  • Objectives : The Illness Intrusiveness Rating Scale (IIRS) is a well-validated self-report instrument for assessing negative impact of chronic illness and/or adverse effects of its treatment on everyday life domains. Although extensive literature probed its psychometric properties in medical illness, little attention was paid for its validity for psychiatric population. This study aimed to test factorial structure of the Korean Version of the IIRS (IIRS-K) in a consecutive sample of psychiatric outpatients. Methods : Data set of 307 first-visit patients of psychiatric clinic at Guri Hanyang univ. Hospital were used. Exploratory and confirmatory factor analysis, internal consistency were tested in IIRS-K. We also checked Spearman's correlation analysis between IIRS-K, Zung's self-report anxiety scale and Zung's self-report depression scale. Results : 76.9% of the patients were with anxiety disorder and depressive disorder. The principal component factor analysis of the IIRS-K extracted three-factor structure accounted for 63.2% of total variance that was contextually similar to the original English version. This three-factor solution showed the best fit when tested confirmatory factor analysis compared to the original IIRS, two-factor model of IIRS-K suggested from medical outpatients, and one-factor solution. The IIRS-K also showed good internal consistency (Cronbach's α=0.90) and good convergent validity with anxiety and depression scales. Conclusions : The IIRS-K showed the three-factor structure that was similar but not identical to original version. Overall, this study proved factorial validity of the IIRS-K and it can be used for Korean clinical population.

  • This quasi-experimental study was intended to test the effect of self-help group program, which is one of the way to enhance adaptation and quality of life to mastectomy patients. Data was collected from July 14, 1998 to Oct. 31, 1998 at two Medical Center in Seoul. The subjects for this study were the patients who had undergone mastectomy and were follow-up ; 14 in experimental group and 14 in control group matched with age and treatment. The instruments for this study were adaptation in Lee(1994)'s physical symptom questionnaire, Zung's Self-rating Depression Scale(SDS, 1965), and Self-rating Anxiety Scale(SAS, 1970), quality of life in Spranger(1996)'s and No(1988)'s Quality of Life Questionnaire. The self-help group program for mastectomy patients was developed based on literature review and pilot study by the investigator. The subjects of experimental group were participated in 6 weeks self-help group program and were received arm and shoulder exercise, informational support, and interpersonal support by group members. The control group were received no intervention, Both group answered questionnaires prior to intervention and 6 weeks later. The data analyzed by frequency, $X^2$-test, Mann-Whitney U test. Wilcoxon Signed Rank test, Pearson's Correlation Coefficient and Stepwise Multiple Regression using SPSS WIN. The results are as follows ; Hypothesis 1. "The experimental group with the self-help group program will have a higher score on adaptation state than control group." was not supported. But the post test score of anxiety and depression in experimental group were declined and the depression score was reduced relatively. Hypothesis 2. "The experimental group with the self-help group program will have a higher score on quality of life than control group." was not supported. But the posttest score of quality of life in experimental group was reduced relatively. Hypothesis 3. "The higher adaptation state of mastectomy patients, the higher quality, of life." was supported(r=,80, p<.001). Additionally, the lower physical symptom, depression and anxiety, the higher quality of life And depression, which was the main predictor of quality of life, accounted for 59.5%, depression and anxiety accounted for 65.5% of the variance in quality of life. In conclusion, when the self-help group program was intervened to mastectomy patients, it was tended to increase quality of life and to reduce depression and anxiety. So self-help group program can be considered useful nursing inter vention effect on adaptation and quality of life of mastectomy patients. With discussion, I suggest repeated further re search on self-help group with appropriate sample size and longitudinal study. Also during adjuvant therapy, it is needed to develop convenient method to be supported from peer group and family, such as computer mediated support group.

  • This study aims to investigate the related variables of self-esteem and depression among the Korean elderly people living alone and examine the relationships or interactions between those variables. Participants were 676 elderly men and women who were at least 65 years of age(M=76.17, sd=7.60) and lived in Seoul, Korea. Three hundred seventy eight participants of this study were living alone. Participants completed questionnaires and psychological tests including: Rosenberg's Self-Esteem Scale, Zung's Self-Rating Depression Scale, The Index of Activities of Daily Living(IADL), Social Support Index(SSI), and Life Satisfaction in the Elderly Scale(LSES). Main statistical designs were 2(gender)×2(residential types), Pearson-product moment and regression analysis. Results indicated that the elderly living alone recognized their health poorer, have lower economic status, and received less social supports than the elderly lived with others. The elderly men who had children were more likely to lived alone. And the elderly men living alone received less social supports than the elderly women living alone, and significant gender difference was found in the reason of living alone. The elderly men living alone had lower self-esteem than the elderly women living alone, while the elderly living alone showed more depressive symptoms than the elderly living with others. There were 2-way interactions both in self-esteem and depression by gender and residential types. There was highly significant gender difference in self-esteem only for the elderly living alone, and it was found that there was no significant difference in depression between elderly men living alone and women living with others. Regression analysis revealed that physical function and self-reported health are predictors of self-esteem, and physical function, self-reported health, and social support are predictors of depression for the Korean elderly living alone. These findings reiterate the role of physical function, social support, health in self-esteem and depession among the elderly and suggest the gender role for quality of life among the Korean elderly living alone.

  • The nuclear family is no longer the typical Korean Family. In recent years, stepfamilies have been of the most rapidly growing family forms in Korea. Census Bureau data show that 5.9% of marriage were in 1980, 8.0% in 1990, 10.3% in 1995, 18.0% in 2000. Especially it is remarkable that women's remarriage have been increased. In spite of the fact, the stepfamily is not an urgent issue of social welfare in Korea. The stepfamily is more vulnerable than the first-marriage family in many reasons such as vague family rules, boundary ambiguity, and stepparent role ambiguity, which provides rationale for social welfare services. This study categorizes determinants of psychological distress in remarried mothers into individual, family, and environment level and tries to prove the relationships between psychological distress and determinants. Also, it explores the degree of psychological distress in remarried mothers using Zung's Self-Rating Depression Scale. The respondents of survey research are 62 cases. The result shows relatively severe level of depression among the subjects. 46.8% of them are clinically diagnosed as mild, moderate, severe depression. Our findings suggest that the variables of marriage satisfaction, ex-wife's remarriage, role strains, marriage experience, and income are significantly associated with the level of psychological distress in remarried mothers. The most powerful predictor of psychological distress is the variable of marriage satisfaction. According to the result, it is desperately needed to pay attention to social welfare services or programs for stepfamilies.

  • Objectives: The present study was directed at revealing the influence of various life styles on the subjective well-being and depressive states, and their related factors among the elderly. Methods: The interviews were given to 454 elderly people aged over 65 (197 male and 257 female) in rural areas of Chungnam Province during the 3-month period from July 1st to Sep. 30th, 2002. The interview contents for the elderly included social demographic characteristics, activities of daily living(ADL) whether independent of others or not, subjective well-being, Zung's self-rating depression scale(SDS), etc. Results: The male elderly showed significantly higher scores in subjective well-being than the female, and with regard to Zung's depression scores, they were significantly higher in the female than the male elderly. Based on the correlation between scores of subjective well-being and its related factors, there was a significant, positive correlation in both sexes with whether or not participation in the social gatherings and the degree of satisfaction with subjective health status. The factors shown to be negatively correlated with depression scores in both sexes were whether or not participation in the social gatherings, the degree of satisfaction with the subjective health status, economic conditions, and ADL scores. The factors influencing on the subjective well-being included depressive states, eating habits, dwelling states, ADL scores, and physical activity. Those influencing on the depressive states were the degree of satisfaction with the subjective health status, physical activity, sexes, smoking, ADL scores, economic conditions, eating habits and whether or not participation in the social gatherings. Conclusions: Consequently, the subjective well-being as well as physical activity and economic conditions were shown to be the important factors for the healthy elderly life.

  • The authors studied depression using Zung's Self-Rating Depression Scale(SDS), in the subjects of 3,499 male and 1,335 female college freshmen of Yeungnam University. The authors collected the reports of SDS during the periods from January to February, 1986, and applied ANOVA and t-test on depression scores in order to compare them between various psychosocial factors, and sexes. The results are as follows : There was significant difference in the mean averages of total depression scores between male and female students: male students scored 35.68+7.46, female students scored 38.93+7.26(P<0.001). The depression scores relating to the items of indecisiveness, psychomotor retardation, decreased libido, and diurnal variation were relatively higher in both groups. One hundred and sixty-one male students(4.6%) showed seriously high degree depression scores of 50 or higher, while one-hundred and seventeen female students(5.8%) showed the same scores. Female students attending in liberal arts showed higher level of depression scores(P<0.01). There was a strong tendency toward higher depression scores in the student who were dissatisfied with their home atmosphere, colleges, department and familiarity of parents, and those who had pessimistic views of self image in the past, present or future in both groups(P<0.001).

Methods:Three hundred twenty-six student(161 persons from industrial wemem's school, 165 persons from cultural wemen's school), who were supposed to different from in psychosocial environment, entered this study. To assess the attitudes toward suicide, Suicide Opinion Questionnaire(SOQ) were done at study. To estimate anxiety, depression, and self-image, Zung' Self-rating Anxiety Scale (ZAS), K-Beck Depression Inventory(KBDI), Offer's Self Image Questionnaire(OSIQ), respectively, were executed in this study. Results:1) We extracted 5 factors after factor analysis based on SOQ results with the response of the 326 subject;ethics, religion, mental illness, acceptability, revenge. Revenge factor were stastically significant between both groups. 2) In OSIQ, family relationship factor & superior adjustment factor were stastically significant between both groups, but, both groups didn't have a significance on ZAS & KBDI. 3) According to correlative analysis between SOQ results and OSIQ, ZAS, KBDI results, ethics factor(SOQ) were converse to BS factor, ET factor, and ME factor in OSIQ. Mental illness factor(SOQ) were in proportion to PP factor(OSIQ). Acceptability factor(SOQ) were in proportion to ZAS & KBDI result. Conclusion:Our subject mostly regarded suicide as sinful ethically ,and as mental illness. But, In that there was neuropsychiatric history and suicidal idea in them, the subjects looked upon suicide as permissive. With significantly high score on ZAS & KBDI, there were more acceptable attitude toward suicide. So, We consider suicide to be used as coping strategy about more stressful event to young adolescent.

  • Objectives: This study aimed to explore the influence of depression severity, disease course, treatment status, and other factors on cognitive function in adolescents with depressive disorders. Methods: Participants who met the inclusion criteria were enrolled in the study. Sociodemographic data of each participant were recorded, including age, sex, and family history of mental disorders. Zung's Self-Rating Depression Scale was used to assess depression status in adolescents. Moreover, P300 and mismatch negativity (MMN) were used to objectively evaluate the participants' cognitive function. Results: Only 26.8% of the adolescents with depression received standard antidepressant treatment. The latencies of N2 (267.80±23.34 ms), P3 (357.71±32.09 ms), and MMN (212.10±15.61 ms) in the adolescent depression group were longer than those in the healthy control group (p<0.01). Further analysis revealed that the latency of MMN was extended with increased levels of depression in adolescents. The MMN latency was short in participants with depression receiving standardized treatment. Furthermore, the latency of MMN was positively correlated with the severity and duration of depression (correlation coefficients were 0.465 and 0.479, respectively) (p<0.01). Conclusion: Receiving standardized treatment and shortening the course of depression can reduce cognitive impairment in adolescents with depression.

  • Purpose: To identify characteristics of chronic fatigue, difference between the duration of fatigue and characteristics of chronic fatigue, and association between chronic fatigue and related factors in adults. Method: The subjects for this study were 180 adults who had experienced fatigue for over one month The measurement tools were the Revised Fatigue Scale by Chalder et al. (1993), the Visual Analogue Scale-energy developed by Lee et al. (1991), the BEPSI (Brief Encounter Psychosocial Instrument) by Frank & Zyzanski (1988), Zung's self rating depression scale (1965) and life style items including exercise sleep, drinking smoking and diet as developed by the researchers. Results : Those who complained of fatigue over six months experienced higher disturbances in their daily life compared to those who had suffered from fatigue less than six months. There were a significant correlation between severity of fatigue and depression (r=.46, p<.001), stress (r=.41, p<.001), and sleep (r=.20, p<.01) Statistically significant relationships were found between severity of fatigue and amount of exercise per week (F=3.79. p<.05) disturbed sleep (t=-2.66, p<.01), number of times awakened during the night (F=3.48, p<.05) types of drinking (F=2.65, p<.05), and diet regularity (F=5.83, p<.01). The construction of a multiple regression model revealed an adjusted $R^2$ of .27 with the depression score serving the major predictor variables for severity of fatigue. Men and people in the younger age group were more energetic than women and those in other age groups. Also married people experienced more fatigue than people who were single. divorced or separated. Conclusion: In nursing interventions for fatigue, medical personnel should consider sociodemographic characteristics of the clients, ways to reduce of stress and counter disturbances in daily life and develop strategies for a health promoting life style.

  • The purpose of this study was to identify factors related to daily activities of post myocardial infarction patients. Data were collected from Aug. 1 to Aug. 31, 1988 through an interview schedule lasting for about 30~60 minutes by the investigators. Tools for this study were a daily activities scale and a subjective physical symptom scale developed by the researchers, and Zung's self rating depression scale. The subjects were 45 men 18 women post myocardial infarction patients who were receiving follow up care at SNU Hospital. The data were analyzed by percentage, mean t-test ANOVA, the Pearson moment Correlation Coefficient test, and Cronbach's $\alpha$ reliability test. The results were as follows. \circled1 Reliability of the daily activities scale was 0.91 by Cronbach's $\alpha$. In the daily activities scale, items about transfer, exercise, and job related activities were most highly rated as meaningful to the post myocardial infarction patients. \circled2 The average daily activities score of these patients was 3.30 (maximum point ; 4) all scores were high except for exercise and job related activities. \circled3 There was a significant difference in daily activities according to sex ; men had a higher daily activities score than women (T=2.32, p<0.05). \circled4 There was a significant difference in daily activities according to job status. Subjects having a job had a higher daily activities score. \circled5 The lower the depression score, the higher the daily activities score(${\gamma}$=-0.5748, P<0.05). \circled6 The lower the subjective physical symptoms score, the higher the daily activities score(${\gamma}$= -0.6015, p<0.05).
