중대재해 사례에 기반한 건설업의 작업 및 위험분류체계 통합 프레임워크 개발 (Development of Framework for Integrated Work-Risk Breakdown Structurebased on Fatal Incident Cases in Construction Industry)

  • 건설업에서의 재해는 수십 년 동안 다른 산업보다 많이 발생하였기 때문에, 건설업에서의 재해는 반드시 줄여야 한다. 이러한 문제를 해결하기 위해서 작업분류체계(WBS) 및 위험분류체계(RBS)를 제시하였다. WBS와 RBS는 계층적 구조로 작업 및 위험 단위를 쉽고, 빠르게 찾을 수 있다. 그래서 본 연구에서는 건설업에서의 재해를 예방하기 위해 통합 WBS-RBS 프레임워크를 개발하고자 한다. 연구의 순서 다음과 같은 3단계로 진행되었다. ① 데이터 수집 ② 데이터 분류 ③ 통합 WBS-RBS 프레임워크 개발 순으로 진행되었다. 연구의 결과 가장 사고가 많이 발생한 건물용도, 건설공종 및 재해요인을 제시할 수 있었다. 연구 결과를 통해 의사결정자는 건물용도, 건설공종, 및 재해요인의 위험 수준을 고려할 수 있다.

사망재해의 원인분석에 관한 연구 (A Study on Analysis of Causes for Fatal Accidents)

  • This study is conducted to analyze exact causes of occupational fatalities and results are as follows : 1) The number of fatalities in establislment which employed less than 50 workers is 605(40.1% ). Age of sixties and seventies is occupied by 10 percent with there being old-aged trend. Fatal workers who were employed less than 3 months are 39.5 percent and fatal workers who were employed more than 10 years are 11.9 percent. Safety training is necessary for newly recruited and long employed workers. 2) Accident type is, in sequence, fall from elevation, traffic accident, disease, contact with electric current, and hazardous condition is, in order, inadequate guard, hazardous method or procedure, public hazard, and agency of accident is, in sequence, temporary structure/building/structure, powered transport vehicle, passenger vehicle, hoisting or lifting apparatus, and unsafe act is, in order, driving error, failure to secure or warn. 3) The results of cross-tabulation for hazardous condition and accident type is, in sequence, traffic accident by public hazard, fall from elevation by hazardous method or procedure, fall from elevation as well as contact with elctric current by inadequate guard. The result of cross-tabulation for agency of accident and accident type is, in order, fall from elevation by temporary structure/building/structure, traffic accident of passenger vehicle and powered transport vehicle, contact with electric apparatus. The result of cross-tabulation for agency of accident and hazardous condition is, in sequence, public hazard of passenger vehicle, hazardous method or procedure of temporary structure/building/structure, publit hazard of powered transport vehicle, inadequate guard of temporary structure/building/structure. The result of cross-tabulation for unsafe act and accident type is, in sequence, traffic accident by driving error, fall from elevation by inattention to footing or surrounding, contact with electric current by secure or warn.

말비계 건설재해 특성 분석에 관한 연구 (A study on characteristics of accidents involving horse scaffolds at construction sites)

  • Many fatal and non-fatal injuries occurred as a result of unexpected falls from horse scaffolds during structural or finishing works such as painting, plastering, or plumbing. Therefore, this study is to improve safety standards for horse scaffolds used mostly in the interior finishing works. But hazard elements for the horse scaffolds has not been specifically identified. This study has analyzed the accidents of horse scaffolds to prevent construction accidents and the characteristics of a serious accidents related the horse scaffolds. It was suggested a safety management plan and a group of accidents characteristics. The results of this study are expected to be utilized as basic data for prviding a reference related to the horse scaffolds and to be unified the regulations associated with the horse scaffolds.

전로사고 예방을 위한 인적오류 분석 (A Case Study on the Human Error Analysis for the Prevention of Converter Furnace Accidents)

  • Occupational fatal injury rate per 10,000 population of Korea is still higher among the OECD member countries. To prevent fatal injuries, the causes of accidents including human error should be analyzed and then appropriate countermeasures should be established. There was an severe converter furnace accident resulting in five people death by chocking in 2013. Although the accident type of the furnace accident was suffocation, many safety problems were included before reaching the death of suffocation. If the safety problems are reviewed throughly, the alternative measures based on the review would be very useful in preventing similar accidents. In this study, we investigated the converter furnace accident by using human error analysis and accident scenario analysis. As a result, it was found that the accident was caused by some human errors, inappropriate task sequence and lack of control in coordinating work by several subordinating companies. From the review of this case, the followings are suggested: First, systematic human error analysis should be included in the investigation of fatal injury accidents. Second, multi man-machine accident scenario analyis is useful in most of coordinating work. Third, the more provision of information on system state will lessen human errors. Fourth, the coordinating control in safety should be performed in the work conducting by several different companies.

지게차 사망재해의 인적오류에 대한 대안 (Alternative Prevention on Human Error of Fatal Injuries by the Folk Lifts)

  • According to the statistics, occupational fatal injuries by the fork lifts were about 30 per year in whole industrial. Fork lifts are widely used in various parts of industries to improve the efficiency of the work. In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by the fork lifts. Fatal injury analysis were conducted with several accident cases by the fork lifts. For each accident, the causes of the injuries were examined and proper safety measures were proposed. In this study, the fork lift showed a high fatality rate in industrial accidents and no detailed cause analysis of fatal accidents was conducted in terms of unsafe acts or conditions. First, fork lifts were the highest of the machines caused the accidents. In order to prevent fatal injuries by the fork lifts, the tarket was manufacturing industry. Second, the order of the cause of cognitive engineering agenda in the manufacture industrial was visibility, responsibility and affordance, and revision of acts was proposed. Third, there was not a lots of different points of human error between occurrence types and business sizes. Forth, number of fatalities by the attacker was more than by the inducer.

  • In order to reduce fatal accidents, a systematic safety management should be performed at a construction site. A lot of researches on direct causes of fatal accidents and indirect causes which affect the direct causes have been conducted for effective safety management. In the previous studies, these causes are considered separately because of the difficulty of recognition of indirect causes. It is also hard to measure the negative impact of these causes and notice their occurrence the perception of its problem. However, a study on identification of complicated relationship between these two types of causes is necessary since an accident occurs due to the chain reaction that indirect causes trigger the direct causes which lead to the fatal accidents. Therefore, this study proposes a safety management plan that considers indirect and direct causes of fatal accidents cases in the Korean construction industry. Then this paper suggests a plan of safety management by identifying major causes of the accidents.

  • 본 연구는 국내 건설공사에서 발생되었던 최근 5년(2002${\sim}$2006)간 한국산업안전공단의 중대재해조사목록을 대상으로, 건설 중대재해의 공사종류별 현황을 직종, 경력, 발생형태, 발생시점별로 분석하여 국내 건설 중대재해의 프로젝트 유형별 발생특성을 도출함으로써 건설공사 현장에서 중대재해에 대한 안전계획 수립 및 관리활동과 관련된 정책수립에 활용될 수 있는 기초자료를 제시한다. 본 연구를 통해 발견된 사실들은 인간존중의 인도주의적 차원뿐만 아니라, 경제적으로도 건설재해예방의 토대를 확립하는데 직 ${\cdot}$ 간접적 기여를 하게 될 것이다. 기존에 건설재해예방을 위한 국가적차원의 노력에도 불구하고 건설재해가 여전히 감소추세를 보이지 않고 있으나, 본 연구는 국내 건설재해예방과 관련된 정책의 유효성을 향상시키는데 기여할 수 있다.

철근콘크리트조 건축공사의 공종별 위험성 평가 (The Risk Assessment of Work Type in Reinforced Concrete Construction Work)

  • Construction work accidents have not decreased in spite of much effect such as new work methods education and research related to the safety management. Specially, many fatal accidents were mainly caused by unanticipated risk factors in reinforced concrete construction work, which was involved in apartment, building. school church, hotel hospital, bank work and the other works. According to the construction fatal accident book by KOSHA 1,903 workers were killed by the fatal accidents in reinforced concrete construction work during the 13 years$(1992\~2004)$. From these reasons, this study researched the cause of fatal accidents which were happened in reinforced concrete construction work during last 13 years and surveyed construction managers and specialists about risk factors according to the work type in the reinforced concrete construction work. Therefore, this study will suggest the counterplan about the construction safety management and prevent fatal accidents from construction site related to the reinforced concrete construction work.

건설업에서의 감전재해 감소 방안을 위한 연구 (A Study on the Decreasing Method of Electric Shock Accident in the Construction Industry)

  • Regarding the characteristics of fatal electric shock accident during the past 10 years, fatalities occurred more frequently in July & August, and AM 10-12 PM & 14-16. Also, the electric shock accident is occurred more frequently in the age group from 30 to 40. Especially, the fatal electric shock accident in construction industry covered more than half of total industry. Therefore, it is strongly requested for the protection technology of an electric shock from the low voltage electric path, grounding method and workers. The aim of this study is to propose the policy about the equipment performance standards and/or worker's safety standards to revise the standards for preventing electric shock accidents on safety workings.

이동식 크레인 사고사망재해 심층분석 연구 (A study on Cause Analyses of Fatal injuries by the Mobile Cranes)

  • In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by mobile cranes. Fatal injury analyses were conducted with several accident cases by the mobile cranes. For each accident, the causes of the injuries were examined and proper safety measures were proposed. This study proposed a revision of the standard guideline as an accident prevention measures through in-depth analysis of fatal accidents. First, fatality rate per 10,000 persons by the mobiles cranes was 0.57 in 2011. Second, the in-depth analysis of the mobile crane accidents showed that the main factors of the accidents were the overturn of mobile crane, the fracture of connection part between mobile crane boom and rope, the breaking of rope and the misunderstanding over the signal between a mobile crane driver and workers around a mobile crane. Third, the order of high defection of connecting parts was bucket bolt, bracket and extension parts. Fourth, since there were several cases of overturn of mobile crane by itself in the in-depth analysis of the mobile crane, the over slop-proof device was proposed as the main alternative in this study. Fifth, the revision of standards of safety factor that shall be more than 10, of connecting parts of mobile crane was proposed.
