There are many advantages to unit-care welfare facilities' care services for the elderly in Japan. The field research was conducted after holding interviews with employees at five elderly welfare facilities in Japan. This research analyzes the space arrangement of unit-care welfare facilities in Japan's Tohoku rural area. The purpose of this study is to provide design data on the space arrangement of a unit-care facility for Korea. The results of research are as follows. 1. Cafes, restaurants, and stores were operated in the elderly welfare facilities, which were open to the general public as well. Therefore, local residents frequently visited. 2. The kitchen, living rooms, private bedrooms and construction of the elderly welfare facilities were similar to that of normal residential houses. 3. The event hall is conveniently located at the center of the facility. 4. It was easy to understand the health status of the elderly by having a health office in the open living room. 5. There were open spaces which are frequently used by the homebound elderly, including room rehabilitation, daycare and short term residence. The above results will be used for space planning data in Korean unit-care facilities.
Purpose: This study was to identify factors that influence the life satisfaction of solitude elderly. Method: The subject were 100 homebound the elderly living alone(age=76.58) who were live in M city located in Jeollanamdo. Data were collected from June 1 to September 30, 2002. The instrument used for this study were a survey of general characteristics, residential environment, MUNSH, Health Self-rating scale, ADL, loneliness and POMS. The data was analyzed using descriptive statistics, t-test, and stepwise Multiple Regression. Result: In general the perceived health status was poor, but ADL was moderate., loneliness was high but mood and life satisfaction were low. In regression analysis, life satisfaction was significantly influenced by mood(51%), ADL(4%), loneliness(3%), and perceived health status(1%). These variables explained 59% of the variances in the life satisfaction. Conclusion: The result suggests that health care professionals should give more attention to helping the elderly raise their life-satisfaction. A further study is necessary to find out an effective nursing intervention for a better those in a comfortable residential environment, decreasing the loneliness and to promoting the mood for those elderly who live alone.
The purpose of this research was to observe the management system of 18 free meal service centers for the low-income homebound elderly in Chungcheongbuk-Do In order researchers interviewed staff members of the free meal service centers. Based on the results of interviews with staff members of the meal service organization. only two centers used a standard recipe. and most of the meal preparation was controlled under the experience of volunteers. Only two meal service centers employed dietitians due to the lak of budgets. The cost of a meal per day ranged from ₩ 556 to ₩2,750 and the number of attendants at meal service ranged from 35 to 350 persons. The budget for most of the meal service centers is not enough to provide meals for the elderly who want to participated in free meal service programs. Home delivery meal service was not considered due to the lake of manpower and areal dispersion in rural ares. Most meal service organizations did not cooperate with other community service organizations. Several improvement strategies are recommended for the effective running of the free meal service centers. First, a nutrition specialist should be included in the staff members of the service organization to provide nutritious meal service to the elderly Second, a joint control system might be introduced into the present system to reduced the cost and the recover the limitations of areal dispersion. Third, service, coordination should be considered to overcome the problem of lake resources.
Purpose: This study was conducted to contribute on nursing practice for elderly people by identifying the effects of hand moxibustion and hand press pellet therapy on the alleviation of constipation. Methods: The data were collected from October to November, 2009, and the subjects included 39 elders (experimental group: 20, control group: 19) from a senior citizens center in B City who reported constipation. Experimental group received an intervention of hand moxibustion and hand press pellet therapy three times a week for six weeks (total: 18 times). Data about the number of defecations per week and constipation degree scores were collected one week after each treatment. Results: 1. The number of defecation in the experimental group were different from that of the control group (F=228.26, $p$ <.001). 2. The degree of reported constipation in the experimental group differed from the control group (F=170.59, $p$ <.001). Conclusion: Hand moxibustion and hand press pellet therapy was shown to be effective in alleviating the constipation of elderly people by increasing the number of defecation per week and reducing the constipation degree scores. These two interventions can thus be used for alleviating the constipation of elderly people, replacing the stool softener and enema.
The purposes of this study were to : a) examine the current foodservice management practices of twenty-one seniors centers in charge of hoke-delivered meal programs, b) evaluate the attitudes of one hundred and ten recipients of meal service program, and c) provide feedback for the efficient and effective foodservice management for the elderly. Statistical data analysis was completed using the SAS package program for descriptive analysis, T-test, and ANOVA. The results of quantitative analysis indicated that the costs of meals, containers and special foods were mostly dependent on the support from local government(Seoul city). The centers where the volunteers were over seventy five p ercent of the workers were frtty-eight percent and sixty-seven percent of the subjects in food preparation and food delivery to the homebound clients, respectively. Meal preparation and food purchasing were mainly practiced by social welfare worker. Standardized recipes were not developed and meal preparation was controlled under the cook' and volunteers' experiences. The survey results of recipients who participated the home-delivered meals program showed that the mean of meal satisfaction score was rated over three point five in the five-point scale. There were significant differences between dependent variables(volunteer's kindness, plate waste, menu variety) and independent variables(sex, receiving periods and family type of the subjects). The purpose of this study is to activate rehabilitation exercises to prevent MCI (mild cognitive impairment) in the elderly at home. Through the convergence of AI and rehabilitation exercise, we will be able to provide integrated services of medical care and rehabilitation exercise that enable integrated health management by activating rehabilitation exercise linkage in medically vulnerable communities such as rural areas and establishing exercise data. To this end, as a convergence development plan for AI and rehabilitation exercise, interdisciplinary experts will participate to produce and distribute game and rehabilitation exercise instruction manuals by type to improve cognitive function and musculoskeletal function, and systematize rehabilitation exercise programs needed for the elderly at home. The development, operation, and education of physical fitness assessment manuals can be expanded and will be of great help in early prevention of dementia.
Purpose: This study was done to develop and test a scale to measure the homeboundness for low-income aged who live in the community. Methods: This was nursing methodology research. Homeboundness Scale development process was composed of construct identification based on concept analysis using the Hybrid model, 35 initial items. This number was reduced to 31 items through face validity tests by 7 experts. The preliminary Homeboundness Scale for low-income aged was administered to 240 aged who registered and received visiting health care service in the community health center located in S city. Data were analyzed using item analysis, factor analysis, Pearson correlation coefficients, and Cronbach's alpha. Results: Twenty-two items were selected for the final scale. Three factors evolved from the factor analysis, which explained 66.0% of the total variance. The internal consistency, Cronbach's alpha, was .945 and reliability of the subscales ranged from .890 to .934. Conclusion: Homeboundness Scale demonstrated acceptable validity and reliability. It can be used to assess the Homeboundness of the low-income aged in practice and research.
The problem of care of patients and families with Alzheimer's disease has become a conscious raising social policy issue in Korea. The government of the Republic of Korea has become cognizant of the situation and has begun searching for ways to remedy it. Thus, there is a need for a comprehensive under-standing of the situation in which patients and their families are struggling and the enormous problems of care. With a realization of the urgent need, this study was done to investigate the situation and the care needs of families with patients with Alzheimer's Disease, and to compare the effectiveness of services utilized by the families in terms of cost and effects on patient's conditions and on family live. The Subjects for the study were 29 families with hospitalized patients, 25 families utilizing hospital outpatient clinics, 14 families utilizing day care facilities, and 16 families with homebound patients. A total of 84 families were interviewed by four trained interviewers using structured and semistructured questionnaires. The data produced from these interviews included : the patient's stage of Alzheimer's disease, patient's bizarre behavior, hours spent on patient care per day, family burden and quality of life, direct and indirect costs encountered in the care of patients, and the families' evaluation of the effectiveness of the services received. The data were analyzed to determine the relationships between family charactersistics, patient's conditions and services utilization. The effectiveness of each of the service entities was assessed through families evaluation and hoped for service and comparisons were made between services in terms of the cost-effectiveness ratios. After initial comparison of cost-effectiveness ratios, further analysis was done to compare between groups for incremental effectiveness for each incremental unit of cost to determine the most cost-effective service entities. The findings of the study are as fellows : 1. The choice of living arrangement and the types of services are a function of the stage of Alzheimer's condition and the economic status of the family. 2. Comparision of the cost of care showed that most expenses were encountered in by families with hospitalization, families using outpatient services, and families using day care services in that order. The least expense was involved in the care of homebound patients. The economic burden felt by families was in the same order as expenses. 3. The average number of hours spent on daily patient care was 9.9 hours for the outpatient clinic users, 9.7 hours for homebound patients, and 5.4 hours for day care users. 4. There were significant differences in the patient's conditions (CDRL), bizarre behaviors and the families's burden by living arrangement and /or types of service. However, no significant difference was found between groups in the family's quality of life. 5. The families rated the services of day care center as most effective for the care of the patients and families, except for a few families who had experienced some improvement in the patient's conditions. The outpatient clinic users expressed psychological comforts mainly in that the patient was being taken care of. For those hospitalized patients, families expressed the comfort of being relieved of the burden of care and that the patient is being professionally cared for. Form the analysis of the costs, hours of patient care, patient's bizarre behaviors, family's quality of life and burdens, and family's evaluation of services, it is concluded that up to the mid stage of Alzheimer's condition, the utilization of day care center services is found to be the most cost-effective, and toward the end stage of the Alzheimer's disease, it is hoped that there will be a establishment of long term or short term in-patient facilities for the protection of patients and preservation of the integrity of families for less cost. Thus. it was concluded that the family centered system of care is the most effective for Korea with systematic support systems developed for the care of patients and their families according to the needs of families as the patient's condition deteriorates.
In 2015, the population of elderly people in Thailand was 16% of the total population and is predicted to be over 20% by 2021 and nearly 28% by 2031. The increase of the elderly population in Thailand has also increased the proportion of dependent elderly people, and caring for them poses many challenges for both families and the government. This descriptive method research aimed to survey the health status of dependent older people in the rural community of Lampang province in northern Thailand. The participants consisted of 62 older people and 62 primary family caregivers from Hong Ha Health Promoting Hospital, Lampang, Thailand (totaling 124 people). The researchers assessed the health status of older people and their activities of daily living (ADL). In addition, researchers assessed the health status and stress of caregivers. All the participants were interviewed about their experiences with caregiving. The results showed that most of the older respondents were female with an average age of 78.15 years. Based on the ADL assessment, 50 of the 62 older persons were homebound while the rest were bedridden. The majority of older people had chronic or long-term conditions that required hospitalization from time to time. Their frequent health problems included oral disorders such as tooth decay or caries/gum disease/no teeth, reduced sight, psychological disorders, knee pain and risk of falling, low BMI, risk of malnutrition, and urinary leakage and incontinence (58.06%, 66.13%, 62.90%, 70.97%, 38.71%, 66.13%, and 37.10%, respectively). Usual care provided by the family members included personal hygiene care, food preparation and feeding, medication management, housekeeping and organizing necessary equipment, supply of needed equipment, prevention of falls, helping with travel for medical checkups and treatment, and providing companionship. Families experienced shortages of medical supplies, daily use equipment, lack of employment, inadequate income, and difficulty accessing health care services due to lack of transportation. Some caregivers experienced caregiving stress related to a lack of social interactions as well as routine caregiving activities. Families need different types of support in order to promote the well-being of older people and caregivers. This highlights the need for a community participation model for the care of older people in order to reflect sustainable long-term outcomes.

Objectives: This study was to investigate fear of falling and related factors in elderly living alone based on fall experience. Methods: Participants were 404 elderly women(faller=148, non-faller=256) over 65 years who were homebound living alone in Jeollanam-do Province. Face-to-face interviews were conducted using questionnaires from April $23^{th}$ to June $9^{th}$ 2013. The questionnaires consisted of demographic variables, fall experience, Fear of falling questionnaire(FOFQ), Falls Efficacy Scale(FES) and Center for Epidemiologic Studies Depression(CES-D). The collected data were analyzed using the SPSS version 14.0. Results: The fear of falling and the influential factors were different according to fall experience. Regression model for fear of falling in fallers significantly accounted for 46.1%(F=6.71, p<0.001); difficulty of performing activity, depression, fall-efficacy, static balance and assistive devices. Regression model for fear of falling in non-fallers significantly accounted for 55.2%(F=15.16, p<0.001); fall-efficacy, environmental hazards, difficulty of performing activity, risk of nutrition, housing type, dizziness and assistive devices. Conclusion: Results demonstrate that fall is an important health problem for elderly women living alone, and show fall experience for factors influencing fear of falling. These results could be used in the developing fall prevention programs.
