Fibulin-3 as a Diagnostic Biomarker in Patients with Malignant Mesothelioma

  • Background: New tumour biomarkers are being intensely investigated for malignant mesothelioma (MM). Fibulin-3 is produced in MM but its role remains uncertain. The aim of this study was to evaluate the validity of measuring serum fibulin-3 in the diagnosis and prognosis of MM. Materials and Methods: This prospective study was performed on 43 patients and 40 healthy controls who were admitted to our hospital between January 2012 and January 2014. Data from MM patients, including demographic and clinical features, routine laboratory data, levels of serum fibulin-3, and treatment outcomes were defined as potential prognostic factors. The receiver operating characteristic (ROC) curve for fibulin-3 was used to detect the cut-off value with highest sensitivity and specificity. Univariate survival analysis was performed using the Kaplan-Meier method in patients with MM. Afterwards, the possible factors identified with univariate analyses were entered into the cox regression analysis. Results: Our results revealed that patients with MM had significantly higher serum levels of fibulin-3 than controls. The results showed that the best cut-off point was 36.6 ng/ml with an AUC (area under the curve)=0.976, sensitivity=93.0% and specificity=90.0. In our study, the initial significant poor prognostic factors were advanced stage, high white blood cell count, high platelet count, high C-reactive protein (p<0.05 for each variable). Later, according to multivariate analysis the results showed only advanced stage as significant parameter (p=0.040). Conclusions: We determined that real use for serum fibulin-3 was not for prognosis but for diagnosis in MM. Also advanced stage was associated with poor MM prognosis.

  • Prostatic acid phosphatase (PAP)는 전립선 암의 진단에 널리 사용되는 표지자로서 1935년 처음으로 동정되었고 인체 전립선에 가장 많이 존재하는 탈 인산화효소이다. PAP는 prostate epithelial cells에서 합성되는 전립선 특이적인 효소로서, 산성 환경에서 효소활성을 띠는 acid phosphatase 그룹에 속한다. PAP는 전립선액에 풍부히 존재하여 수정, 정자부족증, 만성통증의 감소에 관여한다. 그러나 가장 눈에 띄는 기능은 ERK1/2와 MAPK 경로에 관계된 HER-2와 PI3P의 탈 인산화를 유도하여 세포 성장 신호를 억제하고 전립선 암의 억제자로 작용하는 것이다. 최근 PAP DNA 백신을 이용하는 임상시험이 현재 진행 중이고, PAP를 이용한 immunotherapy를 통해 전립선 암을 치료하는 방법이 FDA의 승인을 받아 시행되고 있다. 이러한 PAP의 임상적 중요성에도 불구하고 현재까지 PAP의 분자적 조절기작에 대한 이해는 제한적이라 PAP에 대한 많은 연구가 필요한 실정이다. PAP는 NF-${\kappa}B$, TNF-${\alpha}$, IL-1 및 androgen과 androgen receptor에 의하여 promoter region이 조절된다고 알려졌다. 본 총설에서는 현재까지 밝혀진 PAP 유전자 및 단백질의 특징들과 더불어 전립선 암에서의 PAP의 기능, 발현 조절, 역할들을 종합하였다.

최근 중의학에서 시스템생물학의 발전 현황 - 한의학에 미치는 영향 및 시사점을 중심으로 - (Current Status of Systems Biology in Traditional Chinese medicine - in regards to influences to Korean Medicine)

  • Objectives : This paper serves to explore current trends of systems biology in Traditional Chinese Medicine (TCM) and examine how it may influence the Traditional Korean medicine. Methods : Literature review method was collectively used to classify Introduction to systems biology, diagnosis and syndrome classification of systems biology in TCM perspective, physiotherapy including acupuncture, herbs and formula functions, TCM systems biology, and directions of academic development. Results : The term 'Systems biology' is coined as a combination of systems science and biology. It is a field of study that tries to understand living organism by establishing a theory based on an ideal model that analyzes and predicts the desired output with understanding of interrelationships and dynamics between variables. Systems biology has an integrated and multi-dimensional nature that observes the interaction among the elements constructing the network. The current state of systems biology in TCM is categorized into 4 parts: diagnosis and syndrome, physical therapy, herbs and formulas and academic development of TCM systems biology and its technology. Diagnosis and syndrome field is focusing on developing TCM into personalized medicine by clarifying Kidney yin deficiency patterns and metabolic differences among five patterns of diabetes and analyzing plasma metabolism and biomarkers of coronary heart disease patients. In the field of physical therapy such as acupuncture and moxibustion, researchers discovered the effect of stimulating acupoint ST40 on gene expression and the effects of acupuncture on treating functional dyspepsia and acute ischemic stroke. Herbs and formulas were analyzed with TCM network pharmacology. The therapeutic mechanisms of Si Wu Tang and its series formulas are explained by identifying potential active substances, targets and mechanism of action, including metabolic pathways of amino acid and fatty acid. For the academic development of TCM systems biology and its technology, it is necessary to integrate massive database, integrate pharmacokinetics and pharmacodynamics, as well as systems biology. It is also essential to establish a platform to maximize herbal treatment through accumulation of research data and diseases-specific, or drug-specific network combined with clinical experiences, and identify functions and roles of molecules in herbs and conduct animal-based studies within TCM frame. So far, few literature reviews exist for systems biology in traditional Korean medicine and they merely re-examine known efficacies of simple substances, herbs and formulas. For the future, it is necessary to identify specific mechanisms of working agents and targets to maximize the effects of traditional medicine modalities. Conclusions : Systems biology is widely accepted and studied in TCM and already advanced into a field known as 'TCM systems biology', which calls for the study of incorporating TCM and systems biology. It is time for traditional Korean medicine to acknowledge the importance of systems biology and present scientific basis of traditional medicine and establish the principles of diagnosis, prevention and treatment of diseases. By doing so, traditional Korean medicine would be innovated and further developed into a personalized medicine.

Gene Expression Profiling of Genotoxicity Induced by MNNG in TK6 Cell

  • Genotoxic stress triggers a variety of biological responses including the transcriptional activation of genes regulating DNA repair, cell survival and cell death. In this study, we investigated to examine gene expression profiles and genotoxic response in TK6 cells treated with DNA damaging agents MNNG (N-methyl-N'-nitrosoguanidine) and hydrogen peroxide $(H_2O_2)$. We extracted total RNA in three independent experiments and hybridized cRNA probes with oligo DNA chip (Applied Biosystems Human Genome Survey Microarray). We analyzed raw signal data with R program and AVADIS software and identified a number of deregulated genes with more than 1.5 log-scale fold change and statistical significancy. We indentified 14 genes including G protein alpha 12 showing deregulation by MNNG. The deregulated genes by MNNG represent the biological pathway regarding MAP kinase signaling pathway. Hydrogen peroxide altered 188 genes including sulfiredoxins. These results show that MNNG and $H_2O_2$ have both uniquely regulated genes that provide the potential to serve as biomarkers of exposure to DNA damaging agents.

Salivary microbiota in periodontal health and disease and their changes following nonsurgical periodontal treatment

  • Purpose: The aims of this study were to examine the salivary microbiota in conditions of periodontal health and disease and to explore microbial changes following nonsurgical periodontal treatment. Methods: Non-stimulated saliva samples were collected from 4 periodontally healthy participants at baseline and from 8 patients with chronic periodontitis at baseline and 3 months following nonsurgical periodontal therapy. The V3 and V4 regions of the 16S rRNA gene from the DNA of saliva samples were amplified and sequenced. The salivary microbial compositions of the healthy participants and patients with periodontitis prior to and following nonsurgical treatment of periodontitis were compared based on the relative abundance of various taxa. Results: On average, 299 operational taxonomic units were identified in each sample. The phylogenetic diversity in patients with periodontitis was higher than that in healthy participants and decreased following treatment. The abundance of the phylum Spirochaetes and the genus Treponema in patients with periodontitis was 143- and 134-fold higher than in the healthy control group, respectively, but decreased significantly following treatment. The species that were overabundant in the saliva of patients with periodontitis included the Peptostreptococcus stomatis group, Porphyromonas gingivalis, the Fusobacterium nucleatum group, Parvimonas micra, Porphyromonas endodontalis, Filifactor alocis, and Tannerella forsythia. The phylum Actinobacteria, the genus Streptococcaceae_uc, and the species Streptococcus salivarius group were more abundant in healthy participants than in those with periodontitis. There was a trend toward a decrease in disease-associated taxa and an increase in health-associated taxa following treatment. Conclusions: Our results revealed differences in the taxa of salivary microbiota between conditions of periodontal health and disease. The taxa found to be associated with health or disease have potential for use as salivary biomarkers for periodontal health or disease.

Whole genome MBD-seq and RRBS analyses reveal that hypermethylation of gastrointestinal hormone receptors is associated with gastric carcinogenesis

  • DNA methylation is a regulatory mechanism in epigenetics that is frequently altered during human carcinogenesis. To detect critical methylation events associated with gastric cancer (GC), we compared three DNA methylomes from gastric mucosa (GM), intestinal metaplasia (IM), and gastric tumor (GT) cells that were microscopically dissected from an intestinal-type early gastric cancer (EGC) using methylated DNA binding domain sequencing (MBD-seq) and reduced representation bisulfite sequencing (RRBS) analysis. In this study, we focused on differentially methylated promoters (DMPs) that could be directly associated with gene expression. We detected 2,761 and 677 DMPs between the GT and GM by MBD-seq and RRBS, respectively, and for a total of 3,035 DMPs. Then, 514 (17%) of all DMPs were detected in the IM genome, which is a precancer of GC, supporting that some DMPs might represent an early event in gastric carcinogenesis. A pathway analysis of all DMPs demonstrated that 59 G protein-coupled receptor (GPCR) genes linked to the hypermethylated DMPs were significantly enriched in a neuroactive ligand-receptor interaction pathway. Furthermore, among the 59 GPCRs, six GI hormone receptor genes (NPY1R, PPYR1, PTGDR, PTGER2, PTGER3, and SSTR2) that play an inhibitory role in the secretion of gastrin or gastric acid were selected and validated as potential biomarkers for the diagnosis or prognosis of GC patients in two cohorts. These data suggest that the loss of function of gastrointestinal (GI) hormone receptors by promoter methylation may lead to gastric carcinogenesis because gastrin and gastric acid have been known to play a role in cell differentiation and carcinogenesis in the GI tract.

MTHFR 3'-untranslated region polymorphisms contribute to recurrent pregnancy loss risk and alterations in peripheral natural killer cell proportions

  • Objective: To identify the associations between polymorphisms of the 3'-untranslated region (UTR) of methylenetetrahydrofolate reductase (MTHFR) gene, which codes for an important regulatory enzyme primarily involved in folate metabolism, and idiopathic recurrent pregnancy loss (RPL) in Korean women. Methods: The study population comprised 369 RPL patients and 228 controls. MTHFR 2572C > A, 4869C > G, 5488C > T, and 6685T > C 3'-UTR polymorphisms were genotyped by polymerase chain reaction-restriction fragment length polymorphism analysis or by TaqMan allelic discrimination assays. Natural killer cell proportions were determined by flow cytometry. Results: The MTHFR 2572-5488-6685 (A-C-T) haplotype had an adjusted odds ratio of 0.420 (95% confidence interval, 0.178-0.994; p= 0.048) for RPL. Analysis of variance revealed that MTHFR 4869C > G was associated with altered $CD56^+$ natural killer cell percentages (CC, $17.91%{\pm}8.04%$; CG, $12.67%{\pm}4.64%$; p= 0.024) and folate levels (CC, $12.01{\pm}7.18mg/mL$; CG, $22.15{\pm}26.25mg/mL$; p= 0.006). Conclusion: Variants in the 3'-UTR of MTHFR are potential biomarkers for RPL. However, these results should be validated in additional studies of ethnically diverse groups of patients.

Lomens-P0 (mixed extracts of Hordeum vulgare and Chrysanthemum zawadskii) regulate the expression of factors affecting premenstrual syndrome symptoms

  • BACKGROUND/OBJECTIVES: Premenstrual syndrome (PMS) is a disorder characterized by repeated emotional, behavioral, and physical symptoms before menstruation, and the exact cause and mechanism are uncertain. Hyperprolactinemia interferes with the normal production of estrogen and progesterone, leading to PMS symptoms. Thus, we judged that the inhibition of prolactin hypersecretion could mitigate PMS symptoms. MATERIALS/METHODS: Hordeum vulgare L. extract (HVE), Chrysanthemum zawadskii var. latilobum extract (CZE), and Lomens-P0 the mixture of these extracts were tested in subsequent experiments. The effect of extracts on prolactin secretion at the in vitro level was measured in GH3 cells. Nitric oxide and pro-inflammatory mediator expression were measured in RAW 264.7 cells to confirm the anti-inflammatory effect. Also, the hyperprolactinemic Institute for Cancer Research (ICR) mice model was used to measure extract effects on prolactin and hormone secretion and uterine inflammation. RESULTS: Anti-inflammatory effects of and prolactin secretion suppress by HVE and CZE were confirmed through in vitro experiments (P < 0.05). Treatment with Lomens-P0 inhibited prolactin secretion (P < 0.05) and restored normal sex hormone secretion in the hyperprolactinemia mice model. In addition, extracts significantly inhibited the expression of pro-inflammatory biomarkers, including interleukin-1𝛽, and -6, tumor necrosis factor-𝛼, inducible nitric oxide synthase, and cyclooxygenase-2 (P < 0.01). We used high-performance liquid chromatography analyses to identify tricin and chlorogenic acid as the respective components of HVE and CZE that inhibit prolactin secretion. The Lomens-P0, which includes tricin and chlorogenic acid, is expected to be effective in improving PMS symptoms in the human body. CONCLUSIONS: The Lomens-P0 suppressed the prolactin secretion in hyperprolactinemia mice, normalized the sex hormone imbalance, and significantly suppressed the expression of inflammatory markers in uterine tissue. This study suggests that Lomens-P0 may have the potential to prevent or remedy materials to PMS symptoms.

신평·장림 산단 인근 주민의 PAHs 및 VOCs 노출 (Exposure to PAHs and VOCs in Residents near the Shinpyeong·Jangrim Industrial Complex)

  • Objectives: This study aims to investigate the atmospheric concentration of polycyclic aromatic hydrocarbons (PAHs) and volatile organic compounds (VOCs) and the urinary concentration of biomarkers in residents near the Shinpyeong·Jangrim Industrial Complex to compare them with those of residents in a control area. Methods: Hazardous air pollutants (PAHs and VOCs) were measured in an exposure area (two sites) and a control area (one site). Urine samples were collected from residents near the industrial complex (184 persons) and residents in the control area (181 persons). Multiple linear regression analysis was used to identify which factors affected the concentration of PAHs and VOCs metabolites. Results: The average atmospheric concentration of PAHs in Shinpyeong-dong and Jangrim-dong was 0.45 and 0.59 ppb for pyrene, 0.15 and 0.16 ppb for benzo[a]pyrene, and 0.29 and 0.35 ppb for dibenz[a,h]anthracene. The average atmospheric concentration of VOCs was 1.10 and 0.99 ppb for benzene, 8.22 and 11.30 ppb for toluene, and 1.91 and 3.05 ppb for ethylbenzene, respectively. The concentrations of PAHs and VOCs in residents near the Shinpyeong·Jangrim Industrial Complex were higher than those of residents in the control area. Geometric means of urinary 2-hydroxyfluorene, 1-hydroxypyrene, methylhippuric acid, and mandelic acid concentrations were 0.45, 0.22, 391.51, and 201.36 ㎍/g creatinine, respectively. Those levels were all significantly higher than those in the control area (p<0.05). In addition, as a result of multiple regression analysis, even after adjusting for potential confounding factors such as gender and smoking, the concentration of metabolites in urine was high in residents near the Shinpyeong·Jangrim Industrial Complex. Conclusion: The results of this study show the possibility of human exposure to VOCs in residents near the Shinpyeong·Jangrim Industrial Complex. Therefore, continuous monitoring of the local community is required for the management of environmental pollutant emissions.

이동성 유전인자의 구조 및 생물학적 기능 (Biological Function and Structure of Transposable Elements)

  • 이동성 유전인자는 인간 유전체의 45%를 차지하며 기능성 유전자 내부로 자유롭게 들어갈 수 있다. 이들은 진화과정에서 중복현상으로 다수의 복사수로 생성되며, 생물종다양성 및 계통유전체학 분야에 기여한다. 이동성 유전인자의 대부분은 메틸화 또는 아세틸화 현상과 같은 후성유전학적 조절에 의해 제어된다. 다양한 생물종은 그들만의 고유의 이동성 유전인자를 가지고 있으며, 일반적으로 DNA트란스포존과 레트로트란스포존으로 나뉜다. 레트로트란스포존은 LTR의 유무에 따라 다시 HERV와 LINE으로 구분된다. 이동성 유전인자는 프로모터, 인핸서, 엑손화, 재배열 및 선택적 스플라이싱과 같은 다양한 생물학적 기능을 수행한다. 또한 이들은 유전체의 불안정성을 야기시켜 다양한 질병을 유발하기도 한다. 따라서, 암과 같은 질병을 진단하는 바이오 마커로 사용될 수 있다. 최근, 이동성 유전인자는 miRNA를 만들어 내는 것으로 밝혀졌으며, 이러한 miRNA는 타겟 유전자의 seed 영역에 결합함으로서 mRNA의 분해 및 번역을 억제하는 역할을 수행한다. 이동성 유전인자 유래의 miRNA는 기능성 유전자의 발현에 큰 영향을 미친다. 다양한 생물종과 조직에서 서로 다른 miRNA의 비교 분석 연구는 생물학적 기능과 관련하여 진화학과 계통학 영역에서 흥미 있는 연구 분야라 할 수 있겠다.
