Effect of Dietary Fibers on Retinol Binding Protein (RBP, cRBP I, cRBP II) Gene Expression in Rats Fed High Fat Diet (고지방 식이내 식이섬유질이 흰쥐의 레티놀 결합 단백질 (RBP, cRBP I, cRBP II) 유전자 발현에 미치는 영향)

  • Recently it has been reported that vitamin A and retinol binding proteins (RBPs) in blood and urine were changed in the condition of diabetes mellitus or hyperlipidemia. Fruits and vegetables are recommended to consume for the people suffered from these chronic degenerative diseases. The main components of fruits and vegetables are dietary fibers, for example cellulose and pectin, of which function to affect the absorption and excretion of dietary fat and fat-soluble substances. This study was conducted to investigate the effect of dietary fibers on RBPs mRNA expression in liver, small intestine and serum of rat fed high fat diet during 4 weeks. Sprague-Dawley rats, weighing 121g on average, were divided into four groups; (Control; $17\%$ fat & cellulose supplement diet, HF0: $25\%$ fat & fiber free diet, B:.Uc: $25\%$ fat & cellulose supplement diet and HF0: $25\%$ fat & pectin supplement diet) . The rats fed high fat diet groups (HF0, HFC, HFP) tended to consume the food less than the control group, but FER of HF0 groups was significantly higher than the control (p < 0.05) . The weight of adrenal gland in high fat diet groups (HF0, HFC, HFP) was significantly less than the control. Total lipid in feces daily excreted and in liver did not show any significant differences among the groups. Total cholesterol in HFP group was significantly different from that of HFC group. Serum total cholesterol and triglyceride in other group tended to lower than other groups and HDL cholesterol higher. Consequently, AI (atherogenic index) was the lowest in HFP group. Vit A contents in feces daily excreted tended to lower in high fat diet groups (HF0, HFP) compared to the control group. That content in adrenal gland was the lowest in HF0 group, but not in liver. In HFP group were down-regulated cRBPI mRNA in liver and cRBPII mRNA in small intestine and up-regulated RBP and transthyretin expression in serum compared to the other groups. In conclusion, dietary fibers, especially pectin, in high fat diet might down-regulate the expression of CRBP I, CRBP II mRNA in liver and small intestine, but increase the secretion of RBP into serum and therefore inhance the bioavailability of Vit A through the body. Effects of Medical Nutrition Therapy on Changes of Anthropometric Measurements, Dietary Pattern and Blood Parameters in Over Weight or Obese Women (과체중 및 비만 여성에서 의학영양치료에 의한 체격지수의 변화 양상 및 식사 섭취와 혈액학적 지표의 변화)

  • The purpose of this study was to investigate the effect of medical nutrition therapy (MNT) on anthropometric measurements, quality of diets, and blood parameters through the weight loss program in over weight or obese women. The subjects (n=76, $age\;31.1{\pm}8.4y,\;weight\;70.3{\pm}7.9kg,\;BMI\;27.2{\pm}2.4kg/m^2$) without medical disease were treated with MNT 5 times for 12 weeks by dietitian. Anthropometric measurements included height, weight, fat mass, lean body mass, triceps and thigh skin-fold thickness, mid-arm, waist, hip and thigh circumference. Dietary assessments were accomplished using 3-days food records, diet quality index (DQI), and dietary pattern. The dietary pattern was assessed by daily energy intakes from breakfast, lunch, dinner, and snacks of calorie density, and numbers of eating. Blood levels of leptin, lipid profiles (total lipids, total cholesterol, LDL-cholesterol, HDL-cholesterol), and insulin were analyzed. Anthropometric measurements decreased significantly after MNT (p<0.001). Waist circumference was reduced most rapidly and tricep skin-fold thickness was reduced most effectively after 12 weeks of MNT. Daily energy intake, calorie density and numbers of eating incidence decreased significantly $0796.4{\pm}395.5\;vs\;1402.9{\pm}217.8\;kcal/day,\;162.3{\pm}56.5\;vs\;113.4{\pm}30.1,\;5.0{\pm}1.4\;vs\;3.8{\pm}0.4$, respectively, p<0.05). The daily energy intake from breakfast and snacks significantly decreased (p<0.001). Calories from snack, calorie density of snack and numbers of snacking also decreased (p<0.05). The mean scores of DQI significantly increased ($7.6{\pm}2.1\;vs\;6.5{\pm}1.6$, p<0.05). The plasma levels of total lipid, triglyceride (TG), total cholesterol, LDL-cholesterol and leptin decreased significantly after MNT ($489.8{\pm}100.4\;vs\;447.0{\pm}87.3mg/dL,\;187.7{\pm}34.0\;vs\;175.9{\pm}31.5mg/dL,\;115.2{\pm}29.2\;vs\;109.2{\pm}26.7mg/dL,\;15.7{\pm}7.6\;vs\;12.4{\pm}5.9ng/mL$, respectively, p<0.05). These results indicate that MNT efficiently reduced the body weight through the changes in dietary intake, dietary patterns and dietary quality. MNT also changed body composition and reduced the levels of plasma lipid, cholesterol and leptin.

Evaluation of Nutritional Safety for the Herbicide-Resistant Rice in Growing Male Rats (성장기 흰쥐에서 제초제 저항성 쌀의 급여에 대한 영양적 안전성 평가)

  • This study was conducted to evaluate the safety of herbicide-resistant rice, a genetically modified organism (GMO) developed by the Rural Development Administration, in Sprague-Dawley rats. The nutrient content of herbicide-resistant polished and brown cooked rice was compared with that of conventional Ilpum polished and brown cooked rice to assess composition equivalence. Compositional analysis was performed to measure proximates, fiber, and minerals before animal feeding. Growing male rats were fed one of the following four diets for six weeks: Ilpum polished cooked rice (IP) and Ilpum brown cooked rice (IB) as a non-GMO and herbicide-resistant polished cooked rice (GP) and brown cooked rice (GB) as a GMO. We checked clinical symptoms (anorexia, salivation, diarrhea, polyuria, anuria, fecal change) every day, food intake, change of body weight twice a week, and serum biochemistry and organ weights after 6 weeks of experimental feeding among the four groups. Nutrient content of the herbicide-resistant rice was similar to that of the non-transgenic control and was within the published range observed for non-transgenic rice. We could not find any significant difference in the above-mentioned items as the index to be checked in the animals fed the GMO. These results suggest that the nutrient content of genetically modified herbicide-resistant rice is compositionally equivalent to that of conventional Ilpum rice and that growing male rats fed herbicide-resistant rice are no different from those fed Ilpum rice, non-GMO for 6 weeks.

Contribution of Lateral Interbody Fusion in Staged Correction of Adult Degenerative Scoliosis

  • Objective : Lateral interbody fusion (LIF) is attractive as a less invasive technique to address anterior spinal pathology in the treatment of adult spinal deformity. Its own uses and benefits in treatment of adult degenerative scoliosis are undefined. To investigate the radiographic and clinical outcomes of LIF, and staged LIF and posterior spinal fusion (PSF) for the treatment of adult degenerative scoliosis patients, we analyzed radiographic and clinical outcomes of adult degenerative scoliosis patients who underwent LIF and posterior spinal fusion. Methods : Forty consecutive adult degenerative scoliosis patients who underwent LIF followed by staged PSF at a single institution were retrospectively reviewed. Long-standing 36" anterior-posterior and lateral radiographs were taken preoperatively, at inter-stage, 3 months, 1 year, and 2 years after surgery were reviewed. Outcomes were assessed through the visual analogue scale (VAS), 36-Item Short Form Health Survey (SF-36), and Oswestry Disability Index (ODI). Results : Forty patients with a mean age of 66.3 (range, 49-79) met inclusion criteria. A mean of 3.8 levels (range, 2-5) were fused using LIF, while a mean of 9.0 levels (range, 3-16) were fused during the posterior approach. The mean time between stages was 1.4 days (range, 1-6). The mean follow-up was 19.6 months. Lumbar lordosis was significantly restored from $36.4^{\circ}$ preoperatively up to $48.9^{\circ}$ (71.4% of total correction) after LIF and $53.9^{\circ}$ after PSF. Lumbar coronal Cobb was prominently improved from $38.6^{\circ}$ preoperatively to $24.1^{\circ}$ (55.8% of total correction) after LIF, $12.6^{\circ}$ after PSF respectively. The mean pelvic incidence-lumbar lordosis mismatch was markedly improved from $22.2^{\circ}$ preoperatively to $8.1^{\circ}$ (86.5% of total correction) after LIF, $5.9^{\circ}$ after PSF. Correction of coronal imbalance and sagittal vertebral axis did not reach significance. The rate of perioperative complication was 37.5%. Five patients underwent revision surgery due to wound infection. No major perioperative medical complications occurred. At last follow-up, there were significant improvements in VAS, SF-36 Physical Component Summary and ODI scores. Conclusion : LIF provides significant corrections in the coronal and sagittal plane in the patients with adult degenerative scoliosis. However, LIF combined with staged PSF provides more excellent radiographic and clinical outcomes, with reduced perioperative risk in the treatment of adult degenerative scoliosis.

Effects of macronutrients in mixed meals on postprandial glycemic response (식품 및 음식의 다량영양소 구성 성분에 따른 혈당 반응 연구)

  • Purpose: The aim of study was to determine the effects of carbohydrate, fat, protein, and fiber contents on glycemic responses in a single food item or meal. Methods: Glycemic responses were measured in 30 healthy young adults (17 males and 13 females) with various test foods, including rice, egg whites, bean sprouts, olive oil, noodles, prune, broccoli, Korean dishes, Western dishes, and salad dishes, etc. Test foods were designed to contain various carbohydrate, fat, protein, and fiber contents in single or mixed foods or dishes. After 12 hours of fasting, participants consumed test foods, and the glycemic response was measured for a subsequent 120 min (0, 15, 30, 60, 90, and 120 min). Three hundred and fifty three glycemic responses from 62 foods were collected. The incremental area under the curve (AUC) was calculated for each test food for each subject to examine glycemic responses. Statistical analysis was conducted to identify which macronutrient (carbohydrate, fat, protein and fiber) affected the AUC using a mixed model. Results: Carbohydrates (${\beta}=37.18$, p < 0.0001) significantly increased while fat (${\beta}=-32.70$, p = 0.0054) and fiber (${\beta}=-32.01$, p = 0.0486) significantly reduced the glycemic response. Conclusion: It can be concluded that the glycemic response of a meal can be modified depending on the fat and fiber contents of ingredient foods, even though carbohydrate content is maintained.

The effect of community based participatory obesity management program in obese school children (학령기 비만아동을 위한 지역사회 참여형 비만관리 프로그램의 효과)

  • The aim of this study was to develop a community based participatory obesity control program and measure the effects of the program on obesity, exercise habit, eating habit and life habits among obese elementary school children. The community based participatory obesity control program was provided for the experimental group of $4^{th}$ grade elementary school students for 10 weeks. The program included obesity related health educations and counseling once a week, jump rope classes twice a week and folk game classes held once a week. Method: There were 24 students with obesity in the experimental and 27 in the control groups. The collected data were analyzed by the Kolmogorov-Smirnov test, ${\chi}^2$-test, independent t-test and repeated measure ANOVA. Results: After 10weeks of participation in the program, the experimental group showed significantly lower Body Mass Index (p<.001) and waist hip ratio (p=.036), significantly higher exercise habit (p=.004), eating habit (p=.003) and life habit (p=.001) than those in the control group. Conclusion: This study provides evidence that community based participatory obesity management program is effective nursing intervention for managing child obesity.

Fatty liver associated with metabolic derangement in patients with chronic kidney disease: A controlled attenuation parameter study

  • Background: Hepatic steatosis measured with controlled attenuation parameter (CAP) using transient elastography predicts metabolic syndrome in the general population. We investigated whether CAP predicted metabolic syndrome in chronic kidney disease patients. Methods: CAP was measured with transient elastography in 465 predialysis chronic kidney disease patients (mean age, 57.5 years). Results: The median CAP value was 239 (202-274) dB/m. In 195 (41.9%) patients with metabolic syndrome, diabetes mellitus was more prevalent (105 [53.8%] vs. 71 [26.3%], P < 0.001), with significantly increased urine albumin-to-creatinine ratio (184 [38-706] vs. 56 [16-408] mg/g Cr, P = 0.003), high sensitivity C-reactive protein levels (5.4 [1.4-28.2] vs. 1.7 [0.6-9.9] mg/L, P < 0.001), and CAP (248 [210-302] vs. 226 [196-259] dB/m, P < 0.001). In multiple linear regression analysis, CAP was independently related to body mass index (${\beta}=0.742$, P < 0.001), triglyceride levels (${\beta}=2.034$, P < 0.001), estimated glomerular filtration rate (${\beta}=0.316$, P = 0.001), serum albumin (${\beta}=1.386$, P < 0.001), alanine aminotransferase (${\beta}=0.064$, P = 0.029), and total bilirubin (${\beta}=-0.881$, P = 0.009). In multiple logistic regression analysis, increased CAP was independently associated with increased metabolic syndrome risk (per 10 dB/m increase; odds ratio, 1.093; 95% confidence interval, 1.009-1.183; P = 0.029) even after adjusting for multiple confounding factors. Conclusion: Increased CAP measured with transient elastography significantly correlated with and could predict increased metabolic syndrome risk in chronic kidney disease patients.

Study on the Characteristics of Ordinary Symptoms in Overweight and Obesity Patients according to Sasang Constitution (사상체질에 따른 과체중 및 비만 환자의 소증(素證) 특성 비교)

  • Objectives: This study was aimed to find out the characteristics of ordinary symptoms between the control and patient groups, diagnosed as overweight or obesity, in Soyang, Taeeum, and Soeum Constitutions and among those 3 groups. Methods: As a prospective cross-sectional study based on medical records, 9213 patients, who visited Kyung Hee University Medical Center from May in 2007 to June in 2010, were included. To diagnose the constitution, Revised Questionnaire for the Sasang Constitution Classification II and examinations by oriental medical doctors who majored in Sasang Constitution Medicine were performed. Based on the World Health Organization criteria for Asian obesity assessment, body mass index (BMI) was classified into 3 groups; more than 25 as obesity, 23 to 25 as overweight, and less than 23 as normal. Ordinary symptoms questionnaire, which consists of 46 questions in 8 categories, were given to the subjects. One-way ANOVA test in continuous variables and chi-square test in categorical variables were carried out to analyze statistical significance. Results and Conclusions: 1) In general characteristics, we could find out the significant differences in age, sex, height, weight, BMI, waist circumference, blood pressure, total cholesterol, triglyceride, low density lipoprotein cholesterol, high density lipoprotein cholesterol, fasting blood sugar, and hemoglobin A1c among the 3 Constitutional groups. 2) Soyang Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 3) Taeeum Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 4) Soeum Constitution of overweight and obesity has tendency to have higher appetite and less fatigue in the ordinary symptoms, compared with the control group. 5) Soeum Constitution showed the slightest increase in appetite, preference about room temperature water, less number of feces, and feeling of chill and warmer extremities in ordinary symptoms, compared with Soyang and Taeeum Constitutions. Taeeum Constitution showed the distinct increase in appetite and sweating in ordinary symptoms, compared with Soyang and Soeum Constitutions.

Anti-inflammatory effects of mulberry twig extracts on dextran sulfate sodium-induced colitis mouse model (상지추출물이 Dextran Sulfate Sodium으로 유도된 대장염 마우스 모델에 미치는 항염증 효능)

  • Purpose: Ulcerative colitis is a common inflammatory bowel disease. Prolonged colitis can be a risk factor for the development of colorectal cancer. Mulberry twig (MT, Sangzhi), a dry branch of Morus alba L., which is widely distributed throughout East Asia, has been shown to have anti-inflammatory activities in the cells. However, the effects of MT extracts on colitis in in vivo are limited. Therefore, in this study, we investigated the anti-inflammatory effects of MT extracts in the dextran sulfate sodium (DSS)-induced mouse colitis model. Methods: Six week-old, male ICR mice were divided into 3 groups: Control (n = 5), DSS (n = 7), and DSS+MT (n = 7) groups. Mice in the DSS and DSS+MT groups were administrated 3% DSS in drinking water for 5 days to induce colitis. At the same time, water extracts of MT (5 g/kg body weight/day) were orally administered to mice in the DSS+MT groups for 5 days. Results: The MT extracts significantly reduced the clinical and pathological characteristics of colitis. Disease activity index, mucosal thickness, and colonocyte proliferation were significantly reduced in the DSS+MT group compared with the DSS group. Furthermore, MT administration reduced the levels of plasma $TNF-{\alpha}$, IL-6, and the colonic myeloperoxidase activity as well as mRNA expression of $TNF-{\alpha}$, IL-6, Cox-2, and iNOS. Conclusion: Taken together, these results suggest that MT water extracts have potent anti-colitis activities in the mouse colitis model.

Development and Practical Application of a Psychological Skill Training Program for National Wheelchair Curling Players -Frontal EEG Asymmetry (휠체어컬링 국가대표선수의 경기력 향상을 위한 심리기술훈련 프로그램의 개발 및 적용 -EEG 뇌파활용 연구)

  • The present study aimed to develop a sports psychological skill training program and to test its applicability to improve the performance of national wheelchair curling players. A total of 4 participants completed the study. Subjects have been to 12 sessions of 6 different psychological skills training programs (routine training, attentional focus training, writing practice training in diary, relation training, self-control training, and positive self-talk training). The effectiveness of psychological skills training has been evaluated with 6 questionnaires and EEG measurement. The result was that psychological skills training had improved coping with adversity, peaking under pressure, and concentration, self-management and reduced anxiety of Korean national wheelchair curling player. The result was that the EEG inter-hemispheric asymmetry index showed negative emotional states before psychological skill training but positive emotional states after. The result implies that psychological skill training plays an important mediating role in bringing about positive effects in the psychological elements and competitiveness in national wheelchair curling players.
