Introduction of evidence-based practical medicine through safety classification for herbal medicine(1) (한약의 안전성 등급화를 통한 근거중심실용의학적 연구(1) - Aristolochic acid 함유 한약재를 중심으로 -)

  • Objectives: Evidence-based medicine(EBM) advocates the use of up-to-date "best" scientific evidence from health care research as the basis for making medical decisions. EBM also has been applied to traditional Korean medicine(TKM), especially in the field of safety. Recently, the standard prescription for TKM by Korea Institute of Oriental Medicine was published based on toxic index from various toxicity tests. However, there are some limitations when the results from the study based on EBM are applied in clinics. To overcome these imitations, the term "evidence-based practical medicine" was developed and defined as clinically applicable results from the study based on EBM. And safety classification for TKM was suggested as an example of evidence-based practical medicine. Methods: For safety classification for TKM, the data for $LD_{50}$(50% lethal dose), which was transformed to theoretical $LD_1$(1% lethal dose), was analyzed as one of tools for EMB study and divided by maximum dose used in clinics. Results and Conclusions: As a result, human equivalent dose(HED)-based MOS(margin of safety) for korean traditional medicine was calculated and used for safety classification with 5 categories. These categories would be helpful for oriental medicine clinicians to decide the increase and decrease of dosage according to various factors such as patient's sensitivity, potential toxicity of herbal medicines, clinician's experience for better cure. Thus, this safety classification provides some evidences enough that evidence-based practical medicine should be not the same with EBM and defined differently from EBM.

Extension block and direct pinning methods for mallet fracture: A comparative study

  • Background Mallet fracture can easily occur during sports activities or in daily life; however, the principles and methods of treatment for such fractures remain debated. Among the surgical treatments, various methods of closed reduction have been proposed. We treated patients with the extension block method (EBM) and the direct pinning method (DPM), and then compared the results. We assessed differences in range of motion and measurements of finger movement after surgery. Methods A total of 41 patients who underwent surgery from August 2013 to September 2015 were evaluated retrospectively. Among them, 21 patients were treated with the EBM and 20 patients were treated with the DPM. We then compared extensor lag, range of motion, and outcomes according to Crawford's criteria between before surgery and at 6 to 8 months postoperatively. Results The postoperative extensor lag improvement was $4.28^{\circ}$ and $10.73^{\circ}$, and the postoperative arc of motion was $55.76^{\circ}$ and $61.17^{\circ}$ in the EBM and DPM groups, respectively. The Crawford assessment showed no statistically significant difference between the groups, although the score in the DPM group was higher than that in the EBM group (3.5 vs. 3.1). Conclusions As closed reduction methods for the treatment of mallet fracture, both the EBM and DPM showed good results. However, the DPM proved to be superior to the EBM in that it produced greater improvements in extensor lag and range of motion.

Systematic Studies on Yijin-tang(Erchen-tang) for Establishment of Evidence Based Medicine (표준한방처방 EBM 구축을 위한 이진탕 문헌 연구 분석)

  • Objectives : The purpose of this report was to provide the information activity and safety of Yijin-tang by analyzing domestic/international papers and theses about Yijin-tang. Methods : Domestic/international papers and theses related to Yijin-tang were reviewed and analyzed. These papers were then classified by year, experimental method and subject. Results : 1. Administration of Yijin-tang showed therapeutic effects on diseases of the circulatory system such as thrombosis and hyperlipisemia in in vivo experiments. 2. Administration of Yijin-tang reduced hemorrhagic erosion and ulcer by inhibition of gastric juice secretion and protection effect of gastric mucosa in in vivo experiments. 3. Both clinical and basic science researches, Administration of Yijin-tang showed improvement of digestive function by up-regulated of digestive enzyme and gut-regulated protein in plasma levels. Conclusions : Administration of Yijin-tang showed therapeutic effects on diseases of the circulatory system and protection effect of gastric mucosa in in vivo experiments. Furthermore, both clinical and basic science researches, administration of Yijin-tang showed improvement of digestive function.

Proliferation and Functional Activity of Human Adipose Tissue-Derived CD146 Positive Endothelial Cells According to Culture Mediums (배지 성분에 따른 인간 지방조직기원 CD146 양성 혈관내피세포의 증식 및 기능의 평가)

  • Purpose: This study was to examine the proliferation and function of the adipose tissue-derived endothelial cells according to different culture medium conditions. Materials and Methods: Adipose tissue-derived CD146 positive endothelial cells were cultured in according to different culture mediums (DMEM culture medium with or without osteogenic inductive agents and EBM-2 culture medium with or without osteogenic inductive agents). The proliferation and function of the adipose tissue-derived endothelial cells was examined in different culture medium conditions. Results: Adipose tissue-derived endothelial cells formed tube-like structures on Matrigel in EBM-2 culture medium with or without osteogenic inductive agents. However, the cells did not form tube-like structures on Matrigel in DMEM medium with or without osteogenic inductive agents. After 24 hours of culture, among the culture medium using EBM-2, the proliferation of the cells were promoted in EBM-2 medium without osteogenic inductive agents than in EBM-2 medium with osteogenic inductive agents. However, 72 hours of culture, the proliferation of the cells were promoted in EBM-2 medium with osteogenic inductive agents than in EBM-2 medium without osteogenic inductive agents. Conclusion: These results suggest that the proliferation and function of the adipose tissue-derived CD146 positive endothelial cells could be maintained in EBM-2 with osteogenic inductive agents.

Stimulation of the Immune Response by Herbal Formulas for Wind-Cold and Heat Pain Symptom (대표적인 풍한열(風寒熱)에 의한 통증 치료 처방의 면역 활성화 비교 연구)

  • Three herbal formulas (Bangpungtongsung-san, Ohyaksungi-san, and Ojeok-san) for wind-cold and heat pain symptom were applied to investigate the immunological activities on antigen (Ag)-specific or Ag-non-specific immune responses in murine macrophage cell line (RAW 264.7) and ovalbumin (OVA)-immunized mice. This study was carried out in nitric oxide (NO) synthesis in RAW 264.7 cells and cellular proliferation in mouse splenocytes according to three herbal formulas. C57BL/6 mice were immunized intraperitonially with OVA/aluminium ($100\;{\mu}g/200\;{\mu}g$/mouse) on day 1, 8, and 15. Three herbal formulas were administrated to mice orally for 3 weeks from day 1. On day 22, OVA-, lipopolysaccharide (LPS)-, and concanavalin A (Con A)-stimulated splenocyte proliferation and antibodies (OVA-specific antibodies of the IgG, lgG1, and total IgM classes) in plasma were measured. Ohyaksungi-san increased NO synthesis in RAW 264.7 cells. Ojeok-san and Ohyaksungi-san significantly enhanced cellular proliferation by LPS and Con A in splenocytes from OVA-immunized mice (p<0.001). Three herbal formulas for wind-cold and heat pain symptom also significantly enhanced plasma OVA-specific IgG, IgG1, and total IgM levels compared with the OVA/Alum group. These results suggested that three herbal formulas for wind-cold and heat pain symptom could be used as stimulator of immune response.

Comparative Study of 25 Herbal Formulas on Anti-Inflammatory Effect (한약 처방 25종에 대한 항염증 효능 비교 연구)

  • Purpose: To provide the information of efficacy for herbal formulas of high frequency, it was evaluated the anti-inflammatory effect. In many studies, plantderived anti-inflammatory efficacies have been investigated for their potential inhibitory effects on lipopolysaccharide (LPS)-stimulated macrophages. This study was performed to examine the anti-inflammatory effects of herbal formulas of high frequency on LPS-stimulated RAW 264.7 cells. Methods: Anti-inflammatory activity was investigated in 25 herbal formula extracts in vitro and in vivo. To investigate the anti-inflammatory effect in vitro model, using LPS-stimulated macrophages, RAW 264.7 cell line. The productions of nitric oxide(NO), prostaglandin(PG)$E_2$, interleukin(IL)-6 and tumor necrosis factor(TNF)-$\alpha$ were examined in RAW 264.7 cells, in the presence of the herbal formulas. RAW 264.7 cells were incubated with LPS $1\;{\mu}g/mL$ and herbal formulas for 18 hours. As an in vivo, using a rat model of carrageenin-induced paw edema. The paw volume was measured at 2 and 4 hours following carrageenin-induced paw edema in rats. Results: 8 kinds of herbal formula inhibited NO production by LPS-stimulated in some concentration, but the effect of NO inhibition is weak. 12 kinds of herbal formula inhibited $PGE_2$ production by LPS-stimulated over the 30%. Among them Gumiganghwal-tang, Sagunja-tang, Samchulkunbi-tang, Insampaedok-san and Hwangryunhaedok-tang inhibited IL-6 production by LPS-stimulated but TNF-$\alpha$ was not inhibited. 12 kinds of herbal formula reduced the carrageenin-induced paw edema in rats. Particularly, 3 kinds of herbal formula(Gumiganghwal-tang, Ssanghwa-tang and Soshiho-tang) were better than indomethacin. Conclusion: These results suggest that Gumiganghwal-tang, Sangunja-tang, Samchulkunbi-tang, Insampaedok-san and Hwangryunhaedok-tang have antiinflammatory activity.

Safety on Hazardous Substances of Soeumin Kwakhyangjeonggi-san (소음인(少陰人) 곽향정기산(藿香正氣散)의 위해물질에 대한 안전성 연구)

  • 1. Objective: To compare the contents and transfer rate of hazardous substances in crude, washing solution, crude after washing, decoction and remnant after boiling. 2. Methods: The heavy metal contents of each step were measured by inductively coupled plasma mass spectrometer (ICP-MS) and mercury analyzer (MA-2). In order to analyze pesticides in each sample we used simultaneous multi-residue analysis of pesticides by GC/ECD, which was followed by GC/MSD analysis to confirm the identity of the detected pesticide in each sample. In addition, the contents of sulfur dioxide (SO2) were performed by Monier-Williams distillation method. 3. Results: 1) Contents (mg/kg) of heavy metals in decoction of all herbal medicine prescriptions were not detected. 2) Transfer rates (%) of heavy metals from crude to remnant were As (83.3%), Cd (100.0%), Pb (182.6%) and Hg (100.0%). 3) Contents (mg/kg) of residual pesticides were not detected. 4) Transfer rate (%) of sulfur dioxide (SO2) from crude to remnant was 44.2%. 4. Conclusion: Our results showed that boiled herbal medicine prescriptions which we take is safe from the hazardous substances.

Simultaneous Determination of Seven Compounds in Samsoeum by HPLC-PDA

  • Objectives : To develop and validate HPLC-PDA methods for simultaneous determination of seven constituents in Samsoeum(SSE). Methods : Reverse-phase chromatography using a Gemini C18 column operating at $40^{\circ}C$, and photodiode array(PDA) detection at 254 and 280 nm, were used for quantification of the seven marker components of SSE. The mobile phase using a gradient flow consisted of two solvent systems. Solvent A was 1.0% (v/v) aqueous acetic acid and solvent B was acetonitrile with 1.0% (v/v) acetic acid. Results : Calibration curves were acquired with $r^2$>0.9997, and the relative standard deviation (RSD) values (%) for intra- and inter-day precision were less than 3.0%. The recovery rate of each compound was in the range of 100.07-112.65%, with an RSD less than 4.0%. The contents of seven compounds in SSE were 1.24-10.53 mg/g. Conclusions : The established method will be helpful to improve quality control of SSE.

Contents of Poncirin and Naringin in Fruit of Poncirus Trifoliata according to Different Harvesting Times and Locations for Two Years (2년 동안 지실의 채취시기 및 지역에 따른 Poncirin과 Naringin의 함량)

  • A high performance liquid chromatography (HPLC) method was developed for simultaneous determination of two major flavonoid glycosides (poncirin and nanringin) in Poncirus trifoliata Raf. by different harvesting times and locations for two years. A SunFire $C_{18}$ column (4.6 mm${\times}$250 mm, 5 ${\mu}M$) was used at $40^{\circ}C$ for the determination of poncirin and naringin. The mobile phase using gradient flow consisted of two solvent systems. Solvent A was 1.0% (v/v) aqueous acetic acid and solvent B was acetonitrile with 1.0% (v/v) acetic acid. Flow rate was 1.0 mL/min and injection volume was 10 ${\mu}l$. The chromatogram was monitored by photodiode array (PDA) detection at 280 nm for the identification of two flavonoid glycosides in P. trifoliata. The contents of the two components in P. trifoliata ranged from 0.32~13.02%.

Systematic Studies on Galgeun-tang(gegen-tang, kakkon-to) for Establishment of Evidence Based Medicine (표준한방처방의 EBM 구축을 위한 갈근탕(葛根湯) 문헌 분석 연구)

  • Objectives : The purpose of this report was to provide the information of activity and safety of galgeun-tang by analyzing domestic/international papers and theses about galgeun-tang. Methods : Domestic/international papers and theses related to galgeun-tang were reviewed and analyzed. These papers were then classified by efficacy, or clinical trials. Results : The basic pharmacological experiment showed antipyretic, analgesic and anti-virus and anti-oxidant efficacy of galgeun-tang. In the case report of galgeun-tang, it showed therapeutic effect for patient with chronic rhinitis. But administration of galgeun-tang induced pruritic eruption in the two case report as a side effects of galgeun-tang. Conclutions : galgeun-tang showed tantipyretic, analgesic and anti-virus and anti-oxidant efficacy in the basic pharmacological experiment. Also, galgeun-tang showed therapeutic effect for patient with chronic rhinitis. But it was reported that galgeun-tang induced pruritic eruption in two case report, so physicians should be aware of the potential side effects.
