A Systematic Review of Acupuncture-Moxibustion Treatments for Nonspecific Chronic Neck Pain (비특이적 목 통증에 사용된 침구치료에 대한 체계적 문헌고찰)

  • Objectives : The aim of this study was to describe the details and to assess the clinical evidence of acupuncture and moxibustion for non-specific chronic neck pain. Methods : We searched seven databases including Korean, English, and Chinese databases through July 2016. Studies using acupuncture, moxibustion, pharmacopuncture, electroacupuncture, auricular-acupuncture, acupressure for non-specific chronic neck pain were included. Only controlled clinical trials or randomized clinical trials were assessed. Study design, number of subject, inclusion criteria, intervention, and results were extracted. In addition, details of intervention including needle type, retention time, acupoints were analyzed. Results : Total 64 studies(39 acupuncture, 9 laser, 6 pharmaco-acupuncture, 3 electro-acupuncture, 3 auricular-acupuncture, 3 moxibustion, 1 acupressure) were included. Among 39 acupuncture studies, 35 used acupuncture as sole intervention. Sham treatment was the most common intervention for control group, followed by no intervention. Various outcome including pain, disability, quality of life, range of motion was used as outcome measurement. The effect of acupuncture and moxibustion was different depending on the type of control and outcome measurement. The most commonly used method in acupuncture for neck pain was GB21, SI3, GB20, LI4, BL11 acupoints, 10~30 mm insertion depth, 20~30 retention time, and 1~2 times per week. Conclusions : Analyzing the details of acupuncture and moxibustion treatment could be helpful for researches and clinics. Further studies should consider the characteristics of study design, intervention, and outcome to assess the effect of TKM.

  • The purpose of this study is to report that a patient suffered from Common Peroneal Neuropathy (CPN) caused by ankle sprain was improved by acupuncture plus electro-acupuncture. In this study, the patient was diagnosed with magnetic resonance image(MRI) and electomyography(EMG). We treated at acupoints ST36, ST41, SP9, GB34, GB39 and LR3 by acupuncture plus electro-acupuncture twice a day for 16 days; a total of 30 times. We used evaluation scale including muscle manual testing(MMT) and visual analogue scale (VAS). MMT changed from Grade 0 to Grade 5 and VAS changed from 10 to 2 after treatment for 16days with hospitalization. Though it is a case report, these results suggest that acupuncture and electrical acupuncture might have valid effect on CPN in controlled environment.

  • Heritabilities and genetic correlations of economic traits were estimated with a multiple-trait animal model on 5 different strains of Korea Native Chickens (KNC): Red Brown (RB), Yellow Brown (YB), Gray Brown (GB), Black (B) and White (W). The data used for this study were collected from the Daejeon branch of the National Livestock Research Institute and included 11,233 performance records and 12,729 individual animals. DFREML and SAS BASE/STAT packages were used to estimate genetic parameters and descriptive statistics. The estimated heritabilities for strain RB, YB, GB, B and W, respectively, for age at 1st egg were 0.24, 0.27, 0.12, 0.32 and 0.18; for body weight at first egg were 0.39, 0.43, 0.38, 0.52 and 0.57; for body weight at age of 270 days were 0.43, 0.51, 0.30, 0.52 and 0.67; for egg weight at first egg were 0.08, 0.13, 0.07, 0.06 and 0.07; for egg weight at age of 270 days were 0.37, 0.43, 0.22, 0.34 and 0.41; and for number of eggs laid by age of 270 days were 0.24, 0.25, 0.37, 0.36 and 0.30. The genetic and phenotypic correlations were also estimated.

  • In Oriental Medicine, mononeuropathy belongs to or . Common peroneal neuropathy(CPN) is the most frequently encountered mononeuropathy in the lower extremity. It is usually caused by direct surgery injury, compression, leg crossing, trauma, traction etc, occasionally by nerve tumor. A 47-year-old healthy man was complained of the sudden development of left foot drop and sensory manifestation owing to suspected compression and habitual leg-crossing. Acupuncture along with bee-venom acupuncture, moxibustion was performed mainly at Stomach and Gallbladder Meridian specially ST36, ST37, ST40, GB34 and GB39. Nerve conduction study and electromyography was also performed three times. Symtoms was relieved fast, and full recovery took about 110 days. Acupuncture and bee-venom acupuncture are considered to be beneficial to CPN. More clinical trials and studies are needed.

  • Two cases of in patients with diabetic peripheral neuropathy were reported in this clinical study. After the study, the results were as follows: 1. Diabetic peripheral neuropathy was occurred with both insulin dependent diabetic mellitus(IDDM) patients, long and short durations, and non insulin dependent diabetic mellitus(NIDDM) patients. However, the degree of subjective symptom was stronger with the former patients. 2. Non insulin dependent diabetic mellitus(NIDDM) patients on dosage of Gamisamultang (加味四物湯) showed remarkable decrease of duration of illness and pain; however, plantar causalgic pain was unremarkable. In contrast to non insulin dependent diabetic mellitus(NIDDM) patients, long duration of insulin dependent diabetic mellitus(IDDM) patients showed remarkable decrease of plantar causalgic pain: however, pain decreased unremarkably. 3. The general treatment of diabetic patients was the control of blood glucose level; however it seemed to be no effect on the degree of subjective symptom. When patients were treated with acupuncture, followed by electropucture, on Palpung(八風), Taechung(太衝:Liv3), Chok-imup(足臨泣: Gb41), Hyonjong(縣鐘 : G39), Sungsan(承山 : B57), Chok-Samni(足三里 : S36), and Yangnungchon(陽陵泉 : G34) showed a great effect on decreasing the pain.

The Effects of Nokyongsageunhwan Herbal Acupuncture and Oral Administration on the Growth and the Intellectual Development of Rats (녹용사근환약침(鹿茸四斤丸藥鍼) 및 경구투여(經口投與)가 흰쥐의 성장(成長)과 지능발달(知能發達)에 미치는 영향(影響))

  • Objective: This study was designed to investigate effects of Nokyongsageunhwan and Nokyongsageunhwan aquapuncuture on body weight, body length, intellectual development Methods: We observed body weight, body length, intellectual development after feeding Nokyongsageunhwan and Nokyongsageunhwan aquapuncuture for thirty three days, and mRNA expressions of GHRF, Somatostatin, IGF-1, NPY were analyzed by RT-PCR method. Results: The results were summarized as follows. 1. Body length was significantly increased in Nokyongsageunhwan herbal acupuncture group compared to normal group and normal saline injection group on 4th week, 2. Lower part of hind leg length was significantly increased in Nokyongsageunhwan oral administration group compared to normal group on 3rd week, and it was significantly increased in Nokyongsageunhwan herbal acupuncture group and Nokyongsageunhwan oral administration group compared to that of normal group on 4th week. 3. As results of observing memory aquisition using Morris water maze system, there was significantly difference in Nokyongsageunhwan oral administration group compared to that of normal group on 4th week. 4. As results of observing retention using Morris water maze system, staying time was significantly increased in Nokyongsageunhwan herbal acupuncture group compared to that of normal group. 5. The somatostatin mRNA expression in the hypothalamus was significantly decreased in Nokyongsageunhwan oral administration group compared to that of normal group. 6. The IGF-1 mRNA expression in the hypothalamus is significantly increased in Nokyongsageunhwan herbal acupuncture group and Nokyongsageunhwan oral administration group compared to that of normal group. 7. The NPY mRNA expression in the hypothalamus was significantly increased in Nokyongsageunhwan oral administration group compared to that of normal group, and it was increased in Nokyongsageunhwan herbal acupuncture group compared to that of normal group, but there was no significant difference. Conclusions: According to the above results, it appears that Nokyongsageunhwan oral administration and Nokyongsageunhwan herbal acupuncture on acupoint GB34 and GB39 showed effects on the growth and the intellectual development of rats.

Real-Time Glutamate Release in Rat Striatum of 11-Vessel-Occlusion Ischemia Model Treated with Acupuncture (11개 혈관 차단법을 통한 중증 뇌경색 모델에서 뇌손상 측정과 침치료 효과 실시간 분석)

  • Acupuncture has long been contended to be effective in an ischemic stroke. A real-time monitoring of glutamate, an excitotoxin in the process of ischemic neuronal damage, in the striatum is tried in a rat model of global ischemia. Global ischemia was induced by the 11 vessel occlusion method for 10 minutes, during which acupuncture stimulation on GB34 and GB39 points was executed. Glutamate release in the rat striatum was monitored 256 times per second using real-time amperometric biosensor. Real time measurement data of 10 minutes prior to the induction of ischemia served as baseline data. Data acquisition continued for 30 minutes after the initiation of reperfusion. Peak concentration of glutamate release along with incidentally measured EEG and cerebral blood flow was compared between cases with and without acupuncture stimulation. Peak concentration of glutamate lowered when acupuncture stimulation was executed. A real time monitoring system of 11 vessel-occlusion induced global ischemia model was successfully established. The effect by acupuncture on acute global ischemia was successfully observed in this real-time monitoring setting, which may be one of the neuroprotective mechanism of acupuncture.

A literatual study on the acupuncture and moxibustion for hemiparesis of stroke in Euibujipsung (중풍 후 운동 장애에 대한 『의부집성(醫部集成)』의 침구치료 고찰)

  • Objectives and methods : The Euibujipsung is the one of the huge-scale encyclopedias about Oriental Medicine. To investigate the most frequently used acupoints for hemiparesis after stroke, we used Euibujipsung CR-ROM database with several key words concerned with motor weakness (半身不遂 不遂不隨 癱瘓 中臟 中腑 風痱, etc.). Results : In the result, we found five popular acupoints (GV20, LI11, LI15, ST36 and GB39), and four meridians (Stomach, Gall bladder, Large intestine and Small intestine). We also found that the Yang meridians were cited more frequently than the Yin. Conclusion : Therefore we think that these findings can give further ideas to clinical practice and research fields for stroke rehabilitation in Oriental medicine.

A Case Series of Polycystic Ovary Syndrome with Improved Hyperandrogenism Treated with Korean Medicine (한방 치료로 고안드로겐혈증을 개선한 다낭성 난소 증후군 연속증례연구)

  • Objectives: This study aims to analyze the medical record of eight cases of polycystic ovary syndrome (PCOS) in a local Korean medicine clinic, in order to provide evidence on Korean medicine treatment of PCOS and to suggest the direction of future studies. Methods: The medical records of eight cases diagnosed with PCOS based on 2003 Rotterdam Criteria and whose total testosterone level was 0.53 ng/ml and over were retrospectively analyzed. The primary outcome was total testosterone level, and the secondary outcomes included luteinizing hormone (LH) and follicle stimulating hormone (FSH) levels, LH/FSH ratio, weight, last mentrual period (LMP), numerical rating scale (NRS) score of dysmenorrhea, and other associated symptoms pre- and post-treatment. Korean medicine treatment consists of the complex herbal prescription consisting of Yukmijihwang-tang, Baekho-tang, Daesiho-tang, and Gyejibokryeong-hwan, modified according to symptoms; acupuncture and electroacupuncture on 中脘 (CV12), 下脘 (CV10), 關元 (CV4), 石門 (CV5), 合谷 (LI4), 太衝 (LR3), 三陰交 (SP6), and 懸鍾 (GB39); and indirect moxibustion on 神闕 (CV8). Results: The average total testosterone level lowered statistically significantly after the treatment. The average FSH and LH levels lowered with insignificance, while the average LH/FSH ratio and weight lowered statistically significantly. During the treatment period of three to six months, every patient had the menstrual cycle shortened except for one case; and two cases with infertility problem succeeded in being pregnant. There was no adverse event. Conclusions: The Korean medicine treatment was found effective in treating PCOS, especially regarding hyperandrogenism, amenorrhea or oligomenorrhea, and infertility.

A Clinical Study of Electroacupuncture and Auricular Acupuncture for Abdominal Pain Relief in Patients with Pancreatitis: A Pilot Study (췌장염 환자의 복통 호전을 위한 전침 및 이침 치료 예비 임상연구 프로토콜)

  • Objectives : The purpose of this study is to evaluate the feasibility of further acupuncture research as an effective alternative and safe treatment for abdominal pain control in patients with pancreatitis. Methods and Results : This study is an open-label, assessment-blind, parallel designed pilot clinical trial. Thirty participants will be assigned to the acupuncture group(n=15) and usual care group(n=15). All patients will receive the conventional standard-of-care(SOC) therapy, but only the experimental group will receive acupuncture therapy six times a week, and the duration of acupuncture therapy will be held up to 12 weeks or until the pains are to be resolved. For the conventional SOC therapy, painkiller will be given. In treatment group, the subjects will receive the identical SOC therapy in combination with electroacupuncture therapy on twelve acupuncture points(LI4, PC6, SP6, GB39, ST36, ST37), and auricular acupuncture therapy on five auricular acupuncture points(Sympathetic, Shen Men, Abdomen, Pancreas gall, and Spleen). The primary outcome will be measured using the visual analogue scale(VAS), and the secondary outcome will be measured using the painkiller demand, quality of life index and severity of pancreatitis by abdominal computed tomography(CT). Assessments will be made at baseline and at week 1, 4, 8 and 12. Results of abdominal CT will be evaluated at baseline and at week 12. Conclusions : The result of this trial will provide a basis for the effectiveness and safety of acupuncture treatment for abdominal pain in patients with pancreatitis.
