Development and Application of Simulation Learning Scenario using Standardized Patients: Caring for Neurological Patients in Particular (표준화 환자를 활용한 시뮬레이션 학습시나리오 개발 및 적용 : 신경계 환자 간호를 중심으로)

  • This study was to develop the simulation learning scenario using standardized patients, and to evaluate the effect of it. This study was a pre-post test design with a single group and participants were recruited from fourth-year nursing students at a university. This study has developed the case scenario for a patient with spontaneous intracerebral hemorrhage and the simulation learning using standardized patients, which was applied in this study, had improved problem solving ability and clinical performance ability on nursing students. Through this study, the nursing student will grow up to be a professional nurse by learning the nursing assessment including consciousness assessment. Also, the simulation learning scenario using standardized patient should be developed in the various fields.

  • The purpose of this study is to analyze the effects of simulation education using standardized patients on respiratory infectious disease knowledge and clinical performance of nursing students. A single-group before-and-after design for the nursing of infectious respiratory patients using standardized patients from March 2 to June 15, 2020, targeting 112 senior nursing students. Respiratory infectious disease education program 'knowledge of lung infectious diseases', 'hand washing', 'wearing a mask', 'encouraging patients and caregivers to wear masks', 'intravenous injection', '3-way injection', 'surgical aseptic technique', 'disinfecting medical devices' , 'contaminated linen management', 'infected person management manual', etc., and 10 educational tasks were performed, and consisted of lectures, technical training, simulation using standardized patients, and debriefing. After simulation education using standardized patients, students' knowledge and clinical performance skills on respiratory infectious diseases showed significant improvement, and it was expected that it could be used for various infection control practices.

  • The purpose of this study is to clarify the difference between the scoring results by scorer(doctors and standardization patients) and examination dates. A total of 101 students in the fourth grade of medical school participated in four clinical practice test. Students were randomly assigned to either day-1 or day-2, which was consisted of a standardized patient scoring set or a physician scoring set. Station checklists consisted of history taking, physical examination, patient education, physician-patient relationship and clinical courtesy. The achievement scores of each case and each domain were converted to the standard score, and the differences between groups were compared. Female students' achievement scores were significantly higher than male students' achievement scores in all domains. There was no significant difference between means by the standardized patients' group and doctors group. Day-2 group was significantly higher than day-1 group in both of history taking and physical examination domains. If the principles of checklist are clearly defined, the scorer status (either physician or standardized patients) does not determine the difference of students' practice test scores.

  • This study was conducted to investigate the effects of child nursing simulation using standardized patient on nursing student's anxiety, self-efficacy, and critical thinking disposition. A one group pre test-post test design was used. The subjects of the study were 62 students in the nursing department and 4th year students at the university in C city. The hospitalized child nursing simulation was conducted for approximately 3.5 hours, including 60 minutes of pre-study and orientation, 60-80 minutes of hands-on scenarios, and 30 minutes of reflection diary writing and debriefing. The data collection was conducted from March to December 2016. Statistical analyses of the collected data were conducted, including t-tests, Pearson's correlation coefficient, and Cronbach's ${\alpha}$. Analyses were conducted using SPSS 21.0. The results of the study showed that anxiety (p <.001) was significantly lower and self-efficacy (p<.001) and critical thinking disposition (p<.001) were higher. Therefore, we propose that various scenarios for simulation practice that utilize standardized patients to enhance core nursing abilities be developed.

  • 우리나라의 병원급 이상 의료기관을 이용한 입원환자의 지역별 의료이용의 변이와 의료자원의 공급, 도시규모, 소득수준이 지역별 의료 이용의 변이에 미치는 영향을 살펴보기 위해 연구를 수행하였다. 본 연구에서는 이를 위해 2005년 환자조사 자료를 이용하였다. 지역별 의료이용의 변이에 성, 연령과 같은 지역별 인구구조의 차이에 의한 효과는 직접표준화 방법을 이용하여 보정한 다음 지역별 의료이용의 변이 정도를 파악하였다. 연구결과 병원급 이상 의료기관을 이용한 입원환자의 표준화 퇴원율과 표준화 재원일수는 차이가 있었다. 표준화 퇴원율에 영향을 미치는 요인으로는 도시규모, 인구10만명당 병상수가 주요 요인으로 나타났다.

  • 문제: 퇴원 환자의 문의 전화가 많아지면서 병동 간호사는 이 문의에 응하는 일로 업무량 과다, 전화 상담에 대한 자신감 저하 및 부담 등을 느끼고 있었다. 부적절한 전화문의 관리는 환자와 간호사 모두에게 시간 소모 및 만족도 저하 등의 부정적인 경험을 남길 수 있다. 목적: 전화문의 응대 매뉴얼 구축 및 활용으로 병동 간호사 및 퇴원 환자의 만족도를 향상시키고자 한다. 의료기관: 서울시 종로구에 소재한 대학병원. 질 향상 활동: 표준화된 전화문의 응대 매뉴얼을 개발하였다. 또한 간호사의 업무 효율성 및 지식 향상을 위해 간호사 대상 매뉴얼을 교육하여 실무에서 활용하기 쉽도록 하였다. 개선효과: 표준화된 전화문의 응대 매뉴얼을 안과 병동 간호사와 전보자 및 신규 간호사 교육에 활용할 수 있었다. 간호사의 전화문의 응대 관련 직무 만족도가 향상되었다. 환자 역시 표준화된 매뉴얼에 따라 일치되고 일관성 있는 정확한 정보를 제공받게 되어 환자의 만족도와 이해도가 증가할 것이다.

Effects of Simulation Based Education Using Standardized Patient for Contact Precaution Infection Control for Nursing Students (표준화 환자를 활용한 접촉주의 환자 간호 시뮬레이션 교육의 효과)

  • The purpose of this study was to investigate effect of simulation based education using standardized patient for contact precaution infection control for nursing student. This study was conducted by including 67 nursing student A university from October to December 2019. This study was mixed method research design. Knowledge and performance confidence related to multidrug resistant organism(MDRO) infection control were measured using questionnaires pre and post test, analyzed using paired t-test and reflection sheet was analyzed using content analysis method. After intervention, two variables were increased significantly. Results of the content analysis showed there were 39 significant statements, which were classified into 13 categories. These results suggest that education on simulation program using standardized patient for contact precaution infection control is effective strategy to enhance knowledge and performance confidence related to MDRO infection control and practical nursing infection control skill, patient centered care, interprofessional collaboration.

  • This study was conducted to examine the effects of a simulation-based learning scenario using standardized respiratory patients regarding emergency care on learning satisfaction, and self efficacy, clinical skill competency of health related department students. A quasi-experimental non-equivalent control group pre-post test design was used. A total of 50 students, 25 students for the experimental group and 25 students for the control group, The experimental group received the 2 educations sessions and 1 evaluation session with 180 minutes for each session. It was implemented in November, 2013. Data were analysed using frequency, ratio and t-test by the SPSS/Win 18.0. The experimental group who had the simulation-based learning scenario using standardized respiratory patients showed significantly higher learning satisfaction and self efficacy, clinical skill competency compared with the control group who had a traditional simulation education. Through this study, simulation-based learning scenario using standardized patients was verified to be an effective teaching method to be grow up on professional competency of health related department students. Also the simulation-based learning scenario using standardized patients should be developed in the various fields.

Utility of the Dispatch Protocol to Triage the Emergency Patients who presented with Symptoms of Stroke or Chest Pain (흉통 및 뇌졸중 증상 환자에 대한 전화 중증도분류 지침의 유용성)

  • Delayed treatment of acute cardiovascular and cereb-directrovascular diseases is related to poor prognosis and sequelae. For rapid and adequate treatment, role of prehospital emergency dispatchers for adequate triage and selection of hospital is important. In several advanced countries, emergency dispatchers use standardized protocols for decision of rescuer resources or distribution of patients at each hospital. ut, there has not been developed standardized protocol for emergency dispatchers in Korea. We developed standardized protocol based on NHS-direct and CTAS system for triage of symptoms of chest pain and Stroke. Groups with standardized protocol and without protocol was compared to triage result at emergency department which patient visited. The accuracy of triage on chest pain was 70.0% in group A, 94.0% in group B(p<0.01). The accuracy of triage in stroke symptoms was 64.2% in group A, 84.6% in group B(p<0.01). Conclusion: In this study, the accuracy of telephone triage with the protocol was more accurate than without the protocol. But, more studies are needed to generalize the protocol in South korea.

Reliability of Standardized Patients as Raters in Objective Structured Clinical Examination (객관 구조화 절차 기술 평가에서 채점자로서의 표준화환자의 신뢰도)

  • The purpose of this study is to investigate whether standardized patient(SP) can be used as a reliable examiner in Objective Structured Clinical Examination(OSCE). 4 SPs and 4 faculties who have more than 2 years experience of OSCE scoring were selected. For 1 assignment 2 members of faculty and 2 SPs were designated as raters. SPs were educated for assessing 2 technical skills, male Foley catheter insertion and wound dressing, for 8 hours (4 hours / day, each topic). The definition, method, cautions and complications for each of procedural skills were covered in the education. Theoretical lectures, video learning, faculty demonstration and practical training on mannequins were employed. The 8 raters were standardized for an hour with simulated OSCE scoring using previous videos on the day before the OSCE. Each assessment was composed of 14 checklists and 1 global rate. The allotted time for each assignment was 5minutes and for evaluation time 2 minutes per student. The evaluation from the faculty and SPs were compared and analyzed with the GENOVA program. The overall generalizability coefficient (G coefficient) was 0.839 from two cases of OASTS. The reliability of the raters was high, 0.946. The inter-rater agreement between faculty group and SP group was 0.949 for checklist and 0.908 for global rating. Therefore SPs can play a role of raters in OSCE for procedural skills, if they are given the appropriate training.
