Development of the Korean Form of Zung's Self-Rating Anxiety Scale (한국형 자가평가 불안척도의 개발)

  • This study was carried out to develop a Korean language version of Zung's self-rating anxiety scale(SAS) from august, 1994 to September, 1996. The subjects consisted of 205 normal control subjects from the general population group, and 97 subjects with anxiety disorders. These 97 subjects were chosen from a group by the structured clinical interview for DSM-IV of in patients and out patients. Both normal control subjects and anxiety disorder subjects were drawn utilizing a cluster of sampling methods. In order to analyze the data on anxiety scores, Pearson's product moment correlation coefficient method was carried out, as well as reliability, factor analysis and discriminant function analysis, utilizing the SPSS/PC+ program. The results obtained were as follows: The mean average of the total anxiety scores were 32.36 + 6.35 for the normal control subjects and 50.53 + 7.67 for anxiety disorder subjects. Test-retest reliability(coefficient r=0.98, p < 0.001), and internal consistency(coefficient r=0.96, p < 0.001) were satisfactory. Factor analysis using oblique technique rotation yielded four factors. The normal control subjects scored higher concerning the symptoms such as sweating, restlessness, apprehension, insomnia and dyspnea, and lower for faintness, mental disintegration, paresthesia, dizziness and tremor. On the other hand, for the anxiety disorders, apprehension, restlessness, sweating, dyspnea and insomnia scored higher, and lower for faintness, paresthesia, nightmare, dizziness and tremor.

A Study for the Change and Distribution of Far Sighted and Near Sighted Astigmatism Power according to Age (연령에 따른 원·근거리 난시의 굴절력 변화에 대한 연구)

  • We researched the change of astigmatism power when the fixation point moved from far distance to near distance. Astigmatism power was measured by using both eyes open-view auto-refractometer. We divided the ages between 5 and 67 years old into 12 groups with 1,598 healthy eyes(male-698 eyes and female-900 eyes) without eyes problems and experiences of eyes operations. The mean power in far astigmatism showed that with-the-rule of the total astigmatism: -0.79D, with-the-rule of the corneal astigmatism: -1.07D and against-the-rule of the residual astigmatism : -0.79D were found most respectively. The correlation between cornea astigmatism and total astigmatism was y=0.7493 x + 0.5661 r=0.6510, residual astigmatism and total astigmatism was y=0.248 x - 0.5926 r=0.2598 and corneal astigmatism and residual astigmatism was y=-0.4439 x - 0.1813 r=-0.5551 in the far distance. The mean power in near astigmatism showed that with-the-rule of total astigmatism : -0.92D, with-the-rule of corneal astigmatism : -1.12D, against-the-rule of residual astigmatism : -0.87D were found most respectively. In the near distance, The correlation between corneal astigmatism and total astigmatism was y=0.6872 x + 0.5934 r=0.6204, residual astigmatism and total astigmatism was y=0.303 x - 0.6066 r=0.3165, corneal astigmatism and residual astigmatism was y=-0.46 x - 0.0626 r=-0.5322. When the fixation point moved far distance to near distance, the differences of power according to the type of astigmatism were total astigmatism: $-0.07D{\pm}0.44D$, corneal astigmatism: $-0.04D{\pm}0.54D$ residual astigmatism:$0.01D{\pm}0.53D$. Most of astigmatism refractive power was increased except for oblique-the -astigmatism. When the fixation point moved far distance to near distance, the change of astigmatism refractive power showed total astigmatism: 540 eyes(33.7%), corneal astigmatism: 638 eyes(39.9%), residual: 841 eyes(52.6%).

Implant Isolation Characteristics for 1.25 Gbps Monolithic Integrated Bi-Directional Optoelectronic SoC (1.25 Gbps 단일집적 양방향 광전 SoC를 위한 임플란트 절연 특성 분석)

  • In this paper, we analyzed and measured implant isolation characteristics for a 1.25 Gbps monolithic integrated hi-directional (M-BiDi) optoelectronic system-on-a-chip, which is a key component to constitute gigabit passive optical networks (PONs) for a fiber-to-the-home (FTTH). Also, we derived an equivalent circuit of the implant structure under various DC bias conditions. The 1.25 Gbps M-BiDi transmit-receive SoC consists of a laser diode with a monitor photodiode as a transmitter and a digital photodiode as a digital data receiver on the same InP wafer According to IEEE 802.3ah and ITU-T G.983.3 standards, a receiver sensitivity of the digital receiver has to satisfy under -24 dBm @ BER=10-12. Therefore, the electrical crosstalk levels have to maintain less than -86 dB from DC to 3 GHz. From analysed and measured results of the implant structure, the M-BiDi SoC with the implant area of 20 mm width and more than 200 mm distance between the laser diode and monitor photodiode, and between the monitor photodiode and digital photodiode, satisfies the electrical crosstalk level. These implant characteristics can be used for the design and fabrication of an optoelectronic SoC design, and expended to a mixed-mode SoC field.

A Comparative Study on the Retention of Implant Overdenture According to the Shape and the Number of Magnetic Attachment (자성 어태치먼트의 형태와 수에 따른 하악 임플란트 피개의치의 유지력에 대한 비교 연구)

  • The aim of this study was to compare the retention and stability of implant overdenture according to the shape and the number of magnetic attachment. The experimental groups were designed for the number of implants(1, 2, 4) and shape of magnetic attachments(flat, cushion, dome type) resulting in 9 subgroups. 45 attachments were tested attached to $Br{\aa}nemark$ system implants which were planted on a mandibular model. Each attachment was composed of the magnet assembly embedded in a overdenture sample and the abutment keeper screwed into the implants. Dislodging tensile forces were applied to the overdenture samples using an Instron(cross-head speed 50.80mm/min) in 3 directions simulating function: vertical, oblique, and anterior-posterior. The loading was repeated 10 times in each direction for 45 samples. The values of maximum dislodging force of each subgroup were processed statistically using SPSS V. 12.0 at the 0.05 level of significance. The results of this study were as follows: 1. Flat type magnetic overdenture was the most retentive when subjected to vertically directed forces and dome type was the lest retentive when subjected to obliquely directed forces(p<0.05). 2. In case of planting one implant, flat type had a higher vertically retentive force than anterior-posteriorly retentive force. In case of planting two implants, flat type and dome type had a higher vertically retentive force and in case of planting four implants, flat type and cushion type had a higher vertically retentive force than anterior-posteriorly retentive force(p<0.05). 3. The incremental number of dental implant, without regards to the three types of magnetic attachment shapes, showed higher retention of overdenture(p<0.05). From the results, if a patient need much more retention of implant overdenture, flat type magnetic overdenture would be a good treatment. In case of the bruxism where excessive lateral forces are already present, dome type could be expected to produce better results. In case of planting one implant, flat type is more stable than the other shape of magnet and in case of two implant, flat type and dome type are more stable and in case of four implants, flat type and cushion type are more stable. Planting more than two implants and using flat type magnetic attachment would provide better retention and stability of implant overdenture

Patient Satisfaction and Perception on Nutritional Counseling Services Quality (영양 상담서비스 품질에 대한 환자의 만족도와 인식조사)

  • The objective of this study was to analyze patient satisfaction and perception of nutritional counseling services quality. A patient satisfaction and perception survey was conducted for 1,095 patients from 43 tertiary hospitals and 20 general hospitals. The number of returned questionnaires was 656 (response rate: 60.0%). Data from 633 questionnaires were analyzed after the questionnaires with unanswered items were excluded. Five domains were identified from the result of the factor analysis using the maximum likelihood and oblique rotation. The five domains were named empathy, responsiveness, tangibles, reliability, and skill and specialty. Patient perception mean score of nutritional counseling was 4.54/5.00. Patient satisfaction scores were significantly higher in empathy (P<0.001), responsiveness, and skill and specialty (P<0.01) with a college or graduate school education than in patients with a middle school education. There was no statistically significant difference between satisfaction scores in tertiary hospitals and those in general hospitals, but patients in capital hospitals reported significantly higher scores than their local counterparts in empathy, responsiveness, reliability, skill and specialty (P<0.001), and tangibles (P<0.05). In responsiveness, significantly higher (P<0.01) scores were observed in patients who received nutritional counseling only once compared to patients who received counseling two times. Patients perception of nutritional counseling services was significantly correlated with their satisfaction of five domains, reliability (r=0.721), responsiveness (r=0.697), empathy (r=0.690), skill and specialty (r=0.678), and tangibles (r=0.622).

The Clinical Effect and Construction of a Stereotactic Whole Body Immobilization Device (전신 정위 고정장치 제작과 임상효과에 대한 연구)

  • Purpose: To develop a whole body frame for the purpose of reducing patient motion and minimizing setup error for extra-cranial stereotactic radiotherapy, and to evaluate the repositioning setup error of a patient in the frame. Materials and Methods: The developed whole body frame is composed of a base plate, immobilizer, vacuum cushion, ruler and belts. The dimension of the base plate is 130 cm in length, 50 cm in width and 1 cm in thickness. The material used in the base plate of the frame was bakelite and the immobilizer was made of acetal. In addition, Radiopaque angio-catheter wires were engraved on the base plate for a coordinate system to determine the target localization. The measurement for radiation transmission and target localization is peformed in order to test the utilization of the frame. Also, a Matlab program analyzed the patients setup error by using the patient's setup images obtained from a CCTV camera and digital record recorder (DVR). Results: A frame that is useful for CT simulation and radiation treatment was fabricated. The frame structure was designed to minimize collisions from the changes in the rotation angle of the gantry and to maximize the transmission rate of the Incident radiation at the lateral or posterior oblique direction. The lightening belts may be used for the further reduction of the patient motion, and the belts can be adjusted so that they are not in the way of beam direction. The radiation transmission rates of this frame were measured as 95% and 96% at 10 and 21 MV, respectively. The position of a test target on the skin of a volunteer is accurately determined by CT simulation using the coordinate system in the frame. The estimated setup errors by Matlab program are shown 3.69$\pm$1.60, 2.14$\pm$0.78 mm at the lateral and central chest, and 7.11 $\pm$2.10, 6.54$\pm$2.22 mm at lateral and central abdomen, respectively. The setup error due to the lateral motion of breast is shown as 6.33$\pm$ 1.55 mm. Conclusion: The development and test of a whole body frame has proven very useful and practical in the radiosurgery for extra-cranial cancers. It may be used in determining target localization, and it can be used as a patient immobilization tool. More experimental data should be obtained in order to improve and confirm the results of the patient setup error.

  • Purpose Mammography is the most widely used scan for the early diagnosis since it is possible to observe the anatomy of the breast. however, The sensitivity is markedly reduced in high-risk patients with dense breast. Molecular Breast Imaging (MBI) sacn is possible to get the high resolution functional imaging, and This new neclear medicine technique get the more improved diagnostic information through It is useful for confirmation of tumor's location in dense breast. The purpose of this study is to evaluate the usefulness of MBI for tumor diagnosis in patients with dense breast. Materials and Methods We investigated 10 patients female breast cancer with dense breast type who had visited the hospital from September 1st to Octorber 10th, 2015. The patients underwent both MBI and Mammography. MBI (Discovery 750B; General Electric Healthcare, USA) scan was 99mTc-MIBI injected with 20 mCi on the opposite side of the arm with the lesions, after 20 minutes, gained bilateral breast CC (CranioCaudal), MLO (Medio Lateral Oblique) View. Mammography was also conducted in the same posture. MBI and Mammography images were compared to evaluate the sensitivity and specificity of each case utilizing both image and two images in blind tests. Results The results of the blind test for breast cancer showed that the sensitivity of Mammography, MBI scan was 63%, 89%, respectively, and that their specificity was 38%, 87%, respectively. Using both the Mammography and MBI scan was Sensitivity 92%, specificity 90%. Conclusion This research has found that, The tumor of dense tissue that can not easily distinguishable in Mammography is possible to more accurate diagnosis since It is easy to visually evaluation. But MBI sacn has difficulty imaging microcalcificatons, If used in conjunction with mammography it is thought to give provide more diagnostic information.

  • The numerical model of Boussinesq approximation, which is mainly used for evaluating the port calmness due to the irregular waves, has a limit of applicability of lattice size in ports such as marinas with narrow port openings of around 30m. The SWASH model controls the partial reflection according to the depth, porosity coefficient and structure size when applying the reflected wave incident on the structure and terrain. In this study, the partial reflection evaluation at the front of the structure according to the bottom shape and the shape of the structure are examined. In order to evaluate the reproducibility of the model due to the diffraction waves entering the term, the area of incidence at right angles and inclination of the structure is constructed and compared with the diffraction theory suggested by Goda et al. (1978). The experimental results of the sectional structure reflectances calculated as the depth mean show reflectances similar to the approximate values of the reflectances presented by Stelling and Ahrens (1981). It is considered that the reflected wave is well reproduced according to the control of the reflected wave at the boundary and the shape and topography of the structure. Compared with previous studies to examine the diffraction of the wave incident from the breakwater opening, the wave incidence angle and the shape of the diffraction wave are very similar to the theoretical values, but both oblique and rectangular incidence In the case where the direction concentration is small, the diffraction degree is underestimated in some sections with the crest ratio of 0.5 to 0.6.

Influence of bone loss pattern on stress distribution in bone and implant: 3D-FEA study (주변 골흡수 양상에 따른 임플란트와 골의 응력분산에 관한 유한요소 분석)

  • Purpose: This 3D-FEA study was performed to investigate the influence of marginal bone loss pattern around the implant to the stress distribution. Material and methods: From the right second premolar to the right second molar of the mandible was modeled according to the CT data of a dentate patient. Teeth were removed and an implant ($\Phi\;4.0{\times}10.0mm$) was placed in the first molar area. Twelve bone models were created; Studied bone loss conditions were horizontal bone loss and vertical bone loss, assumed bone loss patterns during biologic width formation, and pathologic vertical bone loss with or without cortification. Axial, buccolingual, and oblique force was applied independently to the center of the implant crown. The Maximum von Mises stress value and stress contour was observed and von Mises stresses at the measuring points were recorded. Results: The stress distribution patterns were similar in the non-resorption and horizontal resorption models, but differed from those in the vertical resorption models. Models assuming biologic width formation showed altered stress distribution, and weak bone to implant at the implant neck area seams accelerates stress generation. In case of vertical bone resorption, contact of cortical bone to the implant may positively affect the stress distribution.

Comparison between Manifest Refraction and Cycloplegic Refractin for the First-time Spectacle Wearers (최초 안경착용자의 현성굴절검사(MR)와 조절마비굴절검사(CR)의 비교)

  • Purpose: To study the difference between refractive errors obtained from manifest refraction (MR) and cycloplegic refraction (CR) in first-time spectacle wearers. To study the difference between manifest refractive errors and cycloplegic refractive errors in first-time spectacle wearers. Methods: From January 2002 to December 2002, manifest and cycloplegic refractions were carried out on the patients who visited an ophthalmology clinic for a spectacle prescription for the first-time. The patients were 509 male and 499 female patients aged between from 3 to 15 years old. Results: The cycloplegic refraction showed a less myopia and a more hyperopia compared with the non-cycloplegic refraction. The differences were more in female patients. The CR showed a less myopic and a more hyperopic refractive errors than the MR. The differences were more in female patients. The average results from a pre- and a post-cycloplegic refraction showed a reduction of -0.22D in male, and -0.20D in female for the myopic group. For the myopic group, the myopic refractive errors by MR were -0.22D in male and -0.20D in female higher than the refractive errors by CR. Hyperopic group showed an increase of +0.37D in male, and +0.56D in female. For hyperopic group the hypropic refractive errors by CR were +0.37D in male and +0.56D in female higher than the refractive errors by CR. This difference between the results of a preand a post-cycloplegic refraction was more if the patients were younger. This difference between refractive errors by MR and by CR showed the younger the more and the proportions of pseudo-myopia and or latent hyperopia were also higher with younger patents age. The amounts of with-the-rule astigmatism and the oblique astigmatism were increased for the post-cycloplegic refraction in the CR refraction. Simple astigmatism reduced, but there was no difference found in the amount of astigmatism. The prevalence of simple astigmatism reduced, but there was no difference in the amount of astigmatism. Conclusions: The difference between manifest refraction and cycloplegic refraction was more in younger group. The difference of refrative error between by MR and CR increases with ageing decrease. Pseudo-myopia and latent hyperopia was also found in the younger group. Simple astigmatism reduced after cycloplegic refraction, there was no difference found in the amount of astigmatism. The prevalence of simple astigmatism reduced, but there was no difference in the amount of astigmatism.
