Survey of research on the optimal design of sea harbours

  • The design of harbours, as with any other system design, must be an optimization process. In this study, a global examination of the different constraints in coastal engineering was performed and an optimization problem was defined. The problem has multiple objectives, and the criteria to be minimized are the structure cost and wave height disturbance inside a harbour. As concluded in this survey, the constraints are predefined parameters, mandatory constraints or optional constraints. All of these constraints are categorized into four categories: environmental, fluid mechanical, structural and manoeuvring.

  • From January, 1990 to December, 1994, we have operated 10 patients with cardiac rupture. The patients are divided into two groups according to their clinical manifestation ; five patients in each hemorrhage and tamponade group. The patients in both groups could maintain their vital signs with closed thoracostomy and fluid resuscitation. The effect of pericardiocentesis was especially dramatic in three patients of tamponade group. The average time from injury to admission was 101 minutes and that of the patients who came our hospital via one or two other hospitals was 170 minutes comparing 31 minutes of those who came directly. The average time from admission to operation was 211 minutes. Considering 98 minutes for the diagnosis and preoperative management and another 30 minutes for the preparation for operation, operations were delayed by 83 minutes to get permission. We conclude that this delaying time for transport and operation of heart-ruptured patients should be shortened in order to manage them more effectively.

  • Spontaneous intracranial hypotension, a syndrome of low CSF pressure, occurs without preceding events such as lumbar puncture, back trauma, operative procedure or medical illness. The most characteristic symptom is an occipital or frontal headache which is aggrevated in the erect position and relieved in the supine position. This syndrome usually resolves spontaneously or with strict bed rest. When the headach persists or is incapacitating, more aggressive treatment may be necessary. Autologous epidural blood patch is highly effective in the management of spontaneous intracranial hypotension. Epidural blood produces an organized clot which effectively tamponade any dural CSF leak. The rapid relief of headache immediately after the infusion of blood occur by some other mechanism, such as an increase in subarachnoid pressure that is known to occur with infusion of fluid into the lumbar epidural space. We report three cases of spontaneous intracranial hypotension successfully treated with epidural blood patch.

Flow Properties of Doenjang (시판된장의 리올로지(Rheology) 특성에 관한 연구)

  • Flow properties of doenjang samples at various total solid contents (30, 32, 34, 36, 38, 40%) were evaluated in this study. Flow properties of doenjang samples was determined by using Haake concentric cylinderical viscometer and Instron testing machine with capillary extrusion viscometer, and consistency index(K), and flow behavior index(n) was also determined from power models, and yield stress was derived form Casson models and vanes methods. Doenjang samples showed shear-thinning (pseudoplastic) fluid with small magnitude of flow behavior index(n) (n=0.30-0.55). Casson yield stress was from 2.11 to 64.02(Pa). Vane yield stress was more effective than casson yield stress in property of reactivation. Apparent viscosity was decreased with the increase in temperature and activation energy was in the range of 6.58 to 10.70 kJ/mole. From the capillary extrusion method, K and n was increased with the increase in solid content with good correlation with. The result revealed that capillary extrusion method is useful for measuring the flow properties of doenjang.

  • The organizational theory and design in future encourages us to bring a fluid perspective to the problems and challenges face. Organizational structure, strategy, management style, teamwork, organizational change, and even products and services can be vitalized and re-formed through creative images that allow us to act in new ways. Leaders and educators sat all levels must gain comfort in dealing with the insights and implications of diverse perspectives. In a leadership paradigm in action, leaders and educators who have more flexibility and willingness to create a learning organization are successful in improving productivity and student empowerment. The key element to organizational structure and changes for interior design education becomes communications. Finally, we need to recognize that despite its roots in mechanistic thinking, organization is a creative process of imagination. We organize as we imagine, and it is always possible to imagine in new ways.

Gas-Forming Brain Abscess Caused by Klebsiella Pneumoniae

  • Gas forming brain abscess is a rare disease caused by Klebsiella pneumoniae occurring in patients with impaired host defense mechanism such as diabetes mellitus or liver cirrhosis. A 59-year-old man with 2-year history of diabetes mellitus and 20-year history of liver cirrhosis presented to the hospital with headache. On the day after admission, severe headache was developed and he deteriorated rapidly. Brain CT showed a non-enhanced mass including multiple air density as well as surrounding edema seen in the right occipital lobe, and isodensity air-fluid level seen in the right lateral ventricle. Despite emergent ventricular drainage and intraventricular and intravenous administration of antibiotics, his condition progressively worsened to sepsis and to death after 5 days. Bacterial culture of blood and ventricular fluids disclosed a Gram (-) rod, Klebsiella pneumoniae. In this report we review the pathogenic mechanism and its management.

Diffuse Pneumocephalus : A Rare Complication of Spinal Surgery

  • The common etiologies of pneumocephalus, presence of air in the intracranial cavity, are trauma and cranial surgery. Pneumocephalus after spinal surgery is an unusual postoperative complication. We report the case of a male 59-year-old man who developed a pneumacephalus after posterior lumbar surgery for spinal stenosis. Intraoperatively, a cerebrospinal fluid leak following a dural tear was noted and immediately repaired. The next day, the patient complained of headache and dizziness. Head and lumbar computed tomography scans revealed significant air in the frontal region, several cisterns, intraventricle, and extra-dural area in the spine canal. Symptoms were spontaneously resolved within 2 weeks with conservative management.

Axisymmetric dynamic instability of polar orthotropic sandwich annular plate with ER damping treatment

  • The axisymmetric dynamic instability of polar orthotropic sandwich annular plate combined with electrorheological (ER) fluid core layer and constraining layer are studied in this paper. And, the ER core layer and constraining layer are used to improve the stability of the annular plate system. The boundaries of instability regions for the polar orthotropic sandwich annular plate system are obtained by discrete layer annular finite element and the harmonic balance method. The rheological property of an electrorheological material, such as viscosity, plasticity, and elasticity can be controlled by applying different electric field strength. Thus, the damping characteristics of the sandwich system are more effective when the electric field is applied on the sandwich structure. Additionally, variations of the instability regions for the polar orthotropic sandwich annular plate with different applying electric field strength, thickness of ER layer and some designed parameters are investigated and discussed in this study.

TREATMENT OF SECONDARY FACIAL ABSCESS SUSPECTED ORIGINATING FROM LIVER ABSCESS : A CASE REPORT (간농양에 의해 이차적으로 발생하였다고 생각되는 안면부 농양)

  • In oral & maxillofacial surgery, careful management is necessary to avoid infection for old aged person or patient with diabetes mellitus(DM). We experienced a 69 years old female patient with DM, who has liver abscess and appendicitis, caused by Klebsiella pneumoniae, was developed into left mid-facial abscess and peritonitis. We report this case along with review of article for we have gained fairly good results in our attempt to resolve the patient's abscess by means of I & D of the left mid-face and percutaneous catheter drainage of the stomach, supplemented with fluid and antibiotic therapy.

Optimization of Thermal Performance in Nano-Pore Silicon-Based LED Module for High Power Applications

  • The performance of high power LEDs highly depends on the junction temperature. Operating at high junction temperature causes elevation of the overall thermal resistance which causes degradation of light intensity and lifetime. Thus, appropriate thermal management is critical for LED packaging. The main goal of this research is to improve thermal resistance by optimizing and comparing nano-pore silicon-based thermal substrate to insulated metal substrate and direct bonded copper thermal substrate. The thermal resistance of the packages are evaluated using computation fluid dynamic approach for 1 W single chip LED module.
