Radiosensitivity Enhancement by Arsenic Trioxide in Conjunction with Hyperthermia in the EC-1 Esophageal Carcinoma Cell Line

  • Objective: To explore the effect on radiosensitivity of arsenic trioxide ($As_20_3$) in conjunction with hyperthermia on the esophageal carcinoma EC-1 cell line. Method: Inhibition of EC-1 cell proliferation at different concentrations of $As_20_3$ was assessed using the methyl thiazolyl blue colorimetric method (MTT method), with calculation of $IC_{50}$ value and choice of 20% of the $IC_{50}$ as the experimental drug concentration. Blank control, $As_20_3$, hyperthermia, radiotherapy group, $As_20_3$ + hyperthermia, $As_20_3$ + radiotherapy, hyperthermia + radiotherapy and $As_20_3$ + hyperthermia + radiotherapy groups were established, and the cell survival fraction (SF) was calculated from flat panel colony forming analysis, and fitted by the 'multitarget click mathematical model'. Flow cytometry (FCM) was used to detect changes in cell apoptosis and the cell cycle. Results: $As_20_3$ exerted inhibitory effects on proliferation of esophageal carcinoma EC-1 cells, with an $IC_{50}$ of 18.7 ${\mu}mol/L$. After joint therapy of $As_20_3$ + hyperthermia + radiotherapy, the results of FCM showed that cells could be arrested in the $G_2$/M phase, and as the ratio of cells in $G_0/G_1$ and S phases decreased, cell death became more pronounced. Conclusion: $As_20_3$ and hyperthermia exert radiosensitivity effects on esophageal carcinoma EC-1 cells, with synergy in combination. Mechanistically, $As_20_3$ and hyperthermia mainly influence the cell cycle distribution of EC-1 esophageal carcinoma cells, decreasing the repair of sublethal damage and inducing apoptosis, thereby enhancing the killing effects of radioactive rays.

임펄스 잡음 환경 하에서 FNOM와 MD를 이용한 새로운 시지연 및 시간-주파수 지연 복합 추정 방법 (New Methods for Estimation of Time Delay and Time-Frequency Delay in Impulsive NOise Environment Using FNOM and MD Criterion)

  • 본 논문에서는 임펄스 잡음이 부가되는 환경 하에서 시지연 추정 및 시지연과 주파수 지연 목합 추정을 위한 새로운 강건 알고리즘들을 제안하였다. 제안된 방법은 ${\alpha}-stable$ 분포 이론을 바탕으로 하여 음의 분수 차수 모멘트(FNOM: fractional negative order moment)와 최소 산란(MD : minimum dispersion) 법을 이용한 강건 시지연 추정법들과 이를 응용한 시지연과 주파수 지연의 복합 추정법인 음의 분수 차수 모호함수법과 복합 최소 산란 법으로 구성되었다. 제안한 방법들을 평가하기 위하여 컴퓨터 모의 실험을 통해서 실제 응용 능력을 기존의 여러 방법들과 비교. 검토하였다. 실험 결과 2차 모멘트 이론을 바탕으로 한 관례적인 방법들은 가우시안 잡음 환경 (${\alpha}$=2인 $S{\alpha}S$ 잡음) 하에서만 시지연 추정이 가능하였고, 최근 제안된 Nikias[7]의 강건 시지연 추정법 들도 대부분이 제한된 임펄스 잡음 ($1<{\alpha}{\le}2$$S{\alpha}S$ 잡음)에 대해서만 그 성능이 입증된 반면, 본 연구에서 제안한 방법들은 가우시안 잡음 및 심한 임펄스 잡음 ($0<{\alpha}{\le}2$$S{\alpha}S$ 잡음) 하에서도 강건하게 시지연 추정이 가능함을 입증하였다.

후두 악성종양에 대한 내시경적 레이저 수술의 적용 (Endoscopic Laser Surgery in Laryngeal Carcinoma)

  • Background and Objectives: To determine if laser endoscopic microsurgery is a reliable and appropriate approach in the treatment of laryngeal carcinomas. Materials and Methods: Retrospective study of 62 patients treated with CO2 laser from June 1988 to November 2000 at Seoul National University Hospital for laryngeal squamous cell carcinoma. All patients were treated with curative intention. Fifty three untreated patients with laryngeal carcinoma (39 glottic and 14 supraglottic carcinoma patients) had primary carbon dioxide laser microsurgery. Nine radiation failure patients were treated. Postoperative radiotheray was done for 17 patients. Neck dissection was performed simultaneously for 4 supraglottic cases with cervical nodal metastasis. Mean follow-up duration was 40 months. Results: In primary laser surgery group, distribution of tumors (American Joint Committee on Cancer, 1997) were 38 cases with Tl, 13 cases with T2, 2 cases with T3. Cure rate was 88.7%(47/53) and local control rate was 92.5%(49/53). Larynx was preserved in 94%(50/53) of patients. The overall 5-year survival rate(Kaplan-Meier) was 81.5%. In radiation failure group, 56% of patients were recurred after laser surgery. Conclusion: Laser surgery could be a better treatment modality for early laryngeal cancers and selected advanced cases. Additional radiation therapy should be considered if resection margin is not satisfactory.

Inflammatory Breast Cancer: a Single Centre Analysis

  • Background: Inflammatory breast cancer (IBC) is an aggressive form of locally advanced breast cancer characterized by rapidly progressive breast erythema, pain and tenderness, oedema and paeu d'orange appearance. It accounts for 1-3% of all newly diagnosed cases of breast cancer in the west. Data on IBC from India are lacking. The aim of our study was to assess the clinical-pathological parameters and outcome of IBC at, All India Institute of Medical Sciences, a large tertiary care centre. Materials and Methods: We screened 3,650 breast cancer cases registered from January 2004 to December 2012 and found 41 cases of IBC. Data included demographics as well as clinical, radiological and histopathological characteristics, and were collected from clinical case records using the International Classification of Diseases code (C-50). Patients who presented with IBC as a recurrence, or who had a neglected and advanced breast cancer that simulated an IBC were excluded from this study. Results: The median age was 45 years (range 23-66). The median duration of symptoms was 5 months. The American Joint Committee on Cancer stage (AJCC) distribution was Stage III - 26 and IV - 15 patients. Estrogen receptor (ER), progesterone receptor (PR) positivity and human epidermal growth factor receptor 2 (HER2/neu) positivity were 50%, 46% and 60%, respectively. Triple negativity was found in 15% of the cases. All the non metastatic IBC patients received anthracycline and/ or taxane based chemotherapy followed by modified radical mastectomy, radiotherapy and hormonal therapy as indicated. Pathological complete remission rate was 15%. At a median follow-up of 30 months, the 3 year relapse free survival and overall survival were 30% and 40%respectively. Conclusion: IBC constituted 1.1% of all breast cancer patients at our centre. One third of these had metastatic disease at presentation. Hormone positivity and Her2 neu positivity were found in 50% and 60% of the cases, respectively.

재난 공동 대응을 위한 지자체 컨소시엄 구성 방안 (Consortium Configuration of Local Governments for Disaster Joint Response Plan)

  • 최근 자연재해의 발생빈도가 높아지고 그 규모가 점차 대형화되고 있어, 방재에 대한 관심이 높아지고 있다. 우리나라의 대표적인 자연재해는 태풍과 호우로, 강도가 증가하는 추세에 있으며 이에 따라 매년 발생하는 피해의 규모도 커지고 있다. 자연재해 피해발생시 지자체가 상시적으로 보유하고 있는 방재자원으로는 그 피해 규모를 감당하기 어려운 실정이다. 따라서 대규모 재난발생시 피해 특성에 맞는 장비와 인력을 신속하게 동원, 배분할 수 있도록 지역 특성에 맞는 방재자원 공동 활용체계를 구축할 필요가 있다. 본 논문에서는 전국 각 지역의 특성을 분석하고 이에 따라 '재난 공동 대응을 위한 지자체 컨소시엄 구성 방안'을 제안하였다. 먼저, GIS를 이용하여 2002년에서 2011년까지의 재해연보에 수록된 지자체별 피해액을 나타낸 재해지도를 제작하였다. 다음, 전국을 행정구역에 따라 크게 5개 권역으로 구분하고, 각 권역의 지형적, 인문적 특성 및 태풍과 호우에 의한 피해 규모를 분석하여 다시 12개의 클러스터로 세분하였다. 마지막으로 각 권역의 재난대책거점을 선정하였다. 본 연구의 피해 분석자료는 방재대책 수립시 참고자료로 활용될 수 있을 것이다.

슬관절 통증 환자의 한의학적 치료 호전도와 BMI 와의 상관성 (The Correlation between Korean Medical Treatment on Knee Joint Pain and BMI)

  • Objectives : This study was designed to assess the general distribution, clinical effectiveness of Korean medical treatment on knee pain and the correlation between Korean medical therapy on knee pain and BMI. Methods : This is an observational study. 65 patients admitted to Daejeon Jaseng Hospital of Korean Medicine with musculoskeletal disorders were observed from July, 2014 to July, 2015. They were analyzed according to sex, age, pain lesion, body mass index(BMI) and treatment efficacy. All patients received a combination of treatments during hospitalization, including acupuncture, pharmacopuncture, herbal medicines and physical therapy. A zero to ten numerating rating scale (NRS) assessing pain, Western Ontario and McMaster Universities Arthritis Index(WOMAC) index and Range of Motion(ROM) was used before and after treatments. Statistical correlations among assessment measurements were evaluated by examining the paired t-test and the Pearson's correlation coefficients. Results : The average BMI of all patients was $24.03{\pm}2.95kg/m^2$. According to clinical definition of obesity by Korean Society for the Study of Obesity, 33.8% of all patients was within normal weight, 1.5% was underweight, 33.8% was overweight, 24.6% was obesity and 6.2% was morbid obesity. For knee pain patients, NRS (Numeric Rating Scale) decreased from $6.28{\pm}1.63$ to $3.94{\pm}2.03$(p<0.001). WOMAC index decreased from $48.14{\pm}17.63$ to $40.37{\pm}18.28$(p<0.001). But, there were no significant correlations in statistics among BMI, knee pain index and knee pain index improvement. Conclusions : Korean medical combination treatment might be effective in reducing pain and improving functional disorders for patients with knee pain. This study further confirmed the efficacy of Korean medical treatment on knee pain. But, more studies on correlation between knee pain and obesity and development of assessment measurement are needed.

상지이용유무에 따른 현대무용 아라베스크 턴 동작의 운동역학적 분석 (The Kinetic Analysis of Arabesque Turn Motion in Modern Dance by Upper Extremity Usage)

  • 본 연구는 현대무용 아라베스크 동작 시 팔의 사용 유.무에 따라 아라베스크 동작 바로직후의 턴으로 이어지는 동작에 영향을 미치는 운동역학적 변인을 분석하여 무용 동작에 대한 과학적 근거를 마련하고자 하는 연구의 목적이 있다. 상지를 이용하지 않은 아라베스크 턴 동작이 상지를 이용한 아라베스크 턴 동작 보다 머리의 회전력과 몸통의 회전력을 더 사용 하였고, 오른쪽 어깨를 이용한 회전력을 얻었다. 상지를 이용한 아라베스크 턴 동작은 왼쪽의 고관절 범위와 왼쪽 발끝의 위치변화가 수직축으로 크게 상승되어 있고, 상지를 이용하지 않는 아라베스크 턴 동작은 발끝의 위치가 이벤트별 아래로 떨어졌다. 신체중심 변위에서는 상지를 이용한 아라베스크 턴 동작이 회전하는 축으로 더 크게 이동하였고, 상지를 이용하지 않은 아라베스크 턴 동작의 신체중심이 낮게 위치하였다. 또한 최대 수직지면반력의 결과 상지를 이용한 아라베스크 턴 동작이 상지를 이용하지 않은 동작보다 낮은 값으로 나타나, 상지를 이용하지 않은 아라베스크 턴 동작이 상지를 이용한 동작보다 하지의 체중부하를 더 사용한다는 것으로 나타났다.

APPLICATION OF FINITE ELEMENT ANALYSIS TO EVALUATE IMPLANT FRACTURES

  • Statement of problem. Higher fracture rates were reported for Branemark implants placed in the maxilla and for 3.75 mm diameter implants installed in the posterior region. Purpose. The purpose of this study was to investigate the fracture of a fixture by finite element analysis and to compare different diameter of fixtures according to the level of alveolar bone resorption. Material and Methods. The single implant and prosthesis was modeled in accordance with the geometric designs for the 3i implant systems. Models were processed by the software programs HyperMesh and ANSA. Three-dimensional finite element models were developed for; (1) a regular titanium implant 3.75 mm in diameter and 13 mm in length (2) a regular titanium implant 4.0 mm in diameter and 13 mm in length (3) a wide titanium implant 5.0 mm in diameter and 13 mm in length each with a cementation type abutment and titanium alloy screw. The abutment screws were subjected to a tightening torque of 30 Ncm. The amount of preload was hypothesized as 650 N, and round and flat type prostheses were 12 mm in diameter, 9 mm in height were loaded to 600 N. Four loading offset points (0, 2, 4, and 6 mm from the center of the implants) were evaluated. To evaluate fixture fracture by alveolar bone resorption, we investigated the stress distribution of the fixtures according to different alveola. bone loss levels (0, 1.5, 3.5, and 5.0 mm of alveolar bone loss). Using these 12 models (four degrees of bone loss and three implant diameters), the effects of load-ing offset, the effect of alveolar bone resorption and the size of fixtures were evaluated. The PAM-CRASH 2G simulation software was used for analysis of stress. The PAM-VIEW and HyperView programs were used for post processing. Results. The results from our experiment are as follows: 1. Preload maintains implant-abutment joint stability within a limited offset point against occlusal force. 2. Von Mises stress of the implant, abutment screw, abutment, and bone was decreased with in-creasing of the implant diameter. 3. With severe advancing of alveolar bone resorption, fracture of the 3.75 and the 4.0 mm diameter implant was possible. 4. With increasing of bending stress by loading offset, fracture of the abutment screw was possible.

상관결과의 왜곡 방지를 위한 재양자화 방법에 관한 연구 (A STUDY ON THE RE-QUANTIZATION METHOD FOR PREVENTING DISTORTION OF CORRELATION RESULT)

  • In this paper, we propose a new re-quantization method after FFT processing to prevent the distortion of correlation result of VCS (VLBI Correlation Subsystem). The re-quantization is used to rearrange the data bit so as to reduce the data rate processed as 16-bit of FFT result of VCS. Having done this procedure, we found that the distorted spectrum of correlation result occurred in the delay tracking experiments by the re-quantization method introduced for initial design of VCS. In order to solve this, two kinds of re-quantization method, that is, the comparison and selection-type, are proposed. The first is to re-quantize the FFT result as a valid-bit by comparing with the input data after determining the adequate threshold. The second is manually to select the valid-bit of FFT result after finding the valid-field of data according to the bit-distribution of input data. We confirmed that the second is more effective compared with the first through the experimental result, and it will be implemented without so much modification of applied method in the condition of the limited resource of FPGA. The re-quantization is, however, carried out with 4-bit in the proposed second method for FFT result, and then the distortion of correlation result is also appeared. To fix this problem, the bit for re-quantization is extended to 8-bit. The proposed 8-bit selection-type is effectively verified so that the distortion of correlation result disappeared by applying to VCS in consequence of the simulation and correlation experiments.

지반조사를 위한 loop-loop 전자탐사 기법의 적용 (An Application of loop-loop EM Method for Geotechnical Survey)

  • 토목이나 건설 그리고 환경 분야에서 요구하는 지반 조사에 대한 공학적인 적용을 위하여 주파수 영역 루프-루프 전자탐사 연구를 수행하였다. 이를 위하여 송신 루프와 수신 루프가 동일면을 이루는 수평동일면 배열과 수직동일면 배열을 이용하였고, 각 배열에 대해 세 개의 이상성분을 측정 자료로 하여 영상을 구성하였다. 본 연구에서의 모형 반응 계산은 확장된 Born 근사를 이용한 2.5차원 적분방정식을 이용하였고, 역산 과정에서 분해능의 향상을 위해 ACB(Active Constraint Balancing)를 채택하였다. 모형 반응을 통한 1차원 및 2차원 역산 알고리듬을 적용시켜 본 결과, 층서구조에서는 비교적 1층의 전기전도도가 높을수록 모델 변수의 추정이 용이하였으며, 2차원 고립 이상체에 대한 역산 결과는 전도성 이상체와 비전도성 이상체의 위치를 잘 확인할 수 있었다. 또한 VCP배열 자료의 역산 결과보다 HCP배열 자료의 역산 결과에서 보다 나은 해상도를 보였으며, HCP배열과 VCP배열의 자료를 동시에 역산하였을 경우에 분해능의 향상을 확인하였다. 실제 루프-루프 전자탐사의 현장 자료 획득을 위해서 캐나다 Geonics사의 전자탐사 장비 EM34-3XL을 이용하였으며, 전기비저항 탐사 자료와 동일 측선상에서 비교 분석하였다. 역산 결과, 1차원 역산 보다는 2차원 역산 알고리듬을 이용하여 구성한 영상에서 전기비저항 탐사 결과와 매우 유사한 만족할만한 전기비저항 분포를 확인할 수 있었다. 따라서 현장에서의 개략적인 지반 조사를 위한 루프-루프 전자탐사의 응용이 기대된다.
