영양 상담서비스 품질에 대한 환자의 만족도와 인식조사 (Patient Satisfaction and Perception on Nutritional Counseling Services Quality)

  • 전국의 43개 상급종합병원과 20개 상급병원의 환자 1,095명을 대상으로 환자의 영양 상담서비스에 대한 인식을 조사하고, 만족도는 서비스 품질 개념을 적용하여 분석하였다. 환자의 영양 상담서비스 인식 평균점수는 4.54/5.00점이었다. 환자의 영양 상담서비스 만족도를 요인 분석한 결과 서비스 품질 5개 영역으로 추출되었고, 각각 공감성, 반응성, 유형성, 신뢰성, 전문성으로 명명, 분류하였다. 5개 영역의 전체 평균 점수는 4.56점/5.00이었고 공감성 4.62점, 반응성 4.52점, 유형성 4.53점, 신뢰성 4.65점, 전문성 4.61점이었다. 학력에 따른 차이에서 영양 상담서비스 품질 만족도 총 평균은 대졸 이상이 중졸 이하보다 유의적(P<0.001)으로 높았다. 수도권 병원의 만족도 점수는 비수도권 병원보다 전체 평균 점수와 반응성, 공감성, 전문성, 신뢰성(P<0.001), 유형성(P<0.05)에서 유의적으로 높았다. 상담횟수에서는 반응성 영역에서 상담을 1회 받은 군이 2회보다 유의적(P<0.01)으로 높은 만족도를 보였다. 환자의 영양 상담서비스 인식과 만족도 간에 신뢰성(r=0.721), 반응성(r=0.697), 공감성(r=0.690), 전문성(r=0.678), 유형성(r=0.622)에서 양(+)의 상관관계를 보였다. 이상을 살펴볼 때 비수도권 병원에서는 임상영양사 수의 확보를 통해 환자의 영양 상담서비스 품질을 향상시키는 것이 필요하겠다. 또한, 영양 상담이 반복될수록 환자의 기대치가 높아질 수 있으므로 임상영양사는 영양 상담 내용 및 기법에 변화를 주고 다양한 자료를 활용하여 영양 상담서비스 품질을 향상시킬 수 있을 것이다. 영양 상담서비스 품질 만족도와 영양 상담 인식 간에는 높은 상관관계가 있는데 서비스 품질 만족도에서 유형성과 반응성은 다른 영역보다 상대적으로 낮은 점수를 보였으므로, 상담실 분위기를 쾌적하게 하고 다양한 교육매체를 활용하며 환자와의 여유 있는 상담시간 및 선택적 상담시간 확보 등 해당 영역에 대한 관리가 특히 필요할 것으로 여겨진다.

서울소재 일개 종합병원의 CRM에 대한 기대만족도와 실제만족도 비교와 VOC활용 (A Comparison Research on the Expected Satisfaction and Actual Satisfaction on VOC of one General Hospital)

  • 본 한국 보건산업은 병원들의 경쟁과 함께 급격히 변화하고 있다. 과거에 병원은 경쟁력있는 전략방법없이 이윤을 창출하였다. 게다가 병원은 고객들의 요구와 원하는 것을 중요하게 여기지 않았다. 그러나, 요즈음 점점 심해지는 병원들의 경쟁은 경쟁적인 관리 전략과 고객관계관리를 매우 중요시 다루고 있다. 본 연구는 의료기관을 방문하는 외래환자와 입원환자에 대해서 의료서비스를 받기 전의 기대만족도와 의료서비스를 받은 후의 만족도를 비교하고 VOC에 대한 환자들의 반응을 분석한 후 환자만족도를 높이기 위한 방안을 찾고자 한다. 연구의 방법은 환자의 만족도를 좀 더 자세하게 파악하기 위해 본 연구의 목적에 맞게 개발된 설문지를 외래, 입원환자 각각 100명을 조사하였으며 분석은 의료서비스의 전, 후의 만족도 비교이므로 Paired - Test와 기술통계등을 이용하였다. 연구결과 외래환자는 간호사의 친절도에서만 만족도의차이가 통계적으로 유의한 것으로 나왔으나 입원환자의 경우 의료진과 직원의 친절도 그리고 회진서비스에서 만족도의 차이가 통계적으로 유의한 것으로 나왔다. 따라서, 조사된 입원, 외래환자 만족도 항목 중 의료서비스관련 만족도에 차이를 보이고 있으므로 그에 대한 좀 더 상세한 분석과 대책 방안 강구를 위해 VOC 를 지속적으로 발전 활용하는 것이 환자만족도를 높이고 그에 따른 의료기관의 경영효율화를 높이는데 매우 도움이 클 것으로 사료된다.

치위생과정에서의 일부 프로그램 적용에 따른 환자의 구강건강지식, 인식, 실천과 만족도 조사 (A Study on Oral Health Knowledge, Recognition, Practice and Satisfaction of Patients by Applying a Targeted Program within a Dental Hygiene Process)

  • 본 연구는 치과 임상의 예방진료실에서 임상예방진료를 받은 환자의 구강건강인식도, 지식도, 실천도, 만족도를 비교분석하였다. 구강건강지식, 인식, 실천은 임상예방진료 전후 유의한 차이가 있었다. 임상예방진료프로그램 중 가장 만족도가 높은 것은 계획단계의 개별 예방계획과 실행단계의 PTC였고, 그 다음으로 평가단계의 평가결과 알림이었다. 만족 이유는 의료진이 친절해서가 가장 높았고, 재방문 점수는 10점 만점에 9.34점, 지인 추천 점수는 9.75점이었다. 지인 추천과 정의 상관관계가 있는 것은 임상예방진료프로그램 중 치아확인, 사진촬영, 계획, PMTC, TBI, 평가였다. 위의 결과를 통해 치위생과정을 근거한 임상예방진료프로그램은 환자의 구강건강에 대한 지식, 인식, 실천에 긍정적으로 효과가 있었고, 임상예방진료프로그램에 대한 만족도가 높아 치과 재방문과 지인 추천에 긍정적인 효과가 있었다. 따라서 치위생과정을 근거한 임상예방진료프로그램을 예방진료실에서 적극 활용하여 널리 적용되기를 바란다.

종합병원 외래환자 진료시 의사의 보건교육활동 평가 (An Evaluative Study on Physician's Health Education Activities in Outpatient Medical Care)

  • The main objectives of the present study is to evaluate Physician's Health Education Activities by means of physician's direct response to the prepared questionnaire and patient's perception to the physician in the course of medical care. For the data collection, the present study was conducted from Aug. 16 to Oct. 7, 1983 for 739 patients and 91 physicians who were attended outpatient clinics of 5 general hospitals in Seoul. The major findings are summarized as follows: 1. Self-evaluation on Physician's Health Education Activities (1) In consideration of health education services for the patient, the data revealed that 9.9% of the sampled physician wanted to strength public health and preventive medicine lecture in the curricula at medical education. On the other hand, only 1.1% expressed that they wanted to make it short. (2) In consideration of the necessity of health education service, it was shown that 95.6% of physicians agreed to take it into consideration. Self expression for the practice of health education was placed on the 3.15 score when 5 point scale used. (3) To evaluate the degree of an explanation about medical care for the patient, Index score with 4 point scale was employed. The Index score for the first time was shown that scale was placed on 3.23 for 'diagnosis', 3.12 for 'progress of the disease', 3.11 for 'discription of procedure' and 3.02 for 'cause of the disease' respectively. In comparison of the physician's explanation about the status of disease for the first and the second visitors to clinic, they evaluated themselves as giving more detailed explanation for the second visitors rather than the first visitors. 2. Physician's Health Education Services evaluated by patients (1) To evaluate physician-patient communication at beginning time for taking history about disease, the Index score with 5 point scale was employed. The data on taking history have shown that the score placed on 3.07 for those patients who visited the first time and 2.53 for second visitors. And the score about listening from the patients was placed on 3.52 and 3.42 respectively. (2) The Index score with 5 point scale, as used before, was also employed to evaluate medical care services for the patient. The data evaluated by the patients was shown that the score placed on 4.21 for patient treatment in general, 4.58 for physician's credibility, and 3.6 for physician's kindness. However, approximately 80% of those who failed to understand physician's explanation was caused by highly sophisticated medical terminology. (3) According to the Index score with 4 point scale, to evaluate physician's explanation, the data was shown that the patient who visited the first time gave 2.51 for 'diagnosis', 2.35 for 'progress', 2.11 for 'cause of the disease' and so on. It is acknowledged on the whole that the patients who visited the second time have more satisfaction in physician's explanation about their disease, than those who visited the first time. 3. Comparison of self-evaluation of Physician's Health Education Activities and patient's perception. (1) There was communication barriers between physicians and patients in expressing some medical terminology. For example physician understood that they explained more than 50% of medical terminology into common words for the patient, but 30% of patient complained medical terminology used by physician. (2) Comparing the index score of health education practice recognized by patients and physicians for both first visit and revisit groups, it was shown that the Index score of health education activities evaluated by physicians themselves were slightly higher than the score evaluated by patients.

가골 신연술로 치료한 제 1 중족골 단축증 (Brachymetatarsia of the First Metatarsal treated by Callotasis)

  • Purpose: To analyze the outcome of metatarsal lengthening of first brachymetatarsia by callotasis using an external fixator. Materials and Methods: Between January 1998 and February 2004, 10 patients (17 cases) were reviewed. The mean age at operation was 17.3 years. Seven patients had bilateral first brachymetatarsia and eight patients had combined 4th brachymetatarsia. The operations were performed with a monoexternal fixator, and distraction was started at a rate of 0.75 mm/day after 7 days. The radiographic results were evaluated by lengthening amount and percentage, fixation time, and healing index. Complications and AOFAS score were evaluated. Results: The average lengthening amount was 17.7 mm and the average lengthening percentage was 43.4%. The external fixation time was 107 days and average healing index was 69.8 days/cm. The evaluation according to AOFAS score was excellent in 12 cases and good in 5 cases. Complications were 4 cases of hallux valgus, 4 of metatarsophalangeal joint stiffness, 3 of medial angular deformity, 3 of pes cavus, 2 of pin breakage, 2 of pin site infection, and 1 of skin hyperpigmentation. Conclusion: Callotasis for 1st brachymetatarsia is a very useful treatment method with high patient satisfaction, excellent healing rate and early ambulation without bone graft. Nevertheless, great care must be taken to minimize the various possible complications.

산후 조리원 실내 환경 계획에 관한 연구 (Research on the Interior Environment Planning of a Postpartum Care Center.)

  • 산후조리원은 불과 십여 년 전부터 등장한 시설이지만, 현재 우리나라의 산후 조리 문화를 바꿔놓았다고 해도 과언이 아니다. 저 출산이 사회적 현상이 되어버린 지금 대부분의 산모들은 산후 조리원을 이용하고 있다. 하지만 산후조리원은 법적, 제도적 뒷받침이 되어있지 않은 상태로, 세무서에 사업자 신고만 하면 누구나 시설을 운영할 수 있기 때문에 기본적인 시설을 갖추지 못한 산후조리원이 난립하고 있는 실정이다. 이에 본 연구는 대상자나 운영면에서 일반 시설과 다른 특성을 갖고 있으나 보건 복지법 상이나 의료법 상에서의 시설 기준이 명확치 못한 산후 조리원의 효율적 공간 구성과 사용자의 편의을 고려한 실내 환경 계획을 제시 하였다. 진행은 서울 경기지역의 산후조리원 6곳을 선정하여 조리원 공간의 구성을 진입공간, 거주공간, 공용공간, 간호공간, 관리 및 부속공간 등의 5가지로 구분하여 그 실태를 조사하고, 개선방향을 제시함으로써 향후 건립될 산후 조리원의 실내 환경 계획을 수립하고자 한다. 본 연구는 산후 조리원 이용자들의 만족도를 높이고, 보다 효율적으로 시설이 운영되어 질 수 있도록 도움을 주는 실내 건축 계획의 기초적 자료가 될 것으로 기대한다.

병원선택에 미치는 요인과 사용자 집단 간의 인식차이 - 대학부속병원 입원환자, 보호자, 간호사에 대한 분석적 계층화 의사결정 평가를 중심으로 (Factors that Affect Decisions for Selecting Hospitals and Different Awareness - Focusing on Inpatient, Care-giver, Nurse in University Hospital using AHP)

  • Purpose: Hospitals for patients and their guardians can, from the concept of healing, be removed from just gaining profits, but suggest a future-oriented direction for the hospital. Accordingly, there have been studies related to the selection of hospitals, but most were related to preference and satisfaction, and only recently did research from the concept of tradeoffs of factors for selection began to grow rapidly. Methods: From this context, this study evaluates the level of importance for factors of selecting hospitals using the analytical hierarchy process, and identifies the correlation with users, gender, age group, and outpatient features in order to identify the difference of awareness among different groups for selecting hospitals. In the factors for selection 26 factors in six categories were set through studies of preceding research, and after surveying 144 people, the following results were attained. Results: 1) The overall analysis results were found in the order of medical level, medical service, and fame, and low for facilities, which is similar to the cases of preceding studies. 2) For user analysis, it was similar between patients and guardians, but there was a slight difference in awareness among nurses, who are also medical service providers. Nurses showed relatively high level of importance in direct factors such as medical technologies and medical services, while guardians of patients showed higher importance in indirect factors such as facility environments and convenience. 3) Women showed higher assessments of importance levels in environmental factors, while men in physical factors. 4) The older the age group, the lower level importance there was on medical level, while the importance on fame reduced the further the commute to the hospital was.

간호진단 프로토콜(Protocol)의 임상적용 효과에 관한 연구 (The Effects of Clinical Application of a Nursing Diagnosis Protocol)

  • This study was conducted to measure the effects of clinical application of a Nursing Diagnosis Protocol. The dependent variables were the degree of patient's satisfaction and the degree of nurse's satisfaction with the nursing activity. Analysis of the effect of the use of the nursing diagnosis protocol was based on the nursing record. The subjects for this study were 61 nurses(experimental group 31, control group 30) and 155 patients (experimental group 55, control group 100) on four internal medicine wards in K University Hospital in Seoul. Data collection was done from August to October 12,1988. The results obtained in this study can be summarized as follows, 1, Effect of the clinical application of the nursing diagnosis protocol. 1) The first hypothesis ; “nurses who use the nursing diagnosis protocol will have higher degrees of satisfaction than those who use traditional methods” was rejected (t=.54, df=58, p=.59). 2) The second hypothesis ; “patients nursed by nurses using the nursing diagnosis protocol will have higher degrees of satisfaction than those nursed with traditional methods” was supported(t=1.93, df=154, p=.05). 3) The third hypothisis : Major hypothesis ; “the nursing records of the experimental group, who used the nursing diagnosis protocol, will be more detailed than those of the control group” was supported (t=6.40, df=79.90, p=.000). (1) The first subhypothesis ; “The recorded data collection of the experimental group will be more detailed than that of the control group” was rejected (t=1.79, df=118, p=.07). (2) The second subhypothesis ; “The recorded patient's problem statement of the experimental group will be more detailed than that of the control group”, was supported. (3) The third subhypothesis ; “The nursing record of the experimental group will be more convenient for implementation than that of the control group” was supported. 2. Factors related to the nurse's degree of satisfaction with protocol. 1) No general characteristics(age, religion, education level, duty career, present duty career) were related to the nurse's degree of satisfaction. 2) Variables related to the nurse's degree of satisfaction were “satisfaction as a nurse” and “consider nursing as lifelong job” (t=-2.6, df=13.22, p=.02, t=2.41, df=23.85, p=.02). 3. Factors related to the patient's degree of satisfaction. 1) General characteristics related to the patient's degree of satisfaction with nurses using the protocol were age, educational level, and being married.(F=5.17, df=3/153, p=.00, t= -2.39, df=154, p=,01, f=5.91, df=2/153, p=.00) 2) The variables previous hospitalization, duration of hospitalization, the hospital unit presence of a relative, medical insurance, or medical diagnosis were not related to the patient's degree of satisfaction. 1. The experimental group's nursing record was more detailed than the control group's record with regard to the physical and psychological state of the patients. As noted above, the experimental group nurses, who use a nursing diagnosis had protocol were less satisfied than the control group who used traditional methods of the recording, but experimental group patients had a higher degree of satisfaction than the control group patients. The nursing records of experimental group, using the nursing, diagnosis protocol was more detailed than that of the control group. If the nursing diagnosis protocol is used in clinical nursing practice, the quality of nursing care may be improved.

부산·경남지역 의료소비자들의 전문병원에 대한 지각 만족도와 기대요소 간의 인과관계 (The Cause-and-Effect Relationship between Perceptual Satisfaction and Expectation Factors for Special Hospitals by Busan and Gyungnam Medical Consumers)

  • 본 연구는 중소병원들이 지속적으로 안정성과 효과성을 유지할 수 있는 전략방안의 검토를 위하여 전문병원에 대한 기대수준 대비 이용 후 지각 만족도와 기대요소 간의 관계를 살펴보았다. 연구방법은 20세 이상의 부산 및 경남 거주자를 대상으로 구조화된 설문지를 이용하여, 카이제곱 검정, 로지스틱 회귀분석을 실시하였다. 연구결과, 첫째, 응답자의 68.0%가 특성화를 표방하는 병원을 전문병원으로 인식하였고, 보건복지부 지정병원이라 인식한 응답자는 21.3%로 확인되었다. 둘째, 주관적으로 인식하는 전문병원 이용 경험이 있는 경우에는 대부분의 이용자들이 기대수준 대비 지각하는 만족도가 충족되었으며, 이용했던 병원을 계속 이용 할 의사가 있는 것으로 확인되었다. 셋째, 기대수준 대비 이용 후 지각 만족도에는 치료에 대한 안전성과 치료의 질적 수준이 유의하게 영향을 미치는 인과관계가 있음을 확인하였다. 이 결과는 의료소비자들의 지각 만족도 향상과 감정적 애착 관리를 통한 장기적이고 지속적인 효과를 위한 전략 수립에 유용한 정보로 활용될 수 있을 것으로 사료된다.

비소세포성 폐암 환자의 항암화학요법을 위한 Critical Pathway개발과 적용효과 (Development of a Critical Pathway for the Chemotherapy of Non-small Cell Lung Cancer Patients and Its Effects)

  • The purpose of this study was to develope a critical pathway for the chemotherapy of non-small cell lung cancer patients and to identify its effects after implementation. Critical pathway was developed through 5 steps including content and clinical validity tests with collaborative efforts of nurses, clinicians, and other allied healthcare professionals with the aim of improving the quality of patient care, while minimizing cost to the patients. This paper was described an evaluation of the impact of a developed critical pathway on complication rate, length of stay, costs, the interval of treatment and patient satisfaction by nonequivalent control group posttest-only non-synchronized research design.Results were compared between the two groups of patients. There were no significant differences in demographic variables and the occurrence of bone marrow suppression between experimental group and control group(t=-0.01, p=0.992). There were statistically significant decreases in the average length of stay(t=-10.45, p=0.000), in the average cost(t=-2.988, p=0.004), and in the interval of treatment(t=-6.75, p=0.000) after implementation of the critical pathway compared to control group. Also, there was a statistically significant improvement of the patient satisfaction after implementation of the critical pathway compared to control group(t=4.57, p=0.000). This paper concludes that critical pathway in chemotherapy for lung cancer, implemented in the context of an general hospital, is the useful tool to shorten the hospital stay, reduce treatment costs, and improve the quality of life in cancer patients. Further study needs to be conducted to identify other clinical outcomes including job satisfaction, collaboration among health professionals and potential for use in education. Also, it is recommended that nurses should revise continuously the developed critical pathway through clinical implementation and maintain their role of patient advocacy through monitoring pathway compliance.
