종격동염의 증례보고 (Mediastinitis: a case report)

  • Acute mediastinitis is a fatal infection which occurs related to connective tissue of mediastium, in the thoratic organs. Occurrence of mediastinitis due to craniocervical infection is very rare, and is defined as descending necrotizing mediastinitis. November 8th, 2008, man in his early fifties visited ER due to severe swelling on left neck area and dyspnea. Antibiotic were administered immediately, and vast amount of abscess formation on pneumomediastinum and adjacent tissues were observed via chest computed tomography. With cooperation of thoracic and cardiovascular surgery department, emergency incision and drainage with drain insertion was done to remove abscess, and control the infection. After surgery, everyday saline irrigation through drain was done during hospitalization, with continues antibiotic therapy. Descending necrotising mediastinitis is a most rare and dangerous infection which occurs on oropharyngeal area. In case of descending necrotising mediastinitis, accurate diagnosis, airway maintenance, remove of abscess by incision and drainage, aggressive antibiotic therapy and continuous saline irrigation is necessary to increase patient survivability. Also, computed tomography with contrast media is essential to figure out the size and location of the infection and abscess formation.

  • Antibiotic is one of the mainly prescribed drugs in dental office. The substantial part of all antibiotics used in dental office is given to patients without signs or symptoms of infection to prevent infections, and antibiotics became the most widely abused prescribed drugs on the basis of inappropriate indications, dosages and durations. Considering that antibiotics are one of the drugs that affect not only a single patient but also entire populations of individuals through their collective effects on microbial ecology, the importance of proper use of antimicrobial therapy can hardly be overemphasized. Therefore, the main considerations for antibiotic therapy in denial office were summarized here.

  • Titanium and titanium alloys are the most common materials used for dental and biomedical implants, owing to their biocompatibility and favourable mechanical properties. However infection of the region surrounding a dental implant by pathogenic microorganisms is a significant factor in implant failure. Prevention and control of microbial colonization of implant surfaces is considerable interest to the biomedical community. One important strategy is to render the implant surface antibacterial by impeding the formation of biofilm. A number of approaches have been proposed for this purpose. Therefore, we reviewed the researches of antibacterial coatings on titanium implants in this articles.

Infectious Myositis of the Jaw Presenting as Trismus of Unknown Origin

  • Infectious myositis, an infection of the skeletal muscles, is a rare condition and potentially life-threatening if not detected and treated in the early stages. This clinical entity may arise from various pathogens, such as bacteria, fungi, parasites, and viruses. A propagation of contiguous infection, penetrating trauma, vascular insufficiency, or hematogenous spreading of microorganisms can cause infectious myositis. Though several cases have been reported in large muscle groups in the lower extremities, there are only a few reports on infectious myositis of the masticatory muscles. We report three cases of infectious myositis presenting jaw pain and trismus. Unlike a common head and neck infection caused by the spreading of odontogenic origin, the early diagnosis of infectious myositis was difficult because no specific lesion suspected to be the infection source was observed in the physical examination and the plain radiographs. Advanced imaging modalities such as computed tomography and magnetic resonance imaging, and laboratory evaluation is useful for the early detection of infectious myositis.

  • The Government is going to enforce assessment of dental institutions in addition to assessment of medical institutions so that it can provide good-quality medical service to people having much interest in quality of medical service. But the empirical research on the assessment of dental institutions which is still in a model assessment stage, is insufficient. Accordingly, the present research aims to help dental institutions preparing the main assessment by researching an acceptance level of workers of dental institutions in a process performing assessment of dental institutions, based on the preceding researches that the perceived utility has influence on implementation intention. The present research proved the influence that job relevance, result demonstration, usability and education & training of workers and manager's leadership of an infection management part affects perceived utility and implementation intention. As a research result, all the job relevance, result demonstration, usability have a positive(+) influence on perceived utility and implementation intention, and the influence of job relevance most affects especially. The leadership and education & training have influence on what workers get to have implementation intention after perception of utility of infection management according to the order. According to the above results, it can be understood that medical institutions preparing for assessment of dental institutions take charge of the assessment part having high relevance with work of workers and need to make a manager ordering performance of its assessment be able to foster a leadership for improving effectiveness of assessment performance.

  • Coronavirus is an enveloped virus with positive-sense single-stranded RNA. Coronavirus infection in humans mainly affects the upper respiratory tract and to a lesser extent the gastrointestinal tract. Clinical symptoms of coronavirus infections can range from relatively mild (similar to the common cold) to severe (bronchitis, pneumonia, and renal involvement). The disease caused by the 2019 novel coronavirus (2019-nCoV) was called Covid-19 by the World Health Organization in February 2020. Face-to-face communication and consistent exposure to body fluids such as blood and saliva predispose dental care workers at serious risk for 2019-nCoV infection. As demonstrated by the recent coronavirus outbreak, information is not enough. During dental practice, blood and saliva can be scattered. Accordingly, dental practice can be a potential risk for dental staff, and there is a high risk of cross-infection. This article addresses all information collected to date on the virus, in accordance with the guidelines of international health care institutions, and provides a comprehensive protocol for managing possible exposure to patients or those suspected of having coronavirus.

  • Acquired Immune Deficiency Syndrome is the state which develops after complications with the infection from HIV. Irrespective of their state, all HIV infections have infectivity. According to a 2013 U.N. AIDS global report, the number of reported newly infected with HIV is constantly falling, while that of Korea has been increasing over recent years and it surpassed 10,000 in 2013. This phenomenon might be attributed to the unusualness of the blood test for early detection of HIV infection. From this fact, we can assume that we have strong possibilities for encounters with infections in the office. But many misconceptions about the disease makes patients try to hide their medical history, which can lead to a nationwide spread of the infection without proper management. Even though it may be difficult to take care of HIV patients in smaller dental offices, large scale hospitals have the means to arrange protocols to treat them. We present a case about dental treatment of a patient with AIDS that has a history of cerebral infarction and hepatitis type B under general anesthesia. The purpose of this case report was to discuss the special considerations of dental care for patients with HIV.

Identification of Enterococcus faecalis antigens specifically expressed in vivo

  • Objectives: Molecular mechanism of the pathogenicity of Enterococcus faecalis (E. faecalis), a suspected endodontic pathogen, has not yet been adequately elucidated due to limited information on its virulence factors. Here we report the identification of in vivo expressed antigens of E. faecalis by using a novel immunoscreening technique called change-mediated antigen technology (CMAT) and an experimental animal model of endodontic infection. Materials and Methods: Among 4,500 E. coli recombinant clones screened, 19 positive clones reacted reproducibly with hyperimmune sera obtained from rabbits immunized with E. faecalis cells isolated from an experimental endodontic infection. DNA sequences from 16 of these in vivo-induced (IVI) genes were determined. Results: Identified protein antigens of E. faecalis included enzymes involved in housekeeping functions, copper resistance protein, putative outer membrane proteins, and proteins of unknown function. Conclusions: In vivo expressed antigens of E. faecalis could be identified by using a novel immune-screening technique CMAT and an experimental animal model of endodontic infection. Detailed analysis of these IVI genes will lead to a better understanding of the molecular mechanisms involved in the endodontic infection of E. faecalis.

치과위생사의 진료실 감염방지에 대한 행태 분석 (The Analysis of the Prevention against Virus Infection in Dental Hygienist at Medical Treatment)

  • 1. 조사 대상자는 총 128명으로 연령은 24세~28세가 71.1%(91명)으로 가장 많았으며, 임상경력은 2년~5년 사이가 36.7%(47명) 우세하게 나타났으며 일 진료환자수는 56.3%(72명)가 25명 이하이고, 대부분 치과위생사수가 3명 이하인 치과의원에 근무하는 것으로 조사되었다. 2. 감염노출 행태 및 감염방지처치실태는 응답자의 76.6%(98명)가 날카로운 기구나 주사바늘에 찔린 경험이 있으며 대부분이 즉시 소독하는 방법으로 감염방지 처치를 하며 진료시 감염은 경험하지 않은 것으로 조사되었다. 3. 감염방지교육 실태에 대한 조사결과 응답자의 75.8%(97명)가 감염관리에 대한 이론과 시술법에 대한 교육경험이있는 것으로 응답하였으며 89.1%(114명)가 병원의 감염 관리에 대한 교육이 필요하다고 생각하며 학생시절(49.2%, 63명)에 학교교육을 통해서(45.3%,58명) 경험하였으며 응답자의 81.3%(104명)가 감염방지교육의 확대를 주장하고 있는 것으로 나타나 학교교육을 통해서 뿐만 아니라 지속적인 보수교육이 필요하다고 판단된다. 4. 치과위생사의 감염방지 행태분석결과 의료용 장갑을 사용하는 비율은 24세~28세의 임상경력이 5년~10년이면서 치과병원에 근무하는 치과위생사일수록 높았으며 진료 후 대부분이 폐기하며 진료시 마스크를 착용하는 비율은 29세, 임상경력이 10년 이상의 경우에서 높게 나타났으며 근무하는 병원규모에 상관없이 높게 나타났으며 진료시 마스크 습기가 찰 경우 대부분이 교체하는 것으로 나타났다. 5. 환자진료시 보안경사용 및 기구세척 과 재처리시 고무장갑을 사용하는가에 대한 조사 결과 보안경을 사용하는 비율은 나이, 임상경력, 근무하는 병원규모에 상관없이 낮은 것으로 나타났으며 대부분이 기구세척과 재처리시 고무장갑을 사용하는 것으로 나타났다. 6. 진료감염방지 교육 경험자와 비경험자에 대한 진료시 감염방지행위의 차이를 분석하기 위하여 각종 감염방지행태 항목을 변수로 하여 독립표본 T검증(Independent -sample T Test)을 실시한 결과 대부분의 항목에서 진료감염방지 교육을 경험한 치과위생사가 진료시 감염방지 행위를 잘하고 있는 것으로 나타났다. 7. 진료 시 의료용 장갑을 사용 및, 진료 후 의료용 장갑폐기, 기구세척 및 재처리시 두꺼운 가사용 고무장갑 사용의 항목에서 두집단 간에 유의수준 0.05, 0.01에서 각각 의미 있는 차이가 있었으며 나머지 3개 항목은 유의수준 0.05에서 의미 있는 차이를 보이지 않았다. "진료시 마스크 사용"에 대한 항목에서는 두집단 간에 큰 차이를 보이지 않았는데 이는 감염방지교육과 상관없이 진료시 기본필수사항으로 인식하기 때문인 것으로 판단된다.

두경부 감염증에 나타난 내경정맥혈전증의 장기적 추적 결과: 증례보고 (ABSTRACT: LEMIERRE SYNDROME: A CASE REPORT)

  • Abscesses are common in the oral and maxillofacial area. However, secondary thrombosis of the internal jugular vein accompanying the primary abscess is rare. In 1936, Andre Lemeierre studied 20 patients who showed an initial oropharyngeal infection, septicemia, internal jugular vein thrombosis, and secondary spread of the infection, and after then this condition Lemierre syndrome. Clinically, these patients present with tonsilitis lasting several days, continuous fever, and cervical pain. In the past, ligation and excision of the internal jugular vein was often performed. Current therapeutic modality for this condition is appropriate antibiotic prescription and surgical drainage of abscess. This case report presents a patient who showed symptoms of Lemierre syndrome, initiated as an oropharyngeal infection then developed thrombosis of the internal jugular vein. This patient was admitted into Seoul National University Dental Hospital. In addition to routine antibiotic therapy, surgical incision and drainage of the infection site was performed. Without ligation or excision, the thrombosed IJV disappeared eventually. As the Lemierre syndrome is not a common disease, this case report and review of the literature would be useful regarding a treatment of patients with Lemierre syndrome.
