중환자실 환자가 경험한 인간중심 간호에 관한 질적 내용분석 (A Qualitative Content Analysis of the Person-Centered Care Experienced by Critical Care Patients)

  • Purpose : To explore the person-centered care (PCC) experienced by critical patients. Method : This qualitative study deductively examined the attributes of the PCC model proposed by Jakimowicz and Perry. The participants were 16 patients who were recently discharged from intensive care units at a university hospital. Data were collected through in-depth interviews and were analyzed using the deductive method of content analysis. Results : A total of 4 categories, 16 subcategories, and 33 codes were generated from 171 meaningful statements. The final 4 categories were "compassionate presence," "professional interaction," "outstanding competency," and "patient identity." These were consistent with the main attributes of Jakimowicz and Perry's model. However, most of the codes belonged to the "compassionate presence" and "patient identity" categories. Among the attributes of the model, "continuity of nursing," "therapeutic relationship," "expert knowledge," "clinical knowledge," "evidence-based intervention," and "patient's rights" were not derived as codes. Conclusion : These findings deepen the understanding of the PCC model from the patient's point of view. The main attributes of PCC identified in the current study can be applied to the development of practical guidelines for intensive care nursing. In addition, we recommend the development of a PCC measurement tool for critical care patients.

중환자실 간호사의 구강간호에 대한 인식 및 수행 실태 (A Survey on Oral Care among ICU Nurses)

  • Purpose: Providing oral care is one of the most important nursing interventions in ICU. Good oral health prevents various complications including pneumonia and improves patients' well-being as well. The purpose of this study was to indentify the ICU nurses’awareness and performance on oral care. Methods: Using a convenient sampling, 170 ICU nurses from 5 university hospitals in two metropolitan cities were selected. Results: Of the 170 nurses in this study, 95.9% were aware of the importance of oral care. Subjects responded that providing oral care three times or more a day was appropriate to prevent ventilator associate pneumonia. 43.5% of the subjects acknowledged that they didn’t know what the most effective oral care solution was. In oral care performance, 61.8% of subjects provided oral care once a day for patients with intubation, and 74.7% used oral solutions without knowing the exact pharmacological effects of them. Conclusion: As the results of this study, most ICU nurses realized the importance on oral care for ICU patients, but the frequency of performance was low. To improve oral care performance in the ICU, structured education should be provided to ICU nurses and evidence based oral care protocols need to be developed.

한국형 중환자간호 분류도구 개발 (Development of Korean Patient Classification System for Critical Care Nurses)

  • Purpose: This study was performed to develop a valid and reliable Korean Patient Classification System for Critical Care Nurses (KPCSC). Methods: Tertiary and general hospitals with various levels of ICU nurse staffing were included. To verify interrater reliability, data collectors and staff nurses of 15 ICUs in 11 hospitals classified 262 patients. To verify construct validity, the staff nurses classified 457 patients according to KPCSC comparing difference by medical department and type of stay in ICU. For conversion index, 195 patients from 10 ICUs in 7 hospitals were classified and nursing time was measured by 174 nurses, 7 head nurses, 18 charge nurses, 37 nurse aids and 1 secretary. Results: The developed KPCSC has 11 categories, 82 nursing activities and 115 criterias. Reliability was found to have high agreement (r=.96). Construct validity was verified by comparing differences in medical department and type of stay in ICU. According to scores, four groups in the KPCSC were identified. One score on the KPCSC indicates 6.12 minutes of nursing time. Conclusion: The findings show that the KPCSC can be used to measure new and complex nursing demands including rehabilitation and the safety of ICU patients.

중환자실 간호사의 중환자간호 실무표준 이행에 대한 조사 (ICU Nurses' Compliance with Standards for Critical Care Nursing Practice)

  • Purpose: To explore nurses' compliance with standards for critical care nursing practice in intensive care units (ICUs) and to provide basic data for high quality of critical care nursing. Method: A total of 616 participants from 61 ICUs which are graded from 1 to 7 throughout the nation were surveyed. Data were collected from February 9 to February 27, 2009 using a questionnaire consisting of 58 questions including 50 nursing activities as indicators. Results: The rate of ICU nurses' compliance with standards for critical care nursing practice was high. As for individual standards, compliance with the standard of assessment was the highest, followed by implementation, diagnosis, and planning in that order. There were differences in compliance according to nurses' ICU experience, work place (unit), and ICU grade. A shortage of manpower was considered as a main cause for noncompliance. Conclusion: The present compliance rate with standards for critical care nursing practice by ICU nurses was identified. Therefore, compliance rate can be used to promote quality of critical care nursing and development of educational programs for ICU nurses.

MicroSim(R)을 병용한 시뮬레이션기반 중환자간호교육의 운영 및 평가 (Implementation and Evaluation of Simulation Based Critical Care Nursing Education Used with MicroSim(R))

  • Purpose: This study was conducted to evaluate the results after implementing a simulation based critical care nursing education with $MicroSim^{(R)}$. Method: Simulation based education was used for a clinical scenario on a patient with chronic obstructive pulmonary disease(COPD) and acute coronary syndrome(ACS). Self-learning program was used for an acute asthma attack and acute myocardial infarction(AMI) in the $MicroSim^{(R)}$. A total of 97 nursing students were chosen. A pretest and posttest was conducted to evaluate learning achievement, clinical performance ability and self-directed learning. Result: Learning achievement and clinical performance ability significantly increased but self-directed learning did not. Conclusion: Simulation based education used with $MicroSim^{(R)}$ was useful for improving learning achievement and clinical performance ability of nursing students. Further studies are needed to compare the effects of simulation based education.

내과계 중환자의 비계획적 기관내관 발관과 계획적 기관내관 발관의 관련 특성 비교 (Comparison of Related Characteristics between Unplanned and Planned Extubation of Patients in Medical Intensive Unit)

  • Purpose: This retrospective study was designed to examine the frequency of unplanned extubation, and to identify the related factors of unplanned extubation in medical intensive care unit patients. Methods: Data were collected from medical records of patients who received intubation in a medical intensive care unit. One hundred eighteen patient charts were selected for the study. The Patient Severity Classification Scale and unplanned extubation risk factors were examined. Data were analyzed with descriptive statistics, $x^2$-test, Fisher's exact test and Mann-Whitney U test. Results: The incidence of unplanned extubation was 11 (9.32%) out of 118 patients who had undergone intubation. There were statistically significant differences between the unplanned extubation and work shift ($x^2$=61.52, p=.001), ventilation mode (p=.001), number of days of ventilator application (U=366.00, p=.038) and administration of sedatives (p=.025). Conclusion: Unplanned extubation is affected by the following variables: a) whether a nurse is in night shift, b) whether ventilation is mandatory, c) duration of ventilation use and d) administration of sedatives.

중환자 간호단위의 환자분류군별 간호원가 산정연구 (Estimation of Nursing Costs by a Patient Classification System(PCS) in ICU)

  • Purpose: The objective of our study was to figure out costs of nursing services in ICU based on the PCS in order to determine an appropriate nursing fee schedule. Method: Data was collected from 2 hospitals from April 15-16 to April 22-23, 2003. The costs of nursing services in the ICU were analyzed by nursing time based on the nursing intensity. The inpatients in the ICU were classified by a PCS tool developed by the Korean Clinical Nurses Association(2000). Results: The distribution of patients by PCS in the ICU ranged from class IV to Class VI. The higher PCS in ICU consumed more nursing time. As a result, the higher nursing intensity, the more the daily average nursing costs in the ICU. Conclusion: Our study provides evidence to refine the current nursing fee schedule that does not differentiate from the volume of nursing services based on nursing time. We strongly recommend that the current reimbursement system for nursing services should be applied not only to the general nursing units but also to the ICU or other special nursing units.

일 대학 간호대학생이 지각한 중환자 간호 역량 조사 (Senior Nursing Students' Perceived Competence of Intensive and Critical Care Nursing)

  • Purpose: The purpose of this study was to examine the degree of intensive and critical care nursing competence in senior nursing students. Methods: The research method for this study comprised a descriptive survey study with a convenience sample of 113 senior nursing students from one university. The instrument entitled ICCN-CS-1, which measures intensive and critical care nursing competence, was translated by the authors following WHO guidelines for translation process. The data were collected using ICCN-CS-1. Results: The mean score of intensive and critical care nursing was above the intermediate level. The competence indices of knowledge, skill and attitude/value were also above the intermediate level, which suggests that their competence is good. For two intensive and critical care nursing domains, the nursing students' mean score of clinical competence is higher than that of professional competence. The competence of intensive and critical care is also influenced by nursing students' satisfaction in their major and clinical practice as well as their confidence in clinical practice. Conclusion: The senior nursing students' competence of intensive and critical care nursing is above the intermediate level. A consideration of the influencing factors of satisfaction of nursing major/ICU practice and confidence of ICU practice is needed in intensive and critical care nursing education.

중환자 가족의 부담감, 건강증진행위 및 건강상태에 관한 연구 (A Relation Study on Burden, Health promotion Behavior and Health Status of the Family Caregiver of Intensive Care Unit Patient)

  • The purpose of this study was to identify the burden, health promotion behavior and health status and to describe the relationship of the burden, health promotion behavior and health status of the family caregivers of intensive care unit patients. Method: The subjects were 48 family caregivers of ICU patients in a University Hospital. Data were collected between June, 1 and July, 31, 2000 using structured questionnaires. Research tools used were Suh and Oh's Burden Scale, Revised Walker, Sechrist, & Pender's HPLP(1987) ; Revised Nam's Health State Scale(1965). Result: The mean score of burden of family caregiver was 3.01(full score was 5). The mean score of health promotion behavior of family caregiver was 2.52(full score was 4). And the mean score of health status of family caregiver was 0.68(full score was 1.00). The score of psychological health state was a little higher than the physiological one. In correlational analysis, the burden and the health status of caregivers were reversely correlated . The correlation between the burden and the health promotion behavior, and the health behavior and health status were not significant. Conclusion: The more burden caregivers of ICU patients felt, the worse their health status. So nurses need to understand the family caregiver's burden and apply nursing care that can reduce burden, in order to improve the health status of family caregivers.

중환자실 간호사 근무환경과 환자가족 만족도의 관계 (Critical Care Nursing Work Environment and Family Satisfaction)

  • Purpose: The purpose of this study was to investigate the nursing work environment and family satisfaction in Korean intensive care units (ICUs). Methods: The study participants were 190 critical care nurses and 133 family members of ICU patients who were randomly chosen from four of the hospitals located in B city. The Korean Nursing Work Environment Scale was used to assess the work environment of critical care nurses. Family satisfaction was measured with the Korean version of the Critical Care Family Needs Inventory. Results: Critical care nurses reported moderate satisfaction with their work environment. The mean score for family satisfaction was 3.59 on a 5-point scale, and satisfaction with information provision received the highest score. Family satisfaction was higher in hospitals where the critical care nurses evaluated their work environment positively. Conclusion: This study revealed that the work environment of nurses affects family satisfaction in ICUs. Therefore, it is necessary to explore various methods of improving the critical care nursing work environment in order to provide the highest possible level of nursing care.
