China's Diplomatic Challenges and Prospect in the Xi Jinping Era (시진핑 시대 중국의 외교적 과제와 전망)

  • This article aims to analyze the diplomatic challenges currently facing China as well as the foreign policy the Xi Jinping administration is adopting to address them. With these purposes in mind, it will look into three areas: first, diplomatic tasks confronting the Xi leadership; second, foreign policy that the Xi administration has implemented since the 18th Party Congress in 2012; and third, the prospects for China's foreign policy under the Xi leadership. As the Xi Jinping administration entered into office, it has encountered two major diplomatic challenges. One is the searching for a new foreign policy, and the other the restructuring of the existing foreign affairs and national security systems. The Xi administration, during its first year in office, has responded actively to tackle these issues. To begin with, it has attempted to make some adjustments on foreign policy while maintaining the Deng Xiaoping line of foreign policy. One of these modifications is placing emphasis on national "core interest," as illustrated by changes in guideline for maritime conflicts, pushing for building maritime power, and proclamation of Air Defense Identification Zone in the East China Sea. Second is the decision on the new guideline for peripheral diplomacy. That is, the administration regarded creating peaceful and stable environments to realize "Chinese Dream" as an important goal of foreign policy, and proposed such new guiding ideology as "familiarity, integrity, benefit, and accommodation." In additoin, the Xi administration restructured the existing foreign affairs and national security systems. Establishing the Central State Security Committee and the Internet Leading Small Group (LSG) are cases in point. As comprehensive organizations for policy coordination that encompass party, government, and military, the two LSGs are in charge of enacting related policies and fine-tuning policy implementation, based upon leadership consisting of chiefs of each relevant organs. Moreover, since Xi himself became the chief of these two newly-found organs, the conditions under which these LSGs could demonstrate unified leadership and adjusting role in its implementation of military, diplomatic, and security policies were developed. The future Chinese diplomacy will be characterized with three main trends. First, peripheral diplomacy will be reinforced. The peripheral diplomacy has become far more important since the Work Conference on Peripheral Diplomacy in October 2013. Second, economic diplomacy will be strengthened with an eye on reducing the "China Threat Theory," which still exists in Asia. Third, the policies to isolate the Philippines and Japan will continue in regard to maritime disputes. All in all, Chinese diplomacy in the Xi Jinping era is likely to feature practical diplomacy which combines both hard and soft approaches to best realize Chinese national interest.

Dioxins and Health: Human Exposure Level and Epidemiologic Evidences of Health Effects (다이옥신과 건강: 인체 노출 수준 및 건강영향에 대한 역학적 연구)

  • General information is summarized, that is necessary to introduce a scientific assessment of the human health and exposure issue concerning dioxin and dioxin-like compound. Scientific literatures were reviewed to assess the background exposures to the dioxin-like compounds for normal residents. Epidemiologic studies were also reviewed to assess malignant and nonmalignant sweets of dioxins. In 1997, the International Agency for Research on Cancer classified 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) as a human carcinogen, primarily based on occupational cohort studies. The US Environmental Protection Agency made the same decision in it's Draft Dioxin Reassessment. Epidemiologic evidences point to a generalized excess of all cancers, without any pronounced excess at specific sites. Reported non-cancer effects included a range of conditions affecting most systems. Among them, chloracne, elevation in gamma glutamyl transferase(GGT), and alterations in reproductive hormones are related to TCDO, Other adverse outcomes, such as lipid concentrations, diabetes, circulatory and heart diseases, immunologic disorders, neurobehavioral effects, and developmental outcomes require further study before their respective relationships to TCDD can be more definitively assessed.

The Effect of the Health Belief and Efficacy Expectation Promoting Program on Osteoporosis Preventive Health Behavior in Women with Rheumatoid Arthritis (건강신념 및 효능기대증진 프로그램이 류마티스 관절염환자의 골다공증 예방행위에 미치는 영향)

  • Osteoporosis has been known as a common complication of rheumatoid arthritis and a major preventable health problem. Lots of studios have demonstrated that changes in life style can help delay or prevent osteoporosis. Therefore nursing intervention related osteoporosis prevention have consisted of education programs aimed at changing dietary and exercise habit. However knowledge gained from education haven't always leaded to behavior change. Therefore it is important to consider other psychological variables in effecting behavior change. Numerous research have found self efficacy and health belief to be an important factor in individual decision making behavior. The purpose of the study was to develop health belief and efficacy expectation promoting program based on Health Belief Model & Self Efficacy Model and to investigate its effects in women with rheumatoid arthritis. For this purpose, one group pretest-post design was used. The subject of the study were 16 women with rheumatoid arthritis in Pusan city and data collection was carried out from April, 1997 to May, 1998. The intervention program was consisted of educating on osteoporosis and enhancing and reinforcing self efficacy by verbal persuasion during the period of 4 weeks. The instruments were used to collect data in this study were Osteoporosis Health Belief Scale, Osteoporosis Self Efficacy Scale, and Osteoporosis Preventive Behavior Scale. Data was analyzed by Wilcoxon signed rank test using SPSS $PC^+$ program. The results are as follows : 1) The behavior should be increased after intervention was supported(Z=-3.5162, p=.0004, diet : Z=-3.2942, p=.0010, exercise). 2) The sub-hypothesis that perceived sensitivity should be increased after intervention was supported (Z=-2.3854, p=.0171). 3) The sub-hypothesis that perceived severity should be increased after intervention was rejected(Z=-1.4327, p=.1520). 4) The sub-hypothesis that perceived benefit should be increased after intervention was supported(Z=-2.6410, p=.0083). 5) The sub-hypothesis that perceived barrier should be decreased after intervention was supported (Z=-2.4138, p=.0158). 6) The sub-hypothesis that efficacy expectation should be increased after intervention was supported(Z=-3.5162, p=.0004). As a conclusion, it was found that health belief and self efficacy promoting program was an effective nursing intervention for preventing osteoporosis of rheumatoid arthritis.

  • Targeted killing is a modern euphemism for the assassination of an individual by a state organization or institution outside a judicial procedure or a battlefield. Targeted killing using armed drones has raised profound anxieties in legal, policy, and advocacy communities in the United States and abroad, including among UN officials. The bottom line for targeted killing supporters is that targeted killing works as part of a larger counter-terrorism strategy. Targeted killing does what it is supposed to and removes the leader of a group. And despite growing legal, moral, and ethical issues concerning targeted killing, scholars agree that drone strikes and targeted killing operations will stay. The ACLU has sued top CIA and Pentagon decision-makers to seek accountability for the unlawful killings of three U.S. citizens in Yemen last year. Also, strikes by drones are associated with serious problems such as collateral damage to ordinary citizens and friendly fire. Targeted killings by drones also involves several issues to be resolved, including suspicions that they may run counter to domestic law prohibiting assassination, the opacity concerning their definitions and military actions, and the impact of whiplash transition. Finally, targeted killing program and the need for transparency. The assembly referring to resolution invites the committee of ministers to undertake a thorough study of the lawfulness of the use of combat drones for targeted killings and if need be develop guidelines for member states on targeted killings with a special reference to those carried out by combat drones. These guidelines should reflect the states duties under international humanitarian and human rights law in particular the standards laid down in the EC on human rights as interpreted by the european court of human rights.

Korea's Designation of Living Happiness Sphere in the Local Area from the Perspective of New Institutionalism (지역행복생활권 획정에 대한 신제도주의적 접근)

  • Park Geun-hye Administration has been leading HOPE Area(Living Happiness Sphere) policy as a part of strategy for the regional development. Recent study on HOPE Area policy mainly focuses on how to enhance cooperation among local governments or how to activate the HOPE area. However, as far as I found, the research on how HOPE area guideline as an institution is made and how HOPE area, the effect of the institution, is demarcated has not well been conducted. With based on these issues, this article aims to identify and examine the process of the HOPE area guideline-making and the designation of the HOPE area. For this purpose, new institutionalism is utilized for the analysis. The guideline was made through competition and compromise among a few parties concerned rather than a decision by unitary elite group, and the final outcome of the process reflected the balanced development strategy more than the growth pole development strategy. There was a tendency that the local governments try to make many HOPE areas by overlapping the HOPE areas, which are the outcomes of the bottom-up approach. In Seoul metropolitan area, a new type of HOPE area was introduced one year later than the other local regions.

A Workflow Determinacy Decision Mechanism (워크플로우 결정성 판단 메커니즘)

  • The primary tasks of a workflow management system specify workflow models with respect to resource, control-flow, data-flow, functional, and operational perspectives, and to enact their workcases (workflow instances). In terms of enacting workflow models, the essential criterion grading the quality of the system is "how much is the system able to guarantee the correctness of workflow models' enactment?". Particularly, the workflow determinacy problem, which may be caused by the interference of the control-flow and the data-flow specifications, is the most challenging issue in guaranteeing the correctness of the system. We are able to solve the problem by either of the following two approaches-analysis of workflow model and verification of workflow enactment. In the paper, we propose a technique that guarantee the system's correctness through verifying workflow enactment. In other words, the technique is able to detect the conflicts of control-flow and data-flow enactments existing on a workflow model, which causes the system to be non-determinant in enacting workflow models. Finally, by applying the technique to the e-Chautauque workflow management system developed by the authors' research group, we prove that the technique is a feasible solution for the workflow determinacy problem.

Characteristics of Pre-Service Secondary Mathematics Teachers' Anticipating Through the Task Dialogue Activity (과제대화록 작성하기를 통한 중등수학 예비교사의 예상하기 특징 분석)

  • The present study aims to investigate ways in which pre-service secondary mathematics teachers anticipate 1) students' responses to specific mathematical tasks which are chosen or devised by the participating pre-service teachers as requiring students' higher cognitive demand and, 2) their roles as math teachers to scaffold students' mathematical thinking. To achieve the goal, we had our pre-service teachers to engage in an adapted version of Spangler & Hallman-Thrasher(2014)'s Task Dialogue writing activity whose focus was to develop pre-service elementary teachers' ability to orchestrate mathematical discussion. 14 pre-service teachers who were junior at the time enrolled in the Mathematics Teaching Method Course were subjects of the current study. In-depth analysis of both Task Dialogues which pre-service secondary mathematics teachers wrote and audiotapes of the group discussions while they wrote the dialogues suggests the following results: First, the pre-service secondary teachers anticipated how students would approach a task based on their own teaching experiences. Second, they were challenged not only to anticipate more than one correct students' responses but to generate questions for the predicted correct-responses to bring forth students' divergent thinking. Finally, although they were aware that students' knowledge should be the crucial element guiding their decision-making process in teaching, they tended to lower the cognitive demands of tasks by providing students with too much guidance which brought forth the use of procedural knowledge. The study contributes to the field as it provides insights as to what to attend in designing teacher education course whose goal is to provide a foundation for developing pre-service teachers' ability to effectively orchestrate mathematical discussion.

Optimal Fixation and Decalcification Methods for Bone Marrow Biopsy (골수생검조직을 위한 최적의 고정 및 탈회 방법)

  • A bone marrow biopsy that has undergone decalcification with 10% nitric acid could not be used for various pathological tests and had extremely limited reproducibility. The fixing solution of each experimental group was differentiated in usage, one solution including acid and the other not. The use of the decalcification solution was separated into either acidic or alkaline (EDTA), and further experiments were conducted with differing time phases. When using a fixing solution and decalcification solution which included acid, the specimens were faulty to the extent that all pathological tests were impossible. However specimens that were processed with an EDTA type decalcification solution did not display a non-specific reaction in EBV ISH and were even able to produce results that were at a level suited to various studies or a pathological diagnosis in the FISH, DNA, RNA tests. By improving the fixing and decalcification of bone marrow biopsy, the study was able to make possible ISH, FISH, DNA tests as well as RNA study, and secured the sensitivity, specificity, and reproducibility of various test methods. The stabilization of various test methods that use bone marrow biopsy contributes to the diagnosis, prognosis, prediction, treatment of the patient and provide guidelines for decision-making.

A Study on the Practice Variations According to Physician Characteristics (의사 특성에 따른 외래 진료내용의 변이)

  • It is well known that a physician's personal characteristic affects his practice pattern. Furthermore, a physician's specialty has powerful influences on his practice pattern. However, despite the fact that specialization has received the most attention for its influence on physician's service behavior, few studies have been conducted on the variations of contents and volume of physician's services. This study has intended to identify factors influencing the practice variations according to various physician characteristics. There are some other evidences that medical care providers are different in using of health services and resources in Korea. Four physician characteristics were selected for the analysis, two demographical factors, age and sex, and two practice factors, place of practice and medical specialty. Also, three indicators of service amount (total amount of insurance claim bill, number of visits per case, number of prescriptions per case) were selected. From the pool of insurance claims for ambulatory care received by the Korean National Federation of Medical Insurance(NFMI), 84,898 cases were randomly sampled. In the meantime using physician database of NFMI, 613 general practitioners (GP), 107 regular family physicians (FP), 483 'grandfather' family physicians(GFP), and 1,157 specialist practitioners(SP) were randomly sampled. Their different practice contents were compared concerning the specialty, age groups, sex, and practice sites (urban-rural) Specialist physicians tend to provide more costly care than do generalists. General practitioners and family physicians usually make fewer following visits and prescriptions. Age is also the important factor in determining the amount of services, which is highest at the physician's age group of 40's. Female doctors and urban practitioners use much more resources than their counterparts respectively. Research findings suggest that physician's characteristics particularly the specialty can affect practice patterns and resource utilizations. Other characteristics such as age and sex are not controllable but physician's specialty is relatively easily controllable during the entire phases of policy implementation. This is all the more true in the individual's initial decision of his specialty. Specialization therefore should receive policymaker's attention for its potential influence on medical care utilization and health care expenditure.

Clinical Practice Guideline for the Treatment of Traumatic Shock Patients from the Korean Society of Traumatology

  • Purpose: Despite recent developments in the management of trauma patients in South Korea, a standardized system and guideline for trauma treatment are absent. Methods: Five guidelines were assessed using the Appraisal of Guidelines for Research and Evaluation II instrument. Results: Restrictive volume replacement must be used for patients experiencing shock from trauma until hemostasis is achieved (1B). The target systolic pressure for fluid resuscitation should be 80-90 mmHg in hypovolemic shock patients (1C). For patients with head trauma, the target pressure for fluid resuscitation should be 100-110 mmHg (2C). Isotonic crystalloid fluid is recommended for initially treating traumatic hypovolemic shock patients (1A). Hypothermia should be prevented in patients with severe trauma, and if hypothermia occurs, the body temperature should be increased without delay (1B). Acidemia must be corrected with an appropriate means of treatment for hypovolemic trauma patients (1B). When a large amount of transfusion is required for trauma patients in hypovolemic shock, a massive transfusion protocol (MTP) should be used (1B). The decision to implement MTP should be made based on hemodynamic status and initial responses to fluid resuscitation, not only the patient's initial condition (1B). The ratio of plasma to red blood cell concentration should be at least 1:2 for trauma patients requiring massive transfusion (1B). When a trauma patient is in life-threatening hypovolemic shock, vasopressors can be administered in addition to fluids and blood products (1B). Early administration of tranexamic acid is recommended in trauma patients who are actively bleeding or at high risk of hemorrhage (1B). For hypovolemic patients with coagulopathy non-responsive to primary therapy, the use of fibrinogen concentrate, cryoprecipitate, or recombinant factor VIIa can be considered (2C). Conclusions: This research presents Korea's first clinical practice guideline for patients with traumatic shock. This guideline will be revised with updated research every 5 years.
