Hospital Workers' Experience with Hospital Evaluation Program: A Focus Group Study (의료기관 평가에 관한 병원 종사자들의 경험: 포커스 그룹 연구)

  • Purpose: The purpose of the study was to understand and describe the hospital workers' experience related to the hospital evaluation program implemented in Korea between 2004 and 2009. Methods: During 2010, data were collected using focus group interviews. Four focus group interviews were held with a total of 28 hospital workers participating. All interviews were recorded and transcribed as they were spoken, and data were analyzed using qualitative content analysis. Results: Nine themes emerged from the analysis: 1) Positive change in the necessity of the evaluation; 2) Improvement in the hospital system, facilities, and human resources; 3) Unity through cooperation among departments; 4) Nursing work overload; 5) Lack of physicians' awareness and responsibilities; 6) Unfair and unrealistic evaluation items; 7) Lack of credibility of the outcome; 8) Shifting responsibility for negative outcomes to the workers; 9) Lack of pragmatic utility. Conclusion: The results of the study demonstrate that the hospital evaluation program played a key role in improving some work environments and communication among departments. At the same time, they show various negative themes resulting from the context of very authoritarian hospital systems and a connection-oriented society in Korea.

Effects of Empowerment and Regarding Standard Precautions for Healthcare associated Infection Control in Healthcare Workers on Performances (의료종사자의 임파워먼트와 의료관련감염 관리 표준주의지침 인지도가 수행도에 미치는 요인)

  • The purpose of this study was to examine relationships among empowerment, awareness and performance on standard precautions and identify the factors that affect performance of standard precaution in the health care workers. The data were collected from 200 health care workers working in the general hospital located Y city. The data were analyzed by descriptive statistics, t-test, ANOVA, Scheffe, Pearson's correlation coefficient, and stepwise multiple regression. The result of the stepwise multiple regression indicates the awareness predict 49.0% (F=191.98, P<.001) in the performance of standard precaution. Therefore, it is necessary to develop awareness of the standard precaution strategy program for improving the performance of standard precautions in health care workers.

The Association between Perceived Job Performance of Hospital Coordinators and the Satisfaction of Medical Workers (의료기관 종사자가 인지한 병원코디네이터의 업무수행도와 종사자의 직무만족도와의 관련성)

  • Objectives: This study was conducted to investigate general characteristics, job characteristics, characteristics of hospitals, and hospital coordinators, and to investigate the factors affecting the job satisfaction of medical workers. Methods: The data was collected through a self-reported questionnaire among 435 workers in plastic surgery and dermatology clinics in metropolitan city G. We investigated general characteristics, job-related characteristics, characteristics of medical institutions and hospital coordinators, job performance of hospital coordinators, and job satisfaction of medical workers. T-tests, ANOVA, correlation, and multiple regression analyses were conducted to investigate the factors related with job satisfaction. Results: The job satisfaction of the medical workers and the perceived job performance of hospital coordinators were $3.55{\pm}0.40$ and $3.74{\pm}0.43$, respectively. In multiple regression analysis, drinking frequency, number of night shifts, number of work days, and hospital coordinator introduction years were associated with the job satisfaction of medical workers. The more the overall job performance of hospital coordinators increased, the higher was job satisfaction(B=0.458, p<0.001). In detail, the more job roles(B=0.218, p<0.001), core competency(B=0.145, p=0.005), and leadership(B=0.099, p=0.037) increased, the higher was the job satisfaction of medical workers. Conclusions: The job satisfaction of the medical institution workers was associated with the perceived job performance of the hospital coordinator. In order to increase the job satisfaction of medical institution workers, a hospital coordinator with specialized job performance will be able to improve job satisfaction by carrying out efficient work in the medical institution.

Convergence Factors Influencing Attitude Toward Advance Directive in Healthcare Workers (의료종사자의 사전의료의향서에 대한 태도에 미치는 융합적 영향요인)

  • This study was a descriptive study to investigate the effects of healthcare workers' consciousness of biomedical ethics, recognition of good death, and self-esteem on attitude toward advance directive. Participants in this study were 292 healthcare workers in the three general hospitals located Y city. The data were collected from April 2 to April 23, 2019 and analyzed descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression using the SPSS Win. 22.0 program. The result of the multiple regression indicates the intention to awareness regarding advance directives, intention to execute advance directives, time to complete advance directives and recognition of good death predict 23.5% (F=12.39, P=.000) of attitudes toward advance directives. Therefore, it is necessary to develop a program to establish attitudes toward advance directives by providing education on recognition of good death of healthcare workers.

The Measurement of Spatial Dose Rate by Gravity Ventilation after Technegas Scanning (Technegas 스캐닝 후 중력환기에 의한 공간선량율 측정)

  • Because examination with technegas produces images through simple diffusion accumulation, the examination room can become contaminated after scan. Therefore, radiation workers and patients awaiting examination will be affected by internal exposure from technegas inhalation. Before and after gravity ventilation, I am trying to find a way to reduce the exposure dose of waiting patients according to a comparative analysis of horizontal spatial dose rates over time. Spatial dose ratio were measured for 10 minutes from various distances and angles around ventilator's location before and after gravity ventilation. Then, mean values, standard deviation and reduction ratio were calculated. The highest reduction rate of gravity ventilation was 95.31% and the highest reduction ratio was 1 to 3 minutes. Therefore, the gravity ventilation could reduce the exposure dose of radiologic technologists, waiting patients, patient guardians and nurses. In conclusion, the reduction of the exposure dose during the technegas ventilation study through gravity ventilation will play a role in optimiging the protection and it is in accordance with the recommended reduction of the medical exposure by ICRP 103.

A Study on the Construction of Curriculum Content for Training Assistive Product Specialist (복지용구 전문가 양성을 위한 교육과정 내용구성에 관한 연구)

  • The purpose of this study is to develop curriculum contents for assistive product specialists and to prepare professional standards for said curriculum. First, after identifying and targeting employees in the assistive product center, hospital and medical facilities staff, users of the assistive product, and instructors for the assistive product, the task importance and education required for each duty of the assistive product specialist were examined. Based on a demand analysis, the subjects(plan) for assistive product specialist was drawn and then the curriculum(plan) was developed. Regarding the developed subjects(plan) and curriculum(plan), a Delphi method was conducted with five professional panels to verify the validity. Finally, a curriculum(plan) for assistive product specialist was developed to include a total of 21 courses(17 compulsory courses, 4 elective courses) totaling 44 hours (36 hours for compulsory courses, 8 hours for elective courses). The construction of systematic content for of assistive product specialist curriculum for qualitative training of assistive product specialists is important. Through this study, it will now be possible to secure professionalism and qualitative improvements in curriculum for assistive product specialists.

Comparative evaluation of radiation exposure in radiation-related workers (방사선 작업종사자의 피폭선량 비교 평가)

  • The purpose of this study is to investigate the dose of radiation exposure to radiation-related workers in a hospital setting, thus increasing awareness of the health risk to the radiation-related workers. The result of the analysis showed the average dose of radiation exposure to radiation-related workers in hospital K was $0.75{\pm}0.26mSv$ in 2008, $0.67{\pm}0.30mSv$ in 2009, and $0.92{\pm}0.33mSv$ in 2010. The average dose of radiation exposure in hospital P was $0.43{\pm}0.13mSv$ in 2008, $0.43{\pm}0.20mSv$ in 2009, and $0.33{\pm}0.85mSv$ in 2010. The average dose of radiation exposure in hospital K by age group was 13.39mSv for age 20 to 29, 8.37mSv for age 30 to 39, 1.19mSv for age 40 to 49, 0.28mSv for age 50 to 59, and 0.32mSv for age 60 to 69 The average dose of radiation exposure in hospital P by age group was 0.33mSv for age 20 to 29, 1.41mSv for age 30 to 39, 0.83mSv for age 40 to 49, 1.66mSv for age 50 to 59, and 1.12mSv for age 60 to 69. Moreover, the average radiation exposure to radiation-related workers over 3 year period by gender group in hospital K was $2.92{\pm}1.03mSv$ for male group and $0.94{\pm}0.93mSv$ for female group. The average radiation exposure over 3 year period by gender group in hospital P was $0.66{\pm}0.18mSv$ for male group and $1.80{\pm}0.60mSv$ for female group. Persons working in diagnostic radiology department received mean of $1.65{\pm}1.54mSv/year$, mean $1.17{\pm}0.82mSv/year$ in radiation oncology, mean $1.79{\pm}1.42mSv/year$ at nuclear medicine department and mean $0.99{\pm}0.51mSv/year$ at other departments. Radiation exposure was higher than that of other departments(p<0.05). Doctors and technologists received higher radiation exposure (mean $1.75{\pm}1.17mSv/year$, $1.60{\pm}1.39mSv/year$ each) than other workers(p<0.05). Measurement and evaluation of radiation exposure in radiation-related workers should be widely conducted accurately and consistently in the radiation-related occupational setting so that people in these occupational settings are more aware of the risk from radiation exposure, and thus give more attention and caution to decrease radiation exposure. It would be essential to minimize accumulated radiation dose in the radiation-related occupational setting in order to maintain and improve the health of radiation-related workers.

Effect of Career Management Activity in Hospital Organization on the Job Instability for Hospital Administrative Staff's (병원조직의 경력관리활동이 행정직종사자들의 고용불안 심리에 미치는 영향)

  • This study has performed to propose the plan of effective career management systematically for the enhancement of mutual communication with internal and external customers based on the effect of career management activity in hospital organization on the job instability for administrative staff's. Data was collected from 192 hospital administrative staff's located in Busan area where 7 general hospitals and 7 hospitals were involved, and examined the factor analysis, the reliability, the regression analysis and the correlation analysis using SPSS statistical program. The organizational career management activities were classified into direct and indirect activities through the verification of validity and internal consistency, and the job instability was influenced by the direct career management activities. "Consider of countermeasure of necessary training required for the career of staff's" has the strongest relationship with the job instability among the organizational direct career management activities, and "Provide sufficient information service for new job opportunity" and "Develope career management plan participate with the staff's" were also relatively shown the higher relationship. Thus, it was indicated that the organizational effective career management activities for the enhancement of mutual communication with customers were necessary for the strategy establishment of career management activities.

Knowledge, Attitude and Practice on Maternal Immunization with Tetanus Toxoid, Reduced Diphtheria Toxoid, and Aellular Pertussis (Tdap) among Pregnant Women (임신 중 Tdap 접종에 대한 임부들의 인식, 태도 및 행동)

  • Purpose: Despite effectiveness and safety of maternal tetanus toxoid, reduced diphtheria toxoid, and acellular pertussis (Tdap) vaccination, Tdap vaccination coverage during pregnancy remains quite low. We assessed the knowledge, attitude and practice on maternal Tdap vaccination among pregnant women. Methods: This study was a cross-sectional survey of pregnant women who visited tertiary obstetrics and gynecologic units in Seoul and Gyeonggi province of Korea. Individual questionnaires were administered to assess knowledge, attitude and practice on maternal immunization with Tdap. Results: The questionnaires were completed by 184 pregnant women; 158 (86%) had not received information from doctors about pertussis and Tdap, and 166 (90%) did not know the need for Tdap vaccination. Only 7% of pregnant women unlikely to receive Tdap vaccine during current pregnancy answered 3 or more of the 5 knowledge-based questions correctly. By logistic regression analysis, recommendation by doctor (adjusted odd ratio [OR], 236.2; 95% confidence interval [CI], 12.6-4,432), belief that the vaccine is effective (adjusted OR, 40.21; 95% CI, 2.35-687.7), and belief that the vaccine is safe (adjusted OR, 19.83; 95% CI, 1.54-255.9) were significantly important factors to respondents' intention to be vaccinated. Conclusions: Most pregnant women seem to be neither recommended nor adequately informed about Tdap vaccination. Information given by health care professionals is very important to increase Tdap coverage among pregnant women.

Behaviors-Based Safety Management of Dental Professionals on Dental Amalgam and Resin (치과용 아말감과 레진에 대한 치과종사자의 안전성에 대한 관리행동 실태)

  • This study investigated the safety management behavior of dental professionals to minimize exposure to harmful elements of amalgam and resin. As for the use of amalgam and resin, 85.8% of them used amalgam. 100% of used composite resin 100%. 97.8% used acryl resin, and 95.7% used tray resin. 95.2% and 36.5% of them were aware of the harm of amalgam and resin respectively. When using resin, their safety management behavior score was 1.99 on average, and the lowest score was 2.50 on average for 'ventilate the indoor air when handling resin'. Their average score of the safety management behavior was 1.83 when using amalgam. The lowest score was 2.89 on average for 'use the rubber dam for patients when handling amalgam'. As a result of the safety management behavior based on general characteristics, all characteristics were significant with regard to amalgam, and my position at work and work experience were significant with regard to resin (p<0.05). The correlation between the safety management behavior and general characteristics was analyzed when dental amalgam and resin are used. Amalgam was statistically correlated with academic background (p<0.05) and position at work (p<0.05), whereas resin was statistically significantly correlated with age (p<0.05) and position at work (p<0.05). Accordingly, education about the harm of dental materials must be continuously provided for dental professionals when they handle dental materials so that safety management behavior will be conducted correctly, and active efforts to protect the health of dental professionals from harmful chemicals must be made.
