  • Purpose : After planning the Respiratory Gated Radiotherapy for Lung cancer, the movement and volume change of sparing normal structures nearby target are not often considered during dose evaluation. This study carried out 4-D dose evaluation which reflects the movement of normal structures at certain phase of Respiratory Gated Radiotherapy, by using Deformable Image Registration that is well used for Adaptive Radiotherapy. Moreover, the study discussed the need of analysis and established some recommendations, regarding the normal structures's movement and volume change due to Patient's breathing pattern during evaluation of treatment plans. Materials and methods : The subjects were taken from 10 lung cancer patients who received Respiratory Gated Radiotherapy. Using Eclipse(Ver 13.6 Varian, USA), the structures seen in the top phase of CT image was equally set via Propagation or Segmentation Wizard menu, and the structure's movement and volume were analyzed by Center-to Center method. Also, image from each phase and the dose distribution were deformed into top phase CT image, for 4-dimensional dose evaluation, via VELOCITY Program. Also, Using $QUASAR^{TM}$ Phantom(Modus Medical Devices) and $GAFCHROMIC^{TM}$ EBT3 Film(Ashland, USA), verification carried out 4-D dose distribution for 4-D gamma pass rate. Result : The movement of the Inspiration and expiration phase was the most significant in axial direction of right lung, as $0.989{\pm}0.34cm$, and was the least significant in lateral direction of spinal cord, as -0.001 cm. The volume of right lung showed the greatest rate of change as 33.5 %. The maximal and minimal difference in PTV Conformity Index and Homogeneity Index between 3-dimensional dose evaluation and 4-dimensional dose evaluation, was 0.076, 0.021 and 0.011, 0.0 respectfully. The difference of 0.0045~2.76 % was determined in normal structures, using 4-D dose evaluation. 4-D gamma pass rate of every patients passed reference of 95 % gamma pass rate. Conclusion : PTV Conformity Index was more significant in all patients using 4-D dose evaluation, but no significant difference was observed between two dose evaluations for Homogeneity Index. 4-D dose distribution was shown more homogeneous dose compared to 3D dose distribution, by considering the movement from breathing which helps to fill out the PTV margin area. There was difference of 0.004~2.76 % in 4D evaluation of normal structure, and there was significant difference between two evaluation methods in all normal structures, except spinal cord. This study shows that normal structures could be underestimated by 3-D dose evaluation. Therefore, 4-D dose evaluation with Deformable Image Registration will be considered when the dose change is expected in normal structures due to patient's breathing pattern. 4-D dose evaluation with Deformable Image Registration is considered to be a more realistic dose evaluation method by reflecting the movement of normal structures from patient's breathing pattern.

  • Background: Dosimetric comparison of two dimensional (2D) radiography and three-dimensional computed tomography (3D-CT) based dose distributions with high-dose-rate (HDR) intracavitry radiotherapy (ICRT) for carcinoma cervix, in terms of target coverage and doses to bladder and rectum. Materials and Methods: Sixty four sessions of HDR ICRT were performed in 22 patients. External beam radiotherapy to pelvis at a dose of 50 Gray in 27 fractions followed by HDR ICRT, 21 Grays to point A in 3 sessions, one week apart was planned. All patients underwent 2D-orthogonal and 3D-CT simulation for each session. Treatment plans were generated using 2D-orthogonal images and dose prescription was made at point A. 3D plans were generated using 3D-CT images after delineating target volume and organs at risk. Comparative evaluation of 2D and 3D treatment planning was made for each session in terms of target coverage (dose received by 90%, 95% and 100% of the target volume: D90, D95 and D100 respectively) and doses to bladder and rectum: ICRU-38 bladder and rectum point dose in 2D planning and dose to 0.1cc, 1cc, 2cc, 5cc, and 10cc of bladder and rectum in 3D planning. Results: Mean doses received by 100% and 90% of the target volume were $4.24{\pm}0.63$ and $4.9{\pm}0.56$ Gy respectively. Doses received by 0.1cc, 1cc and 2cc volume of bladder were $2.88{\pm}0.72$, $2.5{\pm}0.65$ and $2.2{\pm}0.57$ times more than the ICRU bladder reference point. Similarly, doses received by 0.1cc, 1cc and 2cc of rectum were $1.80{\pm}0.5$, $1.48{\pm}0.41$ and $1.35{\pm}0.37$ times higher than ICRU rectal reference point. Conclusions: Dosimetric comparative evaluation of 2D and 3D CT based treatment planning for the same brachytherapy session demonstrates underestimation of OAR doses and overestimation of target coverage in 2D treatment planning.

  • The study investigates the necessity of 3 dimensional dose distribution evaluation instead of point dose and 2 dimensional dose distribution evaluation. Treatment plans were generated on the RANDO phantom to measure the precise dose distribution of the treatment site 0.5, 1, 1.5, 2, 2.5, 3 cm with the prescribed dose; 1,200 cGy, 5 fractions. Gamma analysis (3%/3 mm, 2%/2 mm) of dose distribution was evaluated with gafchromic EBT2 film and ArcCHECK phantom. The average error of absolute dose was measured at $0.76{\pm}0.59%$ and $1.37{\pm}0.76%$ in cheese phantom and ArcCHECK phantom respectively. The average passing ratio for 3%/3 mm were $97.72{\pm}0.02%$ and $99.26{\pm}0.01%$ in gafchromic EBT2 film and ArcCHECK phantom respectively. The average passing ratio for 2%/2 mm were $94.21{\pm}0.02%$ and $93.02{\pm}0.01%$ in gafchromic EBT2 film and ArcCHECK phantom respectively. There was a more accurate dose distribution of 3D volume phantom than cheese phantom in patients DQA using tomotherapy. Therefor it should be evaluated simultaneously 3 dimensional dose evaluation on target and peripheral area in rotational radiotherapy such as tomotherapy.

  • Among the 4th Industrial Revolution technologies, evaluated bolus applicability through dose assesment according to the materials of 3D printing technology. Dose assesment was using MCNPX which was applied Monte Carlo method and 3D print materials were ABS, PC and PLA. Thus, the thickness with the same effect as the bolus 10 mm was found to be ABS 10 mm, PC 9 mm and PLA 9 mm for the 6 MeV electron. For 6 MV X-ray, ABS 11 mm, PC 10 mm and PLA 9 mm were shown. This study showed that tissue equivalent materials made from 3D printer materials can replace bolus.

  • The purpose of the study was to evaluation of the radiation dose reduction and the possibility of the maintainability of the adequate image quality using various automatic exposure control (AEC) systems in multi-detector computed tomography (MDCT). We used three AEC systems for the study: General Electric Healthcare (Auto-mA 3D), Philips Medical systems (DoseRight) and Siemens Medical Solutions (Care Dose 4D). The general scanning protocol was created for the each examination with the same scanning parameters as many as possible. In the various AEC systems, the evaluation of reduced-dose was evaluated by comparing to fixed mAs with using human phantom. The image quality of the phantom was evaluated with measuring the image noise (standard deviation) by insert regions of interests. Finally, when we applied to AEC for three manufacturers, the radiation dose reduction decreased each 35.3% in the Auto-mA 3D, 58.2% in the DoseRight, and 48.6% in the Care Dose 4D. And, there was not statistical significant difference among the image quality in the Strong/Weak of the Care Dose 4D(P=.269). This applies to variety of the AEC systems which will be very useful to reduce the dose and to maintain the high quality.

  • In nuclear medicine, radioactive isotope tracers are administered to the human body to obtain and evaluate disease morphological information and biological function information. Dose calibrator is a device used to measure the radioactivity of a single nuclide in medical institutions. Administration of the correct dose to the human body acts as an important factor in diagnosis and treatment, and measurement through a dose calibrator before administration is the most important factor. Dose calibrator performs daily quality control after installation in each medical institution. Quality control is a means of guaranteeing quality control after installation, and is essential for improving the quality of treatment and promoting patient safety. Therefore, accurate and standardized performance evaluation methods should be established. In this study, 3D printing was used for quantitative evaluation of quality control by increasing the accuracy and standardization of quality control. When the 3D printer was installed and reproducibility was tested, the error range of the expected value and reading value decreased by 0.302% in the F-18 nuclide and 0.09% in the 99mTc-pertechnate nuclide than when the 3D printer was installed. The error rate for other nuclides was also found to have a low error rate for reproducibility tests when 3D printing was installed.

  • Objective: This study aimed to evaluate the image quality and dose reduction of low-dose three-dimensional (3D) rotational angiography (RA) for evaluating intracranial aneurysms. Materials and Methods: We retrospectively evaluated the clinical data and 3D RA datasets obtained from 146 prospectively registered patients (male:female, 46:100; median age, 58 years; range, 19-81 years). The subjective image quality of 79 examinations obtained from a conventional method and 67 examinations obtained from a low-dose (5-seconds and 0.10-μGy/frame) method was assessed by two neurointerventionists using a 3-point scale for four evaluation criteria. The total image quality score was then obtained as the average of the four scores. The image quality scores were compared between the two methods using a noninferiority statistical testing, with a margin of -0.2 (i.e., score of low-dose group - score of conventional group). For the evaluation of dose reduction, dose-area product (DAP) and air kerma (AK) were analyzed and compared between the two groups. Results: The mean total image quality score ± standard deviation of the 3D RA was 2.97 ± 0.17 by reader 1 and 2.95 ± 0.20 by reader 2 for conventional group and 2.92 ± 0.30 and 2.95 ± 0.22, respectively, for low-dose group. The image quality of the 3D RA in the low-dose group was not inferior to that of the conventional group according to the total image quality score as well as individual scores for the four criteria in both readers. The mean DAP and AK per rotation were 5.87 Gy-cm2 and 0.56 Gy, respectively, in the conventional group, and 1.32 Gy-cm2 (p < 0.001) and 0.17 Gy (p < 0.001), respectively, in the low-dose group. Conclusion: Low-dose 3D RA was not inferior in image quality and reduced the radiation dose by 70%-77% compared to the conventional 3D RA in evaluating intracranial aneurysms.

  • In this study, four types of composite added filtration (aluminum, nickel, copper, and zinc) were combined for each thickness to evaluate dose reduction and optimal images due to X-ray attenuation. To evaluate dose and image quality. X-ray generators, Dose Area Product(DAP) and ICY programs with RQR9 standard quality were used. In the image quality evaluation element (PSNR, RMSE, SSIM), only images with PSNR value of 30 dB or more were analyzed. As a result, the best combination in dose evaluation was 3 mmAl + 0.6 mmNi (0.16µGy㎡), and the best filter in image quality evaluation was 0.9 mmAl (PSNR 34.24dB, RMSE 79.52, SSIM 0.24). In this study, the dose aspect and the image quality aspect are mentioned, So it is considered that further studies on patient's exposure dose and optimal image will be needed in the future.

  • Image-based three dimensional radiation treatment planning(3D RTP) has a potential of generating superior treatment plans. Advances in computer technology and software developments quickly make 3D RTP a feasible choice for routine clinical use. However, it has become clear that an evaluation of a 3D plan is more difficult than a 2D plan. A number of tools have been developed to facilitate the evaluation of 3D RTP both qualitatively and quantitatively. For example, beam's eye view(BEV) is one of the most powerful and time-saving method as a qualitative tools. Dose-volume histogram(DVH) has been proven to be one of the most valuable methods for a quantitative tools. But it has a limitation to evaluate several different plans for biological effects of the tissue and critical organ. Therefore, there is a strong interest in developing quantitative models which would predict the likely biological response of irradiated organs and tissues, such as tumor control probability(TCP) and normal tissue complication probability(NTCP). DVH and NTCP of hepatoma were evaluated for three dimensional conformal radiotherapy(3D CRT). Also, 3D RTP was analysed as a dose optimization based on beam arrangement and beam modulation.

  • The purpose of this study was to evaluate the clinical efficacy of 128 MDCT (multi-detector computed tomography) for reducing the CareDose 4D dose and comparing the image quality with the fixed tube current technique. For this purpose, we conducted the phantom and clinical studies to evaluate the exposure dose and image of the subject before and after applying the CareDose 4D system in abdominal examination using 128 MDCT. In the phantom study, ROI (Region of interest) was located at the center, 3, 6, 9, 12 o'clock, into two groups: group A without CareDose 4D and Group B applied were measured. In the clinical study, ROI was located at the liver 8 segments, divided into two groups too. The measured items were CT number, noise, and dose length product (DLP) dose. The result of CTDIvol (CT Dose Index volume) measurements in phantom and clinical studies were lower than those before CareDose 4D application, and dose and effective dose were also measured lower (p<.05). There was no difference in CT number before and after application (p>.05). In conclusion, using CareDose 4D, we can obtain optimal image information without deteriorating image quality while reducing patient dose.
