Effects of the Respiration Exercise Program through the Pan-flute on the Physiological and Psychological Status of the Elderly (목관악기 호흡운동 프로그램이 노인의 신체적, 심리적 기능에 미치는 영향)

  • Purpose: This study was intended for the enhancement of the elderly's quality of life helping them overcome their physiological changes about aging and reinforcing their vitality. Methods: An nonequivalent control group post-test design was used for the study. The methods to perform the abdominal respiration exercise and to blow the pan-flute were demonstrated to 31 experimental subjects and 36 control subjects. The eight-week 30 minute daily home respiration exercise recipes were prescribed to the subjects. Mobile spirometers were used to measure FVC, $FEV_1$, $FFV_1$ / FVC, $SaO_2$, breathing discomfort. Daily life satisfaction scores were recorded. Music therapy accompanied the eight-week respiration program. A post-test was performed in the same manner as the pre-test. The gathered data were analysed by SPSS/WIN program. Results: 1) The pulmonary function and daily life satisfaction were significantly improved in the experimental group. 2) Both the experimental and control groups did not show significant differences in $SaO_2$. 3) The breathing discomfort was significantly reduced in the experimental group. Conclusion: The respiration exercise program for the elderly through the pan-flute improves the respiration activity of the elderly and enhances their daily life satisfaction scores. Therefore, this program produces effects in enhancing the quality of life for the elderly.

The Effect of Singing Intervention for Women Elderly with Dysphagia Risk (연하장애 위험 여성노인의 노래중재 적용 효과)

  • Purpose: This study was conducted to describe the risk of dysphagia and to evaluate the effects of a singing intervention for women elderly in community. Methods: One-group pre test-post test design was used. The subjects were 29 women over the age of sixty and residing in a local area. A singing intervention consisted of breathing, vowel production and singing. The one hour intervention occurred once a week for eight weeks. The variables of dysphagia risk, a swallowing test, and nutritional status were measured. Analysis was done by paired t-test. Results: There was a significant decrease in the score of dysphagia risk (p<.001). There were significant increases in the swallowing test scores [modified water swallowing test (p=.032), food test (p=.001)]. There were no significant differences in nutritional status (triceps skin fold thickness, mid arm muscle circumference). Conclusion: The findings support that a singing intervention can be helpful in reducing the risk of dysphagia may improve deglutition for the women elderly. These results suggest that this study may contribute to the use of a singing intervention for women elders with dysphagia risk.

  • From the study on the relation of voice and O-jang(五臟廳), the conclusion is as follows. The function of breathing out in larynx is involved with lung and kidney. Lung press out the air, at the same time, kidney supports that to maintenance the density of air constantly. So the person who has short breathing can not make strong voice, and also if having a weak kidney in upper Cho[下蕉], their voice will be changed so very softly to cannot make hard type of voice and lingering sounds. Opening and Closing of vocal cords are controled by liver and spleen. when vocal cords are contracted, liver makes tension, whereas spleen does in slackness. So liver is likely to make high-piched sounds in tension of vocal cords, and spleen is the same to do smoothy and husky sounds through relaxing the vocal cords. In the rising condition of liver function, the temp of speech become fast, and more the pronunciation is clearer, it means that the function of thinking by spleen is operated well. Otherwise, indistinct pronunciation means that in the body seup-sa(濕邪) spreads. Also depression of emotion in speech often makes Gi(氣) of heart weaker, whereas it means regular condition of heart including Simpo(心包) that expression of emotion looks happy.

History of Laryngology (후두학의 역사)

  • An understanding of the history of laryngology is both interesting and useful for those in the field so this study attempts to uncover some of the interesting aspects of its history. The oldest reference of laryngology in the Orient is in the medical documents called the "Whangjainaekyung",(황제내경) which was written over a time period between 3000-4000 B.C., and described the laryngeal function of respiration, protection of the airway and phonation. In the West, a drawing that seems to portray a tracheostomy was found in medical tombs in the plains of Saqqara in Egypt. These drawings date from approximately 3600 B.C. A watershed in laryngology occurred when a spanish music professor named Manuel Garcia in first successfully used a mirror to inspect the larynx. Since that time, laryngology has developed relatively quickly and clinical laryngology made it possible by means of a number of favorable developments. Great advances in laryngological diagnosis and treatment has occurred since the 1970s thanks to improvements in technology leading to the introduction of an operating microscope, endoscopes and lasers. Despite our recent advances in laryngology, we still have not achieved uniformly favorable outcomes and there is much that we do not know. Our future promises continued advances in the field of laryngology such as gene therapy to improve wound healing and tissue engineering to allow the recreation of normal mucosa. In this review, I divide the history of the larynx into the past, present and future. In the last section, I described the history of laryngology in Korea briefly.

Literature Review on the Development of Cognitive Function Improvement Program for the Elderly in Community (지역사회 노인의 인지기능 향상 프로그램 개발에 대한 문헌적 고찰)

  • Objective: This study was to compares and analyzes programs applied to improve cognitive function in patients with mild cognitive impairment and early dementia in the community to find out their effectiveness. Methods: In this study, 12 papers were finalized by searching for "elderly", "cognitive", "community", and "program" using the database of the Research Information System (RISS), National Assembly Library, and Korean Studies Information (KISS). Results: Programs for cognitive function were in the order of cognitive stimulation program, arts and crafts, and exercise program. In the program, rather than applying the cognitive stimulation program alone, the program was operated by combining leisure or exercise, music, art, and handicraft. The time was shown to be 30 minutes. The most frequently used evaluation tool was MMSE, followed by GDS and BBS. By cognitive domain, cognitive stimulation program and memory, satisfaction in psychology, and balance ability in exercise were evaluated the most. In the cognitive area, various cognitive stimulation areas were included, and in the exercise area, basic exercise, muscle strength exercise, joint exercise, and balance exercise were applied. Conclusion: Therefore, developing a program to improve cognitive function for mild cognitive impairment, it will be possible to prepare guidelines to establish and development.

  • 21세기 핵심 키워드 중 하나로 두뇌 산업이 떠오르고 있다. 미국, 일본 등 선진 국가에서는 이미 뇌 연구에 활발한 투자가 진행되고 있다. 이에 따라 본 논문에서는 출생과 성장기 뇌 발달에 문제가 발생하는 발달 장애 아동을 위한 음악 치료 프로그램을 개발하고자 한다. 효과적인 발달 장애 치료를 위해, 조기 발견 후 인지 학습 치료가 필요하다. 그 중 인지 기능과 자가 관리 기능을 기르는 것이 중요한데, 리듬 타이밍 훈련이 발달 장애 아동의 기억 능력 개선에 도움이 된다는 여러 입증된 연구 결과가 있다. 그러나 아직까지 발달 장애 아동을 위한 적절한 치료 방법이 없기에 본 논문에서는 인지 학습 치료가 필요한 아동에게 도움을 주기 위해 동요의 정확한 리듬, 음정을 학습하는 프로그램을 제안한다. 아동의 지속적인 흥미를 끌 수 있는 게임과 인지능력 훈련을 결합하였기에, 보다 좋은 학습 효과를 유도할 수 있을 것이다.

A Study on the Effects of the Breath Efficiency & Lung Capacity Improvement Using Recorder Playing Focused on Breath Training (리코더 연주를 통한 뇌졸중 노력성 폐활량과 호흡 효율성 향상 연구)

  • The purpose of the study was to improve the efficiency of Breath & Forced Vital Capacity(FVC) of Stroke Patients Using Record Playing Focused on Breath Training. The Training of Forced Vital Capacity(FVC) is focused on trunk muscles reinforce for Breathing. So the study tried to play Recorder for reinforcing FVC. The play included the two parts. One was for solo play, the other was play in concert base on solo play using Korea country song, Arirang. The Recorder play used abdominal breath, Pursed-lip & Paced methods. The course of play included Long Tone, Staccato, Crescendo, Decrescendo and throughout the course we tried to improve the efficiency & Quantity of breathe. Experiment performed at Social welfare institute for the old. They played & checked FVC, $FEV_1$, $FEV_1/FVC$ before & after breathe 19 times for 6 weeks using Spirovit SP-1, Schiller. After experiment we performed statical process(p<0.05) using statical Package software. The findings were as follows: Analysis for Experiment showed the numerical Value of increasing Of FVC, $FEV_1$, $FEV_1/FVC$. Increasing Percentage of Measured/Predicted FVC, $FEV_1$ (or calculated based on theory) ration was 25%, 13% and Quantity of FVC, $FEV_1$ 35%, 40%. Also $FEV_1/FVC$ Ratio(showed efficiency of breathe) raised 13%. And experiment data was confirmed by statical process. The experiment data was valuable under p<0.05. In Conclusion we think the results of experiment showed the efficiency of Music therapy for Breathe(FVC, $FEV_1$, $FEV_1/FVC$) reinforce.

  • The advent of digital media has facilitated easy access for adolescents to environments conducive to the purchase of narcotics. In particular, there's an increasing trend in the purchase and consumption of narcotics mediated through Social Network Services (SNS) and messenger services. Adolescents, sensitive to such environments, are at risk of experiencing neurological and mental health issues due to narcotic addiction, increasing their exposure to criminal activities, hence necessitating national-level management and support. Consequently, the quest for sustainable treatment methods for adolescents exposed to narcotics emerges as a critical challenge. In the context of high relapse rates in narcotic addiction, the necessity for cost-effective and user-friendly treatment programs is emphasized. This study conducts a literature review aimed at utilizing digital platforms to create an environment where adolescents can voluntarily participate, focusing on the development of therapeutic content through art. Specifically, it reviews societal perceptions and treatment statuses of adolescent drug addiction, analyzes the impact of narcotic addiction on adolescent brain activity and cognitive function degradation, and explores approaches for developing digital therapeutics to promote the rehabilitation of the addicted brain through analysis of precedential case studies. Moreover, the study investigates the benefits that the integration of digital therapeutic approaches and art therapy can provide in the treatment process and proposes the possibility of enhancing therapeutic effects through various treatment programs such as drama therapy, music therapy, and art therapy. The application of art therapy methods is anticipated to offer positive effects in terms of tool expansion, diversification of expression, data acquisition, and motivation. Through such approaches, an enhancement in the effectiveness of treatments for adolescent narcotic addiction is anticipated. Overall, this study undertakes foundational research for the development of digital therapeutics and related applications, offering economically viable and sustainable treatment options in consideration of the societal context of adolescent narcotic addiction.

The Influence of Musical Activities on Social and Emotional Behavior of Infants (음악활동이 영아의 사회·정서적 행동에 미치는 영향 - 만 2세(25-36개월) 영아를 중심으로)

  • The purpose of this study was to verify the influence of musical activities on social and emotional behavior of infants, through providing musical activities to the infants who were cared in a nursery and observing the interactions between peer group, or a therapist and an infant derived during them. The subject is 24 infants who are under 2 years of age(25~36months) at two nurseries located in Bundang area, and 13 infants among them are randomly assigned as study group and 11 infants as control group. Pretest and posttest about social and emotional behavior are performed, and ITSEA developed by Brggs-Gowan and Carter(2001) and amended by Shin Ji Yeon(2004) was used as an evaluation tool. Infants' changes of interaction behaviors during musical play were also analyzed quantitatively and qualitatively, and for the analysis, infant's social play behavior examination tool developed by Holloway and Reichhart-Erickson(1988) was adopted. Based on time sampling method, each item of this tool was evaluated. Each session was performed for 15 minutes, and 60 times of analysis about interactions per session was conducted at every 15 seconds. The analysis result was showed with a table and a graph, and described qualitatively about behavior changes. When compared social and emotional positive behavior average figures and negative behavior average figures between study group and control group, this study showed that the positive behavior figure of study group was increased and the negative behavior figure was decreased. While concentration and empathy among positive behaviors increased meaningfully, aggression, defiance, separation anxiety and rejection to new things among negative activities also decreased meaningfully. The conclusion of this study is as follows. First, interactions with peers or a therapist based on music and musical experience make an effect on strengthening positive behavior among social and emotional behavior and decreasing negative behavior. Second, music has influence on negative behaviors more than positive behaviors of an infant, and produces a good effect on sub behaviors of negative behaviors specially.

The Changes of the Heart Rate, Hand Function, and Health related Quality of Life of the People with Intellectual Disability through the Increase of the Auditory Stimulation (청각자극 증가에 따른 지적장애인의 심박수와 손 기능, 건강관련 삶의 질의 변화)

  • The purpose of the study is to analyze the changes of the heart rate, hand function, and health related quality of life of the people with intellectual disability through the increase of the auditory stimulation. The subjects were 16 people with intellectual disability. The increase of the auditory stimulation was modulated through the music sound and the intensity was consisted of 60dB, 65dB and 70dB. The wist heart rate and blood pressure monitor was used to measure the heart rate during the application of the auditory stimulation increase. The hand function of right and left hand was measured by the Purdue pegboard. Health related of quality of life was measured by the short form-8 health survey. As the results, the increase of the heart rate was showed by the increase of the auditory stimulation and the decrease of the hand function and health related quality of life was showed. For these, the increase of the auditory stimulation acts as a psychological distress, thereby the increase of the heart rate and the reduction of the hand function and health relate quality of life showed. Thus, to decrease of the heart rate and increase the hand function and the health related quality of life of the people with intellectual disability, the intensity of the auditory stimulation should be considered in the environmental stimulation.
