The Effects of Sterilization on Jakyakgamcho Decoction(Shaoyaogancao Decoction) Pharmacopuncture: an Investigation to Minimize Loss of Principal Components (멸균에 따른 작약감초약침의 지표 성분변화 및 해결방안)

  • Objectives : Jakyakgamcho decoction is a traditional prescription known to be an effective pain control medication and muscle relaxant. For more localized treatment outcomes achieved in a shorter period of time, Jakyakgamcho decoction was reprocessed into a form of pharmacopuncture. An analysis of Jakyakgamcho decoction pharmacopuncture showed that there was a significant loss of paeoniflorin(Jakyak's index component). This study was designed to investigate ways to minimize this loss. Methods : After making changes to the processing methods of Jakyakgamcho decoction pharmacopuncture, we measured the quantity of paeoniflorin using high performance liquid chromatography(HPLC) for a before-and-after analysis Results : Paeoniflorin loss was observed 15 minutes after sterilization with $Na_2HPO_4$ at $121^{\circ}C$ Conclusions : It was found that paeoniflorin loss did not occur when pH was not controlled for during processing.

  • Background: Due to the aging population in Korea, knee osteoarthritis (KOA) has become an increasingly common condition. Many patients with KOA prefer analgesics, herbal medicines, acupuncture, or exercise, rather than arthroscopic surgery or a knee replacement. Gyebutang (GB) granules are a herbal extract widely used to treat KOA in traditional Korean medicine, but there is insufficient evidence of its efficacy and safety. Methods: A multicenter, randomized, assessor-blinded, 2-armed parallel, controlled clinical trial has been designed to investigate the efficacy and safety of GB combined with acupuncture for the treatment of KOA. There will be 100 patients with KOA enrolled in the study from 3 traditional Korean medicine hospitals. The participants will be randomly allocated to an experimental group (GB and acupuncture) or a control group (celecoxib and acupuncture) in a 1:1 ratio. Both groups will receive acupuncture treatment once a week for 6 weeks; one group will receive GB and the other will receive celecoxib for the same duration. Results: The primary outcome will be the change of knee osteoarthritic pain, based on scores on a 100 mm visual analog scale. The secondary outcomes will be scores on a numeric rating scale, the Western Ontario and McMaster Universities osteoarthritis index, patient global assessment, European quality of life 5-dimension 5-level scale, and adverse events. Conclusion: The results of this study will provide evidence of efficacy and safety of GB as a treatment for patients with KOA.

Effects of Preoperative Psychological fare on Postoperative Recovery - An Exploratory Experiment - (수술 전 심리간호가 회복에 미치는 효과에 관한 실험적 연구)

  • The purpose of this study was to investigate the preoperative anxiety of surgical patients and to observe the effects of an experimental nursing process on the incidence of vomiting, the number of analgesics used and the voiding ability of surgical patients within a 48 hour postoperative period and also the number of postoperative days in the hospital. This study me based on a sample of 51 patients who were scheduled for abdominal surgery. They were assigned alternately to experimental and control groups. Patients assigned to the experimental group were given nursing care by the writer, using an experimental nursing process. This is a process directed toward helping the patient obtain a suitable psychological state for surgery. This process was performed by use of the following steps: 1. The nurse approached the patient with interest and concern, and observed the patient's behavior to determine the presence of anxiety. 2. The nurse explored further to find out what was causing the anxiety and what was needed too relieve it. 3. The nurse listened carefully lo the patient. 4. The nurse gave an opportunity for expression of individuality. 5. The nurse showed friendliness and reassured the patient 6. The nurse gave instruction about deep berating, turing and coughing and gave an explanation of routine preoperative preparation which decreased the unknown and so decreased the patient's anxiety- The findings of this study are as follows: 7. The investigation of preoperative anxiety through observing and interviewing revealed patients were concerned about the following: a. Length of stay in the hospital 69.7% (31 cases) b. Condition unimproved o. worse 48.,8% (30 cases) c. Postoperative pain and discomfort .56.8% (29 cases) 2. The results of the study regarding the four hypothesis were as follows: a. Preoperative psychological care not reduce the number of analysis needed during the 48 hour period following surgery. b. Preoperative psychological care did not reduce the Postoperative incidence of vomiting c. Preoperative psychological care had no influence on voiding ability following surgery. d. Preoperative psychological care significantly reduced the number of postoperative days in tile hospital. The experimental nursing process proved to be successful in decreasing the length of postoperative hospitalization (improving rate of healing). This study has demonstrated further that the relief of anxiety (emotional distress) is part of the nurse's professional role. In order to be sure psychological care is given it is important to make a nursing care plan which .includes a plan for the psychological care needed by the patient. As this care is given it can be charted off by each nurse just as medication and treatments are checked after they are given.

  • In this clinical research, the fluoride tape(SCMC-T-5) using fluoride(NaF) was developed and manufactured and the treatment effect of the fluoride tape in dentin hypersensitivity patients was evaluated and compared with the effect of existing fluoride varnish($CavityShield^{TM}$). Twenty two healthy adult patients(88 teeth) having dentin hypersensitivity participated in this clinical research and they were divided into two groups. The fluoride product was applied according to the manufacturer's instruction and the level of pain in the tooth after giving irritation using compressed air and ice stick was measured just after the application, after 3 days, after a week and after 4 weeks each using visual analog scale(VAS). In the experimental group, compared with the early VAS scores, all other VAS scores showed the significant decreases statistically. In the control group, all VAS scores except the VAS score of 34.091(air) measured 3 days after(using the irritation examination by the compressed air) showed the significant decreases statistically when compared with the early VAS scores. The fluoride tape and fluoride varnish used in this clinical research were able to treat the dentin hypersensitivity effectively.

The Effects of Exercise Programs on Gait Ability of Elderly Dementia Patients (치매노인을 위한 운동프로그램이 보행능력에 미치는 영향)

  • The purpose of this study is to investigate the effects of the practice exercising and cycling on the gait abilities of dementia elderly patients. Forty forty elderly patients with dementia were participants. Participants were divided into three groups as A, B, and C, Group A was a standard group which had been treated with electric treatment, hot pack, and exercise. The second group B had been treated with cycling added to the treatments of group A. Finally, treatments of group C were arranged by subtraction of exercise from those of group A. The test was a 'timed up and go test' used for measuring the rates of the gait ability through four months. The gait ability of each group was measured after each month. The results of this study are as following: 1. After first month, the averages of changes in gait ability of group C, A and B were -1.69, -1.67, and -1.13 seconds respectively. That means that dementia control was achieved significantly in group A and B (p<0.05)whereas it was not in group C. 2. The averages after two months were -4.00 seconds for group C, -2.60 seconds for group A, and -1.56 seconds for B, respectively. All groups acquired significant effects in treating dementia (p<0.05). 3. After three months, -6.38, -3.00 and -2.31 seconds were average values of group C, A and B respectively. Same as results after two months, there were significant effects in all groups (p<0.05). 4. After four months, the averages of changes in gait ability of three groups were -8.00. -3.93 and -3.00 seconds for group C, A and B respectively. In the all patients treated analysis, treatments showed significant results (p<0.05). 5. Compared with each other through four months. three groups testing the timed up and go test showed more efficient in maintaining gait ability, in order of group B. A, and C

The Infection Status and Perception toward Intestinal Trematodes in an Island Inhabitants (일부 도서지역 주민의 장흡충 감염실태 및 인식)

  • In order to evaluate prevalence and perception on the intestinal trematodes of inhabitants at Docho-island, Shinan-gun, Chollanam-do, a questionnaire survey for the epidemiological study and stool examination for the detection of the eggs of the trematodes were done on 224 individuals at the island from January to February, 2000. The results were as follows: 1. Egg positive rate of the intestinal trematodes was 21.0%. 2. The egg positive rate has revealed significantly higher in the elderly over age 70 than other ages, higher in seashore villagers than inland ones, and higher in persons who had a rural and/or marine ph than persons with other jobs, respectively(P<0.05). 3. The egg positive rate of alcoholic drinkers was higher than non-alcoholic persons, and the rate was also higher in smokers than non-smokers. It was statistically significant(P<0.05). 4. The egg positive rate was shown to be significantly higher in inhabitants who ate raw mullet that was well known to be the source of infection with the intestinal trematodes than people who did not eat the raw marine fishes(P<0.01). 5. The egg positive rate was also significantly higher in humans who had the complaints of frequent abdominal pain and diarrhea than healthy people(P<0.05). In conclusion, the prevalence of intestinal trematodes at the island located in the south and west area in Korea was too high to eradicate the parasitic infections. So a special program is necessary for the control of the trematode infections in many adjacent islands at Chollanamdo.

A New Approach to the Whole Body Intervention Program(General Coordinative Manipulation Program) of Nonspecific Back Disorder

  • Since areas of pain and dysfunction of musculoskeletal typically suffered by the patients with back disorders spread all over the body, WBIP(GCM Program) for the primary treatment and management is required. The purpose of this study is to analyze if WBIP(GCM Program) based on the hyper/hypomobility pattern of Four Body Types can identify the effective treatment of back disorders and the effect on the postural balanced restoration of the spine and extremities. Non-specific back disorder is still a major reason for sick leave. And moreover, its been reported that there was often recurrence to the patients whose symptom had been diminished. As a WBIP(GCM Program) based on kinematic chain patterns of Four Body Types, this study gave a new information on the effective diagnosis, treatment and management of non-specific back disorders. 337 patients above the twenty-five years old with the non-specific back disorders at the hospital and oriental medical clinics at Kyungnam and Busan areas in South Korea from August 24th, 2000 to Feb 23rd, 2001 have randomly been assigned to four experimental groups such as Whole Body Intervention Program Group, Physical Therapy Group like modality treatments, Acupuncture-Treatment Group, and Placebo Control Group. According to intervention program applied to the each four group for three times per week(twelve times per 4weeks), as the time-series methods, we compared and evaluated the body status of the pretest with that of post treatment completion of four week, three month, and six month, respectively. As the analytical method of measurement, our researchers used the Moire Interferometry Unit and Postural Kit that could measure the postural balance of spine and extremities. The collection of data was performed in the designated hospital and oriental medical clinics. For the analysis of the data, the SPSS 10.0 package program was used. X2-test has been taken in order to compare and analyze characteristics and GPES of the patients in four experimental groups. Repeated Measure ANOVA and Tukey post hoc test has been adopted in order to compare the effects of the balanced restoration of the spine and extremities among four Groups categorized for this study. Statistical significance was accepted at the 0.05 level of confidence The effect of the balanced restoration on the spine and extremities of the patients with non-specific back disorders has been proved in all of the Groups. As for the restoration degree, however, WBIP(GCM Program) Group produced the highest effectiveness in terms of the fact that it had a dense moire in comparison with the other three Groups and that the Moires of both sides had the same level by the time(p<0.01). WBIP(GCM Program) based on four tilting types of scapular and ilium and hyper/hypomobility pattern took a higher effect on the balanced restoration of the spine and extremities through a whole body as well as the treatment of back disorders than the other three Groups which the usual remedy without classification of body type had been applied to.

Protective Effect of Corni Fructus Extracts on MIA-induced Animal Model of Osteoarthritis: Effect of Corni Fructus Extracts on OA (MIA로 유발된 골관절염 동물모델에서 산수유 추출물의 골관절염 개선 효과)

  • Objectives: Osteoarthritis (OA) is a chronic and degenerative joint disease characterized by progressive degeneration of articular cartilage. Inflammation is a recognized and important factor of OA progression. The present study was designed to investigate the protective effect of Corni Fructus water extract (CFW) on a monosodium iodoacetate (MIA)-induced rat model of OA. Methods: Osteoarthritis was induced by injection of MIA (50 µL; 80 mg/mL) into the knee joint cavity of rats. After an adaptation period for seven days, the rats were divided into 4 groups (n=8/group): normal, control, indomethacin-treated (5 mg/kg), and CFW-treated (200 mg/kg) groups. The rats were treated orally for 14 days. Pain was evaluated by determining hind paw weight distribution. For biochemical analyses, we measured the changes in reactive oxygen species (ROS) and peroxynitrite (ONOO-) in the knee joint. The presence of anti-oxidant proteins and inflammatory proteins was determined by western blotting. Results: The administration of CFW significantly improved the hind paw weight distribution. The ROS and ONOO- levels of knee joint were significantly decreased in the CFW group. CFW inhibited the production of inflammatory mediators, such as COX-2, and inflammatory cytokines, including IL-6 and IL-1β, via the NF-κB signaling pathway. The expression of anti-oxidant enzymes, such as catalase and GPx-1/2 also increased significantly. Conclusions: The findings indicate that CFW has a therapeutic and protective effect on OA by suppression of inflammation. Therefore, CFW could represent a potential and effective candidate for OA treatment.

Study on cyberbullying among adolescent bystanders - focusing on the empathy of middle school students - (사이버불링을 목격한 중학생 주변인의 실태 및 영향 변인 연구)

  • This study aims to explore the reality of middle school students witnessing cyberbullying and their types of behaviors after witnessing and to examine the role of empathy affecting the behaviors of bystanders of cyberbullying. For this purpose, we collected the survey data from 595 middle school students in the Gyeong-Gi region. Results show that 61.2% of the respondents report the witness of cyberbullying, Among which cyber slander is the most common, followed by spreading false information, outcast, sexual harassment, and extortion. The behaviors of bystanders who witnessed cyberbullying are categorized into spectating, defending, agreeing, or doing nothing. When it comes to the role of the students' empathy, findings show that the 'empathic concern' and 'personal pain' -, parts of emotional empathic abilities, can significantly influence behaviors of cyberbullying bystanders. These findings not only call the attention to the seriousness of cyberbullying among middle school students, but they also suggest the implication that emotion empathy education is required along with emotion control education.
