Development of Standardized Predictive Models for Traditional Korean Medical Diagnostic Pattern Identification in Stroke Subjects: A Hospital-based Multi-center Trial

  • Objectives: To develop a standardized diagnostic pattern identification equation for stroke patients, our group conducted a study to derive the predictive logistic equations. However, the sample size was relatively small. In the current study, we aimed to derive new predictive logistic equations for each diagnostic pattern using an expanded number of subjects. Methods: This study was a hospital-based multi-center trial recruited stroke patients within 30 days of symptom onset. Patients' general information, and the variables related to diagnostic pattern identification were measured. The diagnostic pattern of each patient was identified independently by two Korean Medicine Doctors. To derive a predictive model for pattern identification, binary logistic regression analysis was applied. Results: Among the 1,251 patients, 385 patients (30.8%) had the Fire Heat Pattern, 460 patients (36.8%) the Phlegm Dampness Pattern, 212 patients (16.9%) the Qi Deficiency Pattern, and 194 patients (15.5%) the Yin Deficiency Pattern. After the regression analysis, the predictive logistic equations for each pattern were determined. Conclusion: The predictive equations for Fire Heat, Phlegm Dampness, Qi Deficiency, and Yin Deficiency would be useful to determine individual stroke patients' pattern identification in the clinical setting. However, further studies using objective measurements are necessary to validate these data.

A Pilot Study to Evaluate the Reliability of Pattern Identification Tool for Knee Osteoarthritis and to Analyze Correlation between Pattern Identification Tool and Knee Range of Motion, Visual Analog Scale and Western Ontario & Mcmaster Universities Osteoarthritis Index (퇴행성 무릎관절염 변증도구의 신뢰도 평가 및 무릎 ROM, VAS, WOMAC과의 상관관계에 대한 탐색적 연구)

  • Objectives To evaluate the pattern identification tool for knee osteoarthritis and to investigate the relationship between pattern identification tool and knee ROM (range of motion), VAS (visual analog scale) and WOMAC (Western Ontario & McMaster Universities osteoarthritis index). Methods We studied 50 patients who are diagnosed as degenerative osteoarthritis. With one patient dropping out, total of 49 patient went through pattern identification tool. Re-test of the pattern identification tool was implemented after about a week. The reliability was analyzed by calculating ICC (intraclass correlation coefficient). Also, reliability of each pattern identification score was calculated along with influence factor and correlation between knee ROM, VAS and WOMAC. Results Reliability of the pattern identification tool was evaluated as 'poor agreement beyond chance' with ICC value of 0.396. Reliablility of each pattern identification score was calculated and four out of five were measured 'good' with one 'moderate' reliability. Two of inter-item consistency were 'good', two were 'acceptable' and one was 'questionable'. Correlation between each pattern identification and knee ROM showed 'clear negative linear relationship' with two patterns. VAS showed 'clear positivie linear relationship' with all five patterns. WOMAC showed 'positivie linear relationship' with all five patterns. Conclusions According to the results, pattern identification tool for knee osteoarthritis seems to have meaningful relationship with other agnedas. Also, further research is needed to develop the tool.

Preliminary Study to Develop the Instrument of Pattern Identification for Jing Ji and Zheng Chong (경계정충(驚悸怔忡) 변증도구 개발을 위한 기초 연구)

  • Objectives : This study was performed to develop a standard instrument of Pattern Identification for jing ji and zheng chong. Methods : The advisor committee on this study was organized by 15 neuropsychiatry professors of oriental medical colleges. The items and structure of the instrument were based on review of published literature. We took consultation 2 times from the advisor committee and we also took additional advices by e-mail. Results : 1. We divided the symptoms and signs of jing ji and zheng chong into 9 pattern identification. - heart deficiency with timidity(心膽虛怯), heart qi deficiency(心氣虛), heart blood deficiency(心血虛), heart yang inactivity(心陽不振), heart blood stasis(心血瘀阻), phlegm turbidity obstructing(痰濁阻滯), yin deficiency with effulgent fire(陰虛火旺), water qi intimidating the heart(水氣凌心), dual deficiency of the heart and spleen(心脾兩虛). 2. We got the mean weights that reflect standard deviation to each symptom of 9 pattern identification which had been scored on a 100-point scale. 3. We made out the Korean instrument of the pattern identification for jing ji and zheng chong. It was composed of 17 questions in question-and-answer form. Conclusions : Instrument of Pattern Identification for jing ji and zheng chong was developed through experts' disscussion. If the validity and reliability of this instrument is confirmed through additional clinical trial, the instrument of pattern identification for jing ji and zheng chong is expected to be applied to the subsequent research.

Study to Develop the Instrument of Pattern Identification for Hwa-byung (화병변증도구 개발 연구)

  • Hwa-byung is a form of psychogenic illness among people in Korea and is listed as a culture-bound syndrome of Korea in the DSM-IV. Despite increased clinical researches for Hwa-byung in the oriental medicine of Korea, there has been no agreement of pattern identification for Hwa-byung. The purpose of this study is to develop a standard instrument of pattern identification for Hwa-byung which will be applied to clinical research. The items and structure of the instrument were based on review of published literature. The advisor committee on this study was organized by 15 neuropsychiatry professors of 11 oriental medical colleges nationwide. The experts attended 2 consultation meetings and discussed developing the instrument. and we also took professional advices by e-mail. The results were as follows; First, we divided the symptoms and signs of Hwa-byung into five pattern identification - stagnation of liver Qi, flare-up of the liver fire, disharmony between heart and kidney, deficiency of both Qi and blood, malfunction of gallbladder due to phlegm stagnation. Second, we got the mean weights to each symptom of five pattern identification which had been scored on a 5-point scale - ranging from 0 to 4 by the 15 experts. Third, we made out the Korean instrument of the pattern identification for Hwa-byung. It was composed of 34 questions and decided on question-and-answer form. Though there are some limits in this study, the instrument of pattern identification for Hwa-byung is meaningful and expected to be applied to the subsequent research. And also, we hope to improve the instrument and make up for this study through various research and discussion.

Pattern Identification of 97 Functional Dyspepsia Patients and the Characteristics of Each Pattern Type (기능성 소화불량 환자 97명의 변증유형별 특성)

  • Objective: This study was designed to identify and explore the pathological patterns of functional dyspepsia (FD) patients. We also evaluated the usefulness of the Pattern Identification Questionnaire by comparing it with other assessment tools for FD. Methods: We recruited 97 FD patients based on the Rome III criteria for FD diagnosis. The pathological patterns of the subjects were determined by the Pattern Identification Questionnaire. Their dyspepsia-related symptoms were assessed using the Gastrointestinal Symptom Questionnaire (GIS) and the Pyeongwi-san (Pingwei-san) Patternization Questionnaire. Depressive symptoms were evaluated with the Beck Depression Inventory (BDI) and quality of life with the Functional Dyspepsia-Related Quality of Life (FD-QoL) Questionnaire. Tongue coating was measured by the Digital Tongue Diagnosis System (DTDS). Results: The male to female ratio was 1:1.1, and the forties and fifties age groups were largest in number. The spleen deficiency and phlegm-dampness pattern was the most common pattern found among the FD patients. No significant differences in the GIS, BDI, FD-QoL, and DTDS scores were found among the five pattern types. All pattern types showed significant correlation with GIS, Pyeongwi-san Patternization Questionnaire, and FD-QoL scores. Conclusions: Pattern Identification Questionnaire can not only identify the pathological pattern types of FD patients but also evaluate the severity of their symptoms. Compared to conventional assessment tools for FD, it could enable a more dynamic evaluation of FD patients reflecting the severity of dyspeptic symptoms and the quality of life. Further studies on the Pattern Identification of FD patients are anticipated in order to improve the diagnosis and therapy for Korean FD patients.

Characteristics of Patients with Allergic Rhinitis through the Pattern Questionnaire Items (변증 설문지 문항을 통해 살펴 본 알레르기 비염 환자의 특성)

  • Objective : We performed a clinical study to investigate pattern characteristics in persistent allergic rhinitis depending on Korean Medicine pattern questionnaire items as a pattern identification diagnostic tool. Method : 32 patients with persistent allergic rhinitis were asked to interview with doctor of Korean Medicine and perform the 4 pattern questionnaires(Cold-Heat Pattern, Phlegm Pattern, Yin Deficiency pattern, bloodstasis pattern). Then, we analyzed the response rate of each pattern questionnaires. Results : After diagnosis of Korean Medicine Doctor's pattern identification, 17 individual items have higher response rate, 7 of 17 items have a common tendency in allergic rhinitis. The other 8 of 10 items belong to Lung qi deficiency cold and Lung-spleen qi deficiency group, these have higher tendency of deficiency. In bloodstasis pattern questionnaires, we don't decide the tendency of patients with allergic rhinitis. Conclusion : The result may provide that we don't use Korean Medicine pattern questionnaires as a major tool in the pattern identification of allergic rhinitis. Continuous studies are needed to develop the standardized pattern identification diagnostic tool.

A Review of Korean Medicine Treatment for Hyperhidrosis (다한증의 한의학적 변증 및 치료에 대한 국내 임상 논문 고찰)

  • Objectives The purpose of this study is to figure out the tendency of the commonly-used-pattern-identification and treatment for Hyperhidrosis by reviewing Korean clinical studies. Methods 18 articles which were published from August, 2004 to December, 2018. were obtained from the National discovery for science leader (NDSL), Research information sharing service (RISS), and Oriental medicine advanced searching integrated system (OASIS) by using keyword 'hyperhidrosis'. Results The most commonly-used-pattern-identification were the patterns with 'Heart' and 'Spleen-stomach'. Hyungbangsabaek-san and Taeeumjowi-tang were the most frequently used herbal medicine. The most common acupoints was LI4. The most common method of assessment was VAS. Conclusions This study identifies the most common pattern identification and treatment for hyperhidrosis. Developing systematic standards of pattern identification and treatment can be possible with further studies.

Tongue of Fatigue by Classification of Sasang Constitution and Qi Blood Pattern Identification (특발성 피로의 사상체질 및 기혈변증 설진 분석)

  • Objectives The purpose of this study was to investigate the relevance of Sasang constitution, Qi Blood pattern identification, and tongue diagnosis in subjects complaining of fatigue.Methods Seventy-three subjects who complained of fatigue were assessed using the Chalder Fatigue Scale, tongue diagnosis, pattern identification questionnaire and Sasang constitution diagnosis. The association of tongue diagnosis with Qi Blood pattern identification and Sasang constitution was evaluated.Results 1. There was no significant association between tongue diagnosis and Sasang constitution.2. Tongue color, which is one of the diagnostic indicators in tongue diagnosis, was redder in the Qi stagnation group than in the Qi deficiency and Blood deficiency groups.Conclusions Tongue diagnosis can be utilized in future if proper research regarding Sasang constitution and Sasang constitution pattern identification is conducted.

Study on the Relationship between Korean Standard of Pattern Identification (II) and Pattern Identification of Cold-Heat and Deficiency-Excess (한국형 중풍 변증 표준안 - II와 한열허실 변증지표의 연관성 연구)

  • Previously standardization study for identifying 5 types of pattern identification of stroke patients has been performed and the Korean standard of pattern identification (II) was developed. In the present study we investigated the interactions between total indices designated by the Korean standard of pattern identification(KSPI II) and indices for PI of Cold-Heat and Deficiency-Excess. Indicators for Cold-Heat and Deficiency-Excess are isolated from 58 indices through the survey of oriental medicine doctors and their relationship with KSPI-II indices was analyzed by corresponding analysis method using data of 1581 stroke patients. Means and standard deviations indicated that 2 Cold indices, 14 Heat indices, 12 Deficiency indices, and 5 Excess indices were included for Cold-Heat and Deficiency-Excess pattern identification. The results of corresponding analysis shows the relationship of 57 indices and 4 types of pattern identification (excluding 1 index and 1 pattern among 58 indices and 5 patterns) using the cross-tabulation which was obtained from the clinical data. Most of Cold and Heat index were divided to dimension 1(inertia 51.9%) obtained from the result of corresponding analysis. Deficiency and Excess index were partially associated with dimension 2(inertia 31.7%). These data suggest that pattern identification of Cold-Heat plays an role in the standardization of pattern identification in stroke, although further studies are required by various trials such as analysis of surveys and clinical data.

Identification of Potential Prognostic Biomarkers in lung cancer patients based on Pattern Identification of Traditional Korean Medicine Running title: A biomarker based on the Korean pattern identification for lung cancer

  • Objective : We studied prognostic biomarkers discovery for lung cancer based on the pattern identification for the personalized Korean medicine. Methods : Using 30 tissue samples, we performed a whole exome sequencing to examine the genetic differences among three groups. Results : The exome sequencing identified among 23,490 SNPs germline variants, 12 variants showed significant frequency differences between Xu and Stasis groups (P<0.0005). As similar, 18 and 10 variants were identified in analysis for Xu vs. Gentleness group and Stasis vs. Gentleness group, respectively (P<0.001). Our exome sequencing also found 8,792 lung cancer specific variants and among the groups identified 6, 34, and 12 variants which showed significant allele frequency differences in the comparison groups; Xu vs. Stasis, Xu vs. Gentleness group, and Stasis vs. Gentleness group. As a result of PCA analysis, in germline data set, Xu group was divided from other groups. Analysis using somatic variants also showed similar result. And in gene ontology analysis using pattern identification variants, we found genes like as FUT3, MYCBPAP, and ST5 were related to tumorigenicity, and tumor metastasis in comparison between Xu and Stasis. Other significant SNPs for two were responsible for eye morphogenesis and olfactory receptor activity. Classification of somatic pattern identification variants showed close relationship in multicellular organism reproduction, anion-anion antiporter activity, and GTPase regulator activity. Conclusions : Taken together, our study identified 40 variants in 29 genes in association with germline difference of pattern identification groups and 52 variants in 47 genes in somatic cancer tissues.
