Statement of problem. Wear as a result of repeated closing/opening cycles may decrease the friction coefficient of screw head, threads, and other mating components and, consequently, resistance to opening gradually decreases. It may cause screw loosening, which is one of the most common failures in implant prosthesis. Purpose. The purpose of this study is to evaluate the changes on the head and thread surface of the abutment screws after repeated closing and opening through the examination of tested screws in SEM(scanning electron microscope). Materials and methods. Five species of abutments were selected (3i-three, Avana-two) respectively by two pieces. The implant fixtures were perpendicularly mounted in liquid unsaturated polyesther(Epovia, Cray Valley Inc.) with dental surveyor. Each abutment was secured to the implant fixture by each abutment screw with recommended torque value using a digital torque controller. The abutment screws were repeatedly tightened and removed 20 times with a digital controller. FESEM (field emission scanning electron microscope, Netherland, Phillips co., model:XL 30 SFEG) was used to observe changes of each part caused by repeatedly closing/opening expeiment. First, the Photomicrographs of pre-test screws provided by each manufacturer were taken. The changes of each screw were investigated after every fifth closing and opening experiment with FESEM. Scaning electron microscope photomicrographs of each screw were taken four times. Results. As the number of closing and opening was increased, the wear or distortion of hexed or squared slot that contacted with the driver tip was more severely progressed. Wear or distortion of hexed slot was more severe than that of squared slot and it was more remarkable in the titanium screw than in the gold screw. All the tested screws showed that the width in the crest of their screw thread decreased gradually as the test was proceeded. Conclusions. Conclusively, we recommend the clinical use of gold screw, a periodic exchanges of abutment screws and avoiding repeated closing/opening unnecessarily. We also suggest a more careful manipulation of the abutment screw and screw-driver and using of abutment screw with an acute-angled slot design rather than an obtuse-angled one. Finally, it is suggested that the new slot design and the surface treatment for enduring wear or distortion should be devised.
Purpose: The object of this study was to determine if the low-priced alloy and metal UCLA abutment could be available for manufacturing bar-retained framework of implant prosthesis. Materials and methods: Bar structure was classified into 4 groups, The specimen of group 1 and 2 were based on casting high noble metal alloys and noble metal alloys with gold UCLA abutment. The specimen of group 3 and 4 were based on casting noble metal alloys and base metal alloys with metal UCLA abutment. Cast bar structure was installed in an acrylic resin model and only the screw on the hexed abutment side was tightened to 20 Ncm. On the opposite side, vertical discrepancy was measured with stereo microscope from front, back, and lateral side of the implant-abutment interface. One-way ANOVA was performed to analyze the marginal fit discrepancy. Results: One-way ANOVA test showed significant differences among all groups ($P$<.05) except for Group 1 and 3. Among them, difference between Group 1 and 2 was noticeable. Measured vertical discrepancies were all below $70{\mu}m$ except to Group 2. Conclusion: Base metal alloy and metal UCLA abutment could be used as an alternative to high-priced gold alloy for implant bar-retained framework.
Purpose: The purpose of this study was to investigate screw joint stability and sagittal fit between internal connection implant fixtures of two different manufacturers and customized abutments. Materials and methods: Internal connection implant systems from two different manufacturers (Biomet 3i system, Astra Tech system) were selected for this study (n=24 for each implant system, total n=48). For 3i implant system, half of the implants were connected with Ti ready-made abutments and the other half implants were connected with Ti CAD-CAM custom ones of domestic-make (Myplant, Raphabio Co., Seoul, Korea) and were classified into Group 1 and Group 2 respectively. Astra implants were divided into Group 3 and Group 4 in the same way. Micro-CT sagittal imaging was performed for fit analysis of interfaces and preloading reverse torque values (RTV) were measured. Results: In the contact length of fixture-abutment interface, there were no significant differences not only between Group 1 and Group 2 but also between Group 3 and Group 4 (Mann-Whitney test, P>.05). However, Group 2 and Group 4 showed higher contact length significantly than Group 1 and Group 3 in abutment-screw interface as well as fixture-screw one (Mann-Whitney test, P<.05). In addition, RTV was lower in CAD-CAM custom abutments compared to ready-made ones (Student t-test, P<.05). Conclusion: It is considered that domestically manufactured CAD-CAM custom abutments have similar fit at the fixture abutment interface and it could be used clinically. However, RTV of CAD-CAM custom abutments should be improved for the increase of clinical application.
Purpose: The biocompatibility and bio-adhesive property of a dental implant abutment are important for proper soft tissue healing and maintenance of osseointegration of implant. However, studies of soft tissue healing and mucosal attachment of various materials of implant abutment other than titanium are still needed. In this study, cell attachment, proliferation, cytotoxicity of human gingival fibroblast for ceramic, gold alloy, Ni-Cr alloy and, commercially available pure titanium as a control were evaluated, using MTS and scanning electron microscopy. Materials and Methods: Specimen was designed to disc, 4mm diameter and 1mm thickness, made of ceramic, gold alloy, Ni-Cr alloy and commercially available pure titanium. Primary culture of human gingival fibroblasts were grown in Dulbecco's modified Eagle's medium with 10% fetal bovine serum and 1% antibiotics. Cells were inoculated in the multiwell plates placed the specimen disc. Cell Titer 96 AQucous One Solution Cell Proliferation Assay were done after 1hour 3hours, 24hours, 3days, 5days of incubation. The discs were processed for scanning electron micrography to evaluate cell attachment and morphologic change. Results: The results were obtained as fellows. 1. The ceramic showed high cell attachment and proliferation and low cytotoxicity, which is as much bioadhesive and biocompatible as titanium. 2. The gold alloy represented limited proliferation of human gingival fibroblast and the highest cytotoxicity among tested materials (p<0.05). 3. The Ni-Cr alloy limited the proliferaion of the human gingival fibroblast compared to titanium(p<0.05) but cytotoxicity on the bottom of well was not so considerable, compared to titanium. 4. On the scanning electron micrographs , the ceramic showed good attachment and proliferation of human gingival fibroblast, which was similar to titanium. But gold alloy and Ni-Cr alloy showed the shrinkage of gingival fibroblast both after 24 hours and 3 days. On 5th day, small amount of the human gingival fibroblast proliferation was observed on the Ni-Cr alloy, while the shrinkage of gingival fibroblast was still observed on the gold alloy. Conclusions: These results suggest that the ceramic abutment is as biocompatible as titanium to make proper mucosal seal. The gold alloy has a high cytotoxicity to limit proliferation of gingival fibroblast, which suggest limited use on the anterior tooth where soft tissue healing is recommeded.
Purpose: In this study, the retrospective radiographic study is executed to evaluate amount of bone loss of various conditions in patients using customized abutment for 4 years of follow-up. Materials and methods: The subjects of this study were implant fixed dental prosthesis using CAD/CAM customized abutments. CAD/CAM customized abutment and fixed dental prosthesis were manufactured by the Prosthodontics Department of Chosun University Dental Hospital from August 1, 2011 to July 31, 2012. Radiological assessments were performed on the patients who were treated by the fixed prosthodontics. After each treatment, a retrospective study was performed for a total of 4 years at 3 months, 6 months, 1 year, 2 years, 3 years, and 4 years. Results: As a result of the study, the customized abutment using CAD/CAM showed less bone loss than the results of existing research. There was no statistically significant differences at alveolar bone loss between splinting group and non-splinting group (respectively 0.27 mm, 0.5 mm). Also, there were statistically significant differences at alveolar bone loss in mx. anterior, mx. posterior, mn. anterior and mn. posterior part (respectively 1.37 mm, 0.39 mm, 0.00 mm, 0.30 mm). Conclusion: The customized abutment using CAD/CAM showed less bone loss than the results of existing research, there were statistically significant differences at alveolar bone loss in implant positions.

The purpose of this study was to clarify the clinical efficiency of tungsten carbide/carbon coating on obtaining optimal preload of abutment screw compared with non-coated screw in external-hex implant system. In the present work, rotational value and the compressive force between abutment and fixture of abutment screws without coating and with coating tightened to 30Ncm were measured. Mean compressive force of coated screw was 504.6N. Then uncoated screw was tightened to clamping the abutment and the implant to 504.6N, and the tightening torque value and the rotational value was recorded. The following conclusions were drawn within the limitation of this study. Compressive force of coated screw was higher than that of uncoated screw after tightening at 30Ncm. The tightening torque that was nedeed to clamping the uncoated screw equal to coated screw tightened to 30Ncm was 55.6Ncm. In case of equal compressive force, there was no significant difference in rotation value between coated and uncoated screw.
The digital workflow of optical impressions by the intraoral scanner and CADCAM manufacture of dental prostheses is actively developing. The complex process of traditional impression taking, definite cast fabrication, wax pattern making, and casting has been shortened, and the number of patient's visits can also be reduced. Advances in intraoral scanner technology have increased the precision and accuracy of optical impression, and its indication is progressively widened toward the long span fixed dental prosthesis. This case report describes the long span implant case, and the operator fully utilized digital workflow such as computer-guided implant surgical template and CAD-CAM produced restoration after the digital impression. The provisional restoration and customized abutments were prepared with the optical impression taken on the same day of implant surgery. Moreover, the final prosthesis was fabricated with the digital scan while utilizing the same customized abutment from the provisional restoration. During the data acquisition step, stl data of customized abutments, previously scanned at the time of provisional restoration delivery, were imported and automatically aligned with digital impression data using an 'A.I. abutment matching algorithm' the intraoral scanner software. By using this algorithm, it was possible to obtain the subgingival margin without the gingival retraction or abutment removal. Using the digital intraoral scanner's advanced functions, the operator could shorten the total treatment time. So that both the patient and the clinician could experience convenient and effective treatment, and it was possible to manufacture a prosthesis with predictability.
The objective of this finite element method study was to analyze the stress distribution induced on a supporting bone by 3.75mm, 4.0mm, 5.0mm diameter of dental implant fixture(13mm length). 3-dimensional finite element models of simplified gold alloy crown(7mm height) and dental implant structures(gold cylinder screw, gold cylinder, abutment screw, abutment, fixture and supporting bone(cortical bone, cancellous bone) designs were subjected to a simulated biting force of 100 N which was forced over occlusal plane of gold alloy crown vertically. Maximum von Mises stresses(MPa) under vertical loading were 9.693(3.75mm diameter of fixture), 8.885(4.0mm diameter of fixture), 6.301(5.0mm diameter of fixture) and the highest von Mises stresses of all models were concentrated in the surrounding crestal cortical bone. The wide diameter implant was the good choice for minimizing cortical bone-fixture interface stress.

Purpose: This retrospective study was performed to analyze the relationship between complications of the posterior mandibular single crowns and distance from the adjacent teeth to the implant. Subjects and Methods: Of the patients who presented Ewha Womans University Mokdong Hospital & Yonsei University Dental Hospital with missing of the posterior mandibular molar and restored with implant-supported 18 Single crowns between 1996 thru 2007, 115 patients had been followed after crown delivery. The subjects were divided into complication-followed group and a control without any problems. The distance from the most distal tooth to the implant were measured. The prosthetic & biologic complications were reviewed by the cantilever distance and analyzed by abutment type, age & gender statistically using SAS version 9.1 (SAS Inc., USA). Results and Conclusion The results were as follows; 1) The posterior mandibular single crown with cantilever showed higher incidence of follow-up complications upon logistic analysis (p<0.05). 2) The prosthetic and biologic complications are related with the cantilever distance with 2.1 odds ratio and 3.39 cut-off value of specificity & sensitivity by SPSS 12.0. 3) The complications are neither significant in abutment types nor age & gender.

The external contour of an implant can have significant effects on the load transfer characteristics and may result in different bone failure rates for different implant system. The purpose of this study was to investigate the effects of crest module shape and occlusal load direction on bone failure modes of five commercially available dental implant systems. Five different implant systems with internal connection; ITI (Model 1), Astra (Model 2), Bicon (Model 3), Friadent (Model 4), and Paragon (Model 5), comparable in size, but different in thread profile and cest module shapes, were compared using the finite element method. Conclusively, in the internal connection system of the implant-abutment connection methods, the stress-induced pattern at the supporting bone according to the abutment connection form had differenence among them, and implants with narrowing crestal module cross-sections at the top of the cortical bone created more favorable load transfer characteristics in this region. But it is considered that the future study is necessary about how this difference in the magnitude of the stress have an effect on the practical clinic.
