Study on Treatment Options for Pediatrics Based on DongEuiBoGam (동의보감(東醫寶鑑) 소아문(小兒門) 처방(處方)에 대한 고찰(考察))

  • Objectives The purpose of this study is to find a way to apply current oriental pediatrics treatment guidelines based on a pediatric chapter in DongEuiBoGam. Methods I learned pediatrics prescription formula, indications of drugs, dosage regimen from DongEuiBoGam, and I classified herbs that can be used for pediatrics prescriptions based on their types, effects, tempers, flavors, toxicities, and attributive channels. Results & Conclusions I learned similarities among pediatrics prescription regimens and the herbs commonly used in pediatrics. Also, I found out physiological and pathological characteristics in pediatrics. I expect that this study would be helpful for current oriental pediatric treatments.

  • Objectives: The purpose of this study is to find a way to apply current oriental pediatrics treatment guidelines based on a pediatric herb chapter in Dong-eui-bo-gam. Methods: We classified herbs based on the four properties, five flavors, attributive channel, a dosage form of the medicine, diagnosis, toxicity and character. Results & Conclusions: The result of this study is that there were lots of things that we have to learn - characteristics of common herbs in pediatrics and pediatric diseases and pathology. From this study, we also learn about changes of pediatric physiology and pathology. More related studies can be helpful for updating the current oriental pediatric guidelines.

  • Historically, children have inevitably been under different treatments depending on their circumstances. In noble families, overprotection rather damaged children; in general, children undergoing important stages of life were exploited as laborers and not cared for. In order to help these children grow whole and healthy, pediatrics has provided information on the right environment and medical treatments for children. In traditional Korean medicine, it is said, "the reason I put gynecology and pediatrics in the first chapters $\cdots$ of this book (Chapter on Pediatrics, Euibangyoochui) is to show my intention of respecting the origin", emphasizing the significance of children as the basis of a person. This is distinguished not only by philosophical implication, but also by its practical contribution to children's health. Euibangyoochui 醫方類聚 is the major book of 15th-century Korean medicine, compiled medicine until Joseon. Through Chapter on pediatrics, which collected medical theories and treatments on children, this paper studies how children had been perceived in traditional Korean medicine, and how it was followed by corresponding medical treatments.

A Study On Pediatrics Contents In 『Maijing』 -Focusing On 「Chapter 9: Determining Pediatric Diseases 9th」- (『맥경(脈經)』의 소아과학(小兒科學) 내용 연구 - 「권제구(卷第九)·평소아잡병증제구(平小兒雜病證第九)」를 중심으로 -)

  • Objectives : To study the pediatric contents in the 『Maijing』, the most comprehensive compilation of pulse theory. Methods : First, the original meaning was understood comprehensively through careful translation of the original text. Next, the original texts from which 『Maijing』 quoted certain verses were traced. Then, contents of 『Maijing』 were analyzed through comparison with contents from later period texts such as 『Beijiqianjinyaofang』, 『Zhubingyuanhoulun』, 『Xiaoeryaozhengzhijue』, 『Zhengzhizhunsheng』. Results : The study of pediatric contents of Wangshuhe's 『Maijing·Chapter9·Determining Pediatric Diseases 9th』 revealed that he set the standards of 'normal pulse' in terms of number of pulsation and pulse xiang[脈象] differently for children compared to adults. He summarized the most common disease patterns to be wind epilepsy[風癎], indigestion of breast milk[乳不消], and fright seizure[客忤氣], and described the pulses that reflected these conditions's physical characteristics. He also described the pulse and symptom patterns of 'growth fever[變蒸]' and 'heat in bone part[骨間有熱]' based on his observation, which contents were quoted and developed in 『Zhubingyuanhoulun』 and 『Xiaoeryaozhengzhijue』. For other miscellaneous pediatric conditions, he quoted prior texts such as 『Lingshu』 while adding words or making modifications to better reflect characteristics of children based on his observations in clinical pediatrics. Conclusions : It is concluded that 『Maijing·Chapter9·Determining Pediatric Diseases 9th』 not only describes pulse diagnostics but reflects in its contents pediatric theories and clinical knowledge of the Jin(晉)period, which affected pediatrics development of following periods.

Studies on application of Radix Puerariae main blended Prescription from Donguibogam (동의보감(東醫寶鑑) 중(中) 갈근(葛根)이 주약(主藥)으로 배오(配伍)된 처방(處方)의 방제학적(方劑學的) 고찰(考察))

  • Through investigation on prescriptions from Dongeuibogam with Radix Puerariae as an ingredient, the following conclusions were reached. The 59 prescriptions with Radix Puerariae as an ingredient are mentioned from chapter of Pediatrics to chapter of Intestinal Convulsions. And 85% of those appears from chapter of Pediatrics to chapter of coughs. Prescriptions with Radix Puerariae as the main ingredient are used for skin diseases, cold, epidemics, abscess, diabetes, stroke, feverish diseases, intestinal diseases, weakness and cough. The dosage of Radix Puerariae varies from 0.875g to 11.25g. 3.75g was the most frequently used dosage which accounts 44%. And those with small dosage of 0.875 to 1.125g were for use on infants or pregnant women. The grounding pathology used for prescriptions with Radix Puerariae as an ingredient appeared to be mostly the pathology related to the Six External Qi. Among this, wind with cold(風寒), wind, cold with heat and humidity(風寒署濕), catching cold(傷寒), successively catching cold(兩感傷寒), cold of the Yangmyung and heat clogging(陽明傷寒火鬱), abnormal weather(時令不正) are the majority. Through the prescriptions with Radix Puerariae as an ingredient, it could be concluded Radix Puerariae that has various functions such as sweating and removing rash(發表透疹), exhalation and curing muscular ache(發散解肌), producing body fluid and ceasing thirst(生津止渴), opening abscess and healing the scar(托瘡生肌), removal of fever(解熱), resolving hangover(醒酒). Seungmagalguntang(升麻葛根湯) is used as the base prescription the most, and various prescriptions and herbs are combined according to the case.

Introducing the general management of glomerular disease from a pediatric perspective based on the updated KDIGO guidelines

  • In 2021, a new chapter on the general management of glomerulonephritis (GN) was added to the Kidney Disease: Improving Global Outcomes (KDIGO). It emphasizes the importance of early general management of GN for improving long-term kidney outcomes and prognosis. The chapter introduces the management of glomerular diseases in 18 subchapters. Here, kidney biopsy for the diagnosis and evaluation of kidney function and the management of complications, such as hypertension, infection, and thrombosis, are presented. Moreover, the adverse effects of glucocorticoids and immunosuppressive therapy, which are commonly used drugs for glomerular disease, are mentioned, and a guideline for drug selection is presented. Each subtheme focused on items reflecting the interpretation of the "practice points" of the expert working group are introduced. In this review of the general treatment for GN in the KDIGO guidelines, excluding pregnancy and reproductive health, we focused on and compared various references pertaining to pediatric GN management.

A Study on Application of Semen Armeniacae Amarum Main Blended Prescription from Donguibogam (동의보감(東醫寶鑑) 중(中) 행인(杏仁)이 주약(主藥)으로 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察))

  • This report describes 61 studies related to the use of Semen Armeniacae Amarum main blended prescriptions from Donguibogam. The following conclusions were reached through investigations on the prescriptions that use Semen Armeniacae Amarum as a key ingredient. Semen Armeniacae Amarum blended prescriptions are utilized for 16 therapeutic purposes, for example, in symptoms of cough, excrements, wind, and pediatrics. In particular, 52.46% of the prescriptions appear in the chapter of cough, and 9.84% of those appear in the chapter of excrements. Prescriptions that utilize Semen Armeniacae Amarum as the main ingredient are used in the treatment of cough, asthma, lung disease, and constipation, and they are also used for treating 15 different types of diseases. Semen Armeniacae Amarum is used in pathogenic factors such as wind, wind with cold, and wind with moisture, in pathologies related to lung and digestive systems, and in those related to weakness, extravasated blood, and alcoholic drink-food. The actual amount of Semen Armeniacae Amarum blended has ranged at a wide variety of amounts from 5 pun to 5 don. 47.62% of the prescriptions used 1 don. When Semen Armeniacae Amarum is combined with base prescriptions such as Samyotang, Banhaenghwan, ljintang, etc and Herba Ephedrae, Rhizoma Pinelliae, and Pericarpium Citri Reticulatae, it ceases cough and asthma. In addition, when Semen Armeniacae Amarum is combined with Fructus Ponciri Seu Aurantii and Fructus Cannabis, it brings a relief in constipation.

A prescription study in 『Dongeuibogam』 for the Applications of Cnidii Rhizoma (천궁(川芎)의 활용(活用)을 위한 『동의보감(東醫寶鑑)』 처방연구(處方硏究))

  • Objectives : Until now the study of Cnidii Rhizoma, hemorrhage, brain waves, such as ischemic brain injury, analgesic, effect overcome of the stress from pregnancy melanin formation and inhibiting effects skin whitening have been published regarding this article. Cnidii Rhizoma demonstrates its different abilities depending on the characteristics. This paper reported that effect of Cnidii Rhizoma in Dongeuibogam blended prescriptions as main medicine. In addition, by analyzing data, we studied about utilizing of Cnidii Rhizoma. Methods : Cnidii Rhizoma in Dongeuibogam Prescriptions as the main ingredient was built with database of 202 prescriptions. Thus analyzed data was summarized in detail.(Table-1) If there is no difference in the title of the prescription but in other case the configuration information is different, formulations 1 and 2 were divided by the table. Results : The following results were reached through investigations on the prescriptions usikng Cnidii Rhizoma as a key component. 1. Prescriptions taking Cnidii Rhizoma as a monarch drug are utilized for 40 therapeutic purposes. In particular, 12.3% of prescriptions appear in the chapter of head, and 10.8% of those appear in the chapter of women, and 9.4% of eye, 8.9% of child, 6.4% of wind disease respectively. 2. Prescriptions utilizing Cnidii Rhizoma as the main ingredient are used in the treatment of headache, dizziness and pregnancy hemorrhage fetal movement, premature birth and they are also used for treating 131 different types of disease. 3. The dosage of Cnidii Rhizoma in formulas is from 2pun(about 0.75g) to 5don(nearly 18.75g), however 1don(nearly 3.75g) has been taken the most for clinical application. 4. We find out that according to herbs or prescriptions, Cnidii Rhizoma has a variety of functions such as ascending & descending of energy. Samultang is the most useful base prescription which used the Cnidii Rhizoma as the main component. Conclusion : These results suggest that, Cnidii Rhizoma once-amount use (don nearly 3.75g) 4g in head, gynecology, ophthalmology, pediatrics and paralysis disease associated with oriental medicine resource development can be considered to be widely used These results suggest that Cnidii Rhizoma was used most with 1 don(4g) and can be widely used for the resource development to the disease such as brain, gynecology, ophthalmologhy, pediatrics and wind-associated symptoms.

A Study on the Organization of the 『Yosandangsinjipeuibangkeumnangjibo』 (『요산당신집의방금낭지보(樂山堂新集醫方錦囊至寶)』의 편제에 대한 연구)

  • Objectives : This paper aims to examine the overall form of the 『Yosandangsinjipeuibangkeumnangjibo』 (『Yosandang』). Methods : The table of contents, organization of the body, citations in the 『Yosandang』 were examined, then compared to the organization of 『Donguibogam』 (『Bogam』) and 『Jejungsinpyeon』 (『Jejung』). Results & Conclusions : Based on the organization of the body, it could be determined that Byun Gwangwon referenced 『Bogam』 and 『Jejung』. However, it was found that 『Yosandang』 strived to be more faithful to the 『Bogam』 than the 『Jejung』 while it also tried to minimize the complicatedness of 『Bogam』. Its organization was unique in that it created new chapters and added contents. Also, through examination of the citations, it could be assumed that those in the 『Yosandang』 were secondary citations of the 『Bogam』 and 『Jejung』. Despite these features, it also cited the 『Zhengzhizhunsheng』 and showed efforts to include new knowledge with citation labels such as '新增[newly added]' '自述[original writing]' and 'Yosan(樂山)'. Discussion : The findings of the comparison and examination process of the organization and citations of the book revealed that the uniqueness of the organization of 『Yosandang』 is most obvious in the pediatrics chapter. This indicates that the author's intention was to deal with pediatric diseases professionally.

4 Weeks Repeated Oral Dose Toxicity Studies with LMK02-Jangwonhwan in SD Rats (LMK02의 Sprague-Dawley 랫드를 이용한 4 주간 반복 경구투여 DRF 독성시험)

  • The oriental medicine Jangwonhwan, which is a boiled extract of 12 medicinal herbs/mushroom, has been prescribed for patients with cognitive dysfunction and it is originally from the Korean medical text, DonguiBogam(amnesia chapter). Recently, a modified recipe of Jangwonhwan (LMK02-Jangwonhwan) consisting of seven medicinal plants/mushroom, was shown to reduce ${\beta}$-amyloid deposition in the brain of Tg-APPswe/PS1dE9 mouse model of Alzheimer disease. The toxicity of LMK02 was investigated in SD rats by oral repeated adminstration for 4 weeks and we tried to determine test does for 13 weeks repeated study. Quality control of tablet form of LMK02 was established by estimating indicative components, Ginsenoside Rg3 of Red Ginseng and Decursin of Angelicagigas Nakai. The toxicity of LMK02 was investigated in 6 weeks old specific pathogen free (SPF) Sprageu-Dawley rats by oral administration. Each test group were consist of 5 male and 5 female and they received doses of 500, 1,000 and 2,000 mg/kg/day of test substance for 4 weeks. The clinical signs, death rate, body weight, food consumption, ophthalmic examination, urinalysis, hematological and serum biochemistry, organ weight and pathological changes were examined and compared with those of control group. Urinalysis : We observed increase of PRO(p<0.01), SG(p<0.01) in female rats of 1,000 mg/kg/day and 2,000 mg/kg/day(p<0.01). Also, we observed increase of pH and KET in female rats of 1,000 mg/kg/day(p<0.05) and of 2,000 mg/kg/day(p<0.01). WBC in female rats in 1,000 mg/kg/day and 2,000 mg/kg/day were on increase. Hematological test : We observed increase of MCV in male rats of 250 mg/kg/day. (p<0.05) Serum biochemistry test : We found increase of CHO in female rats of 2,000 mg/kg/day(p<0.05). During the experimental period, there were no animals dead or moribund. There were no treatment related changes of general symptom, food and water consumption, organ weight and autopsy According to the results of 4-week repeated dose range finding study, the highest dose was established as 1000 mg/kg for 13-week repeated dose toxicity study and we determined to put 2 more groups by common ratio two.
