A Case Study on the Potential Severity Assessment for Incident Investigation in the Shipbuilding Industry (잠재 심각도 평가 기반의 조선업 재해 분석 활동에 관한 사례 연구)

  • Korean shipbuilding companies have taken many efforts for safety over the years by developing Health, Safety & Environment (HSE) Management Systems, Procedures, Training, and studying Programs for prevention of incidents. As a result, the shipbuilding industry has succeeded in reducing overall injury rates. Nevertheless, the industry also noticed that incident rates are still not at zero and more importantly, serious injuries and fatalities are still occurring. One factor that may be attributing to this is the lack of managing potential severity during incident investigations, most incident investigations are implemented based on the actual result. Generally, each shipbuilding company develops their customized incident investigation programs and these are also commonly being focused on actual result. This study aimed to develop a shift in strategy toward safety to classify the criteria of potential severity from any incidents and manage that to prevent any recurrence or causing any serious injuries or fatalities in the shipbuilding industry. Several global energy companies have already developed potential severity management tools and applied them in their incident investigations. In order to verify the necessity of improvement for current systems, a case study and comparative analysis between a domestic shipbuilding company and several global energy companies from foreign countries was implemented and comparison of two incident investigation cases from specific offshore projects was conducted to measure the value of a potential severity system. Also, a checklist was established from the data of fatalities and serious injuries in recent 5 years that occurred in Korea shipbuilding industry and a proposal to verify high potential incidents in the incident investigation process and comparative analysis between the assessment by appling proposed checklist and the assessment from a global energy company by using their own system was implemented. As a measure to prevent any incidents, it is required to focus on potential severity assessment during the incident investigation rather than to only control actual result. Hence, this study aims to propose a realistic plan which enables to improve the existing practices of incident investigation and control in the shipbuilding industry.

  • Incident investigation is regarded as a means to improve safety performance. For the prevention of industrial accidents, measures such as providing safety education, enhancing management interest and participation, establishing a safety management system, and conducting inspection of the work site are necessary. In particular, accident investigation activities, which are an important element of safety management, help to prevent similar accidents, thereby minimizing damage and enhancing work safety. They are critical for understanding business-related incidents and the vulnerabilities and opportunities associated with them. Therefore, it is clear that accident investigation activities are important for accident prevention. The primary focus of many incident investigation processes is on identifying the cause of an event. While considerable research has been conducted on potential accident investigation tools there has been little research on including the views and experiences of practitioners in the accident investigation process. In this study, a questionnaire survey was conducted among safety managers in the domestic manufacturing/construction industry to understand the practice of accident investigation. The investigation pertained to companies' accident investigation systems, the competence of investigators, and the identification and recommendations of the cause of accidents. From the analysis results of accident investigations, investigators' competence, the difficulty level of investigations, and the root causes of accidents were identified from the viewpoint of the participants of the accident investigations. In particular, the development of standardized and simple accident investigation methods and their dissemination to companies were found to be necessary for activating the root cause of accidents. Based on this, it can be used as basic data for the development of root cause analysis investigation techniques that are easily applicable to organizations.

  • Embezzlement incident of staff at the casino occurred. Staff of some casinos have lower job satisfaction, moral gap is seriously considering a change jobs. In addition, cash lure of large amounts and a lack of money management system causes embezzlement incident. In this paper, the uniqueness of the casino industry and that employee job satisfaction is investigated. Content analysis of occurrence for casino embezzlement incident and tracking that bank account and bank check, suspect's call list, and so on that digital forensic investigation technology will be studied. Problems and solutions suggest that conducted a loss prevention program, a digital forensics technology and introduce of investigator. Through this study, the computerization of the casino business to embezzlement accident prevent will contribute to that give back profits of property to society, the develop of forensic investigation technology.

  • ICAO Universal Safety Oversight Audit Programme is a comprehensive evaluation for aviation safety matters, including aircraft operation, licensing and aircraft incident/accident investigation. As this programme is measures aviation safety level of States, there is a need to meet international standards, especially in the field of aircraft incident/accident investigation. This research discusses points required to satisfy international standards, which are independence and autonomy of an accident investigation board, the lack of the enough number of investigators and efficient management of those personnel, the effective composition and use of an investigation report, an accident investigation itself to build big data and the creation of a cooperation system among States. This paper suggests various opinions in order to enhance aviation safety especially through aircraft accident investigations, using case studies from the US, UK and Japan.

  • Purpose: Natural Disaster has well formed framework more than social disaster, because natural disaster is controlled by one department, such as MOIS, but social disaster is distributed. This study is on the design of the integrated service platform for the social diaster data. and then, apply to the local governments. Method: Firstly, we design DB templates for the incident cases considering the incident investigation reports. For the risk management, life-damage oriented social disaster risk assessment is defined. In case of the real-time incident data from NDMS, AI system provides the prediction information in the life damage and the cause of the incident. Result: We design the structured and unstructured incident data management system, and design the integrated social disaster and safety incident management system. Conclusion: The integrated social disaster and safety incident management system may be used in the local governments

  • North Korea's discharge of water from a dam into the Imjin River that flows through the inter-Korean border swept away a riverside camping site early Sunday morning, killing six people. This tragic incident might have been prevented if the North had given prior notice of the release from its Hwanggang Dam to the South. Investigations are under way to determine the reasons behind the unexpected act. This incident was a man-made disaster not least death of six people. A water level monitoring system(WLMS) of the Imjin River was installed the bridge of Pilseung that operate three public institutions. The WLMS of the A institution set up warning siren and broadcasting as the water level has been rising the bridge of Pilseung in the Imjin River. But the A institution's system was already out of side before discharged of water a dam into the Imjin River and the operators were culpable negligence. The B institution's office employee on charges of negligence that might have contributed to the tragedy and one of the A institution's employees ignored 26 warning messages on the WLMS. This tragic incident was a man-made disaster not least death of six people and might have been prevented if the WLMS was normally operated and the system operators must be worked a tight.

  • Standard guidelines for marine accident investigation have been prepared through the enforcement of the Casualty Investigation code as of January 2010. In addition, as the International Maritime Organization (IMO) recommended contracting the state to manage a marine incident system established under this code, Korea also has newly established provisions for a marine incident system in the 'Act on the Investigation of and Inquiry into Marine Accidents' also as of 2010. The Korean Maritime Safety Tribunal (KMST) has made a multilateral effort to prevent marine accidents through the efficient operation of a marine incident system, but this system has not been properly activated. This study examines the operational status and problems of a marine incident system and analyzes the marine incident systems of foreign countries and similar transportation agencies such as railroads and aviation. Options include switching to voluntary reporting of marine incidents, transferring responsibility to a non judicial private organization, expanding incentive systems for a marine incidents, revising regulation and preparing detailed implementation guidelines.

  • It is necessary to analyze the railway accidents and incidents for the purpose of understanding present safety state and enhancing its system. Korea National Railroad has its accident/incident reporting codes, but it is relatively not sufficient for detail classification and investigation of accident and incident compared with foreign countries. This paper suggests how to classify the railway accident/incident, and describes the analysis result for domestic recent railway accidents and incidents according to the new suggested classifying system.

  • A numerical investigation was made to examine characteristics of rectangular pontoon type floating breakwaters in oblique waves. Sway and heave wave exciting forces, roll moment acting on the floating breakwater and three motion reponses decrease as the incident wave angle increases for the most of the wave ranges. There exists a minimum wave transmission coefficient which is a function of wave frequency. In short wave range wave transmission coefficient increases as the incident wave angle increases. In long wave range, however, wave transmission coefficient decreases as the wave incident angle increases.

  • Comparison for various empirical models of normal incident absorption was made, along with experiments. Comparative result indicates that Voronina model which is function of fiber diameter and porosity is more suitable than the other models. An investigation for correlation between normal and random incident absorption was carried out by experiment and analysis. It appears that at the low frequencies, the random incident absorption is higher than the normal one, whileas at the high frequencies, the random incident absorption is decreased due to the effect of grazing incident components.
