합병원 수술실에서 근무하는 간호사로 수술실 경력이 6개월 이상인 자 9명을 대상으로 하였다. 심층면담으로 자료를 수집하였고, Giorgi(2004)의 현상학적 분석 방법으로 분석하였다. 연구 결과 수술실 간호사의 방사선 노출 경험은 "반복되는 방사선 노출로 질환 발생을 걱정함", "육체적, 정신적으로 소진됨", "방사선 방어를 통해 자기보호를 하고자 노력함", "내적 혼란과 갈등을 느낌", "근무환경 개선을 요구함", "현실을 받아들이고 적응함"의 6개 구성요소로 도출 되었다. 결론으로 이들의 경험은 방사선 노출 수술에 참여하면서 질환 발생을 걱정하고 방사선 피폭과 관련된 신체적, 정신적 소진을 경험하고 갈등을 느끼면서 이직을 고려하기도 하지만 자기보호를 하고자 노력하고 현실을 받아들이고 적응하는 과정을 보였다. 본 연구결과의 바탕으로 방사선 안전 환경조성을 위해서는 조직적 차원에서 체계적으로 구체적인 해결 방법을 모색하는 것이 바람직하다고 볼 수 있다.

가정간호실무에 적용가능한 이론적틀 (Appling Nursing Theory to Clinical Practice of Home Health Care)

  • The home health care industry has grown rapidly and can be expected to continue to grow in the foreseeable future. Home health care refers to the practice of nursing applied to clients with a health condition in the clients place of residence. clients and their designated care givers are the focus at home health nursing practice. The goal of care is to initiate. manage and evaluate the resources needed to promote the clients optimal level of well-being and function. Nursing activities necessary to achieve this goal may warrant preventive maintenance and restorative emphases to prevent potential problems from developing. Many project program were suggested home health care model for Korea's health care system and policy direction for expansion and establishment of home health care .But the aim of this paper is to provide on overview for theoretical frame work in home health care. Theories and conceptual frameworks or models are important nursing because they define and guide the boundaries of professional practice and identify key nurse-patient-caregiver relationships that emerge with caring. Following is the research with an investigation of the literature review in the University of Arizona international medline database, In conclusion, are as followers: First, many nursing theorists have had a tremendous impact on nursing practice. the following highlights those nursing theorists that are particularly helpful in understanding home health care. 1. Florence Nightingale : Our earliest theoretical legacy. Nightingale's believes are reflected in basic infection control practice such as hand washing and infectious waste disposal and are key nursing interventions in home care. 2. Martha Roger's :Science of unitary human beings theory. Rorger's believed that the focus of shared. non invasive healing modelities is the human environmental field rather than direct physical care. These modelities continue to evolve as our awareness (reflecting greater diversity, faster rhythms, motions, and ways of knowing) transcends time and space, allowing individuals to get in touch with their integral nature of unbroken wholeness. On people as ever changing energy fields have special relevance in home care especially with hospice and palliative care applications. 3. Madeline Leininger's; Transcultural nursing theory. Home care nurses move through a variety of communities and often care for patients from different cultural back grounds. Therefore Leininger's work has a good that with home care because home care nursing practice is very culturally focused. 4. Dorothea Orem's : Self care deficit theory. Orem's theory views care as something to be performed by both nurses and patients. The role of the nurse is to provide education and support that help patients acquire the necessary activities to perform self-care. Orem's theory is foundational to have care because it begins to truly acknowledge the role of the patient in managing his or her own health. which is referred to as self-care. 5. Margaret Neuman's; Health as expending consciousness theory. Neuman believes that health compasses disease and reflects an underlying pattern of person-environment interaction. A key application of 'Neuman's work to home care is for nurses to understand that health and illness do not necessarily exist at opposite ends of a continuum. 6. Jean Watson's: Theory of human caring. Watson's theory of human caring in nursing proposes human caring as the moral ideal of nursing. Nurses participate human caring to protect, enhance and preserve humanity by assisting individuals to fing meaning in illness. pain and existence and to help others gain self knowledge. self control. and self healing such thinking lends richness to theory development. as well as clinical practice in home care. Second, Robin Rice : Dynamic self determination for self care. (A theoretical framework for home care) Dynamical self determination for self care can be useful to home care nurses in a variety of ways. As research tool it can be reflected in the interview process when the home visit. The home care nurse's role is that of facilitator of patient self-determination for self care through numerous strategies. including patient education and case management.

가정전문간호사의 직무스트레스 측정 도구 개발 (Scale Development of Job Stress for Home Care Nurses)

  • Purpose: The purpose of this study was to develop a job stress scale for hospital-based home care nurses in Korea. The process was construction of the conceptual framework, development of the preliminary items, verification of the content validity, item analysis and test of the reliability. The preliminary items were based on literature review and in-depth interviews with home care nurses. As a result, eight categories and sixty items were selected. These were reviewed by seven specialists for content validity and finally fifty one items were chosen. Data was collected from 180 home care nurses who were engaged in 87 hospitals from August to September 2003. The result of item analysis one was excepted, The final item count was 50. Categories were as follows: overload work(8 items), lack of specialized knowledge and technique(5 items), ethical dilemma(4 items), role conflict(5 items), interpersonal relationships(6 items), visiting home environment(9 items), driving conditions(4 items) and lack of administrative support(9 items), The reliability of the scale by Cronbach's alpha was .948 and the domain's reliability ranged from .649 to .841. The result of this study could be used to measure the job stress of home care nurses. However, for further validity and reliability, repeated studies will be necessary.

간호사의 사회심리적 건강과 동료의 지지가 환자 돌봄 능력에 미치는 영향 (The Impacts of Nurses' Psycho-social Health and Social Support from Colleagues on Patient Caring Ability)

  • Purpose: The purpose of this study was to identify the effect of nurses' psycho-social health and social support from colleagues on patient caring ability. Methods: A cross-sectional survey design was used and the participants in this study were 422 nurses from 3 general hospitals. Data were collected using a self-administered questionnaire and collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple regression analysis with the SAS 9.4 program. Results: Total mean score for psychosocial health was $3.15{\pm}0.57$, for social support from colleagues, $3.43{\pm}0.55$, and for caring ability, $4.04{\pm}0.53$. Psychosocial health, social support from colleagues, and married status were identified as significant factors influencing patient caring ability. Conclusion: Findings from this study indicate that psychosocial health and social support from colleagues are related to patient caring ability, and thus, to improve nurses' caring ability for patient and for self, it is important to create a work environment that keeps nurses healthy and promotes collaboration with colleagues.

임상실습 경험 내러티브에 나타난 간호학생의 자리매김 (Analysis of Positioning in the Nursing Students' Narrative of the Experiences of Clinical Practice)

  • Purpose: The purpose of this study was to understand the positioning which was perceived by nursing students from their clinical practice experiences. Methods: The data were 80 narratives of practice experience written by 20 nursing students. Using qualitative contents analysis, nursing students' reflective narratives were analyzed in the aspects of the positioning which including personal characteristics, designated social role, and ethical order. Results: In personal characteristics, nursing students positioned themselves as they had vague fear and cold feet about the physical and personal environment of the clinical setting. In the aspects of the designated social role, they positioned themselves to learn more knowledge and skills of the nursing, to put effort in maintaining good relationship with their patients and other nurses, and to evaluate the nurses on their job performances and professional attitudes. In ethical order, nursing students positioned themselves as a person who worries about their work as a future professional nurse. Conclusion: The result of this study can be used to better understand nursing students and application of the nursing students' narratives in the clinical practice education, and to facilitate positive outcomes and transitions from nursing students to nurses.

요양병원 간호사의 역할갈등 경험 (Experience of the Role Conflict of Nurses in Long-term Care Hospitals)

  • 본 연구의 목적은 요양병원 간호사의 역할갈등 경험을 기술하기 위함이다. 연구 참여자는 요양병원에 근무 중인 간호사 9명이었으며, 자료 수집은 심층면담을 통해 이루어졌다. 수집된 자료는 Colaizzi의 현상학 연구 방법을 적용하여 분석하였다. 연구결과, 12개의 주제모음과 5개의 범주가 도출되었다. 5개의 범주는 '과중한 임무,' '불명확한 규정의 간호업무 위임,' '보호받지 못하는 의사영역의 업무수행,' '비체계적인 간호 보조 인력관리' 그리고 '업무능력의 한계'로 구성되었다. 본 연구결과는 한국의 보건의료 환경을 고려한 요양병원 간호사의 업무환경 개선과 직무만족도 향상을 위한 전략 개발의 근거를 제공함으로써 요양병원 간호사의 역할정립과 업무의 질적 향상에 기여할 것이다.

간호리더십에 대한 개념 분석 (Concept Analysis of Nursing Leadership)

  • Purpose: The purpose of this study was to provide a concept analysis of nursing leadership. Methods: The concept analysis process of Walker and Avant (2011) was used. A review of the literature was conducted using several databases, including PubMed, CINAHL, Ovid-Medline, DBpia, and KISS. The databases were searched using the following terms 'nursing leadership', 'nursing' & 'leadership', and 'nurse & 'leadership', and for studies published from January 2000 to March 2015. Strict inclusion and exclusion criteria were applied. Results: Analysis of these studies indicated nursing leadership attributes were individual growth, collaboration, nursing excellence, creative problem solving, and influence. The antecedents of nursing leadership were the individual maturity level, construction of mutual trust and confidential relationships, and a job requirement. The consequences were improvement in nursing productivity, development of a supportive work environment, and satisfaction with the nursing profession. Conclusion: The findings in this study provide a definite concept of nursing leadership. The findings also provide the theoretical evidence for the development of nursing leadership competencies in nursing students and professional nurses.

중소병원 간호사의 직장 내 약자 괴롭힘과 자아존중감 및 이직의도와의 관계 (Relationships of Workplace Bullying to Self-esteem and Turnover Intention among Clinical Nurses at Small and Medium-sized Hospitals)

  • Purpose: This study was to assess the prevalence of workplace bullying and the relationship workplace bullying to self-esteem and turnover intention among clinical nurses. Methods: The subjects were 223 clinical nurses at 3 small and medium-sized hospitals in K and B cities. The data were collected from February 1st throughout 28th, 2013. The collected data were analyzed using the SAS program through the frequency, percentage, mean, and Pearson's correlation coefficients. Results: The rate of self-reported bullying in the nursing workplace was 24.6% of respondents. There was a negative correlation between workplace bullying and self-esteem (r=-.39, p<.001). Also there was a positive correlation between workplace bullying and turnover intention (r=.14, p=.031). Conclusion: Prevalence of self-reported bullying is high among small and medium-sized hospitals and is clearly associated with lower levels of self-esteem and higher levels of turnover intention. Therefore, an organizational nursing management program as a monitoring regularly the psychosocial work environment and policies and procedures to prevent and manage workplace bullying is strongly recommended for the promotion of self-esteem and reducing the nurse's intention to leave the organization.
