A Study about Compassion Fatigue, Compassion Satisfaction and Burnout in Comprehensive Nursing Care and General Ward Nurses (간호·간병통합서비스 병동과 일반병동 간호사의 공감피로, 공감만족, 소진 비교)

  • Purpose: The purpose of this study was to compare the differences in compassion fatigue, compassion satisfaction and burnout between nurses working at comprehensive nursing care unit and general ward. Methods: The subjects were 35 nurses in general ward and 42 nurses in the Comprehensive Nursing Care unit in one hospital. Measurement instrument included the Stamm's professional quality of life (ProQOL) version 5-Korean. Descriptive statistics, t-test, paired t-test, ANCOVA, and Pearson correlation were used to analyze the data. Results: Nurses working at the Comprehensive nursing care unit demonstrated significantly lower compassion fatigue (F=17.00, p<.001), higher compassion satisfaction (F=14.39, p<.001), and lower levels of burnout (F=40.07, p<.001) than control group. Conclusion: Compassion fatigue and burnout were lower and compassion satisfaction was higher among nurses working at comprehensive nursing unit than general ward. In order to improve quality of the comprehensive nursing care services, there is a need to be concerned with the nurse's compassion fatigue, compassion satisfaction, and burnout.

Factors Influencing Depression of Nurses among Comprehensive Nursing Care Service Ward (간호·간병통합서비스 병동 간호사의 우울 예측 요인)

  • Purpose: The purpose of this study was to identify depression and its influencing factor of nurses among comprehensive nursing care service ward. Methods: The research was cross-sectional descriptive study. The subjects of the study were 173 nurses of comprehensive nursing care service ward in Gyeonggi-do. Data collection was done using self-reported structured questionnaires asking about job stress, emotional labor, job burnout, somatic symptoms, turnover intention, depression and general characteristics from May 16 to June 3, 2016. Data were analyzed with SPSS/WIN 19.0 program. Results: Mean scores of job stress were 159.15(range 43~215), emotional labor 30.90(range: 9~45), job burnout 59.45 (range: 0~132), somatic symptoms 24.03 (range: 12~60), turnover intention 5.61 (range 0~18) and depression 19.25 (range: 0~60) and reported as depression in 63.6%. Somatic symptoms (${\beta}=.26$, p<.001), job burnout (${\beta}=.37$, p<.001) had significant associations with depression and the most important variable was job burnout. Conclusion: It is necessary to apply the developed program to reduce job burnout in hospitals and to prevent and/or control depression. And also, it needs to improve working conditions and increase the nursing staff to reduce somatic symptoms for nurses among comprehensive nursing care service ward.

Effect of Role Conflict on Job Satisfaction by Comprehensive Nursing Care Service Ward Nurse: Moderating and Mediating Effect of Resilience (간호·간병통합서비스 병동간호사의 역할갈등이 직무만족에 미치는 영향: 회복탄력성의 조절 및 매개효과)

  • Purpose : This study aimed to identify the moderating and mediating effects of resilience on the relationship between role conflict and job satisfaction among nurses working in a comprehensive nursing care service ward. Methods : Participants were 130 comprehensive nursing care service ward nurses from four hospitals. Data were collected using structured questionnaires and analyzed by t-test, ANOVA, Scheffe test, Pearson correlation coefficients, and hierarchical multiple regression analysis, using SPSS 25.0. The mediating effect of resilience on the relationship between role conflict and job satisfaction was analyzed using multiple regression analysis according to Baron and Kenny's procedure. Results : Statistically significant negative correlations were found between role conflict and resilience (r=-.25, p<.007) and between role conflict and job satisfaction (r=-.38, p<.001). A significant positive correlation was found between resilience and job satisfaction (r=.54, p<.001). A partial mediating effect of resilience was found between role conflict and job satisfaction (Z=-2.53, p=.012) but no moderating effect was found. Conclusion : To improve the job satisfaction of comprehensive nursing care service ward nurses, interventions for alleviating role conflict and improving resilience as well as evaluations of the two variables are necessary.

Comparative Study for the Clinical Nurses' Work Satisfaction, Job Stress and Turnover Intention - in General ward, Comprehensive nursing care service ward, and Anshim unit (일반병동, 포괄간호서비스병동, 안심병동 간호사의 직무만족도, 업무스트레스, 이직의도 비교연구)

  • The purpose of this study is to compare with work satisfaction, job stress and turnover intention of nurses who work in Comprehensive nursing care service ward, Anshim unit and general ward. The subjects are nurses of general ward, Anshim unit, Comprehensive nursing care service ward from two hospitals in Seoul and one hospital located in Gyeongi province. The summary of the research is as follows. Nurses in a general ward, a Comprehensive nursing care service ward and an Anshim unit don't have meaningful difference in job satisfaction(F=0.50, p=.602), job stress(F=1.12, p=.326), turnover intention.(F=0.02, p=.972). There were some statistic differences of Work satisfaction, job stress and turnover intention according to general characteristics. Henceforth it needs diverse consideration on these factors to reduce turnover intention of nurses.

  • The purpose of this study was to examine the effect of emotional labor, resilience, nursing work environment on turnover intentions of comprehensive nursing care medical service ward nurses. The data were collected from 186 subjects using a structured self-report questionnaires and analyzed using SPSS WIN Version 22.0. The mean score of emotional labor, resilience, nursing work environment, and turnover intentions were 3.66, 3.52, 2.62, and 3.82. Turnover intentions had significant correlation with emotional labor, resilience and nursing work environment. The influencing factors on turnover intentions were sex, emotional labor, resilience and nursing work environment of comprehensive nursing care medical service ward nurses. These result indicates that in order to reduce the turnover intentions of comprehensive nursing care medical service ward nurses, it is necessary to develop and apply an intervention programs considering sex, emotional labor, resilience, and nursing work environment.

Comparing the Emotional Labor, Job Stress, and Nursing Performance of Nurses in the Comprehensive nursing care Ward and the General Ward (간호·간병통합서비스병동과 일반병동간호사의 감정노동, 직무스트레스 및 간호업무수행)

  • Purpose: This study aimed to improve nursing practice efficiency by analyzing and comparing the emotional labor, job stress, and performance of nurses in comprehensive nursing care wards and in general wards. Methods: The study surveyed 151 nurses in comprehensive nursing care and general wards. The results were analyzed by ${\chi}$ 2test, ANOVA, and Pearson's correlation coefficient. Results: There were no statistically significant differences in terms of emotional labor, job stress, and performance for the two different types of nurses. Statistically significant positive correlations were noted between job stress and emotional labor, nursing performance, emotional labor, and job stress in the comprehensive nursing care wards, and between job stress and emotional labor, and nursing performance in the general wards. Conclusion: A longer duration of direct nursing in comprehensive nursing care wards tended to increase, emotional labor and job stress. However, the quality of nursing provided to patients tended to improve. Therefore, it is necessary to establish an institutional plan for carrying out efficient nursing while reducing emotional labor and job stress among nurses in comprehensive nursing care wards.

Effects of the Comprehensive Nursing Care Service Ward Nurses Occupational Stress on Safety Control and Patient Safety-related Nursing Activities (간호·간병 통합서비스 병동 간호사의 직무 스트레스가 안전통제감과 환자안전관리 활동에 미치는 영향)

  • This study is a descriptive research study to identify the relevance of the occupational stress, safety control, and patient safety-related nursing activities of the nurses working in comprehensive nursing care service ward, and to check the effect of the occupational stress on safety control and patient safety-related nursing activities. From March 11 to April 17, 2019, 137 nurses who worked in comprehensive nursing care service ward at four general hospitals were studied. The results of this study suggest that occupational stress has a negative correlation with safety control and patient safety-related nursing activities, and occupational stress negatively affects safety control and patient safety-related nursing activities. Based on the results of this study, it is expected that the work environment will be improved to reduce the occupational stress in order to promote the safety control and patient safety-related nursing activities of nurses in comprehensive nursing care service ward, and the development of various programs for occupational stress management will be necessary.

  • The purpose of this study is to provide basic data for effective human resource management by clarifying the relationship among ward operation support system of ward nurse, nursing performance, job satisfaction, burnout and turnover intention. The subjects of this study were 137 nurses working at comprehensive nursing care service wards of less than 500 beds in B and C city. Data were collected using structured questionnaires, and analysis was conducted using SPSS/WIN 21.0 and AMOS 18.0 programs. As a result of the path analysis, variables directly affecting the turnover intention of nurses working at the comprehensive nursing care service wards are ward operation support system and burnout, and indirect influences are nursing performance and job satisfaction. Burnout has the most direct effect on turnover intention, which is directly influenced by job satisfaction and nursing performance. Ward operation support system was confirmed to have a direct effect on job satisfaction. Therefore, in order to reduce the turnover intention of nurses working at the comprehensive nursing care service wards, strengthening ward operation support system in order to improve job satisfaction and measures to reduce the burnout and burden of nursing performance are needed.

The Emotional Labor Status and Improvement Plans of Nurses Working in the Integrated Nursing Service Ward: Applying Focus Group Interviews (간호⋅간병통합서비스 병동 간호사의 감정노동 파악 및 개선방안: 초점집단인터뷰 적용)

  • Purpose: The purpose of this study was to investigate the status of emotional labor of nursing personnel working in comprehensive nursing service ward and to suggest the way service improvement can be achieved. Methods: A total of 28 nurses working in comprehensive nursing service ward were divided into four groups to conduct focus group interview. All interviews were recorded and transcribed after the interview to perform data analysis in the order of data classification, topic categorization, and keyword derivation. Results: The five categories of subjects and relating keywords drawn from the focus group interviews are as follows: 1) Emotional labor experience: suppressing emotions, expressing emotions or actions that are different from reality, 2) Situations of emotional labor: verbal abuse and assault, sexual harassment, personal needs and errands, 3) Responses to emotional labor: responding directly, responding directly, receiving senior's help, using the organizational system, persevering, 4) Problems caused by emotional labor: work exhaustion, job change intention, job stress, 5) Protection plan against emotional labor: manual or education for nurses, education for patients and carers, compensation, tough sanctions though system strengthening. Conclusion: This study shows that although nurses working in comprehensive nursing service ward generally experience high levels of emotional labor, the problem solving of them relies mainly on personal response. Therefore, it is necessary to develop various measures to protect nurses in an organizational level response, thus to improve the comprehensive nursing service system.

A Comparison of Nursing Work Environment, Role Conflict, and Job Embeddedness of Nurses Working in Comprehensive Nursing Care Service Wards and General Wards in a Tertiary Hospital (상급종합병원에서의 간호·간병통합서비스 병동과 일반병동 간호사의 근무환경, 역할갈등, 직무 배태성 비교)

  • Purpose: The objective of this study was to compare and analyze the work environment, role conflict, and job embeddedness between comprehensive nursing care service (CNCS) ward nurses and general ward nurses. Methods: This descriptive research study involved 70 CNCS ward nurses and 69 general ward nurses working at an advanced general hospital in Seoul. Data were collected using the structured questionnaire from March 27 to April 14, 2019 and analyzed with the SPSS 24.0 program. Results: The work environment of the CNCS ward nurse was higher than that of the general ward nurse (t=4.38, p<.001), and the role conflict of the CNCS ward nurse was lower than that of the general ward nurse (t=-2.09, p=.038). However, job embeddedness did not show any statistically significant difference (t=0.22, p=.824). Conclusion: The results of this study show that the introduction of CNCS ward has shown improvement in the work environment and strengthened the establishment of the roles in their team, while maintaining the job embeddedness of nurses. These results indicate that improvements in the work environment, such as nurse staffing and material support, would contribute to the qualitative enhancement of nursing and that it would need to extend the introduction of CNCS wards.
