  • Backround : The paradigm of recent education has been shifting from existing style of professor-oriented, passive and rote teaching to learner-centered education. Rather than mere delivery of knowledge, today's idea of education uses various audiovisual media to let learners gain more problem-solving skills, judgment, cognitive thinking ability, and creativity to apply to real practice. Also, while current trends and change in policy ask for related industry to require practice-centered teaching learning model, Problem-Based Learning (PBL) is quite effective that it activates problem-solving skills as well as application of National Competency Standards (NCS). Purpose : The purpose of this study was to suggest a teaching learning model article as an approach to apply web-based PBL for patient & medical charge management practices. Discussion & Conclusion : This paper the cases on PBL and presents the teaching learning model on web-based PBL as an approach to applying web-based PBL, which fits Medical Information System Department that combines health-medical treatment and computer applications, to practical health administrative affairs.

Proteomic Analysis of Pancreata from Mini-Pigs Treated with Streptozotocin as Type I Diabetes Models

  • Type 1 diabetes mellitus (T1DM) is an autoimmune disease characterized by extreme insulin deficiency due to an overall reduction in the mass of functional pancreatic ${\beta}$-cells. Several animal models have been used to study T1DM. Amongst these, the mini-pig seems to be the most ideal model for diabetes research, owing to similarities with humans in anatomy and physiology. The purpose of this study was to analyze differentially expressed pancreatic proteins in a streptozotocin (STZ)-induced mini-pig T1DM model. Pancreas proteins from mini-pigs treated with STZ were separated by two-dimensional gel electrophoresis, and 11 protein spots were found to be altered significantly when compared with control mini-pigs. The data in this study utilizing proteomic analysis provide a valuable resource for the further understanding of the T1DM pathomechanism.

  • The medical service, accompanied by various disease and disease of modern people, must collect various information about patients through various tests, and demand continued to increase. As a result, interest in waiting time for medical services is rising, and various studies are underway to reduce waiting time. However, medical procedure is complex and diverse in the procedure of the services need to be conducted for each procedure. In this study, we are going to propose ways to improve the waiting time of the test procedures that are basically experienced by outpatients through the simulation experimental method. Based on the actual measurement data, the Arena Simulation program was used to implement the hospital's ophthalmic examination model and alternative model. Through this analysis, the waiting time of the examination procedure, the size of the queue, and density of the waiting room were analyzed.

A Study on the Development of Collaborative Print Journal Preservation Model for the Medical Libraries in Korea (의학도서관을 위한 인쇄본 학술지 공동보존체제 구축에 관한 연구)

  • The purpose of this study is to develop a collaborative print journal preservation model for the medical libraries in Korea. After reviewing previous studies and advanced preservation practices, a survey was administered to the members of the Korean Medical Library Association to collect data about the present states of journal weeding and storage space as well as their needs and preferences for collaborative preservation. Based on the analysis, a hybrid model, consisting of centralized and distributed archives, is proposed with MEDLIS as infra structure. Detailed characteristics and three phases of implement strategies are described. Finally, recommendations for the Korean Medical Library Association to the successful substantiation are suggested.

Factors Related to the Output of Health Centers (보건소의 사업성과에 관련된 요인)

  • This study was conducted to identify the factors that affect the output of health conters. An analystical model employed in this study was developed by modifying 'input-output model' and 'organizational behavior model'. Data were collected form two source; the 1995 report of thealth center which was submitted to the Ministry of Health and Welfare and a mail survey questionary of officers at health center, including 66 directors and 1,768 staffs of the health centers in southern region. The major findings are as follows: That analysis has identified the factors associated with dependent variables: medical services provided by the health center and health program performance(HPP). The number of primary medical facilities was negatively associated with health center performance while the number of staffs, job satisfaction, and professional background of health center directors were positively associated. These independent variables accounted for 40.1% of the variance of dependent variables. The variance of HPP was significantly explained by the number of health subcenter and primary health post, priority level of public health program by hief executive officers(CEOs) and legislator. A significant relationship was found between leadership types of health center directors and the performance of maternal and child health program. Considering these results, the authors suggested that the role in medical care service of health center in the should be rearranged at local level because medical care service of the health center is competing with primary medical facilities in the same region. It is also suggested that educational efforts be made to improve leadership of the health center directors and concern with public health program by the CEOs and legislators of local governments.

Effects of Cortex Phellodendri on the Metabolic Function in Experimental Mouse Model of Obesity (황백(黃柏)이 비만 유발 mouse의 대사기능에 미치는 영향)

  • Objectives: This study was undertaken to investigate how Cortex Phellodendri affects metabolic functional change in an experimental rat model of obesity.Methods: An obesity model was induced in a C57BL/6 mouse with a high-fat diet. Mice were divided into three groups (n=6) of normal diet, high-fat diet (=control), and high-fat diet with Cortex Phellodendri. After 12 weeks, we measured the three mice groups’ body weight, FBG, FBI, HOMA-IR, OGTT, the weight of epididymal fat and liver, the percentage of ATM, and the gene expression of TNF-α, IL-10, and CD68.Results: Cortex Phellodendri significantly reduced blood glucose and oral glucose tolerance levels. It also reduced ATM numbers and TNF-α and CD68 gene expression and increased IL-10 gene expression.Conclusions: This study suggests that Cortex Phellodendri normalized the blood glucose and reduced the expression of inflammatory markers. However, with respect to other indicators of metabolic function in obesity, there were no significant results.

The Pricing Strategy for the Performance of Medical Service -­ Based on the Segmentation for the N­block tariff Pricing of Medical Examination­ - (의료서비스의 성과 제고를 위한 가격전략 -­건강검진료 다단계가격책정을 위한 시장세분화를 중심으로­-)

  • This research objective is to determine the optimal price break points for n­block tariff, because comparing non­linear pricing with uniform pricing on the basis of profit, n­block tariff outperforms two­part tariff, all unit discount price schedule, and uniform pricing. Although the merits of non­linear pricing are well documented, the attempt to practice the non-linear pricing in medical service sector has been relatively rare. The determination of the parameters under n­block tariff is the interesting decision making agenda for marketers. Under n­block tariff, the marketers should decide the optimal price break points and the optimal marginal price for each price zone. The results can be summarized as follows: The researchers found that mixture model can be the feasible methodology for determining the optimal number of n­block tariff and identifying the optimal segmentation criteria. We demonstrate the feasibility and the superiority of the mixture model by applying it to the database of medical examination. The results appear that the number of patients per month can be the optimal segmentation variable. And 6­block tariff is the optimal price break for this medical service.

A New Medical Lead for Various MRI Systems (다양한 MRI 시스템에서 사용가능한 의료용 리드선)

  • Radio Frequency (RF) coils in Magnetic Resonance Imaging (MRI) systems interact with a patient's tissues, resulting in the absorption of RF energy by the tissues. The presence of an electrically conducting medical implant may concentrate the RF energy and causes tissue heating near the implant devices. Here we present a novel design for a medical lead to reduce this undesired heating. Specific Absorption Rate (SAR), an indicator of heating, was calculated. Remcom XFdtd software was used to calculate the peak SAR distribution (1g and 10 g) in a realistic model of the human body. The model contained a medical lead that was exposed to RF magnetic fields at 64 MHz (1.5 T MRI), 128 MHz (3 T MRI) and 300 MHz (7 T MRI) using a model of an MR birdcage body coil. Our results demonstrate that, our proposed design of adding nails to the medical lead can significantly reduce the SAR for different MRI systems.

Medical Student Career Choice and Career Planning (의과대학생들의 진로선택과 진로지도)

  • Purpose: This study analysed the intention of medical students career choice, educational programmes, and mentoring and counseling system for career planning at Yonsei medical school in Korea. Methods: The data were collected based on four separate graduation questionnaires at Yonsei medical school in the years of 2005, 2006, 2007, and 2008. The number of the survey was 130 in 2005, 130 in 2006, 153 in 2007, and that of the latest was 120 in 2008. We analysed the career intention on medical specialties and activities, and perceptions of important factors in choosing medical specialty. Results : The results which can be drawn from this study are these: firstly. students had more intention for choosing clinical medicine as university faculty than any other activities. While male students preferred to major in surgery, neurosurgery, orthopedic surgery, urology, otorhinolaryngology, female students in internal medicine, neurology, anesthesiology and pain medicine, diagnostic radiology, laboratory medicine. Secondly, students perceived that the most important factor which can influence on choosing a medical specialty was individual factor such as one's interests and concerns, values, and aptitudes. In stead, they relatively less perceived mentor and role model's effects on choosing a medical specialty compared to those of the United States of America. Third, the career planning at Yonsei medical school was evaluated well, especially educational programmes for career planning such as self assessment programme, elective(specialized) courses, and conversation with a senior programme. Conclusions: Unexpectedly, there are high demands for career planning by medical students. Therefore, we will reorganize systematic devices for career planning such as mentoring and counseling system at medical school.

A Study of Orthognathic Surgical Guides with Two-stage Split Path (2단 절개 형태를 가지는 악교정 수술 장치 연구)

  • In this study, the accuracy of the orthognathic surgical guides with single-stage split path was upgraded to realize orthognathic surgical guides with two-stage split path and simulated surgery was performed to verify its accuracy. As a result, the average error distance between the simulation model and the scan model was + 0.289 / - 0.468 mm (standard deviation 0.128), which was confirmed to be within ± 0.5 mm, which is a clinically acceptable level. Also, there was no significant difference compared with the average value of + 0.313 / - 0.456 mm (average standard deviation 0.106) of the conventional single-stage split path type device. It is judged that the use of this device can contribute to the reduction of surgical time and increase in accuracy since a separate finishing operation for bone preparation is unnecessary.
