Development of a Tool to Measure Suffering in Patients with Cancer (암환자의 고통 측정도구 개발에 관한 연구)

  • This study is a methodological research study to develop an instrument to measure in patients with cancer and to test the validity and reliability of the instrument. The research procedure was as follows : 1) The first step was to develop conceptual framework based on a comprehensive review of the literature and in-depth interviews with patients with cancer. This conceptual framework was organized in to three dimensions (the intrapersonal dimension, the significant-other and context related dimension, the transcendental dimension). Initially 59 items were adopted. 2) These items were analyzed through the index of content validity(CVI) and 53 items were selected which met more than 80% on the CVI. 3) The pretest was carried out with 87 patients with cancer. After the pretest results were analyzed by item analysis, 44 items were selected. A second test of content validity was conducted and 6 items were eliminated considering the 80% CVI. 4) To test for reliability and validity, data collection was done during the period from January 25, 1999, to February 26, 1999. The subjects for the test were 160 patients with cancer and 185 healthy persons. analysis, item analysis and multitrait-multimethod method to analyze validity. The findings are as follows : 1) The Cronbach's alpha coefficient for internal consistency was .92 for the total 38 items and .79, .82, .85, for the three dimensions in that order. 2) The item analysis was based on the corrected item to total correlation coefficient( .30 or more) and information about the alpha estimate if this item was dropped from the scale. 3) As a result of the initial factor analysis using principal component analysis and varimax rotation, one item was deleted because of factor complexity (indiscriminate factor loadings). In the secondary factor analysis, 7 factors with eigenvalue of more than 1.0 were extracted and these factors explained 56 percents of the total variance. The seven factors were labeled as 'family relationship', 'emotional condition', 'physical discomfort', 'meaning and goal of life', 'contextual stimuli', 'change of body image', 'guilt feelings'. 4) The convergence effect between this instrument and the life satisfaction scale was identified and there was significant positive correlation(r= .52, p= .00). The discriminant validity between this instrument and the depression scale(CES-D) was tested and there was significant negative correlation(r= -.50, p= .00). The instrument for accessing the suffering of patients with cancer developed in this study was identified as a tool with a high degree of reliability and validity. In this sense, this tool can be effectively utilized for assessment in caring for patients with cancer.

Serum Concentration and Exposure History of Dioxins and Organochlorine Pesticides among Residents around the Camp Carroll Area (캠프캐럴 인근 주민에서 다이옥신류 및 유기염소계 농약의 혈중 농도 및 노출력)

  • Objectives: This study was performed in order to evaluate whether 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) could be detected among residents living near Camp Caroll in Waegwan and whether serum concentrations of dioxins, including 2,3,7,8-TCDD, and organochlorine pesticides (OCPs) are associated with length of residence. Methods: Study subjects totaled 113 (for dioxins) and 190 (for OCPs) adults who were selected from participants in a medical investigation. Serum concentrations of dioxins and OCPs were measured using HRGC/HRMS. Information on length of residence was obtained through questionnaires. Results: 2,3,7,8-TCDD was not detected in serum among all subjects. When length of residence was classified as a categorical variable, after adjusting for confounding variables, only residents living in Waegwan for 40 years or longer tended to have high total TEQ values and 2,3,4,7,8-PeCDF with marginal significances. There was no dose-response relation between length of residence and serum concentrations of these chemicals. In multiple regression models with continuous values of the length of residence, total TEQ value and 1,2,3,4,6,7,8-HpCDF were positively associated with length of residence. However, they explained about 3-5% of total variations of serum concentrations of these compounds, while age, consumption of fatty fish, body mass index, alcohol drinking, and cigarette smoking were main variables affecting serum concentrations of dioxins or OCPs. Conclusions: In the current study, high concentrations of certain compounds were mainly observed among persons who lived in Waegwan for at least for 40 years without a dose-response relation. Therefore, it seems difficult to conclude that length of residence meaningfully contributed to the current serum concentrations of dioxins or OCPs among residents in Waegwan. However, considering the half-life of 2,3,7,8-TCDD and indirect exposure routes, the limitations of the current study design should be considered in the interpretation of the study findings.

Toxicity Assessment of Polygalae Radix Aqueous Extract Orally Administered to Rats for 2 Consecutive Weeks (원지 추출물의 랫드에서 2주 반복 경구투여 독성평가)

  • The objective of this study is to characterize a toxicity of Polygalae Radix (PR) in F344 rats and to find a dose levels for the 13 weeks toxicity study. PR is well known as medicinal herb in many Asian countries for treatment of expectorant, tonic, tranquillizer, antipsychotic agent and functional diet for improving memory. However, there is insufficient background information on toxicological evaluation of PR extract to support its safe use. Therefore, we conducted toxicological evaluation of this drug in compliance with OECD and KFDA guideline in this study. The extract of PR was administered orally to F344 rats at dose levels of 0, 500, 1000, 2000, 3500 and 5000 mg/kg/day for 2 weeks. Each group was composed to five male and five female rats. In the result, there were no treatment PR-related adverse changes in food consumption, hematology, clinical chemistry, urinalysis, gross finding at necropsy, organ weight examination. Four males at 5000 mg/kg/day were found dead during the treatment period. These animals showed salivation. The cause of death is still under investigation. The animals treated at 500, 1000, 2000, 3500 and 5000 mg/kg/day showed salivation and all animals at 5000 mg/kg/day exhibited lower body weight and cumulative weight gain in compared to those of control animals. Therefore, we recommend that a dose group of 3500 mg/kg/day is a highest treatment group in 13-week exposure study.

Path Tracking Control of 6X6 Skid Steering Unmanned Ground Vehicle for Real Time Traversability (실시간 주행 안정성 분석을 위한 6X6 스키드 조향 무인 자율 주행 차량의 경로 추종 제어)

  • For an unmanned vehicle to be driven on the off-road terrain, it is necessary to consider the vehicle's stability. This paper suggests a path tracking controller for simulation of real-time vehicle stability analysis. The path tracking controller uses the preview distance to track the given trajectory. The disturbance moment is estimated using the yaw moment observer, and this information is used for compensation in the yaw moment control. On a curved path, the vehicle's desired velocity is determined from the curvature of the path. Because the vehicle is equipped with six independent motor driven wheels, the driving torques are distributed on all the wheels. The effectiveness of the path tracking controller is verified using ADAMS/MATLAB co-simulation.

Studies on the Marketing of Broilers (육계의 유통에 대한 조사연구)

  • This studies were carried out to obtain the information on the establishment of marketing of broilers. The data for these studies were collected from 16 whole sale traders, 25 retail stores, 12 supermarkets and 3 direct-sales stall located at the suburbs of Seoul, Daejon, Gwangju and Busan. 1. Chickens were generally sold and named for a Spring chicken(0.6-0.9kg), a Boiled chicken of ginseng (1.0-l.3kg), a Semi-bro (1.3-l.7kg) or High-bro(1.8-2.1kg) by their body weight, However, those names were not uniform. 2. Since 47.5% of High-bro chickens, 60.0% of Spring chickens and 16.7% of Semibro chickens were used for domestic use and 66.7% of Semi-bro chickens was taken by butchers, it seemed that most chickens except Highbro and Spring chicken were distributed via butchers. 3. In most cases(75%) when the carcasses were sold a exact measurement was taken, but in some cases(18.8%) eye measurement was still used. 4. For the standardization of carcasses, 37.5% of the answered were for that and 50% of them wanted a gradual standardization and 66.5% of the answered wanted chicks sold by parts.

Evaluation of Cytotoxic Properties of Caffeine Treated with Over-the-counter Drugs in the Intestinal Cells (카페인과 일반의약품의 복합처리에 의한 장관계 세포 독성 평가)

  • Caffeine is a xanthine alkaloid derivative found in many foods and beverages. Dietary caffeine may interact with commonly-consumed over-the-counter (OTC) drugs in body. In this study, cytotoxic effects on the intestinal cells by combined treatment of caffeine with several OTC drugs, including ibuprofen, aspirin, and acetaminophen. Cytotoxic effect of caffeine was more potent in normal intestinal INT 407 cells than in colon cancer HCT 116 cells. Relative toxicity of caffeine and the OTC drugs was significantly enhanced in INT 407 cells when treated together. Intracellular thiol levels of the cells treated with the OTC drugs increased in the presence of caffeine. When HCT 116 cells were incubated with each OTC drug after or before caffeine treatment, the relative cytotoxicity of the OTC drugs increased. The present study may provide basic information about possible health effects through the interactions between caffeine and OTC drugs in the intestinal cells.

The Effect Analysis of Smart City Planning on Urban Dynamics Using System Dynamics Method - Focused on Anyang-city, Korea (시스템 다이내믹스를 이용한 스마트도시계획이 도시동태성에 미치는 영향 분석 - 안양시를 중심으로)

  • Recently, smart cities are attracting attention as a solution for a plethora of urban problems, including transportation, environment, safety, and energy. However, despite a substantial body of research dealt with the concept, trends, policy, and legal institutions of smart cities, few researchers have examined how the smart city services influence the cities from the dynamic perspective that considers the entire cycle of a city, including its growth, stagnation, and decline. Thus, it is vital to understand how the city changes with time from the view that a city is a system of sub-elements-population, industry, transportation, environment, housing, and land-closely interacting together. Within this context, this study explores how the urban dynamics of Anyang-city develop for the long term using the System Dynamics method and analyzes the effect of smart city project investment on the dynamics of Anyang-city. According to the result, Anyang-city is a "mature and stable" type, and its population is expected to decrease slowly by 2040. Specifically, the Anyang-city population will be reduced to 553,000 by 2030. It was analyzed that the number will decrease to 543,000 by 2040. It was also found that the investment in smart city projects in Anyang, based on the Plan for Anyang Smart City, would have the following effects: easing population decline, increasing number of businesses, improving urban safety index, and increasing average driving speed. The population will grow by 4,000 and the number of businesses will increase by 761 than before budget investment. The result of this paper is expected to contribute to identifying and predicting the effect of smart city policies from a long-term perspective.

Older Adults with Type 2 Diabetes Improve Glycemic Control after Nutritional Education Program at the Public Health Center (제2형 당뇨병 환자 식생활습관 및 보건소 영양교육 후 혈당개선효과)

  • This study was carried out to provide information on the effect of nutrition education program for diabetic patients at the Guri City's Public Health Center. Subjects of this study were 31 persons(male 7, female 24) who attended all courses of "2002 Diabetes Education Class". They were indicated as the 'education group'(EG). Eating and living habits of EG were investigated before the education. EG's weight and blood glucose (post prandial 2 hours, PP2) were examined as well. EG's PP2 reduction was compared with a 'control group'(CG) who didn't join any course in that class. All of the subjects were non-insulin-dependent diabetes mellitus(NIDDM) patients. EG's average age was 62.4$\pm$8.8. Before taking the course, EG's PP2 was 251.5$\pm$29.6mg/dl, and body mass index(BMI) was 26.3$\pm$2.3 on average. Most of them were stressed out from their daily lives and usually had no exercise. Most people of EG ate meals rapidly and liked sweet and fatty foods. After the course of training, EG's weight and BMI before the training were not decreased significantly. However, all of the EG's PP2s, which were measured 4 times(before the meal at the special lunch session, after 2 hours at this meal, after 2 weeks and 4 weeks dietary assembly), were decreased in comparison with the PP2 which was checked prior to joining the training. EG's average PP2 was more reduced than CG's one. In addition, all groups' PP2s were decreased for 8 weeks. After all, this nutritional education at the public health center was effective in glycemic control for diabetes mellitus patients. Especially, when the dietary assembly as practical training was included in the educational process, the patient's dietary intake and PP2 was improved more effectively. Therefore, this study suggests that nutrition work at public health centers is necessary for the Health Promotion Policy.

Influence of Different Slope Analysis during Pitching Wedge Swing on Plantar Pressure Distribution Pattern (경사면에서 골프스윙 동작시 족저압력 분석)

  • The study analyzed the mechanism of plantar foot pressure distribution during pitching wedge swinging on a flat, an up hill lie and a down hill lie to provide the fundamental information regarding biomechanical motion data by using plantar foot pressure measuring instrument. In the results, time factor spanning according to slope differences, plantar foot pressure factor and swing motion on the slope could have negative effect on the coiling of lower limbs during back swing, as well as the blocking of the lower limbs to minimize the dispersion of the weight and the release of the lower limbs after the impact during the down swing process. Moreover, since slope is one of many external factors affecting swing motion, address motion on an up hill lie limits the lower limbs movement, therefore, a relatively narrow stance is better on a down hill lie. It is estimated that a relatively wide stance would be better in order to limit the bigger activation of the lower limbs. Not only for the address motion but also during the down swing on an up hill lie it is concluded that the weight should be on the left foot in order to keep the body balance.

Method Validation for Monitoring of Agricultural Worker Exposure to Insecticide Fenthion (살충제 Fenthion에 대한 농작업자 노출 측정을 위한 분석/시험방법 검증)

  • Exposure measurement of agricultural worker to pesticide is one of important part of health risk assessment of pesticide. Therefore exposure matrices, apparatus, instruments and methods must be validated in advance to field experiment. In this study, method validation with an organophosphorus insecticide fenthion was carried out for exposure monitoring of agricultural worker. LOD and LOQ were 0.01 and 0.05 ng, respectively. Calibration curve linearity ($R^2$ > 0.999) and reproducibility (C.V. < 3%) were also excellent. Recovery at LOQ, 10LOQ and 100LOQ levels from gloves, socks, mask, patch, solid sorbent, glass fiber filter was 76~113% (C.V. < 3%). Trapping efficiency was 95~105% while no breakthrough was observed. Method validation for the exposure monitoring was established successfully through several experiments. Such method validation can be usually performed in laboratory and not much different for each pesticide so that, this techniques will be applied widely in research for pesticide exposure monitoring by combination with body surface area and respiration rates.
