응급구조과 대학생의 원격수업 경험 분석 (The experience of distanced synchronous and asynchronous learning in paramedic students through focus group interviews)

  • Purpose: The study was a qualitative study to examine the synchronous and asynchronous distanced learning experience of online paramedic students during the COVID-19 pandemic. Methods: The subjects included 10 students enrolled in the department of emergency medical service at J City C University. Written consent was provided by the subjects prior to the study, and focus group interviews were then conducted with sufficient explanation. The interviews were recorded and were directly transcribed immediately after the interview. Research results were then derived through content analysis. Results: A total of 4 domains and 9 categories were derived from the experiences of paramedic students on distanced learning. The 4 domains included "distanced lectures type," "student's adaptation and non-adaptation," "change of evaluation," and "learning anxiety." Conclusion: Contents of each domain derived from this study are expected to be used as basic data for the design of the distanced learning in the future.

한의학 교육이 의대생의 한의학에 대한 태도에 미치는 영향 (Influences of Education on the Attitudes of Medical Students toward Traditional Korean Medicine)

  • Objective : This study aims to figure out what the medical students' attitudes toward traditional Korean medicine (TKM) are and how TKM education affects them. Method : We performed content analysis on 283 assignments submitted by senior students who took a 'TKM and complementary alternative medicine (CAM)' course in a medical school in 2010 and 2011. The assignment was to write an report after reviewing a research article about 'TKM or CAM' or interviewing a person engaging in a related field. Results : 193(68.2%) students chose a article review and 90(31.8%) students chose an interview. Significantly more students chose TKM topic in interview group than in article review group(p=0.021). Most frequent questions in interview are as follows; 'comparison of TKM and western medicine'(15.7%), 'education in TKM school'(8.8%), and 'TKM and evidence based medicine'(8.8%). Among students who expressed their attitudes toward TKM, 52.4% of them showed positive attitudes before entering medical school, however, 54.6% of them showed negative attitudes in medical school years. Among students who indicated attitude change before and after the TKM course, 35.9% of them changed positively. Conclusion : TKM education might have positive effects on medical students' attitude toward TKM. It is necessary to expand TKM education and introduce effective education stategy in order to alleviate medical students' misunderstanding and prejudice against TKM.

신체형 장애 환자에서 동반되는 인격 장애/성향의 유형 및 빈도에 대한 연구 (A Study on the Types and the Frequencies of Personality Disorders/Traits in the Patients with Somatoform Disorders)

  • This study was conducted to identify the relationships between personality disorders/traits and somatoform disorders. After the patients were screened through self-rated SCID-II Questionnaire(Structured Clinical Interview for DSM-III-R, Questionnaire), the researcher got psychiatric history, performed clinician-rated SCID-P(Structured Clinical Interview for DSM-III-R, Patient Edition), classified and diagnosed mental disorders with SCID-P, and evaluated SCID-II(Structured Clinical Interview for DSM-III-R, Personality Disorders) by direct interview. The prevalencies of avoidant, obsessive-compulsive, schizotypal, narcissistic, paranoid, dependent, self-defeating, borderline personality disorders/traits in patients with somatoform disorders were diagnosed as 67.4%, 48.8%, 44.2%, 41.9%, 37.2%, 34.9%, 34.9%, 32.6%, respectively. The frequencies of self-defeating and schizotypal PD/traits were significantly higher than those of other neurotic control group. The results of this study could be regarded as replicating the results of previous studies that had reported most of all patients with somatoform disorder had presented with personality disorders/traits. This study, however, showed that the patients with somatoform disorders accompanied not with any specific types of personality disorders/traits but with various types of personality disorders/traits, which was much different from the previous usual clinical impressions. Thus, it is necessary for clinicians to approach the patients with somatoform disorders through more flexible and more supportive methods and attitude, in order that they should treat them more effectively.

조사방법에 따른 의료이용 비교 -보완된 자기기입조사와 면접조사 방법의 차이- (A Comparative Study of Two Survey Methods for Health Services Research Modified Self-administered Questionnaire vs. Interview Survey)

  • The purpose of this study was to compare the difference of two survey methods for health services research. Data were collected by means of two types of household survey conducted from March 11 to September 19, 1985. A probability sample of 30,613 persons was taken from 180 Enumerated Districts designated by the Economic Planning Board. The sample was divided into two groups systematically. One group was surveyed by the self-administered questionnaire and the other group was interviewed. Response rates were 81.4% and 90.6% respectively. The data were analysed by the ratio of the results of the self-administered survey to those of the interview survey. No difference was observed in sex, age, residence, or occupation between the two groups. However the respondents' characteristics were statistically different between the two groups. The major findings of this study are as follows : 1. The morbidity rate was 142.5 per 1,000 persons during the two week period by the self-administered questionnaire survey and 74.3 per 1,000 persons with the interview survey method. The ratio of the morbidity rate by the self-administered questionnaire to that by the interview was 1.92, and the difference between the two rates were due to the personal characteristics. 2. The out-patient utilization rate was 10.2 visits per person per year by the self-administered questionnaire survey and 5.4 by the interview survey, and the ratio was 1.89; the admission rate was 3.2 times per 100 persons per year by the self-administered questionnaire survey and 1.9 times by the interview survey, and the ratio was 1.68. Differences due to the sociodemographic characteristics were greater in the out-patient utilization rates than in the adimssion rates. 3. Percentages of effective medical care demand were 90.2% in the self-administered survey and 92.3% in the interview survey; the ratio was 0.98 which was less than that of the morbidity rate and medical care utilization. But, differences of effective medical care demand occurred in persons with no occupation, and aged or low educated respondents. 4. Respiratory illness had the highest frequency in the two survey methods. But there was a slight difference between the two survey methods in morbidity composition. 5. It was concluded that data collected by the interview survey were inclined to be underestimated and this problem can be corrected by a modified self-administered survey.

한의약건강증진사업 발전을 위한 공중보건한의사 역할 탐색 (Exploring the roles of Korean public health doctors in the development of Korean medicine health promotion project)

  • Objectives : In an effort to suggest a developmental plan of Traditional Korean Medicine health promotion project(TKM-HPP), this study was conducted to identify the problems experienced by public health Traditional Korean Medicine(PH-TKM) doctors in duty and to analyze successful cases of the health promotion project. Methods : Two PH-TKM doctors in duty at the Public Health Centre branch and Public Health Centre respectively, and one TKM doctor in charge of Public Health Centre participated in the in-depth interview. The research procedure was as follows; 1) inform study objectives and interview questions to participants, 2) in-depth interview with semi-structured questions, and 3) post-confirmation and verification. The interview was recorded and transcripted. In-depth interview was composed of 6 subsections: 1) Introducing participants and share purpose of interview, 2) Confirmation of the job status and difficulty of public health centre, 3) Reflecting and sharing experiences of public health clinic job performance cases, 4) Current status and improvement needs of TKM-HPP, 5) Discussions on improvement of TKM-HPP in public health center, 6) Sharing and reconfirmation of interview results. Thematic analysis was conducted from the narratives. Results : The study showed that; 1) the support from the local government was critical for the successful TKM-HPP and participation of the PH-TKMs; 2) the major problems experienced by PH-TKMs were lack of training for TKM-HPP in the field and lack of knowledge at local government level; 3) the suggestions to provide PH-TKMs with manual to educate residents effectively and redefine roles of PH-TKMs in the project. It also suggests that TKM-HPP model should be developed by the district in a way of reflecting regional characteristics. Conclusions : Firstly, more efforts should be made on the projects rather than medical treatment in public health centers, except for areas where medical institutions are scarce. Secondly, in order to facilitate participation of PH-TKM doctors, each local office should reduce medical care and increase efforts related with projects. Thirdly, the institution has to develop feasible TKM-HPP plans that can be carried out by PH-TKMs by the manual. Lastly, pre-training should be provided to PH-TKMs to understand contents of TKM-HPP related to public health centre work before the placement.

퇴행성 슬관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 면접조사 (An Interview Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis)

  • Objectives : This interview survey was carried out to identify how Korean medical doctors make acupuncture prescriptions for knee osteoarthritis in clinics. Methods : Survey questions were developed based on a consensus of acupuncture professors at Kyung Hee University, Dongguk University, Daegu Haany University. The interviews were conducted to 72 members of the Korea Oriental Medical Association who answered to prefer remote acupuncture prescription at previous telephone survey. Results : Korean medical doctors prefer to use five shu acupuncture points, especially 'hyung' or , 'shu' points, on the contralateral side of lesion and that the first target organ is liver. Five element points theory was mainly based on 'Nanjing' and 'Hwangdi Neijing'. The De-qi sensation of both doctor and patients was emphasized. Diagnosis and evaluation generally depend on subjective evaluation rather than objective scale. Conclusion : This study showed that Koran medical doctors prefer to follow the Korean traditional acupuncture methods respecting the old classic principles. And these results can guide us to develop advanced clinical trial protocols more close to our acupuncture practice.

가정의학 임상실습과정에 대한 구조화된 평가 (Structured Assessment to Evaluate a Family Medicine Clerkship Program)

  • This study was conducted to analyze the strengths and weaknesses of a 3-week family medicine clerkship program based on the results of an online survey taken by the students (N=127) and a structured interview with a focus group (n=10), aimed to improve the quality of the clerkship program. The online survey contained questions pertaining to goals, schedule, contents, arrangement, atmosphere, environment, evaluation, and satisfaction regarding the clerkship. The focus group interview addressed the schedule and achievements of the program. Scores were reported on a 5-point Likert scale. Most students were highly satisfied with the overall quality of the clerkship. The structured interview results showed that 97.6% of the clerkship program was executed according to the schedule. The focus group reported a perfect score of 5 points on several measures including: accomplishment of the educational goals of the family medicine clerkship, providing many chances to obtain medical histories and perform physical examinations on real patients, experience with various symptoms and diseases, positive attitudes of faculty members when teaching, notification of the guidelines for evaluation beforehand, well-constructed and effective clerkship schedule, and reflection of student feedback. However, the focus group gave low scores on: support for health accidents of students, access to patient information, enough opportunities to practice clinical skills, appropriate rest facilities for students, and fairness of clerkship evaluation process. In conclusion, the structured evaluation performed after the 3-week clerkship program motivated students and helped them ensure an efficient clerkship. This structured evaluation also suggested basic data to make the professor who is subject of the assessment. This study shows that structured assessment is an effective method which can be used to improve the quality of clerkships.

화병(火病)을 시하차(柴河車) 약침(藥鍼)을 사용(使用)하여 치료(治療)한 1례(例) (A clinical report of Hwa-byeong with Jahageo Herbal acupuncture therapy)

  • Hwa-byeong, the syndrome usually found in Korean society, is a culture-related disease. The common symptoms of Hwa-byeong are flushed face with fever, chest discomfort, palpitation, insomnia, headache, dizziness, and dyspepsia. In this case, we diagnosed Hwa-byeong by HBDIS(Hwa-Byeong Diagnostic Interview(Schedule) and treated the patient with Jahageo Herbal acupuncture at Shindo(Shendao) Yongdae(Lingtai), Chiyang(Zhiyang), Kumi(Jiuwei) and Chonjung(Shanzhong). After that, her subjective symptoms improved. With this result, It was considered that Jahageo Herbal acupuncture was very effective for treatment of Hwa-byeong.

한의사의 성폭력피해자 진료경험에 대한 현상학적 질적 연구 (A Phenomenological Qualitative Study on the Traditional Korean Medical Doctor's Experience of Treating Victims of Sexual Violence)

  • Objectives : The purpose of this study is to explore the experiences of traditional korean medical doctors (TKMD) who have treated victims of sexual violence by collecting their qualitative data that cannot be obtained with statistical data. Methods : In-depth interviews were conducted with semi-structured questionnaires for each of 5 TKMDs who had experiences in treating sexual violence victims. For analysis, the interviews were recorded along with their facial expressions and actions during the interview process, and all collected data were analyzed by colaizzi's method. In each process, a 'phenomenological reduction' was applied. Results : The identity of TKMD's experience of treating sexual violence victims was structured into 25 themes, 9 theme clusters, and 4 categories. The 4 categories included 'Professionalism and sense of efficacy', 'Personal life experiences and mental trauma treatment experiences that are mutually influencing each other', 'Factors needed to increase TKMD's participation in treatment of sexual violence victims', 'Deriving the future strategy of traditional korean medicine'. Conclusions : This study derived the essence and meaning of TKMDs' experiences in treating sexual violence victims, and presented future directions of traditional korean medicine in this field. It is expected that the results of this study would be used as the basis for establishing the traditional korean medical support system for sexual violence victims in the context of the korean medical system.

Research on Current Execution of Knowledge Management in Taiwan's Medical Organizations

  • Since the execution of National Health Insurance system in Taiwan, the competition of medical industry is becoming more and more severe. The ways the hospital operate knowledge management (KM) concept, combine current human resources and professional knowledge by information techniques and upgrade the competitiveness through reinvention of organizational culture have become the important issues. This research is based on the relationship between KM and organizational operation, integrates the characteristic of medical institutions and framework of medical knowledge cycle and starts the research subject by questionnaires from three dimensions: current situation of KM construction in medical organizations, executive effect of KM activities and the challenges faced by KM; subsequently, from qualitative interview, this research attempts to understand how a medical organization executes and adjusts in the consideration of theory and reality as well as quality and costs when actually operates the organization. This research accesses to KM system application of medical institutions and the empirical executive benefits and difficulties through questionnaires. The research results are as follows: (1) having initial understanding toward current KM establishment of medical institutions; (2) confirming the most important items of KM establishment of medical organizations; (3) understanding the most difficulty which the medical organizations encounter when executing KM; (4) establishing medical knowledge cycle figure of the hospitals receiving interviews. Through case interview, this research profoundly accessed to the actual operation of KM application of medical organizations. The target hospitals intended to try many medical KM measures; however, during to complicated hospital organizations and cultural characteristics, the promotion was not successful and the results were not apparent. The most difficulty was to change the employees’ behavior. The targets believed that only the continuous promotion of KM can allow it to be an important aspect of organizational culture and the competitiveness could constant be upgraded.
