Object:Nicotine dependence is the most common substance abuse disorder. One of the characteristics of nicotine dependence is craving. Regional activation of the brain induced by craving for nicotine was evaluated by using functional magnetic resonance imaging to investigate neuroanatomical site of smoking craving. Method:A smoker who satisfied DSM-IV criteria for nicotine dependence and a non smoker was studied. MRI data were acquired on a 1.5T Magnetom Vision Plus with a head volume coil. Two sets of visual stimuli were presented to subjects in a random manner. One was the film scenes of inducing smoking craving and the other was neutral stimuli not related to smoking. There were two fMRI sessions before and after smoking or sham smoking. Data were analyzed using SPM99. Results:fMRI showed significant activated area in anterior cingulate and medial frontal lobes in the smoker during smoking craving. Right dorsolateral prefrontal cortex and parietal lobes were activated in the control during visual stimulation before smoking. After smoking, there was no brain activation during visual stimulation in both of smoker and non smoker. Conclusion:Metabolic activity of the anterior cingulate and medial frontal lobes increased during craving for smoking. This result suggests that fMRI may be a valuable tool in the identification of neurobiological process of craving.

Cigarette smoking contributes to lung cancer, cardiovascular diseases, oral diseases, etc. In desire to reduce their risk of disease, many cigarette smokers have tried to quit smoking. Sensory aspects of cigarette smoke are important for providing smoking satisfaction. Previously it was reported that citric acid aerosol significantly reduced craving for cigarettes and enhances smoking reduction and cessation. In this study, we tested whether a newly combined product Ciga-X, an aerosol for cessation aid, had toxicity in human embryonic lung fibroblast (MRC-9). The inhibitory effect of Ciga-X on cytotoxicity induced by cigarette smoke extract (CSE) or nicotine was examined in MRC-9, and craving for cigarettes and smorkers satisfaction after using Ciga-X was estimated. Ciga-X did not affect cell viability and had no toxicity in MRC-9. Ciga-X significantly inhibited not only CSE-induced cytotoxicity but also nicotine-induced cytotoxicity in MRC-9. One hundred and forty smokers rated the satisfaction for Ciga-X aerosol and craving reduction for cigarettes after using Ciga-X. The percentage of over 5 rating was 71.0% and 50.0% of subjects in satisfaction test for Ciga-X compared to their own brand and in craving reduction for cigarette, respectively. Besides, craving reduction for cigarette was highly correlated with the duration of smoking. Subjects have smoked under 10 years were more reduced in craving for cigarettes after using Ciga-X as compared to over 10 years (p=0.049). These results suggest that Ciga-X may be effective in promoting smoking abstinence with the reduction of CSE- or nicotine-induced human lung fibroblasts cytotoxicity.
Research has shown that many smokers experience an increase in the desire to smoke when exposed to smoking-related cues. Cue exposure treatment (CET) refers to the manualized, repeated exposure to smoking-related cues, aimed at the reducing cue reactivity by extinction. In this study, we constructed a virtual reality system for evoking a desire of nicotine, which was based on the results of a Questionnaire of Nicotine-craving. And we investigated the effectiveness of the virtual reality system as compared to classical device (pictures). As a result, we reached the conclusion that virtual reality elicits more craving symptoms than the classical devices.
Purpose: This study was done to analyze the changes in smokers' subjectivity about smoking before and after participation in a 5-day hospitalized smoking cessation program. Design: Q-methodology, which provides a scientific method on subjectivity, was applied. Findings: Nine types of subjectivity changes about smoking were identified; Type I, Nicotine $craving{\to}Confident$ of self control for smoking; Type II, Nicotine $craving{\to}Cognition$ of the link between habituation and smoking, Type III, Nicotine craving ~ Nicotine craving, Type IV, smoking as a social $behavior{\to}Cognition$ of the link between habituation and smoking, Type V, smoking as a social $behavior{\to}Motive$ to turn away from being addicted to smoking to being healthy, Type VI, Habitual $smoking{\to}Confident$ of self control for smoking, Type VII, Habitual $smoking{\to}Cognition$ of the link between habituation and smoking, Type VIII, Habitual $smoking{\to}Motive$ to turn away from being addicted to smoking to being healthy and Type IX, Denial of the negative effects of $smoking{\to}Confident$ of self control for smoking Conclusion: The types of changes in smokers' subjectivity before and after the smoking cessation program in this study suggest a better tailored intervention for individual smokers. The types of changes on smokers' subjectivity provide insight about which factors of the interventions for smoking cessation are especially valued for a particular group of smokers.
Nicotine, primary component of tobaco produces craving and withdrawal effect both in humans and animals. Nicotine shows a close resemblance to other addictive drugs in molecular, neuroanatomical and pharmacological, particularly the drugs which enhances the cognitive functions. Nicotine mainly shows its action through specific nicotinic acetylcholine receptors located in brain. It stimulates presynaptic acetylcholine receptors thereby enhancing Ach release and metabolism. Dopaminergic system is also stimulated by it, thus increasing the concentration of dopamine in nuclear accumbens. This property of nicotine according to various researchers is responsible for reinforcing behavioral change and dependence of nicotine. Various researchers have also depicted that some non dopaminergic systems are also involved for rewarding effect of nicotinic withdrawal. Neurological systems such as GABAergic, serotonergic, noradrenergic, and brain stem cholinergic may also be involved to mediate the actions of nicotine. Further, the neurobiological pathway to nicotine dependence might perhaps be appropriate to the attachment of nicotine to nicotinic acetylcholine receptors, peruse by stimulation of dopaminergic system and activation of general pharmacological changes that might be responsible for nicotine addiction. It is also suggested that MAO A and B both are restrained by nicotine. This enzyme helps in degradation dopamine, which is mainly responsible for nicotinic actions and dependence. Various questions remain uninsurable to nicotine mechanism and require more research. Also, various genetic methods united with modern instrumental analysis might result for more authentic information for nicotine addiction.
Purpose: This study was done to identify types of smoking decision making-temptation in adolescents and characteristics related to type among student smokers. Method: Data collection was done from March to July. 2006. A survey was administered to 275 students in 13 high schools and 15 middle schools in Seoul, South Korea. To identify types and characteristics smoking decision making-temptation in adolescents, cluster analysis using the K-mean method was employed. Characteristics of the influential variables according to the identified types of adolescent smokers were evaluated using ANOVA. Results: Four types of smoking pattern in adolescents were identified: habitual craving (17.7%), nicotine dependence (35.8%), feeblemindedness (28.4%), and self control (18.1%). The score for nicotine dependency was higher in the habitual craving type than any other type (F=11.79, p=.001), while the score for self efficacy for smoking abstinence was higher in the self control type (F=23.06, p=.000). Conclusions: Findings from this study suggest that effective interventions for smoking cessation in adolescents require not only active implementation of nicotine replacement therapy but also development of individualized approaches for each person targeting change in the social environment that may lead to positive smoking decisional balance.
Smokers keep smoking despite knowing that tobacco claims many lives, including their own and others'. What makes it hard for them to quit smoking nonetheless? Tobacco companies insist that smokers choose to smoke, according to their right to self-determination. Moreover, they insist that with motivation and willpower to quit smoking, smokers can easily stop smoking. Against this backdrop, this paper aims to discuss the addictive disease called tobacco use disorder, with an assessment of the addictiveness of tobacco and the reasons why smoking cessation is challenging, based on neuroscientific research. Nicotine that enters the body via smoking is rapidly transmitted to the central nervous system and causes various effects, including an arousal response. The changes in the nicotine receptors in the brain due to continuous smoking lead to addiction symptoms such as tolerance, craving, and withdrawal. Compared with other addictive substances, including alcohol and opioids, tobacco is more likely to cause dependence in smokers, and smokers are less likely to recover from their dependence. Moreover, the thinning of the cerebral cortex and the decrease in cognitive functions that occur with aging accelerate with smoking. Such changes occur in the structure and functions of the brain in proportion to the amount and period of smoking. In particular, abnormalities in the neural circuits that control cognition and decision-making cause loss of the ability to exert self-control and autonomy. This initiates nicotine dependence and the continuation of addictive behaviors. Therefore, smoking is considered to be a behavior that is repeated due to dependence on an addictive substance, nicotine, instead of one's choice by free will.
This study was done to investigate the relation of smoking temptation, stage of change in smoking cessation, nicotine dependency and perceived health status in middle aged men. Convenience samples of 176 subjects who were either smoked or used to smoke, aged between 30 to 64, living in Seoul and Kyungi province area in Korea were selected for the study. The data was collected from December 1, 1999 to June 30, 2000. The research instrument were Stage of Change of Smoking Cessation Measure (DiClemente et al. 1991). Smoking Temptation Measure (Velicer, DiClemente, Rossi, Prochaska. 1990), Perceived Health Status Measure (McDowell & Newell, 1996), and Nicotine Dependency Scale (FTQ: Fagerstrom, 1978). The data were analyzed using the SAS Program. The result of the study are as follows : 1. The analysis of variance and multiple comparison showed that according to the stage of change, there were significant mean differences in the three sub-factors of smoking temptation; 'positive affect situation (F=12.64, p=.0001)', 'negative affect situation (F=16.01, p=.0001)', 'habitual craving situation (F=14.43, p=.0001)' and nicotine dependency (F=4.12, p=.0033) The mean score for smoking temptation for the subjects who were in the precontemplation stage outweighed the mean score for smoking temptation for subjects who were in the maintenance stage. 2. Through discriminant analysis, it was found that negative affect situation was the most influential variable of the smoking temptation sub-factors which can be used to discriminate stage of change. 3. The analysis of Pearson correlation coefficients showed that there was a significant positive relation between nicotine dependency and negative affect situation of smoking cessation((r=0.2182, p=0.0045) and a significant negative relation between nicotine dependency and perceived health status(r=-0.2115, p=0.0059).
Purpose: This study aimed to identify factors influencing nicotine dependence among male college students intending to quit smoking on the basis of the PRECEDE model. Methods: A cross-sectional survey of smokers, enrolled at the Smoking Cessation Support Center in W city in Korea was conducted between September 2015 and December 2016. Data was analyzed using the SPSS program for descriptive statistics, bivariate analyses, and hierarchical multiple regression. Results: The mean score of nicotine dependence was determined to be 2.9. Tobacco craving (β =.34, p<.001), social smoking (β =-.23, p=.004), allowance (β =.22, p=.010) and age of smoking initiation (β =-.20, p=.022) were factors that predicted nicotine dependence of male college students. The final model explained 28.3% of the variation in nicotine dependence (adjusted R2=.28), and was significant (F=7.22, p<.001). Conclusions: We determined that to support smoking cessation, it is necessary to develop anti-smoking programs that consider the social and cultural factors of male college students as well as their personal psychological characteristics.
Purpose: This study attempted to identify influential variables on types of smoking in adult smokers, which have been derived from Q-study. Methods: A survey was conducted with 328 adult smokers at community settings in Seoul, South Korea from April 2006 to July 2006. To identify the influential variables related to smoking, variables from the Trans-theoretical model(i.e., self-efficacy for smoking abstinence, smoking temptation, process of change, decisional balance(pros and cons)) and nicotine dependency were analysed by ANOVA. Post-comparison tests were also done. Results: The scores of nicotine dependency and smoking temptation were higher in habitual craving types than any other types, while the scores of self-liberation and self-efficacy for smoking abstinence were higher in self control types than any other types. Conclusion: Specific smoking intervention methods for each of the smoking types needs to be developed, based on how different groups of individuals gets into a decisions to quit smoking within their contexts.
