2급 응급구조사 양성과정 개선방안 연구 : 양성기관 현황 및 지정기준 검토 (Improvement in emergency medical technician-basic training program : a review of the status of training institutions and designation criteria)

  • Purpose: To discuss the records and legal standards of emergency medical technician training institutions in Korea and abroad, to identify the problems, and to provide the basic resources for improving the EMT-basic training institutions. Methods: We received advice through an advisory meeting of experts (professors of department of emergency medical technicians) and interested parties (Korean Association of Emergency Medical Technicians, officials of emergency medical technician training institutions) and referred to various reports published by governments, official institutions, and other trustworthy organizations. Also, we communicated with the related experts abroad (3 countries) on the phone or by email for surveys. Results: Compared to the abroad, it is necessary to categorize the standards and procedures of designating the emergency medical technician training institutions in Korea and improve the management of training institutions to train competent emergency medical technicians. Conclusion: It is necessary to designate and manage continuously the emergency medical technician-basic training programs for the systematic primary healthcare service.

소방공무원의 조직문화가 직무만족도에 미치는 영향 (The Influence of Organizational Culture on Job Satisfaction of Fire Officials)

  • Purpose : The purpose of this study was to investigate the influence of organizational culture on job satisfaction of fire officials. Method : The subjects of this study were 251 fire officials in Gongju and Choengju province and Daejeon City. Data were collected using self-administering questionnaire from February 20 to March 20 in 2006. Collected data were analyzed using SPSS 14.0 program. Real number, percentage, mean and standard deviation were calculated, and t-test, ANOVA, Pearson correlation coefficient, multiple regression were analyzed. Result : The results of this study were as follows : 1. The job satisfaction was found to be significant positive correlations with full organizational culture(r = .595, p = .000). 2. The job satisfaction was found to be in significant positive with rational goal culture, open system culture and human relation culture. 3. How organizational culture affects the job satisfactions was as follows; 25.2% of core values of manpower, 6.3% of rational goal culture, and 4.9% of job initiative. Conclusion : Enhancing the job satisfaction of fire officials would improve personalized factors and changes rational goal culture.

응급수술을 시행한 응급의료센터 내원 외상 환자 분석; 응급의학과 진료는 외상환자에 대한 외과의 수술과 같은 전문적인 진료를 오히려 지연시키는가? (Clinical Analysis of Trauma Surgery Patients in a Local Emergency Center; Does Emergency Physicians'Treatment Delay the Surgeons'Special Care like Emergency Operations for Trauma Victims?)

  • Purpose: Trauma surgery is not an official medical specialty in the Republic of Korea (South Korea). Thus, a trauma victim transported to an emergency room (ER) is resuscitated and surveyed by an intern, a resident, or an emergency physician (EP) at first. Currently an operative management is decreasing because of multiple factors. Nevertheless, trauma surgery is the key for some patients. Does the EP's treatment in the ER delay the surgeon's emergency operation? Methods: A retrospective study was performed for trauma victims who underwent trauma surgery from March 2004 to February 2005 in a local emergency center of Daegu-city. We reviewed the medical records and analyzed the trauma victim's age, sex, cause of injury, method of transport, time from the trauma to the operation, EP's treatment, surgical department, mortality, and injury severity score (ISS). Results: Of the 223 trauma victims included in this study, males were predominant (83.4%). The mean age was 37.98 years of age. The main Causes of trauma were trauma NOS (not otherwise specified) and motor vehicle accidents (MVA). The main methods of transport was privately owned automobile. The mean time from trauma to operation was 617.46 min. The mean ISS was 7.67. Trauma surgery with the EP's treatment group included 40 trauma victims with higher ISS, and the time from trauma to operation was shorter than it was for the 183 trauma victims not in that group. Conclusion: The EP's treatment of high-ISS multiple-injury trauma victims can shorten the time from trauma to trauma surgery and will help the surgical department treatment. In the trauma care system of the Republic of Korea, and increased role should be encouraged for emergency physician.

국내 주요 수변시설물 EAP 프레임워크 개발 (Development of a Framework of Emergency Action Plan for Domestic Water Front Critical Infrastructure)

  • 강우는 자연과 인간에게 중요한 자원이다. 적절한 강우는 농작물의 성장과 토지의 온도를 일정하게 유지시켜줌으로써 우리 사회가 안정적으로 유지되도록 하는 매우 중요한 역할을 한다. 그러나 태풍 및 집중호우 등과 같이 특정 지역에 계획강수량보다 더 많이 발생한 집중강우는 시설물 및 인간에게 커다란 피해를 입히기도 한다. 이에 따라 미국, 일본 등 국외 선진국의 수재해 대응기관들은 시설운영자와 재난전문가, 응급대응인력들에게 가이드라인을 제공하기 위해 수재해 EAP(Emergency Action Plan)를 작성 운영하여 시설물을 보호하고 강화하는데 활용하고 있다. 대표적인 예로, 미국의 FEMA와 DHS는 시설물과 인명을 보호하기 위해 댐, 제방 등을 위한 다양한 EAP를 운영하고 있는데, 특히 FEMA는 EAP를 시설물이나 인명에 가해지는 피해나 기능적 충격과 같은 응급상황을 감소시키기 위해 작성하는 공식문서로 규정하고 있으며, 이 EAP를 통해 응급 시에 발생할 수 있는 모든 상황뿐만 아니라 평상시 발생할 수 있는 시설물관리상의 문제까지 대응이 가능하도록 규정하고 있다. 이에 본 연구에서는 미국, 일본 등 국외주요국과 국내에서 사용중인 EAP 관련 규정을 분석하여 국내 상황에 적합한 EAP 체계를 제안하였다. EAP는 예방, 준비, 대응, 복구의 재난단계별로 시설물과 인명을 보호하기 위해 재난전문가들과 시설운영자들이 취해야 할 업무, 응급장비 운용 등의 내용을 담고 있다. 본 연구를 통해 개발된 EAP 프레임워크는 테스트베드 적용을 통해 적정성을 평가받았으며, 국내 홍수재난 위험을 감소시키기 위한 절차와 방법 개선에 기여할 것으로 기대된다.

마산만 국지해일 예경보 모의 시스템 구축 (A Warning and Forecasting System for Storm Surge in Masan Bay)

  • 본 연구에서는 태풍과 해일에 의한 범람 피해 영역을 예보하기 위한 동적경보시스템에 대해서 서술하였다. 이 시스템은 연안 지역에 내수범람과 같은 잠재적 위기가 예상되는 태풍 발생 시기에 해안공학적 지식이 부족한 공공 재난관리자들이 직접 신속하게 운용할 수 있도록 일반 PC에서 MATLAB의 GUI(Graphical User Interface)를 이용하여 개발되었다. 신속한 예보를 위한 바람장은 기상청 태풍 예보 자료를 이용하는 단순파라메터 태풍모델로 생성된다. 조위와 수심평균된 조류를 신속하고 정확하게 모의를 하기 위하여 경계 조건을 사전에 준비하였다. 또한 임의의 시점에서도 정확하고 빠르게 수행되도록 MATLAB 수치프로그램을 벡터화하였다. 예상 태풍경로와 관련 태풍 예보 자료를 마우스 등을 이용하여 윈도우 화면에 입력하고 실행하면 마우스로 선정된 특정범람 예상 지점에 대한 실시간 수위정보를 얻을 수 있다. 이 시스템은 지역 재난관리자들에게 가능한 위험 정보를 신속히 파악하고 체계적으로 운용될 수 있도록 개발되었다. 그러나 모델결과는 태풍의 경로에 민감하게 작용하고, 또 지역 재난관리자들이 정확한 정보를 생산하기 위하여 오랜 기간 숙련과 지역에 적합한 경험 계수의 결정 과정이 필요하다.

한국보건행정학회 30주년 기념 특별호 (Special Issue for the 30th Anniversary of the Korean Academy of Health Policy and Management)

  • The Korean Academy of Health Policy and Management (KAHPM) has shown remarkable achievements in the field of health policy and management in Korea for the last 30 years. The KAHPM consists of experts in various fields of health policy and management, and has been the leading academic discussion forum for health policy agendas of interest to the public. Health Policy and Management (HPM), the official journal of the KAHPM, published the first issue of volume 1 in October, 1991 and is publishing the second issue of volume 28 as of 2018. Currently, it is one of Korea' main journals in the field of health policy and management. HPM has published a special issue in commemoration of the 30th anniversary of the KAHPM. The HPM invited authors, including former presidents of the KAHPM and current board members, to write about main issues in health policy and management. Although the HPM tried to set up an invited author on all subjects in the health policy and management field, 19 papers are published, that completed the peer review process by August, 2018. The authors of the special issue of the 30th anniversary of the KAHPM include six former presidents, a senior professor, and 12 board members. The subjects of this issue are reform of the healthcare delivery system, health insurance and medical policy, reform of health system governance, the role of National Health Insurance Service (NHIS), the Korea Institute for Health and Social Affairs (KIHASA) and the National Evidence-based healthcare Collaborating Agency (NECA), ethical aspects of health policy change, regional disparities of healthcare, healthcare accreditation, new healthcare technology evaluation system, globalization of the healthcare industry, the epidemiological investigator system, the quarantine system, safety and disaster, and official development assistance. There are some remaining topics to deal with for the KAHPM: aged society, anti-smoking, non-infectious disease, suicide, healthcare resources, emergency medical care, out-of-pocket money, medical fee payment system, medical aid system, long-term care insurance, industrial accident compensation insurance, community-centered health welfare system, and central government and local government of health. The HPM will continue to publish review articles on the main topics in health policy and management. This is because the KAHPM, which has been the leading academic society of Korea's health policy and management for the last 30 years, feels responsible for continuing its mission for the next 30 years.

WTO/SPS 통보문 분석 연구 - 식품안전조치를 중심으로 - (Study on the Notifications of WTO/SPS - Focused on Food Safety Measures -)

  • According to Article 7 and Annex B of the World Trade Organization(WTO) Agreement on the Application of Sanitary and Phytosanitary(SPS) Measures, WTO members are required to notify their newly established or amended SPS measures that might affect international trade. This study analyzed SPS notifications from 1995 to 2008 to identify international trends for food safety measures. The notifications were collected from the SPS information management system and the official document distribution system of WTO. The 153 WTO members represented 37 developed countries, 84 developing countries and 32 least developed countries. The number of overall notifications was 9,820. The annual notifications increased from 198 in 1995 to 1,264 in 2008. The monthly average notifications were from 44.8 in December to 69.5 in June; however, there were no statistical differences among them. The six leading Members in terms of notification submissions were United States(U.S.), Brazil, New Zealand, Canada, European Community and Korea. Among the regular and emergency notifications, 62.7% notifications were concerned with food safety, followed by animal and plant protections. Among animal protection notifications, 54.8% were emergency situations. Of the 4,821 food safety notifications, 60.4% were from developed countries, and 39.3% were from developing countries. Measures concerning pesticide residues and risk assessments were mainly from developed countries. In contrast, 77.5% of the measures concerning zoonoses were from developing countries. However considering the numbers of developing and developed countries, the average number of measures for each country was similar. Food safety measures were mainly involved pesticide residues followed by food additives, zoonoses, new regulations, labeling. As the overall notifications, zoonosis measures were mainly emergency notifications. Measures concerning pesticide residues, food additives, zoonoses, new regulations, veterinary drugs, and labeling were submitted most often by the U.S., Korea, Albania, U.S., Australia, and U.S., respectively. Identifying the exact trends of food safety measures was complicated by a number of factors. However, WTO notifications might be useful tool in providing a general view of international trends.

국가적 재난 대응에 있어서의 소셜 미디어 활용 방안 연구 (Application of Social Media for Responding to a National Disaster)

  • 2011년 3월 11일에 일본에서 발생한 대지진은 인적, 물적으로 상당한 피해를 남기게 되었다. 이러한 가운데, 안부 확인 및 피해 복구 과정에서 소셜 미디어가 상당한 활약을 보임으로써 재해 발생 시에 매우 유용한 커뮤니케이션 채널로서 부각되었다. 하지만, 재해 발생의 안전지대라 할 수 없는 우리나라에서는 이러한 재해 발생에 대한 구체적인 대응책이나 체계가 갖추어져 있지 않은 것이 현실이다. 따라서, 본 논문에서는 소셜 미디어 활용에 대한 현황을 알아보기 위하여 외국과 국내의 소셜 미디어 활용 관련 문헌을 조사하고, 이를 기본 재료로 삼아 언제 일어날지 모르는 재해에 대응할 수 있는 소셜 미디어의 이상적인 활용 방안을 도출하고자 한다. 본 연구 결과 도출된 소셜 미디어의 활용 방안을 요약해보면 다음과 같다. 첫째, 기상청이나 소방방재청 등의 관련 기관에서 긴급 상황 시 대국민 의사소통 체계 구축에 활용할 수 있다. 둘째, 유사시 구조 요청이나 물자보급 요청 등 위치 정보를 포함한 다양한 재난 정보서비스 체계 구축에 활용할 수 있다. 마지막으로, 페이스북이나 트위터를 통하여 온라인 기부 서비스가 제공될 수 있다. 본 연구에서 얻어진 결과는 재난 대응 관련 담당자나 연구자들에게 유용한 시사점을 제공한다.

재난관리자로서 이장의 역할과 스트레스에 관한 분석 (Ijang's Role and Stress as an Emergency Manager)

  • 본 연구의 목적은 2006년 평창 수해를 겪으면서 마을 대표인 이장이 수행한 역할과 업무 스트레스를 조사하는데 있다. 전화 설문과 인터뷰에 기반한 연구 결과를 보면, 공식적으로 이장의 역할은 행정기관과 마을 주민을 연결하는 역할을 수행하는 것으로 제한되어 있으나, 실제 재난 과정에서 이장은 주민들을 대피시키고, 자원을 배분하고, 복구를 지휘하는데 있어서 중요한 역할을 수행하였음을 알 수 있다. 게다가 예기치 않은 과도한 부담으로 인하여 상당수의 이장들은 심각한 업무 스트레스를 겪은 것으로 밝혀졌다; 60 퍼센트의 이장들은 이장 업무를 그만두고 싶었고, 약 4분의 3에 해당하는 이장들은 전화 벨이 울릴 때마다 놀랐거나, 몸무게 변화가 있었다고 응답하였다. 또한 카이 제곱 검정 결과에 의하면, 사전 교육, 마을 주민들로부터 욕을 들었는가의 여부, 재난 피해는 업무 스트레스와 통계적으로 의미있게 연관되어 있는 것으로 밝혀졌다. 이러한 결과는 농촌 지역의 재난 관리 계획을 수립하는데 있어서 명령 통제 모델보다는 발현적 인적 자원 모델이 더 효과적인 접근이 될 수 있음을 제시한다.

Evaluation of the Public Health Emergency Response to the COVID-19 Pandemic in Daegu, Korea During the First Half of 2020

  • Objectives: This study evaluated the response in Daegu, Korea to the first wave of the coronavirus disease 2019 (COVID-19) pandemic according to a public health emergency response model. Methods: After an examination of the official data reported by the city of Daegu and the Korea Centers for Disease Control and Prevention, as well as a literature review and advisory meetings, we chose a response model. Daegu's responses were organized into 4 phases and evaluated by applying the response model. Results: In phase 1, efforts were made to block further transmission of the virus through preemptive testing of a religious group. In phase 2, efforts were concentrated on responding to mass infections in high-risk facilities. Phase 3 involved a transition from a high-intensity social distancing campaign to a citizen participation-based quarantine system. The evaluation using the response model revealed insufficient systematic preparation for a medical surge. In addition, an incorporated health-related management system and protection measures for responders were absent. Nevertheless, the city encouraged the participation of private hospitals and developed a severity classification system. Citizens also played active roles in the pandemic response by practicing social distancing. Conclusions: This study employed the response model to evaluate the early response in Daegu to the COVID-19 pandemic and revealed areas in need of improvement or maintenance. Based on the study results, creation of a systematic model is necessary to prepare for and respond to future public health emergencies like the COVID-19 pandemic.
