The Effects of Health Promotion Initiatives with Community Participation and Parterships: The Case of Sabong-Myeon in Jinju City (지역사회참여와 파트너쉽에 기반을 둔 지역건강증진사업의 효과: 진주시 사봉면 사례)

  • Objectives: The objective of this study was to identify the effects of area-based health promotion initiatives with community participation and partnerships conducted in Sabong-Myeon, Jinju City for three years. Methods: We used data from the '2008 Sabong-Myeon Health Survey' and the '2011 Health Plus Happiness Plus Community Health Survey'. The study comprised 520 systematically sampled individuals, composed of 300 adults in 2008 and 200 adults in 2011. We compared some health behavior and mental health indicators as well as social capital levels between these two surveys. Results: The prevalences of smoking and walking exercise in 2011 were significantly higher than those in 2008. The prevalences of high risk alcohol consumption in men, stress perception, depressive symptoms experience, and suicide ideation were significantly lower than those in 2008. The proportion of people having a high level of social capital in 2011 was significantly higher than that in 2008, regardless of sex or marital status. However, the difference in the proportion was not statistically significant among people with a high educational background. Conclusions: This study suggested that the 3 years' implementation of area-based health promotion initiatives conducted in Sabong-Myeon might help to improve the mental health and social capital level of this community. The long-term health effects of area-based health promotion initiatives with community participation and partnerships need to be studied further, and lessons from these initiatives can be obtained by adapting proper evaluation methods.

Related Factors with Medication Task Ability in Rural Elderly (일부 농촌 노인에서의 약물복용 수행능력과 관련된 요인)

  • Medication non-compliance among the elderly results in medical problems and substantial cost to the health care system. This study investigate predicted variable related to the medication task ability among elderly. This study was done in the selected 4 villages in Kimchun County of Kyungbuk Province from July to August, 1996. The subject was the resident that 202 adults above 60 years of age. The questionnaire of interview included medication task ability, socio-demographic data, COOP/WONCA chart, family ABGAR score. BDI(Beck depression inventory), ADL(activities of daily living), IADL(instrumental activities of daily living), and MMSE-K(minimental state examination-Korean version). The results were as followed : 1. Approximately 49% of study population was taking drug medication currently. We found that 93% of study population was successful at the medication task all alone, 6% was failure at the medication task all alone, so need help partly or completely. 2. Significant variables between group of medication task ability were age, educational attainment, IADL, and MMSE-K in univariate analysis. And significant correlated variables with medication task ability were ADL, IADL, MMSE-K, and BDI in correlation analysis. 3. Major predictors to medication task ability on multiple logistic regression were IADL and sex finally. Findings suggest that IADL is related to medication task ability than other test battery of health status, so IADL could be used to necessary for medication management and add information to conventional methods of assessing mental status.

Depression and Related Factors of Children Using Community Child Center in Gwangju and Jeollanamdo (광주 및 전라남도 지역아동센터 이용 아동의 우울감 및 관련요인)

  • Objectives: This study examined the current status of depression and related factors among children using community child center. Methods: A cross-sectional study selected children in grades 4-6 who used the Gwangju and Jeollanamdo community child center (n=224) using a convenience sampling method. General characteristics, family characteristics, children's emotional characteristics, children's school life environment and depression status were assessed using a self-reported questionnaire. Results: The average score of depression among children using community child center was 15.31±7.70 out of a total of 27. Fifty-eight (25.9%) children had depression above 22 points. Variables related to children's depression were shown as grade, subjective economic level awareness, after-school activities excluding local children's centers, presence of family members after school, and family structure. Conclusions: The depression prevalence of children using community child center was higher. Policy support such as children's psychological support programs would be needed to reduce children's depression, and community child centers are expected to be effective in reducing children's depression if continuous child psychological support services are developed for children's mental health.

A Study on the Risk of Drug Use by Male Adolescents (남자 청소년의 약물사용 위험성에 관한 연구)

  • Purpose: The purpose was to identify the risk of drug use by adolescents. Method: The participants were 933 male students in the first grade of a high school in D city. The data were collected from Aug. 5th to Oct. 30th, 2004. The instrument was the High Risk Group Adolescent Drug User Screening Test(HIRIGADUST) developed by the Korea Adolescent Society(1996). The data were analyzed using SPSS. Results: For substance use, 64.5% of the students answered that they had drunk, 40.3% that they had smoked, and 2.0% that they had tried drug use. For scores on HIRIGADUST regarding socio-demographic characteristics, there were significant differences depending on school type, personality, academic performance, economic status, and ability to talk with parents. For scores on HIRIGADUST regarding drug using-related characteristics, there were significant differences depending on drinking experience, frequency of drinking, amount of alcohol intake, smoking experience and number of cigarettes smoked. Of the students 27.2% students were in the high risk group. Conclusion: In schools, systematic and intensive assessment of drug use should be done, and if needed, a service system connected to clinics specializing in drug addiction should be established. Prevention education should be carried out continuously.

The Relationship and Mechanism Underlying the Effect of Conscious Breathing on the Autonomic Nervous System and Brain Waves (의식적 호흡이 자율신경과 뇌파에 영향을 미치는 기전에 관하여)

  • Purpose: Breathing can be controlled either unconsciously or consciously. In Asian countries, various conscious breathing-control techniques have been practiced for many years to promote health and wellbeing. However, the exact mechanism underlying these techniques has not yet been established. The purpose of this study is to explore the physiological mechanism explaining how conscious breathing control could affect the autonomic nervous system, brain activity, and mental changes. Methods: The coupling phenomenon among breathing rhythm, heart rate variability, and brain waves was explored theoretically based on the research hypothesis and a review of the literature. Results: Respiratory sinus arrhythmia is a well-known phenomenon in which heart rate changes to become synchronized with breathing: inhalation increases heart rate and exhalation decreases it. HRV BFB training depends on conscious breathing control. During coherent sinusoidal heart rate changes, brain ${\alpha}$ waves could be enhanced. An increase in ${\alpha}$ waves was also found and the synchronicity between heart beat rhythm and brain wave became strengthened during meditation. Conclusion: In addition to the effect of emotion on breathing patterns, conscious breathing could change heart beat rhythms and brainwaves, and subsequently affect emotional status.

A Study on the Location and Area Composition of the Operation Department in Regional Public Hospitals - Focused on Net Floor Area (지방의료원 수술부 배치 및 규모에 관한 연구 - 순면적을 중심으로)

  • Purpose: To establish the building guidelines and to analyze the function of Regional Public Hospitals, the survey was conducted primarily on the current status focusing on the location and relationship of the department, area composition ratio, area per bed, and area per operating room. Methods: The research methods of this paper are as follows. 1) A literature survey on the function of the operating and related departments, 2) A drawing analysis for spatial composition and net area calculation. Results: The area of the Operation Department in the Regional Public Hospitals with 200 to 300 beds are as follows. Area ratio: 3.35%, area per Bed: 2.53㎡/bed, the number of beds per operating room: 64.37bed, area per operating room: 146.46㎡/n, area per operating room by plan type: integration corridor 133.84㎡/n, separation corridor 184.82㎡/n. Implications: This paper analyzed data on the current state of Operation Departments for setting up the function and role for the Regional Public Hospital. In the future, it is also required to provide size that takes into account user behavior and the user's psychological aspect in order to suggest appropriate area.

Development of an Algorithm for Emergency Nursing Care of Dyspneic Patients (호흡곤란 환자 응급간호 관리를 위한 알고리즘 개발 연구)

  • Purpose: This study was to develop an algorithm for emergency nursing care of dyspneic patients. Methods: This methodological study was done through reviews of medical records and literatures, checklists of emergency nursing care for dyspneic patients, interviews with nurses, and experts' validity. Results: Firstly, the initial assessment confirmed the identification of airway patency, accessory muscle usage, RR, $SpO_2$, v/s, skin color, and mental status. Immediate emergency care provided oxygen, checked ABG, EKG, and chest X-ray, established a semi-fowler position, maintained IV routes, administered medication orders, and conducted careful monitoring. Secondly, if the patient exhibited $SpO_2$ of less than 90%, the nurse considered the patient's condition to be aggravated. Thirdly, if the patient showed improvement of more than 90% $SpO_2$, the nurse administered secondary assessment and carried out specific nursing care. However, if the patient continuously showed $SpO_2$ of less than 80%, the nurse assisted the intubation and then executed ventilator therapy. Conclusions: This study suggests that the algorithm is an effective decision tool and utilizing the algorithm is expected to improve the emergency nursing care for dyspneic patients.

Cognitive Function, Physical Function, Problematic Behaviors of Elders using Dementia Daycare Service and Reasons for Stopping Daycare (치매주간보호센터 이용 노인의 인지·신체기능, 문제행동 및 이용중단 이유)

  • Purpose: Purpose of this retrospective study was to investigate cognitive function, physical function, and problematic behaviors of elders who attended dementia daycare centers, and to identify reasons why they stopped using the center. Methods: Participants were 176 elders, 60 years or over, attending one of four dementia daycare centers in Incheon. Data were collected from center documents. Results: Mean age was 80.5 years. When admitted to the centers mean scores for the mini-mental status examination, activity of daily living, and instrumental activity of daily living (IADL) were 12.31, 9.53, and 25.09 respectively. Participants received day care service for an average of 17.98 months. The reasons for leaving the center were worsening dementia and health (40.2%), and problematic behaviors (20.1%). Conclusion: Results show that elders began to use day care services when their cognitive function and IADL had declined considerably. As the ultimate goal of dementia daycare service is to delay the worsening of cognitive capability and decreases in activities daily living, the effect of the service can be maximized when the service is provided as early as possible in the course of progressively severe dementia. Active promotion should be exerted in the community to encourage early use of this service.

A Study of Self-esteem and Stress Coping Skills in Early Adolescence (초기 청소년의 자아존중감과 스트레스 대처방식)

  • Purpose: This study was conducted to investigate the relationship between self-esteem and coping skills among middle school students. Method: The participants were 674 students in three middle schools in GyeongGi-Do. The data were collected from June 10 to 20, 2005. The instruments used were the Rosenberg Self-esteem Scale and the Ways of Coping Checklist. The data were analyzed using the SPSS-Win 12.0 program. Result: Self-esteem showed a significant positive correlation with positive coping skills (r= .162. p= .000). As for the positive coping skills, there was a significant difference by grade (F= 4.69, p= .010), relationship with parent (F=4.036, p= .02), and relationship with siblings (F=3.902, p= .02). As for the negative coping skills, there was a significant difference by grade (F=3.05, p= .04), condition of living with parents (F=3.559, p= .02), economic status (F=3.736, p= .02), relationship with sibling (F=3.408, p= .03), and troubling matters (F=5.422, p= .005). Conclusion: Positive coping skills may be effective for adjustment because they positively related to self-esteem. Self-esteem and coping skills should be considered when an adolescent's mental health enhancement program is designed.

Social support and phychosocial distress among white-collar workers (직장인의 스트레스와 사회적 지지의 상관성에 관한 연구)

  • The effects and roles of social support and stressors (stressful life events and chronic strains) on psychosocial distress were examined in a model with data from a sample of the white-collar workers (N=416) in Korea. The findings of this study present that mean levels of psychosocial distress were higher in the female, youth, unmarried and divorced, and low economic status (monthly income). The analysis also shows that, as expected, stressful life and chronic strains as stressors are positively related to occurrence of psychosocial distress, and social support is negatively related to psychosocial distress. Further, the contribution of social support to predicting psychosocial distress is greater in magnitude than that of stressful events, chronic strains, and seciodemographic variables. There is no evidence that social support buffers the impacts of stressors on psychosocial distress, rather social support takes a role of mai or direct effects on psychosocial distress. Therefore, the strength of the social support is an important and meaningful strategy to prevent from stress and mental, physical illness.
