This paper try to find root-cause of failure in a connector used in transmitting signals for throttle body control in automotives by analyzing possible failure causes and performing experiments to simulate the cable failure in field. The connector comprises fins, wires, and case moldings. The failure is due to degradation of initial clamping force required fixing fins and wires in the connector. Expansion and compression of the case molding material surrounding fins would cause the degradation. Investigations of strict initial claming force and control of thermal expansion property of the molding are required to prevent the failure.
This paper suggests a weibull time delay model to evaluate failure risks in FMEA(failure modes and effects analysis). Assuming three types of loss functions for delayed time in failure cause detection, the risk of each failure cause is evaluated as its occurring frequency and expected loss. Since the closed form solution of the risk metric cannot be obtained, a statistical computer software R program is used for numerical calculation. When the occurrence and detection times have a common shape parameter, though, some simple results of mathematical derivation are also available. As an enormous quantity of field data becomes available under recent progress of data acquisition system, the proposed risk metric will provide a more practical and reasonable tool for evaluating the risks of failure causes in FMEA.
In the calendar and the advertising catalog, the surface is usually coated by coating polypropylene film. The delamination failure of coating film depends on surface roughness and quality of the substrate paper. In this paper, the mechanisms of delamination failure between the coating film and the paper is investigated by using the root cause analysis as one of techniques of reliability evaluation. The papers used in failure analysis are three kind products made by two domestic and one foreign companies. It found that the main causes of delamination failure between the coating film and the paper were the creation of microvoids caused by shape of filler and their growth caused by contraction of paper.

This paper investigates a mathematical model of a system composed of two non-identical unit parallel system with common-cause failure, critical human error, non-critical human error, preventive maintenance and two type of repair, i.e. cheaper and costlier. This system goes for preventive maintenance at random epochs. We assume that the failure, repair and maintenance times are independent random variables. The failure rates, repair rates and preventive maintenance rate are constant for each unit. The system is analyzed by using the graphical evaluation and review technique (GERT) to obtain various related measures and we study the effect of the preventive maintenance preventive maintenance on the system performance. Certain important results have been derived as special cases. The plots for the mean time to system failure and the steady-state availability A(${\infty}$) of the system are drawn for different parametric values.
Power system needs to sustain high reliability due to its complexity and security. The reliability prediction method is usually based on independent failure. However, in practice, the Common Cause Failures(CCF) and Cascading failure occur to the facilities in power system as well as independent failures in many cases. The CCF and Cascading failure turn out the system collapse seriously in a wide range. Therefore to improve the reliability of the power system practically, it is required that the analysis is conducted by using the CCF and Cascading failure. This paper describes the CCF and Cascading failure modeling combined with independent failure. The incorporated model of independent failure, CCF and cascading failure is proposed and analyzed, and it is applied to the distribution power system in order to examine this method.
Failure analysis is necessary to clarify the root cause of a failure, predict the next time a failure may occur, and improve the performance and reliability of a system. However, it is not an easy task to analyze and interpret failure data, especially for complex systems. Usually, these data are represented using many attributes, and sometimes they are inconsistent and ambiguous. In this paper, we present a scalable approach for the analysis and interpretation of failure data of high-performance computing systems. The approach employs rough sets theory (RST) for this task. The application of RST to a large publicly available set of failure data highlights the main attributes responsible for the root cause of a failure. In addition, it is used to analyze other failure characteristics, such as time between failures, repair times, workload running on a failed node, and failure category. Experimental results show the scalability of the presented approach and its ability to reveal dependencies among different failure characteristics.
In the field of nuclear reactor safety study, common cause failures (CCFs) became significant contributors to system failure probability and core damage frequency in most Probabilistic risk assessments. However, it is hard to estimate the reliability of such a system, because of the dependency of components caused by CCFs. In order to analyze the system, we propose an analytic method that can find the parameters with lack of raw data. This study adopts the shock model in which the failure probability increases as the shock is cumulated. We use two-step Expectation and Maximization (EM) algorithm to find the unknown parameters. In order to verify the analysis result, we perform the simulation under same environment. This approach might be helpful to build the defensive strategy for the CCFs.
This paper statistically analyzes the time required for each failure cause and describes a diagnostic method for 159 reports of failure analysis of MV cables that occurred in the distribution system of KEPCO over the past 18 years. In addition, the manufacturer's failure rate compared to 100C-km was calculated using 381 cases of MV cable deterioration failure between 2008 and 2020. It is hoped that this paper will help those in charge of maintaining underground facilities at the business office to use the failure rate to prioritize facility diagnosis.
In recent years, the diminishing of operation and maintenance cost using advanced maintenance technology is attracting many companies' attention. Especially, the heavy machinery industry regards it as a crucial problem since a failure of heavy machinery requires high cost and long downtime. To improve the current maintenance process, the heavy machinery industry tries to develop a methodology to predict failure in advance and to find its causes using usage data. A better analysis of failure causes requires more data so that various kinds of sensor are attached to machines and abundant amount of product usage data is collected through the sensor network. However, the systemic analysis of the collected product usage data is still in its infant stage. Many previous works have focused on failure occurrence as statistical data for reliability analysis. There have been less works to apply product usage data into root cause analysis of product failure. The product usage data collected while failures occur should be considered failure cause analysis. To do this, this study proposes a methodology to apply product usage data into failure cause analysis. The proposed methodology in this study is composed of several steps to transform product usage into failure causes. Various statistical analysis combined with product usage data such as multinomial logistic regression, T-test, and so on are used for the root cause analysis. The proposed methodology is applied to field data coming from operated locomotive and the analysis result shows its effectiveness.
Failure, the research object, means situations when goals and their results are different, which has developed presenting various aspects from ancient times and modern times. Failure is a complex concept which essentially requires judgement, and also a relative concept which can be changed depending on how you set up time base and standard. There are four stages for failure study to have been developed from studying failure through historical facts to the approach of organizational theory and it has been studied by looking at the failure of an organization according to different categories such as a developmental stage, a type of business, a period and a course. Compared with success science, failure study has not been studied sufficiently and also the level of its analysis is low. Thus, since lessons of failure tend to repeat themselves instead of being didactically accumulated, there need more studies on this. This thesis identified the main cause of the failure through various studies regarding failure which have been conducted at home and abroad. What the main cause of the failure that more than three studies mentioned have in common was that it occurred because organizations concentrated on the inside without communicating with external environment. The key point of failure study is to analyze failure, utilize it as assets, and create a frame of failure management. This thesis focused on delivering fragmentary knowledge on failure study, but case studies regarding this subject should be done in the future.
