Risk factors of fisher on stow net fishing vessel using analysis of adjudication (재결서 분석을 통한 안강망어선에 승선하는 선원의 위험요인)

  • The adjudication of Korean Maritime Safety Tribunal (KMST) was analyzed to collect basic data to identify the cause of the risk that did not appear in the current data provided by the fishermen's occupational accidents of the National Federation of Fisheries Cooperative (NFFC) in stow net fishing vessel from 2015 to 2019. The personnel's carelessness was the most common in 29 out of 33 accidents (87.9%), followed by 25 cases (75.8%) of inadequacy of instructions, 24 cases (72.7%) of inadequacy of education on hazard factor, 20 cases (60.6%) of no personal protection equipment, 18 cases (54.5%) of poor guard, 17 cases (51.5%) of inadequacy of work method, 16 cases (48.5%) of absence of emergency stop button, 14 cases (42.4%) of work practice of poor safety precautions that affected more than 40% of all accidents as accident causes. These causes had a strong influence on each other, and the ratio of accident causes is high. With this relationship, accidents can be prevented or the severity of human injury can be reduced if types of accident process can be estimated with a scenario, and the key points before the accident in the scenario are switched to safe points.

Study on Fatality Risk of Older Driver and Traffic Accident Cost (고령운전자 연령구간별 사망사고 발생위험도와 사고비용 분석 연구)

  • Korea is facing a surge in the aging population, showing that population aged 65 and above will be accounted for 42.5% of the total population in 2065 with the emphasis on the over-80 population consisting of 19.2%. In response to this abrupt change in population structure, the number of traffic fatality accident referring to older driver as aged 65+ years had been increasing from 605 fatalities in 2011 to 815 fatalities in 2015 resulting in increases in 34.7% in oppose to happening to decreases in 17.2% about non-older driver. With Logit analysis based on Newton-Raphson algorithm utilizing older driver's traffic fatality data for the 2011-2015 years, it was found that the likelihood of an accident resulting in a fatality for super older driver aged 80 years and above considerably increased compared to other older driver with aging classification: 2.24 times for violation of traffic lane, 2.04 times for violation of U-turn, 1.48 times for violation of safety distance, 1.35 times for violation of obstacle of passing; also average annual increase of traffic accident cost related to super older driver was fairly increased rather than other older driver groups. Hence, this study proposes that improving and amending transport safety system and Road Traffic Act for super older driver needs to be urgently in action about license management, safe driving education, etc. when considering the increase of over-80 population in the near future. Also, implementing a social agreement with all ages and social groups to apply with advanced driver assistance system for older driver groups will be able to become a critical factor to enhance safe driving over the face of the country.

  • Dae-gu subway accident raise whole points in connection with safety facilities and operating of national urban transit system like rolling stocks, facilities, management of human. Rolling stock is made every effort for improvement of performance, guarantee of comfortableness, insurance of economical efficiency. But Security like safety of fire is not thoroughgoing enough. Especially, interior material has used although it is not prove its degree of safety. it is a main cause of Dae-gu subway accident. Safety regulation of urban transit vehicle that legislate for security in March 2000 does not applied manufacturing vehicles before in 2000. It has be prescribed in the regulations that incombustibles must be used. But detailed test standard related with incombustibles is not prescribe. Thus that regulation be required reinforcement of detailed test standard. Main cause of Dae-Gu subway accident is a fire in vehicle. However, many defects are found in infrastructure and operating vehicle of urban transit, such as inexperienced disposal of driver and CCC in early stage of the fire accident, unskilled opening and closing doors, insufficient escape facilities and safety facilities of a station house and tunnel, and incomplete communication system between vehicle and CTC, extraordinary step. Thus the aims of this study are prevention of urban transit accident, improvement plan of safety driving, and proposal of quick action plan through analysis of total faculty of vehicle.

An Exploratory Structural Analysis of the Accident Causing Factors in Railway Traffic Controllers (철도관제사의 사고유발 요인에 관한 탐색적 구조분석)

  • This study intended to exploratively testify human error causing factors for railway traffic controller, using AMOS structural equation model. Through literature survey, fatigue and stress as exogenous variable, errors in information process such as cognitive, memory, storage, and execution error as endogenous variable, and accident and incident(near-miss) as dependent variable were set up. Results based on AMOS using 201 railway traffic controllers' questionnaire showed that a clear causality loop like as 'stress ${\rightarrow}$ memory error ${\rightarrow}$ storage error ${\rightarrow}$ incident(near-miss) ${\rightarrow}$ accident' is formed. This result suggests that for the purpose of mitigation of traffic controller's accident, it is so necessary to reduce memory and execution error in the information processing process based on the effective management of stress, as the precedent of them.

  • Positive effects such as significantly reducing traffic accidents caused by human error can be expected by the introduction of Automated vehicles (AV). However, as new traffic safety issues are expected to occur in the future due to errors in H/W or S/W of autonomous vehicles and lack of its function, it is necessary to establish a scenario to evaluate the driving safety of AV. Therefore, in this study, functional scenario was developed to evaluate the driving safety of AV based on traffic accident data of the National Police Agency. Using the GIS program, QGIS, traffic accident data that occurred in the toll gate and ramp sections of expressway were extracted and accident summary items were checked to classify the types of accident. In addition, based on the results of accident type classification, functional scenario were developed that contains various dangerous situations in the tollgate and ramp sections.

Feasibility Study on Cross-tie Systems in Nuclear Power Plants Using Multi-unit PSA (다수기 PSA를 활용한 원전 안전자원 공유 활용성 평가)

  • Following the accident at Fukushima, the true impact of multi-unit accidents came to light. Accordingly, research related to multi-unit accident effect analysis, risk evaluation, and accident prevention/prevention technology has been conducted. Specific examples are mobile/fixed equipment such as multi-barrier accident coping strategy (MACST) and diverse and flexible coping strategies (FLEX), which have been introduced and installed in multi-units for preventing and mitigating multi-unit accidents. These strategies are useful for enhancing the safety of nuclear power plants (NPPs); however, a more efficient strategy is required in terms of the costs of physical and human resources. To effectively and efficiently mitigate an increase in multi-unit accidents, it is necessary to not only to utilize mobile/fixed equipment but to also use crosstie options with resources that already exist at NPPs. Therefore, we analyzed the current international and domestic status of crosstie systems technology and propose a method to evaluate feasibility alongside risk based on a multi-unit probabilistic safety assessment (PSA). To analyze the international and domestic status of crosstie systems technology, actual cases and related research were studied, and a list of potential crosstie safety resources was derived. Additionally, a case study was performed on crosstie cases of two systems within the assumed six units on-site under a multi-unit accident, and a multi-unit PSA-based risk evaluation method is proposed.

Concept and characteristics of safety information design that reflects human characteristics (인적 특성을 반영한 안전 정보디자인의 개념과 특징)

  • In design studies, there has been extensive prior research on accident and disaster prevention, but it mainly focuses on visualization methods to improve visibility. Most accidents and disasters are caused by human factors. However, there is little design research that considers human characteristics that manifest in dangerous situations. This study aims to identify the concepts and characteristics of safety information design that reflects human traits. It examines the concepts of risk and safety from a social science perspective and looks into human visual information processing from a cognitive neuroscience perspective. Additionally, it explores the relationship between human information processing and accident rates in dangerous situations from a psychological perspective. Based on these analyses, the study distinguishes between conventional information design and safety information design, ultimately identifying the features of safety information design that incorporates human traits. The key findings are as follows: First, information should be presented considering the human tendency to suspend rational judgment in dangerous situations. Second, appropriately dispersing or concentrating attention according to the level of risk can serve as an opportunity to minimize harm. Third, it was found that sensory and perceptual characteristics should be given top priority in the field of risk and safety information design. Through these findings, the study concludes that a design approach that reflects human traits in safety information design can ultimately be a key indicator for improving safety levels.

  • The problem of qualitative reliability system is very important issue in the digitalized nuclear power plant, because the failure of its system brings about extravagant economic loss, extensive environment destruction, and fatal damage of human. Therefore this study is to develop the reliability evaluation model through the normalized scoring model by the quantitative and qualitative factors considering the advanced safety factors In the Advanced Pressurized water Reactor 1400MWe(APR 1400) under uncertainty Especially, the qualitative factors considering the information and human factors for the systematic and rational justification have been closely analyzed. The reliability evaluation model can be simply applied in real fields in order to minimize the industrial accident and human error in the digitalized nuclear power plant.

  • The Human Factors Analysis and Classification System (HFACS) is a general human error framework originally developed and tested within the U.S. military as a tool for investigating and analyzing the human causes of aviation accidents. Based upon Reason's (1990) model of latent and active failures, HFACS addresses human error at all levels of the system, including the condition of aircrew and organizational factors. As a result, this study aims to examine the influence between the latent conditions based on HFACS. This study seeks to verify the factors of "Organizational Influence" effecting the "Precondition for Unsafe Acts" of HFACS. The results of empirical analysis demonstrated that the organizational influence had a positive influence on precondition for unsafe act, especially the "Organizational Climate" of organizational influence had even greater influence on precondition for unsafe acts.

  • Railway system is a word that includes rolling stock, railway infrastructure and signalling System in passenger transfer and rail freight. This system get higher competitiveness in energy efficiency part than other transfer system. But it is comprehensive system has latent loss of lives and property by big accident. First of all, to control this comprehensive system and to confirm safety of it, we need human resource management. This research will suggest how to apply efficiency safety verification and certification system to railway system. human resource management systems is defined as three kinds that is recruitment and management, ensure personnel's competencies and fitness, activity of assigning suitable responsible person for work.
