서울과 경기도 소재 상급종합병원과 종합병원의 필수실무실습 핵심역량교육 성과에 대한 장기적인 고찰 (Long Term Assessment of Outcome of Essential Competencies in CPPE at Tertiary and Secondary Hospitals Located in Seoul and Gyeonggi-do: College of Pharmacy Students' Evaluation from 2014 to 2018)

  • Objective: As the demands of healthcare environment change, it is necessary to advance human health care by improving students' essential competencies including knowledge, skills, abilities, inter-professional collaboration and patient centered care. This study identified long term accomplishment and improvement of the essential competencies in clinical pharmacy practice education (CPPE) at Korean hospitals over time. Methods: This study was conducted for pharmacy students who completed CPPE evaluation related to tertiary hospitals and secondary hospitals located in Seoul and Gyeonggi-do regional area from 2014 to 2018. Results: Over the past 5 years, overall results of student evaluation on the essential competencies in CPPE at both tertiary and secondary hospitals showed a decreasing trend or did not change. Essential competency in CPPE at tertiary hospitals had been identified as superior on 'Learn clinical knowledge in the treatment of diseases' to secondary hospitals [average number of students (%): 210 (72.9%) vs 68 (68.0%)]. On the other hand, essential competencies in CPPE at secondary hospitals had been identified as better at 'inter-professionals collaborative teamwork and direct patient care' than tertiary hospitals [average number of students (%): 64 (64.0%) and 56 (56.0%) vs 121 (42.0%) and 90 (31.3%)]. A total of 176 (61.1%) students in tertiary hospitals and 66 (66.0%) in secondary hospitals evaluated that 'patient-centered care' education was good. Conclusion: In tertiary hospitals, all six essential competency outcomes have not been improved, whereas four essential competency outcomes showed an increasing trend in secondary hospitals. It will be necessary to develop outcome-based CPPE education program to better reflect the essential competencies.

융합 액션러닝 기반 가상 시뮬레이션 프로그램이 간호대학생의 문제해결 능력, 비판적 사고, 의사소통 능력, 임상수행 능력에 미치는 효과 (The Effects of Virtual Simulation Program based Convergence Action Learning on Problem-Solving, Critical Thinking, Communication Skills, and Clinical Competency of the Nursing students)

  • 본 연구는 융합 액션러닝 기반 가상 시뮬레이션 프로그램이 간호대학생의 문제해결 능력, 비판적 사고, 의사소통 능력, 임상수행 능력에 미치는 영향을 검정하기 위해 수행되었다. K시 소재 일 간호대학생 54명에게 비동등성 대조군 사전·사후 유사 실험연구으로 진행하였다. 자료수집 기간은 2021년 4월 12일에서 6월 18일까지이며, 수집된 자료는 IBM SPSS 23.0 program을 이용하여 분석하였다. 융합 액션러닝 기반 가상 시뮬레이션 프로그램 시행 결과, 실험군이 대조군보다 의사소통 능력, 임상수행 능력에 효과가 있는 것으로 나타났다. 본 연구를 통해 간호대학생을 대상으로 다빈도로 활용되는 시뮬레이션 교육을 적용함에 있어 다양한 교수법을 접목한 프로그램이 적극 활용될 필요성이 있으며, 융합 액션러닝 기반 가상 시뮬레이션 프로그램은 시뮬레이션의 효과를 향상시키는 방법으로 사용될 수 있을 것이다.

국내외 병원약사 교육프로그램 현황 및 한 병원의 약사 역량강화를 위한 교육프로그램 개발 (Investigation and Development of Competency-based Training Program for Pharmacists in a Hospital)

  • Objective: This study was to investigate and develop a training program that integrated and strengthened clinical competence and capacity of hospital pharmacists which would be implemented at the pharmacy department of Ewha Womans University Mokdong Hospital. Methods: We assessed the training programs of hospital pharmacists in the United Kingdom, the United States and Japan and compared those of major hospitals in Seoul and Gyeonggi provinces in Korea. The survey assessed the satisfaction on the current educational programs for pharmacists at Ewha Womans University Mokdong Hospital and a newly designed education program was suggested. Results: The major differences of the programs among the Korean hospitals and Mokdong Hospital were the training period and contents of the entry-level pharmacist education program. The overall satisfaction of the training program was >3 points on 5-Likert scale in the survey. There was no significant difference of the satisfaction level based on the employment period of pharmacists. The pharmacists were in more than 7 training programs of clinical support work. We proposed the clinical support training for the new pharmacists to be separated from orientation and concentrated on actual work duty. In order to improve the competency, it was encouraged to participate the in-house seminar or certify the related fields provided by Korean Society of Health-system Pharmacists. Conclusion: The education program was assessed among different countries and hospitals of Korea. We suggested to update education program in the hospital to enhance pharmacists' competency.

간호대학생의 전공만족도와 간호전문직관이 임상수행능력에 미치는 영향 (The Effects of Major Satisfaction and Nursing Professionalism on Clinical Competence of Nursing Students)

  • 본 연구는 간호대학생의 전공만족도와 간호전문직관이 임상수행능력에 미치는 영향을 알아보고자 시행한 서술적 조사연구이다. 일개 대학 간호학과 4학년 123명을 연구대상자로 선정하였으며, 구조화된 설문지를 사용하여 2022년 10월부터 12월까지 자료수집을 하였다. 수집된 자료는 SPSS 22.0 프로그램을 이용하여 기술통계, 상관관계 T검정, 분산분석 및 회귀분석을 실시하였다. 자료분석결과 전공만족도, 간호전문직관 및 임상수행능력은 양적 상관이 있는 것으로 나타났으며, 전공만족도와 간호전문직관은 임상수행능력에 유의한 영향을 미치는 것으로 분석되었다. 임상수행능력에 가장 큰 영향을 미치는 요인은 전공만족도로 나타났으며, 모형의 설명력은 51.9%였다. 연구결과, 간호대학생의 임상수행능력 강화를 위해서는 무엇보다 전공만족도를 높일 수 있는 간호교육과정 개발을 제언한다.

신규간호사의 핵심역량 (Core Competencies for New Nurses)

  • Purpose: The purpose of this study was to identify core competencies for new nurses and valuate the appropriateness and capability of core competencies. Methods: Mixed method was applied for the study. Qualitative data were obtained from preceptor and nurse manager utilizing an open-ended survey question and qualitative data analysis was conducted. The quantitative data were collected from 238 nurses (79 new nurses, 78 preceptors, 81 nurse managers) and descriptive statistics, ANOVA, $x^2$ tests were applied. Results: Three themes (20 contents) were identified as core competencies: competency as an employee, competency to perform nursing care for patient, competency to maintain nursing expertise. New nurses recognized themselves as having higher competency as an employee and to perform nursing care for patient when compared to nurse managers. Conclusion: The findings identified core competencies for new nurses need to be reflected to developing human resource management strategies for hiring new nurses.

응급실 간호사의 중증도분류 전문역량에 영향을 미치는 요인 (Factors Influencing Professional Competencies in Triage Nurses Working in Emergency Departments)

  • Purpose: This study was conducted to identify the professional competency of nurses working in emergency medical institutions that use the Korean Triage Acuity Scale (KTAS), and to identify factors that affect them. Methods: This study collected data from 105 nurses working in emergency medical institutions from June to August 2020. For data analysis, descriptive analysis, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression were performed using the SPSS 25.0 program. Results: As for the professional competency in triage, the higher the self-efficacy (β= 0.58, p< .001), the more experience they have in triage-related education (β= 0.30, p< .001), 2-4 years of clinical experience in emergency department (β= 0.19, p= .002), in case of triage alone (β= 0.24, p< .001), the higher the level of education a nurse has (β= 0.19, p= .003), the higher the professional competency in triage. These variables explained professional competency in a total of 64.2% of the participants (F = 38.30, p< .001). Conclusion: To improve nurses' professional competence in triage, introducing manpower expansion, financial support, and the provision of appropriate places is suggested. In addition, it is necessary to repeatedly provide educational opportunities in an environment similar to actual clinical practice by developing various scenarios and introducing simulations and web-based formats.

스마트 러닝을 활용한 심폐소생술 재교육이 간호대학생의 심폐소생술 지식, 자기효능감, 수행능력에 미치는 영향 (The Effects of Repeated Cardiopulmonary Resuscitation Training using Smart Learning on Nursing Students' Knowledge, Self-efficacy, Clinical Competency.)

  • 본 연구는 스마트 러닝을 활용한 심폐소생술 재교육이 간호대학생의 지식, 자기효능감, 수행능력에 미치는 효과를 확인하기 위한 연구이다. 비동등성 대조군 사전사후 유사실험설계를 사용하였다. 연구의 참여자는 심폐소생술 교육을 받은지 6개월이 지난 간호대학생으로 총 102명이었다. 심폐소생술 재교육을 스마트 러닝, 강의식 교육, 실습교육으로 나누어 실시하였다. 자료수집 기간은 2016년 11월부터 12월이었다. 수집된 자료는 SPSS 21.0을 이용하여 실수와 백분율, 평균과 표준편차, ${\chi}^2$-test, t-test, one way ANOVA로 분석하였다. 심폐소생술 재교육 후 지식은 강의식 교육이 스마트 러닝과 실습 교육보다 높았고, scheffe의 사후검정결과도 통계적으로 유의하게 나타났다(F=8.23, p=<.001). 자기효능감은 실습교육이 스마트 러닝과 강의식 교육보다 높았지만 통계적으로 유의하지 않았다(F=2.46, p=.091). 수행능력은 실습교육이 가장 높았으며, 스마트 러닝과 강의식 교육 순으로 나타났으며, scheffe의 사후검정결과도 통계적으로 유의한 차이가 있었다(F=59.90, p=<.001). 이상의 연구결과를 통해 볼 때, 간호학생의 지식, 자기효능감, 수행능력에 대한 효과적인 교육방법이 다르다는 것을 보여준다. 더욱 효과적인 심폐소생술 재교육을 위해서는 복합적인 교육이 되어야 할 것이다.

음악치료에서의 개인적 역량: 교육과 임상적 훈련에 대한 도전 (Intrapersonal Competency in Music Therapy: Challenges for Education and Clinical Training)

  • 음악치료사의 자질과 능력 증진은 많은 연구자들과 학자들에 의해 연구되어온 분야이며 이를 위한 효과적인 교육 및 임상 전략들이 규명되고 개발되고 있다. 본 연구에서는 기존의 음악치료의 정의를 중심으로 음악치료사의 기본 자질을 규명하고 학술단체와 기관에서 정의하는 음악치료가 강조하는 다른 항목들을 비교분석함으로써 공통적으로 음악치료사가 갖추어야 하는 역량들을 규명하였다. 그 결과 본 연구에서는 음악치료사는 세 가지 역량을 갖추어야 하고 기존의 교육과정은 이러한 역량들을 강화할 수 있는 내용으로 구성되어야 하는데 이에는 학문적 역량, 실기역량, 그리고 개인적 역량이 포함된다. 여기서 개인적인 역량은 음악치료 교육과정에서 다루어야 하는 새로운 역량 분야로 규명되었다. 이는 음악치료가 지식기반 내용만을 다루는 학문이 아님으로 치료를 위한 음악중심의 개입을 제공할 수 있는 내적 성찰과 치료사적 통찰력이 기반이 된 역량을 갖추어야 하기 때문이다. 음악치료가 정의하는 학문과 전문성을 갖추기 위해서 음악치료사는 이 세 가지 역량을 갖추어야 하며 이러한 역량 강화를 위해서는 효율적인 교과내용들이 기존의 교육과정에 반영되어야 한다.

간호대학생의 문화역량 강화를 위한 교수학습모형의 효과 (The Effect of a Teaching Model for Improving Undergraduate Nursing Students' Cultural Competency)

  • Purpose: This study evaluated the effect of a teaching model to improve cultural competency (TMCC) for Korean undergraduate nursing students. Methods: A nonequivalent control group pretest/posttest quasi-experimental study was conducted with a convenience sample of 168 undergraduate nursing students in South Korea. The experimental group of 121 seniors was taught a 13-week teaching model in order to improve cultural competence. A control group with 47 junior students underwent nursing major courses, but did not take this teaching model. Before and after the program, students' level of cultural competency was measured using the Questionnaire for Cultural Competence (QCC) consisting of three sub-scales: "awareness and desire," "encounter," and "nursing skill and knowledge." Results: After the experiment, the experimental group showed significantly higher improvement in the Questionnaire for Cultural Competence in the three sub-scales of "awareness and desire," "encounter," and "nursing skill and knowledge" than the control group (p=<.050). Conclusion: A teaching model to improve cultural competence was effective in improving Korean undergraduate nursing students' cultural competency. Further studies need to be repeated in order to identify the effectiveness of the teaching model to improve cultural competency with graduate or clinical nurses.

외래 간호사의 감정노동, 의사소통능력, 감성지능 및 사회적 지지가 소진에 미치는 영향 (Effects of Emotional Labor, Communication Competency, Emotional Intelligence and Social Support on Burnout among Nurses in Outpatient Department)

  • Purpose: Based on the results of Grandey's Emotion Regulation Process Model and previous studies, this study was conducted to identify the relationship between emotional labor, communication competency, emotional intelligence, social support, and burnout, and to identify factors affecting burnout of nurses in outpatient department. Method: The participants were 190 nurses with more than six months of experience working at the outpatient department of a general hospital in Seoul. Data were collected from April 5 to May 28, 2021, and analyzed using SPSS/WIN 25.0. Results: Significant variables affecting burnout were emotional labor, communication competency, emotional intelligence, education, and total clinical experience. Social support showed a statistically significant negative correlation with burnout, but did not affect burnout. Burnout showed a statistically significant a positive correlation with emotional labor, and showed a negative correlation with communication competency, emotional intelligence and social support. We found a negative correlation between burnout and subjective health status. Emotional labor had a positive effect on burnout. Emotional intelligence, clinical experience for more than 10 years, communication competency, and education for masters or higher negatively affected burnout, respectively. They accounted for 49.2% of the total variance of burnout. Conclusion: Based on the results of this study, it is necessary to reduce emotional labor, one of the major predictors of burnout for outpatient care. In order to prevent emotional labor that results in burnout, an integrated program that improves emotional intelligence and communication competency should be developed.
