임플란트 지대주나사 코팅이 결합안정성에 미치는 영향 (Influence of the Dental Implant Abutment Screw Coating Materials on Joint Stability)

  • 본 논문의 목적은 코팅처리의 기계적 성질과 임플란트 지대주나사의 코팅처리가 고정체와의 결합안정성에 미치는 영향을 연구하고자 함이다. 이를 위해 Megagen 임플란트 시스템과 직경 15 mm, 높이 1 mm의 티타늄원판을 사용하였다. 지대주 나사와 원판에 TiN, TiCN, 및 TiC를 코팅하여 실험군으로, 코팅처리하지 않은 것을 대조군으로 하였다. 실험군은 지대주나사를 32 Ncm로 체결한 후 풀림회전력과 조임회전각을 측정하고, 10만 번씩 저작 모방 시험을 실시하여 풀림회전력의 변화를 관찰하였다. 조임회전각은 TiCN, TiC코팅나사에서 코팅하지 않은 나사보다 크게 나타났다(p<0.05). 그러나 풀림회전력은 TiCN, TiC코팅나사에서 코팅하지 않은 나사보다 낮게 나타났다(p<0.05). 저작 모방 시험후 코팅한 나사에서 코팅하지 않은 나사보다 풀림 회전력이 더 높게 나타났으며 풀림회전력은 건조상태와 습윤상태 사이에 유의한 차이가 없었다. 이상의 결과로 TiC 코팅군은 저작 모방 시험 전에는 풀림회전력이 낮았지만 저작 모방 시험 후에는 낮은 마찰계수로 인한 높은 풀림회전력과 조임회전각을 가져 임플란트 지대주나사의 결합안정성에 영향을 미치는 효율적인 방법임을 시사한다.

IN VITRO STUDY OF THE TENSILE BOND STRENGTH OF CEMENT-RETAINED SINGLE IMPLANT PROSTHESIS BY THE VARIOUS PROVISIONAL LUTING CEMENTS AND THE SURFACE TREATMENT OF ABUTMENTS

  • The main disadvantage of cement-retained implant restorations is their difficulty in retrievability. Advocates of cemented implant restorations frequently state that retrievability of the restoration can be maintained if a provisional cement is used. The purpose of this study was to find the optimal properties of provisional luting cements and the surface treatment of abutments in single implant abutment system. 30 prefabricated implant abutments, height 8mm, diameter 6mm, 3-degree taper per side, with light chamfer margins were obtained. Three commercially available provisional luting agents which were all zinc oxide eugenol type ; Cavitec, TempBond and TempBond NE were evaluated. No cement served as the control. TempBond along with vaseline, a kind of petrolatum (2:1 ratio) was also evaluated. Ten out of thirty abutments were randomly selected and abutment surfaces were sandblasted with $50{\mu}m$ aluminum oxide. Another ten abutments were sandblasted with $250{\mu}m$ aluminum oxide. A vertical groove, 1 mm deep and 5mm long was cut in each twenty abutments. Ten of them were sandblasted with $50{\mu}m$ aluminum oxide. The full coverage casting crowns were cemented to the abutments with the designated provisional luting agent. Specimens were stored in distilled water at $37^{\circ}C$ for 24 hours. Each specimen was attached to a universal testing machine. A crosshead speed of 0.5mm/min was used to apply a tensile force to each specimen. Within the limitations of this in vitro study, the following conclusions were drawn: 1. Tensile bond strength of provisional luting cements in no surface treatment decreased with the sequence of TempBond NE, TempBond, Cavitec, TempBond with vaseline, no cement. 2. Tensile bond strength more increased by surface treatment. Sandblasting with $250{\mu}m$ aluminum oxide exhibited the highest tensile bond strength in the abutment cemented with TempBond NE and sandblasting with $50{\mu}m$ aluminum oxide exhibited the highest tensile bond strength in cemented with TempBond. 3. In the aspect of a groove formation, tensile bond strength significantly increased in TempBond with vaseline only and the others had no significant effect on tensile bond strength.

치과 임플랜트용 고정체 인상법과 지대원주 인상법간의 정확성 비교 (A COMPARISON OF ACCURACY BETWEEN FIXTURE IMPRESSION AND ABUTMENT IMPRESSION FOR DENTAL IMPLANTS)

  • The purpose of this study was to compare the accuracy between future impression and abutment impression using strain gauges. The master model used in this study was a partially edentulous mandibular metal cast with two fixture analogs on both sides. On the left, two future analogs were parallel, whereas right side, posterior future analog exhibiting a 15-degree lingual inclination. From master cast, 10 impressions were made for each of the three impression methods. The master frameworks was fabricated on the master model, and two-element strain gauge was attached to a master framework. The master framework was seated on each cast, and gold screws were tightened to 10 Ncm using a torque controller AI-1600 strain measurement system was used for strain measuring. Impression methods studied were : Group 1:abutment impression Group 2:fixture impression Group 3:combined impression (anterior:fixture impression, posterior:abutment impression) The results were as followed. 1. The strain values on X-axis and Y-axis according to the three impression methods showed no significant difference. 2. The strain values on parallel and angulated groups according to the three impression methods showed no significant difference. 3. The parallel group exhibited significantly higher accuracy in adaptation than angulated group for all experimental groups (p<0.05). In conclusion, it is considered that accuracy of implant prostheses is more affected by implant angulation than impression methods.

Clinical reliability of zirconium abutment in implant restorations in the English and Korean literature

  • Background: This study aimed to evaluate the mechanical, biological, and esthetic stability of a zirconium abutment according to evidence-based dentistry. Main text: An electronic search was performed. Domestic studies were found using the keywords "zirconia abutments" and "zirconium abutment" in KMbase, KoreaMed, and the National Assembly Library, and international studies were found using the same keywords in PubMed. All identified studies were divided by evidence level from the viewpoint of the research type utilizing the evidence-based review manual. A total of 102 domestic studies (with Korean language) were found, and 9 of these studies were selected. In these nine studies, 3 had evidence level 3 and 6 had evidence level 4. A total of 97 international studies (with English language) were found, and 19 were selected. Among these 19 studies, 5 had evidence level 2 and 7 had evidence level 3, whereas the remainder had evidence level 4. According to the studies, zirconium abutments are mechanically, biologically, and esthetically stable, but the evidence level of these studies is low, and the follow-up duration is no longer than 5 years. Conclusions: All examined studies verified the mechanical stability of zirconium abutments for a period no longer than 5 years. Therefore, a long-term clinical observation is needed. Zirconium abutments are thought to be biologically stable, but they are not superior to titanium abutments. As the esthetic stability of such abutments had a low evidence level in the studies that examined here, a much higher evidence level is needed.

일체형 지대주의 Diamond Like Carbon 표면 처리와 나사 조임 시기가 풀림 현상에 미치는 영향 (THE INFLUENCE OF ABUTMENT SCREW TIGHTENING TIMING AND DLC COATING OF CONICAL CONNECTION IMPLANT SYSTEM)

  • 본 연구의 목적은 내측연결형 임플랜트와 일체형 지대주를 사용하여 최종 보철물을 시적한 뒤에 5만 번의 반복하중 후에 다시 체결하고 20만 번의 반복하중을 가한 후의 지대주 풀림토크와 20만 번의 반복하중을 받은 지대주 풀림토크가 어떻게 달라지는지를 비교하고자 하는 것이다. 실험 재료로는 타이타늄 지대주와 DLC 코팅된 지대주를 사용하였다. 실험군은 4군으로 모두 8개의 ITI SLA $4.1{\times}10mm$ 를 사용하였다. A군과 B군은 5만번, 20만번 반복하중을 가하고 Periotest값과 풀림 토크값을 측정하였으며 C군과 D군은 20만번 반복하중을 가하고 Periotest 값과 풀림토크값을 측정하였다. 풀림 토크값의 측정은 디지털 토크 게이지를 사용하였고, 반복하중은 MTS. (Bionix 850 II, MTS, U.S.A.)를 사용하였으며 100N/cm의 힘을 20도의 경사, 14Hz 의 조건으로 가하였다. A군과 C군은 코팅하지 않은 타이타늄 지대주를 사용하였으며, B군과 D군은 DLC 코팅한 지대주를 사용하였다. 1. 5만번 반복하중을 가한 뒤 측정한 풀림 토크는 B군에서 약간 더 큰 평균값을 보였다. 하지만 통계학적으로 두 군간에 유의한 차이를 보이지 않았다 (P>0.05). 2. 20만번 반복하중을 가한 뒤 측정한 최종 풀림 토크 값은 A군은 C군보다, B군은 D군보다 약간 더 큰 평균값을 보였으나 통계학적으로 두 군간에 유의한 차이를 보이지 않았다 (P>0.05). 3. 20만번 반복하중을 가한 최종 풀림 토크 값은 A, B군과 C, D군 사이에 통계학적으로 유의할 만한 차이가 없었다 (P>0.05).

  • Statement of problem: Major objective in making on implant-supported prosthesis is the production of superstructure that exhibits a passive fit when connected to multiple abutments. One requirement to ensure passive fit is to make an accurate impression. Purpose : The purpose of this study was to compare the accuracy of master cast fabricated by using different impression methods at the different impression levels. Material and method: The master model used in this study was resin block having low implant analogs. Impression method studied were 1) direct method on fxiture level (Group FIX-D), 2) indirect method on fixture level(Group FIX-I), 3) modified indirect method on fixture level(Group FIX-M), 4) direct method on abutment level(Group AB-D) and 5) indirect method on abutment level(Group AB-I). Each of the five groups took 10 impressions. Fifty impressions were made for master cast by using Impregum $F^{(R)}$ impression material loaded on individual tray. Three dimensional measuring microscope was used to measure the inter-implant distance. Error rate of each inter-implant distance were calculated and evaluated. Results : The results were as follows. 1. Group FIX exhibited higher accuracy than group AB. 2. In group FIX, modified indirect method showed the highest accuracy, while indirect method showed the lowest accuracy. In group Ab, indirect method showed the higher accuracy than direct method. 3. Group FIX showed larger horizontal error than group AB. But, group AB showed the larger vertical error than group FIX. 4. Group Fix-M showed smallest vertical and horizontal error. Conclusion: An impression method have more effect on accuracy of master model than an impression level. A modified indirect method showed smallest vertical and horizontal error.

경사하중에 따른 시멘트 유지형 임플란트 지지골의 유한요소법 응력 분포 (Finite Element Analysis of Stress Distribution on Supporting Bone of Cement Retained Implant by Oblique Loading)

  • 본 연구는 치과 임상에서 사용하고 있는 시멘트 유지형 치과용 임플란트의 지지골 응력 분포 안정성을 확인하고자 시행하였다. 모델링과 유한요소 응력 분석은 유한요소 해석 프로그램인 Solidworks를 사용하였고, 시멘트 유지형 임플란트 시스템인 지대주와 고정체를 연결하는 지대주 나사를 20 Ncm 나사조임력에 의한 결합조건을 적용시킨 단관 모델을 제작하고, 설측에서 협측으로의 $45^{\circ}$ 경사로 100 N 크기 외부하중을 가하여 지지골 응력 분포 해석을 실시하였다. 경사하중에 따른 임플란트 고정체의 지지골 응력 크기와 분포를 파악하기 위한 유한요소법 분석을 통해 다음 결과를 얻었다. 고정체 직경, 길이의 조건에 관계없이 임플란트 고정체 상부와 골 접촉부인 치밀골에 응력이 집중되는 양상으로 나타났고, 고정체 길이 증가로 인한 응력 감소 폭보다 직경 증가로 인한 감소폭이 큰 것으로 나타났다 따라서 본 연구 결과는 지지골 형태 조건에 대하여 가능한 큰 직경의 고정체 사용이 효과적이라고 판단된다.

Accuracy of a proposed implant impression technique using abutments and metal framework

  • PURPOSE. This study compared the accuracy of an abutment-framework (A-F) taken with open tray impression technique combining cement-on crown abutments, a metal framework and resin cement to closed tray and resin-splinted open tray impression techniques for the 3-implant definitive casts. The effect of angulation on the accuracy of these 3 techniques was also evaluated. MATERIAL AND METHODS. Three definitive casts, each with 3 linearly positioned implant analogs at relative angulations 0, 30, and 40 degrees, were fabricated with passively fitted corresponding reference frameworks. Ten impressions were made and poured, using each of the 3 techniques on each of the 3 definitive casts. To record the vertical gap between reference frameworks and analogs in duplicate casts, a light microscope with image processing was used. Data were analyzed by two-way analysis of variance and the Tukey test. RESULTS. The open tray techniques showed significantly smaller vertical gaps compare to closed tray technique (P < .05). The closed tray and the resin-splinted open tray technique showed significantly different vertical gaps according to the angulation of implant (P < .05), but the A-F impression technique did not (P > .05). CONCLUSION. The accuracy of the A-F impression technique was superior to that of conventional techniques, and was not affected by the angulation of the implants.

  • Although many prosthetic materials exist for fabrication of implant-supported telescopic overdentures, available materials have not been thoroughly evaluated from a functional standpoint. This case report describes the use of polyaryletherketone (PAEK) based polymer for an implant-supported telescopic overdenture, a seldom used material in dentistry. This material is lighter than traditional materials, can accommodate changes in retentive forces, and is an easily retrievable by CAD/CAM fabrication. This case highlights the possibility of using new polymer materials for implant-supported telescopic overdentures.
